Summary
Varicocele is dilatation and reflux of veins in the pampiniform plexus. It is most often left sided because of venous anatomy, but side alone is not a diagnosis and a right-sided, sudden or non-decompressing lesion deserves particular attention. Many varicoceles are painless and clinically insignificant; others are associated with scrotal dragging discomfort, testicular hypotrophy, abnormal semen parameters or impaired fertility. The clinical question is not simply whether a vein looks prominent, but whether a palpable, clinically relevant varicocele is linked to a symptom or a reproductive objective.
Assessment begins with history and standing examination with and without Valsalva. Doppler ultrasound is useful when examination is equivocal, the testes are difficult to assess, symptoms are atypical, recurrence is suspected, or an adolescent has possible volume asymmetry. Routine imaging of every incidental subclinical varicocele is not a substitute for examination. Sudden painful swelling, a hard intratesticular mass, fever or acute right-sided disease needs an alternate urgent pathway.
Observation is reasonable for an asymptomatic adult with normal testes and no fertility concern, and for many adolescents when growth and volume are normal. Repair or embolisation is considered selectively for a persistent palpable lesion with abnormal semen analysis in a man seeking conception, troublesome pain after other causes are excluded, or documented testicular growth impairment in an adolescent. How Common Is It?
Varicocele is common in adolescent and adult males, but estimates vary with age, examination position, examiner technique, referral setting and whether ultrasound-only lesions are counted. A hospital fertility-clinic percentage cannot be presented as the prevalence in all Indian men. The important distinction is between a subclinical Doppler finding and a palpable clinical varicocele: they do not carry the same evidence or treatment implications.
The lesion often becomes apparent around puberty as testicular blood flow and venous loading change. Left-sided findings predominate, while bilateral disease is possible. A proportion of men with varicocele have normal semen and paternity; association does not prove that the varicocele caused infertility. Conversely, a man with infertility should not have every abnormal semen result attributed to it before female-partner factors, endocrine disease, infection, obstruction, medication, heat exposure and laboratory variation are considered.
In India, presentation may be influenced by stigma, delayed marriage, fertility pressure, out-of-pocket testing and uneven access to microsurgery or interventional radiology. Service audits should record clinical grade, testicular volumes, semen results, indication, chosen technique, complications and pregnancy or live-birth outcomes where follow-up is available. Do not promise a fixed pregnancy rate from an international study. Shared decisions should use the couple's reproductive timeline, access and treatment goals rather than a scan label alone.
Risk Factors
Risk is not a simple list of causes. Pubertal development, venous valve incompetence, increased hydrostatic pressure and anatomic drainage patterns contribute. A family history may be relevant but is not a diagnostic requirement. Ask about prior scrotal or inguinal surgery, trauma, infection, cryptorchidism, testicular torsion, renal or retroperitoneal disease, strenuous activity, prolonged standing, heat exposure, tobacco, alcohol, anabolic steroids, testosterone products and medicines that affect fertility. These factors may alter semen or pain without creating the varicocele itself.
Fertility context matters: duration of trying to conceive, previous pregnancies, partner age and ovarian reserve, intercourse frequency, erectile or ejaculatory symptoms, prior semen results and assisted-reproduction plans should be explored respectfully. A semen abnormality may be reversible or unrelated. Ask adolescents about growth, pain, exercise, school impact and body image, with confidential time appropriate to age and safeguarding.
A new varicocele after adulthood, especially on the right, one that does not reduce when supine, or one accompanied by weight loss, abdominal pain, haematuria or leg swelling raises concern for renal, retroperitoneal or venous obstruction. This is not proof of malignancy, but it changes the examination and imaging threshold. Do not label a soft scrotal swelling as varicocele without checking for hernia, hydrocele, epididymal disease and an intratesticular mass.
Diagnosis
History
Clarify duration, side, change over time, ache or heaviness, triggers, relief when supine, urinary and systemic symptoms, fever, trauma, prior surgery and fertility goals. Document pubertal stage or adult reproductive history respectfully. For couples, ask about time attempting conception and whether either partner has been evaluated. Record medicines, testosterone or anabolic-steroid use, smoking, alcohol, heat and occupational standing. Sudden onset, severe pain, a fixed non-reducing swelling, abdominal symptoms, haematuria or constitutional symptoms requires urgent consideration of another cause.
