Summary
Varicose veins are dilated, tortuous superficial veins caused by venous reflux, obstruction or both. They range from an appearance concern to symptomatic chronic venous disease with aching, oedema, skin change, superficial venous thrombosis, bleeding or ulceration. The visible vein is not the whole diagnosis: assess symptoms, pulses, skin, swelling, ulcer history and the deep and superficial venous pathways. Bilateral leg swelling may have cardiac, renal, hepatic, medication or lymphatic causes and should not be labelled venous solely because veins are visible.
A vascular examination and duplex ultrasound guide treatment. Duplex is needed when intervention is being considered, when symptoms or signs suggest significant disease, when recurrence or deep-vein involvement is suspected, or when the diagnosis is uncertain. Compression can reduce venous symptoms and oedema in selected patients, but it is not an automatic treatment before arterial assessment and may be unsafe in significant peripheral arterial disease or some heart-failure states.
For confirmed truncal reflux requiring treatment, contemporary pathways generally offer endothermal ablation first when suitable, foam sclerotherapy when endothermal treatment is unsuitable or for selected tributaries, and surgery when other techniques are unsuitable or unavailable. Ulcers, bleeding, acute painful superficial thrombosis, suspected DVT and pregnancy need tailored pathways. How Common Is It?
Varicose veins and chronic venous symptoms are common, but reported prevalence changes with age, sex, occupation, pregnancy history, examination method and whether thread veins are counted. A clinic sample cannot provide a precise India-wide estimate. Many people with visible veins never develop ulceration, while a minority develop progressive oedema, pigmentation, eczema, lipodermatosclerosis or ulcers. Appearance and clinical severity are related imperfectly.
Symptoms may include aching, heaviness, itching, swelling or night discomfort that worsens with prolonged standing and improves with elevation or walking. Similar symptoms occur in musculoskeletal disease, neuropathy, arterial disease and medication-related oedema. Pregnancy can bring temporary venous enlargement and pelvic or vulval varices; many improve after delivery, but sudden pain, unilateral swelling or bleeding still needs assessment.
Indian care is affected by heat, outdoor work, long standing, transport, cost of duplex and compression garments, and access to vascular or wound services. Services should record ulcer healing, infection, bleeding, intervention type, recurrence and patient-reported symptoms rather than count procedures alone. Do not promise that treatment removes every visible vein or prevents all future varices. A realistic plan includes skin care, mobility, weight and occupational measures, examination for arterial disease, and a referral route for complications.
Risk Factors
Ageing, family tendency, pregnancy, prolonged standing, obesity, prior DVT, pelvic or abdominal obstruction, immobility and occupations with limited calf-muscle activity can contribute to venous hypertension. A history of leg injury, surgery, thrombosis, congenital venous malformation or multiple pregnancies may alter anatomy. Ask about the exact symptom pattern, previous ulcers, bleeding, superficial thrombosis, cellulitis, compression use, walking limitation and prior vein procedures.
Medication review matters: calcium-channel blockers, NSAIDs, hormones and other medicines can contribute to oedema or symptoms, while anticoagulants increase the consequences of bleeding. Smoking and diabetes raise overall vascular risk. Pregnancy and the postpartum period need separate consideration because pelvic venous disease, superficial thrombosis and DVT may overlap. A unilateral sudden change is not explained safely by chronic varicose veins alone.
Check for arterial risk factors such as diabetes, smoking, chronic kidney disease, coronary disease, stroke and claudication before recommending firm compression. Heart failure, renal or liver disease and hypoalbuminaemia can produce bilateral oedema and change fluid management. Occupational advice should be practical rather than blaming: alternate standing with walking, use calf movement and elevate when feasible. Risk factors guide prevention and investigation, but they do not replace a measured pulse examination or duplex when intervention is contemplated.
Diagnosis
History
Ask when veins appeared, whether pain, heaviness, itching or swelling follows standing, and whether elevation or walking helps. Document side, duration, ulcer or wound history, bleeding, superficial thrombosis, fever, trauma, prior DVT, prior procedures, pregnancy or postpartum status, cancer, surgery, travel and mobility. Ask about breathlessness or chest pain, anticoagulants, antiplatelets and compression. Explore work demands, footwear, skin products, cost and ability to attend dressings or procedures. Sudden unilateral swelling, severe pain, redness, bleeding or an open wound changes urgency.
