Indications and dose
**Chronic hepatitis C infection (under expert supervision)**
- **Adult** (By Mouth): 400 mg once daily, for duration of treatment-consult product literature.
**Chronic hepatitis C infection (under expert supervision) for sofosbuvir**
- **Child 3-17 years (body-weight up to 17 kg)** (By mouth using granules): 150 mg once daily, for duration of treatment-consult product literature.
- **Child 3-17 years (body-weight 17-34 kg)** (By mouth): 200 mg once daily, for duration of treatment-consult product literature.
- **Child 3-17 years (body-weight 35 kg and above)** (By mouth): 400 mg once daily, for duration of treatment-consult product literature.
Side effects
Common or very common Alopecia; anaemia; anxiety; appetite decreased; arthralgia; asthenia; chest pain; chills; concentration impaired; constipation; cough; depression; diarrhoea; dizziness; dry mouth; dyspnoea; fever; gastrointestinal discomfort; gastrooesophageal reflux disease; headaches; influenza like illness; insomnia; irritability; memory loss; muscle complaints; nasopharyngitis; nausea; neutropenia; pain; skin reactions; vision blurred; vomiting; weight decreased Frequency not known Hepatitis B reactivation; Stevens-Johnson syndrome Side-effects, further information Side-effects listed are reported when sofosbuvir is used in combination with ribavirin or with ribavirin and peginterferon alfa.
Interactions
**Severe interactions:**
- Co-administration of amiodarone with a sofosbuvir containing regimen may result in serious symptomatic bradycardia.
**Other interactions (53):**
- As Epclusa contains sofosbuvir and velpatasvir, any interactions that have been identified with these active substances individually may occur with Epclusa.
- See Table 4 for examples of interactions with sensitive substrates of P-gp (digoxin), BCRP (rosuvastatin), and OATP (pravastatin).
- Potential for other medicinal products to affect Epclusa Sofosbuvir and velpatasvir are substrates of drug transporters P-gp and BCRP.
- Medicinal products that are strong inducers of P- gp and/or strong inducers of CYP2B6, CYP2C8, or CYP3A4 (e.g.
- John's wort) may decrease plasma concentrations of sofosbuvir or velpatasvir leading to reduced therapeutic effect of sofosbuvir/velpatasvir.