Examination
Examine in a warm room, standing and supine, with inspection and palpation of each testis, epididymis and cord. Ask for Valsalva and describe whether veins are palpable or only visible, whether they empty supine, and whether there is tenderness, hernia, hydrocele or a mass. Measure testicular volume with a consistent orchidometer or ultrasound method and compare sides; document pubertal development in adolescents. A grading system can improve communication, but grade does not alone determine treatment. Examine the abdomen and renal region when the presentation is atypical.
Investigations
Semen analysis is appropriate for a man with a palpable varicocele and fertility concern, usually with repeat testing when abnormal because results vary. Endocrine tests are targeted to low libido, erectile symptoms, severe oligospermia, azoospermia, testicular atrophy or clinical hypogonadism. Scrotal Doppler ultrasound can confirm anatomy, reflux and volume when examination is uncertain, testes are small or a recurrence is suspected; it should answer a clinical question. It does not justify treating an isolated subclinical lesion. Consider renal or abdominal imaging for sudden, right-sided or non-decompressing disease or other red flags.
Differential Diagnosis
A palpable cord swelling can be varicocele, inguinal hernia, hydrocele of the cord, epididymal cyst, spermatocele, lipoma, lymphadenopathy or thrombosed vein. A scrotal mass must be located: intratesticular masses are malignant until evaluated, whereas extratesticular lesions have a different probability and pathway. Epididymo-orchitis, torsion, torsion of an appendage and trauma can cause pain and swelling but should not be collapsed into a chronic varicocele diagnosis.
A hydrocele may obscure the testis; a large or adult-onset hydrocele needs ultrasound to assess the underlying testis. An inguinal hernia may become more prominent with standing or cough and may be reducible or incarcerated. A painful thrombosed varicocele can mimic acute scrotal disease. Referred pain from ureteric colic, hip or spine disease is possible. Scrotal skin infection and Fournier gangrene have systemic or skin signs and need urgent care.
Subclinical ultrasound reflux without palpable abnormality is not equivalent to a clinical varicocele. Bilateral findings can coexist with unilateral pain or testicular asymmetry. A sudden right lesion, failure to decompress supine, abdominal mass, haematuria or unexplained weight loss should prompt investigation for obstruction rather than reassurance. Azoospermia, severe oligospermia or endocrine symptoms may indicate primary testicular or hypothalamic-pituitary disease and require fertility or endocrine expertise. The differential directs urgency and counselling before any repair decision.
Management
Explain that treatment is goal-directed and that observation is active follow-up, not neglect. An asymptomatic adult with normal testicular examination and no current fertility objective can usually be observed with return precautions. Supportive underwear, activity adjustment and simple analgesia may help discomfort, but persistent or severe pain should trigger reassessment rather than indefinite analgesic use. Do not recommend testosterone or supplements as a varicocele treatment; exogenous testosterone can suppress spermatogenesis.
For an adult seeking conception, repair is generally considered when the varicocele is palpable, semen parameters are abnormal and the couple has a realistic reproductive plan after appropriate evaluation. Repeat semen testing and discussion with a fertility team are important. Repair may improve semen concentration, motility or morphology in selected men and may increase the chance of spontaneous pregnancy, but outcomes vary and it may not remove the need for assisted reproduction. A subclinical lesion found only by ultrasound is usually not treated for infertility.
In adolescents, monitor height, pubertal progression, testicular volume and symptoms. Persistent clinically important asymmetry, progressive hypotrophy or troublesome pain may support intervention after paediatric urology review. Techniques include microsurgical inguinal or subinguinal ligation, laparoscopic ligation and percutaneous embolisation; choice depends on expertise, anatomy, recurrence, anaesthesia, radiation exposure and local access. Discuss recurrence, hydrocele, infection, arterial injury, thrombosis, failure to improve pain and the possibility that fertility benefit cannot be guaranteed. Review after repair with examination, volume and semen assessment when appropriate.
Prescribing Information
There is no medicine that reverses venous reflux or reliably restores fertility in a varicocele. Analgesia for a mild ache should follow age, renal, gastrointestinal, hepatic, cardiovascular and anticoagulation considerations; a clinician should choose the product and duration. Avoid prolonged unsupervised NSAID use and avoid using antibiotics without evidence of infection. Treating an incidental varicocele with empiric antimicrobials is harmful stewardship.