Examination
Examine both legs standing and supine. Inspect venous pattern, oedema, pigmentation, eczema, induration, atrophie blanche, lipodermatosclerosis, ulcers, inflammation and scars. Measure ulcer size and location and document exudate, odour, pain and infection signs. Palpate temperature and tenderness, check femoral, popliteal and pedal pulses, capillary refill and sensation, and look for a mass or abdominal collateral veins. A pulse does not fully exclude arterial disease, and visible varices do not prove reflux.
Investigations
Duplex ultrasound maps reflux, deep-vein patency, obstruction, perforators and treatment targets. It is recommended before planned intervention and is urgent when DVT or significant superficial thrombosis is suspected. An ankle-brachial pressure index or other arterial assessment is needed when pulses are abnormal or arterial disease is suspected before compression; interpret results carefully in diabetes or calcification. Ulcer assessment may need glucose, renal tests, inflammatory markers or biopsy when atypical, non-healing or suspicious. Do not order venography or CT routinely when good duplex answers the clinical question.
Differential Diagnosis
Bilateral dependent oedema can reflect heart failure, kidney disease, liver disease, venous insufficiency, lymphoedema, obesity, immobility or medicine effects. Unilateral acute swelling and pain require exclusion of DVT, ruptured Baker cyst, cellulitis, haematoma, trauma and compartment syndrome. A tender cord suggests superficial venous thrombosis, but extension toward the saphenofemoral or saphenopopliteal junction can threaten the deep system.
Arterial ulcers are often painful, distal and associated with cool skin, reduced pulses or dependent rubor; they need arterial assessment and a different dressing and compression strategy. Neuropathic or diabetic ulcers may be painless and occur at pressure points. Pyoderma gangrenosum, vasculitis, malignancy, atypical infection and calciphylaxis should be considered for unusual, rapidly progressive, punched-out or painful wounds. Lymphoedema has tissue thickening and may coexist with venous disease.
Pelvic venous disease can produce vulval or atypical upper-thigh varices, especially after pregnancy; recurrence after technically adequate leg treatment should prompt reconsideration. A visible vein that is hard, red and painful is not simply cosmetic. A bleeding varix may conceal venous hypertension or anticoagulant-related risk. Use history, pulses, skin findings and duplex to assign the pathway; never apply a compression or ablation plan to an unexamined swollen leg.
Management
Explain the goal: symptom relief, ulcer healing, complication prevention or appearance, and clarify that intervention does not guarantee permanent clearance. Encourage walking and calf exercises, weight management where appropriate, skin moisturising around but not into an ulcer, avoidance of skin trauma and leg elevation for comfort. Do not advise prolonged bed rest. Treat eczema and infection according to examination; antibiotics are not routine for a clean venous ulcer. Wound care should be coordinated with compression and vascular assessment.
If truncal reflux is confirmed and symptoms or complications justify treatment, current guidelines recommends offering endothermal ablation when suitable. Ultrasound-guided foam sclerotherapy is offered when endothermal treatment is unsuitable or as a selected adjunct; surgery is considered when endothermal or foam options are unsuitable, unavailable or clinically inappropriate. Tributaries may be treated at the same sitting or later. Discuss bruising, thrombosis, nerve injury, pigmentation, recurrence, skin burns, allergic reactions and the small risk of pulmonary embolism.
Compression is useful for venous oedema, symptoms and ulcer care after arterial assessment, but it is not a universal prerequisite or cure. Avoid or modify it in significant arterial disease and seek advice in decompensated heart failure. Acute superficial thrombosis, bleeding, ulcer infection, suspected DVT and pregnancy require tailored review. After intervention, mobilise as instructed, protect puncture or wound sites, follow compression duration from the treating team and return for worsening pain, swelling, fever, bleeding or breathlessness.
Prescribing Information
No tablet removes established superficial venous reflux. Analgesia should be chosen for age, kidney, gastrointestinal, hepatic, cardiovascular and anticoagulation risk; avoid prolonged self-treatment with NSAIDs. Venoactive products have variable evidence and product quality and must not delay duplex, ulcer care or intervention when indicated. Antibiotics are for clinically infected ulcers or cellulitis under local policy, not for colonisation or routine varicose veins.
A bleeding varix requires direct pressure and urgent assessment, especially when the person takes anticoagulants or antiplatelets. Do not independently stop anticoagulation; the prescriber must balance bleeding against thrombosis. Superficial venous thrombosis treatment may require anticoagulation or intervention depending on length, location, proximity to the deep system, provoking factors, bleeding risk and current guideline. It must not be treated with an arbitrary antibiotic or a few days of aspirin.
