Summary
Urticaria is a mast-cell-mediated disorder producing transient itchy wheals, angioedema or both. An individual wheal usually fades within 24 hours without bruising, while deeper angioedema may last longer. Acute urticaria lasts up to six weeks; chronic urticaria persists or recurs beyond six weeks and may be spontaneous or inducible by pressure, cold, heat, exercise, vibration, water or other reproducible stimuli. The visible rash is not the whole diagnosis: airway, breathing, circulation and gastrointestinal symptoms must be assessed for anaphylaxis.
Most acute episodes are self-limited and may follow infection, medicine, food or an apparently unexplained event. Chronic spontaneous urticaria is not usually caused by a hidden food allergy, and indiscriminate panels, elimination diets and repeated antibiotics can harm. First-line treatment is a non-sedating second-generation H1 antihistamine at a standard licensed dose, with clinician-directed escalation when needed. Short systemic corticosteroids may be considered only for a severe flare under supervision; repeated or prolonged courses are unsafe.
Omalizumab and other specialist therapies belong to an allergy or dermatology pathway with monitoring, eligibility and access checks. How Common Is It?
Urticaria is common across ages. Acute episodes are frequently seen in primary care and emergency settings, often during or after viral infection, but the cause is not always identifiable. Chronic urticaria affects a smaller proportion of the population and can substantially disrupt sleep, work, education, relationships and mental health even when examination between flares is normal. Prevalence estimates vary with definitions, access and whether angioedema is counted separately; this guide does not assert a single Indian national rate.
Wheals are dynamic: photographs, timing and a symptom diary can be more informative than an examination after the rash has disappeared. Chronic spontaneous urticaria may coexist with autoimmune thyroid disease or other conditions, but broad screening without a clinical indication generates false positives and anxiety. Inducible urticaria is diagnosed by a reproducible stimulus-response pattern, not by a patient's suspicion alone.
In India, heat, humidity, infections, over-the-counter medicines, traditional remedies and uneven access to allergy specialists can influence presentation. Financial barriers may encourage sedating antihistamines or steroid injections purchased without review. Ask what has already been taken, including ayurvedic or combination products, and explain that symptom control and trigger evaluation are separate tasks.
Risk Factors
Potential acute triggers include viral or bacterial infection, NSAIDs and other medicines, foods, insect stings, contact exposures and physical stimuli. A temporal association is not proof of IgE-mediated allergy: infection, co-factors, dose, exercise and spontaneous fluctuation can mislead. NSAIDs may aggravate chronic spontaneous urticaria in some patients, while other analgesics may be tolerated; document the actual pattern rather than labelling every painkiller an allergy.
Chronic spontaneous urticaria can be associated with autoimmune disease, but most patients do not have a single removable food trigger. Chronic inducible forms may be provoked by dermographism, delayed pressure, cold, heat, solar exposure, vibration, exercise or water. Coexisting asthma, allergic rhinitis, eczema, thyroid disease, infection, anxiety and sleep deprivation affect burden and treatment decisions without proving causation.
Ask about pregnancy, breastfeeding, age, cardiovascular disease, glaucoma, urinary retention, liver or kidney impairment and sedatives before choosing an antihistamine. Alcohol, driving, machinery and night work matter because “non-sedating” is not identical to zero impairment. Hereditary or acquired bradykinin angioedema is a separate differential when swelling occurs without itch or wheals, particularly with abdominal attacks, laryngeal episodes or ACE-inhibitor exposure.
Diagnosis
The first diagnostic task is to decide whether this is urticaria, angioedema, anaphylaxis or a mimic.
History
Ask when each lesion appears and resolves, whether it itches, bruises or leaves pigment, and whether swelling affects lips, tongue, throat, hands, feet or genitals. Record food, medicine, infection, insect, heat, cold, pressure, exercise and occupational exposures with timing and reproducibility. Ask about breathing difficulty, wheeze, throat tightness, voice change, dizziness, collapse, vomiting or cramping. For chronic disease record sleep, work, photographs, disease-control days, prior antihistamines, steroid use, pregnancy and family history of angioedema.
