FMGE 2026: Complete Guide for Foreign Medical Graduates Returning to India

The FMGE (Foreign Medical Graduate Examination), conducted by the National Board of Examinations (NBE), is mandatory for all Indian citizens and Overseas Citizens of India who have completed their MBBS from a medical college outside India and wish to practise in India.

Who Needs the FMGE?

Any Indian citizen or OCI holder who graduated from a foreign medical university — whether from Russia, China, Ukraine, Philippines, Bangladesh, Nepal or elsewhere — must pass the FMGE to obtain a licence to practise medicine in India. The exam is conducted twice a year (June and December).

Exam Format

• Format: 300 MCQs (single best answer), computer-based
• Duration: 5 hours (2 papers of 2.5 hours each)
• Pass mark: 150/300 (50%)
• Validity: Passing grants eligibility for provisional registration with State Medical Councils
• Attempts: No limit on attempts

What Topics Are Tested?

The FMGE covers the entire MBBS curriculum. Subject-wise distribution:

• Medicine & Allied: 25–30% of questions. Cardiology, nephrology, endocrinology are highest yield.
• Surgery & Allied: 20–25%. Focus on acute presentations, pre-operative assessment, and trauma protocols.
• Pharmacology: 10–12%. Drug interactions, adverse effects, mechanisms of action.
• Obstetrics & Gynaecology: 12–15%. High-yield — covers both clinical and theoretical aspects.
• Paediatrics: 8–10%. Developmental milestones, vaccination schedule (NIS), common paediatric emergencies.
• PSM / Community Medicine: 8–10%. National health programmes, biostatistics, epidemiology.
• Pathology & Microbiology: 10–12%. Haematology, histopathology, bacteriology are most tested.

The 8-Week Preparation Plan

Weeks 1–2: Clinical medicine review. Focus on Medicine using standard resources (Harrison's Principles, API Textbook of Medicine). Do 30 MCQs daily.

Weeks 3–4: Surgery, O&G, Paediatrics. Use Bailey & Love, Dutta, and OP Ghai. Do 40 MCQs daily.

Week 5: Pharmacology sprint. Drug classes, adverse effects, mechanisms of action. KD Tripathi is the gold standard.

Week 6: PSM/Community Medicine and Pathology. National health programmes are high-yield and frequently tested. Park and Robbins are your references.
