Frequently asked questions
Why is the bedside identity check the most critical step?
The commonest cause of a serious ABO-incompatible transfusion is a patient-identification error, not a laboratory error. The final bedside check that this unit is for this patient is the last barrier before harm.
What is the first action in a suspected transfusion reaction?
Stop the transfusion immediately, keep the intravenous line open with saline, check the patient and the unit details, and escalate. Then treat according to the type of reaction and report it through the haemovigilance pathway.