Examination
Assess airway, voice, tongue and oropharynx, breathing, circulation, mental state and oxygen saturation whenever angioedema or systemic symptoms are present. Examine the skin for wheals, dermographism, bruising, purpura, scale, target lesions or fixed lesions. Check for fever, joint symptoms, lymphadenopathy and abdominal tenderness. A wheal that persists in one place beyond 24 hours, burns more than itches or bruises suggests urticarial vasculitis or another disorder.
Investigations
Typical acute or chronic spontaneous urticaria needs limited testing after a careful history and examination. Targeted full blood count, inflammatory markers, thyroid testing or other investigations are guided by symptoms and local protocols; routine food IgE panels, infection screens and imaging are not justified. Provocation testing for inducible forms belongs to trained services. Suspected anaphylaxis is a clinical emergency; tryptase may support selected specialist investigation but must never delay treatment. Suspected bradykinin angioedema needs complement and specialist assessment rather than antihistamine escalation alone.
Differential Diagnosis
Anaphylaxis is urticaria or angioedema with airway, breathing or circulation compromise, or acute skin/mucosal findings with significant respiratory, cardiovascular or gastrointestinal involvement. It requires emergency intramuscular adrenaline according to the local protocol, not oral antihistamine alone. Isolated lip swelling without wheals can be ACE-inhibitor or bradykinin-mediated angioedema; itch and urticaria are usually absent, and response to antihistamines is unreliable.
Urticarial vasculitis causes lesions lasting more than 24 hours, pain or burning, bruising and sometimes fever, arthralgia, renal or abdominal disease. Contact dermatitis, atopic eczema, scabies, insect bites, erythema multiforme, mastocytosis, autoinflammatory syndromes and viral exanthems may mimic the rash. Fixed drug eruption and serum-sickness-like reactions require medicine review.
Mast-cell disorders are uncommon and should be considered with recurrent severe flushing, syncope, unexplained anaphylaxis or systemic symptoms rather than isolated ordinary wheals. Anxiety can amplify breathlessness but must not be used to dismiss airway compromise. A photographed, transient, itchy lesion supports urticaria; a lesion that is painful, persistent, purpuric or associated with systemic disease needs a different pathway and possible biopsy.
Management
Start with reassurance, trigger review and a written safety plan. Remove a clearly implicated new medicine with the prescriber, avoid a proven trigger and treat infection only when clinically indicated. For acute urticaria without anaphylaxis, use a standard-dose second-generation H1 antihistamine such as cetirizine, loratadine, fexofenadine or another locally licensed option, selected for age, pregnancy, renal or hepatic function and sedation risk. Do not combine multiple products casually or exceed labelled dosing without clinician direction.
Chronic spontaneous urticaria is treated stepwise: regular second-generation H1 antihistamine, then clinician-directed up-dosing within guideline and product limits if control is inadequate, followed by specialist assessment for omalizumab or other advanced therapy. Treatment is regular rather than only at the moment of a flare when symptoms are frequent. Avoid routine first-generation antihistamines in children, older adults, drivers and people at risk of falls or cognitive effects.
A brief oral corticosteroid course may be considered for a severe acute flare or specialist-directed rescue, but steroids do not provide a safe chronic strategy and repeated injections can cause diabetes, hypertension, infection, osteoporosis, mood effects and adrenal suppression. Omalizumab and immunomodulators require specialist eligibility, monitoring, adherence and access review. Cool compresses, bland emollients and avoidance of heat may help comfort.
Treat anaphylaxis immediately with intramuscular adrenaline and emergency observation; antihistamines do not reverse airway obstruction or shock. Reassess control, adverse effects, pregnancy, triggers and quality of life at planned review rather than escalating indefinitely.
Prescribing Information
Choose one appropriate second-generation H1 antihistamine and check the product-specific dose, age range, renal or hepatic adjustment and pregnancy information. Even newer agents can impair some patients; warn about driving, alcohol and machinery until individual response is known. First-generation drugs may cause anticholinergic effects, impaired learning, falls, urinary retention and respiratory-depressant combinations, and should not be routine chronic treatment. Avoid duplicate combination cold remedies that conceal antihistamine exposure.
Up-dosing for chronic urticaria is a clinician-supervised, off-label or product-dependent decision in some settings; document the rationale and do not assume that fourfold dosing is appropriate for every drug, child, pregnancy or comorbidity. H2 blockers, leukotriene antagonists and prolonged steroids are not universal substitutes for the guideline stepwise pathway. Check interactions and QT or sedation risks where relevant.
