What are high-alert medications?
Drugs that carry a heightened risk of serious harm when used in error, such as concentrated electrolytes (for example potassium chloride), insulin, anticoagulants like heparin and warfarin, and opioids. They are stored, prescribed and checked with extra safeguards.
What is medication reconciliation?
It is the process of creating an accurate list of all a patient's medicines and comparing it against new orders at admission, transfer and discharge, so that nothing is unintentionally omitted, duplicated or changed.