Do not prescribe testosterone, anabolic steroids, human chorionic gonadotrophin or fertility supplements as a substitute for diagnosis. Testosterone therapy commonly suppresses gonadotrophins and sperm production; a man seeking conception should discuss alternatives with an andrologist or reproductive endocrinologist. Antioxidant combinations have uncertain product quality and inconsistent clinical benefit, and should not delay semen assessment or partner evaluation.
Before a procedure, reconcile anticoagulants, antiplatelets, allergies, renal disease, bleeding history and anaesthesia risk. Embolisation may use contrast and radiation; check kidney function, pregnancy possibility in a partner is not a patient contraindication but reproductive planning and radiation counselling should be clear, and contrast allergy history. After repair, provide the named analgesic plan, wound or puncture-site advice, scrotal-support guidance, activity limits from the operating team and red flags. Do not promise that a postoperative semen result will normalize or that pain will disappear. Any prescription must follow the current Indian product information and supervised specialist plan.
When to Refer
Refer to urology or paediatric urology when examination is uncertain, the testis is small or difficult to palpate, pain persists, a palpable varicocele accompanies abnormal semen parameters, there is adolescent volume asymmetry, the lesion recurs after treatment or the patient is considering repair or embolisation. Refer to an andrology or fertility service when semen is abnormal, azoospermia or severe oligospermia is present, hormonal symptoms exist, testosterone or anabolic steroids have been used, or assisted reproduction may be time-sensitive. Both partners should be offered coordinated assessment rather than assuming a male-only cause.
Urgent assessment is required for sudden severe scrotal pain, a hard intratesticular mass, fever with toxicity, suspected torsion, incarcerated hernia, acute testicular injury or rapidly expanding swelling. New right-sided disease, a lesion that fails to decompress supine, haematuria, abdominal mass, weight loss or renal symptoms warrants expedited urological evaluation and renal or abdominal imaging as indicated. A normal basic scrotal scan does not end evaluation when the clinical concern is obstruction or a mass.
In India, referral planning should name the receiving service, anticipated costs, Doppler or semen availability and transport route. Send examination grade, side, testicular volumes, semen reports, endocrine results, medicines and relevant imaging. Use government medical colleges or district referral networks where specialist access is limited; document safety-net advice and a follow-up owner.
Red Flags
Sudden severe unilateral pain, nausea or vomiting, a high-riding testis, abnormal lie, absent or abnormal cremasteric response, rapidly increasing swelling or pain out of proportion is an acute-scrotum emergency, not routine varicocele. Fever, spreading erythema, skin duskiness, bullae, crepitus or systemic toxicity suggests severe infection. A hard intratesticular lump, loss of testicular contour, rapid enlargement or a new adult hydrocele needs urgent ultrasound and urology review.
A new right-sided varicocele, sudden onset, failure to empty when supine, associated abdominal or flank mass, haematuria, leg oedema, weight loss or unexplained constitutional symptoms raises concern for renal or retroperitoneal obstruction. It is a red flag for investigation, not a diagnosis of cancer. Severe adolescent asymmetry, progressive reduction in testicular volume, delayed puberty or bilateral small testes requires prompt specialist assessment.
After repair, urgent advice is needed for fever, escalating pain, expanding haematoma, wound or puncture-site bleeding, testicular colour change, urinary retention, breathlessness or an allergic reaction. Persistent pain is not automatically a technical failure; check for recurrent varicocele, hydrocele, nerve or musculoskeletal pain and another scrotal diagnosis. A semen result should not be interpreted in isolation, and a delayed pregnancy warrants coordinated couple-based review. Never let a reassuring ultrasound override a high-risk acute history.
Indian Clinical Context
Varicocele care in India ranges from primary clinics and district hospitals to microsurgical urology and interventional radiology centres. Access to high-quality Doppler, semen analysis, endocrine testing, microscopy and embolisation is uneven. A practical plan states which test is necessary, where it can be performed and how an abnormal result changes the next step. Avoid expensive scanning of an asymptomatic man solely because a visible vein has caused anxiety.
Fertility pressure, stigma and cost can encourage unproven herbal products, testosterone use, repeated scans or surgery without semen confirmation. Explain confidentiality, involve both partners with consent and discuss the total pathway, including repeat semen analyses, travel, operating costs, recurrence review and assisted-reproduction options. Laboratories vary in collection instructions and reference intervals; semen should be collected and interpreted according to a current laboratory standard, with repeat testing when an abnormal result will drive intervention.