Before foam or endothermal procedures, reconcile medicines, allergy, thrombosis history, renal disease, mobility, pregnancy possibility, anaesthetic issues and planned travel. Exact local product, concentration, dose and post-procedure medicines belong to the treating service. Compression prescriptions should specify pressure, garment type, application, skin checks and arterial assessment; a tight garment bought without measurement can injure skin or be impossible to use. Written instructions should include missed dressings, wound care, analgesic boundaries and named follow-up.
When to Refer
Refer to vascular or venous services when symptoms persist despite practical measures, there is skin change, recurrent superficial thrombosis, bleeding, a venous ulcer, atypical varices, suspected obstruction, recurrence after treatment or a patient is considering intervention. Duplex referral is appropriate before planned ablation, foam or surgery. Same-day or emergency assessment is required for suspected DVT, phlegmasia, rapidly spreading cellulitis, uncontrolled bleeding, severe pain with a tense limb, threatened skin or a cold pulseless foot.
Ulcer services should assess any non-healing wound, recurrent ulcer, severe pain, malodour, necrosis or uncertain arterial status. Refer urgently for systemic illness, rapidly progressive wound, disproportionate pain, exposed tendon or bone, suspected malignancy or calciphylaxis. Pregnancy with unilateral swelling, chest symptoms, painful vulval or pelvic varices, bleeding or postpartum deterioration needs obstetric and vascular coordination.
In India, specify the referral question and the capacity needed: duplex mapping, arterial pressure, wound dressings, anticoagulation, endothermal treatment, foam, surgery or interventional radiology. Send photographs only with consent, ulcer measurements, pulses, medicines, pregnancy status, previous scans and procedures. A government medical college or district hospital may provide safe assessment when private vascular care is inaccessible; document transport, cost and a contingency for deterioration.
Red Flags
Call for urgent care for breathlessness, chest pain, syncope or haemoptysis with a swollen leg, because pulmonary embolism may coexist. Sudden unilateral swelling, calf or thigh pain, warmth, new collateral veins or marked asymmetry needs DVT assessment. A painful hard superficial cord that extends toward the groin or behind the knee can approach the deep system. Severe whole-leg swelling, cyanosis, blistering, neurological change or escalating pain suggests threatened venous outflow.
Uncontrolled variceal bleeding is an emergency: lie down, elevate the leg, apply firm direct pressure with a clean dressing for at least ten minutes and call emergency services if it does not stop. Do not stand to inspect repeatedly. Anticoagulants, antiplatelets, frailty and a remote location lower the threshold for transfer.
An ulcer with fever, rapidly spreading erythema, increasing pain, crepitus, necrosis, confusion or hypotension may be infected or necrotising. A cold pale foot, rest pain, absent pulses or a new motor or sensory deficit suggests arterial emergency; do not apply tight compression. Rapidly enlarging, atypical, undermined or unusually painful ulcers need specialist and sometimes biopsy assessment. Pregnancy and the first six weeks after delivery lower the threshold for objective DVT assessment; do not attribute sudden symptoms to ordinary varices.
Indian Clinical Context
Indian venous care is heterogeneous: some centres offer duplex and endovenous treatment, while others rely on referral, limited dressings and variable compression supply. A safe plan matches intervention to local expertise and follow-up. Do not sell a procedure as a guaranteed cosmetic cure or assume that a stocking bought online has a known pressure. Measure garments, check arterial status and demonstrate application and skin inspection.
Heat, long standing, agricultural or factory work, travel, toilet access, footwear and the cost of repeated dressings affect adherence. Discuss calf movement during work, seated breaks, leg elevation when feasible and a realistic wound-care location. Nutrition, diabetes care and smoking cessation are part of ulcer healing but do not replace venous and arterial assessment. Explain that compression is commonly helpful after an appropriate vascular examination but may be dangerous with significant arterial disease or some unstable cardiac states.

If symptoms recur, reassess the anatomy and consider deep venous obstruction, pelvic reflux, new thrombosis or a non-venous diagnosis instead of repeating the same procedure automatically. Patient-reported heaviness, pain, sleep disruption, work limitation and confidence using compression are meaningful outcomes alongside appearance.
Frequently Asked Questions
Should everyone with varicose veins wear compression stockings?
No. Compression can relieve venous symptoms and support ulcer care, but it should follow examination of pulses and consideration of arterial disease, diabetes, skin condition and heart failure. A measured garment and clear application instructions are safer than an unmeasured tight stocking. Compression does not remove reflux and is not automatically required for an asymptomatic cosmetic vein.
What is the usual sequence of procedures for troublesome varicose veins?