Omalizumab is specialist-administered or supervised according to the current product information and disease indication; discuss delayed response, observation and access. Cyclosporine or other immunomodulators require specialist monitoring of blood pressure, kidney function, interactions and infection risk. Adrenaline for anaphylaxis should follow weight- and device-appropriate emergency teaching, with a plan for repeat dosing and transfer.
Pregnancy and lactation require a risk-benefit discussion; never stop essential treatment or start a biologic without the treating team. Record suspected medicine allergy precisely, including phenotype, timing and severity, to avoid unnecessary lifelong labels.
When to Refer
Refer urgently or transfer to emergency care for tongue or throat swelling, voice change, stridor, wheeze, respiratory distress, faintness, hypotension, persistent vomiting, collapse or rapidly progressive angioedema. Give emergency anaphylaxis treatment when criteria are met; referral is not a reason to observe without adrenaline.
Dermatology or allergy referral is appropriate for chronic urticaria uncontrolled despite regular standard-dose or clinician-directed increased-dose antihistamine, recurrent angioedema, suspected inducible disease requiring provocation testing, suspected urticarial vasculitis, severe quality-of-life impairment, recurrent anaphylaxis, drug-allergy delabelling or suspected bradykinin angioedema. Specialist review is needed before omalizumab, ciclosporin or other immunomodulators.
Children, pregnant or breastfeeding patients, older adults with falls or cognitive risk, and patients with kidney, liver, cardiovascular or airway disease may need an earlier tailored review. Refer when the diagnosis is uncertain, lesions are fixed or bruising, systemic symptoms occur, or prolonged steroid use has begun. Send photographs, lesion duration, medicines, comorbidities, pregnancy status, emergency episodes, response and adverse effects. Red Flags
Airway swelling, hoarseness, stridor, wheeze, cyanosis, breathlessness, dizziness, collapse, hypotension, severe abdominal symptoms or repetitive vomiting with acute skin or mucosal symptoms may be anaphylaxis. Call emergency services, give intramuscular adrenaline into the outer thigh according to the local protocol, repeat if indicated and arrange observation; oral antihistamine is an adjunct for itch, not a substitute.
Angioedema without wheals or itch, recurrent severe abdominal pain, laryngeal attacks, family history or ACE-inhibitor exposure suggests bradykinin-mediated disease and needs urgent airway awareness and specialist evaluation. A lesion fixed longer than 24 hours, painful or burning, bruising or leaving pigmentation suggests urticarial vasculitis or another inflammatory disorder.
Fever, arthralgia, haematuria, abdominal tenderness, lymphadenopathy, weight loss, blistering, mucosal erosions or a toxic appearance should prompt evaluation for systemic or severe cutaneous disease. A new rash after a medicine with facial swelling, skin pain, blistering or mucosal involvement may be a severe drug reaction. Repeated steroid injections, escalating sedatives, uncontrolled symptoms in pregnancy or a child, and any impaired ability to access emergency care are safety red flags.
Indian Clinical Context
Indian patients may encounter high heat and humidity, dust, infections, self-purchased antihistamines, steroid injections, polyherbal preparations and variable allergy or dermatology access. Ask explicitly about over-the-counter combinations, antibiotics, NSAIDs, traditional remedies and injectable steroids. A brand name does not identify the active ingredient reliably; ask for the packet or photograph. Avoid blaming food, bathing or “toxins” when the evidence supports chronic spontaneous urticaria.
Explain emergency signs in the patient's preferred language and rehearse where adrenaline and emergency transport are available. Heat and pressure triggers may affect outdoor work, uniforms, travel and religious or household activities; advice should be feasible and not require unsafe avoidance of water, food or employment. Cost and availability may influence the choice of licensed antihistamine and specialist biologic therapy.
Indian practice should reconcile current international urticaria guidance with product approvals, pharmacovigilance, antimicrobial stewardship and local referral capacity. There is no single universal Indian test panel or biologic access pathway. Pregnancy, children and rural patients may need coordinated primary, obstetric, paediatric, dermatology and emergency services. Frequently Asked Questions
When is urticaria an emergency rather than a rash to monitor?
