Summary
Trigeminal neuralgia is a disorder of severe, brief, electric-shock-like pain in one or more trigeminal divisions, usually triggered by light touch, chewing, speaking, brushing teeth or cold air. A patient may also have a continuous background ache, but persistent pain, sensory loss, bilateral disease or an abnormal examination should prompt a search for secondary causes. The diagnosis is clinical; MRI is used to investigate the nerve, posterior fossa and causes such as multiple sclerosis or tumour. Do not label dental pain, post-herpetic pain or a painful red eye as trigeminal neuralgia without examination.
Carbamazepine is the usual first-line medicine in guideline-based care; oxcarbazepine is a common alternative when interactions or tolerability make it preferable. Both require a clinician-led titration, safety review and interaction check. Rash, fever, mucosal ulceration, facial swelling, jaundice, severe ataxia, confusion or suicidal thinking needs urgent advice. The purpose of an early visit is to reduce avoidable harm while the diagnosis is clarified. Ask whether the person has stopped brushing teeth, eating, drinking or working; these functional details can expose dehydration, weight loss and depression. Avoid promising that a scan will identify a single cause or that surgery is automatically curative. Explain that pain attacks can be frightening but that a new numb area, visual symptom, weakness or fever is a change requiring review. Keep a written record of the pain map, attacks, triggers, treatment response and adverse effects. This makes specialist triage more efficient and helps distinguish recurrent attacks from medication toxicity. Every treatment conversation should include who will review the patient, when blood tests are due and what the person should do if a rash or severe neurological symptom occurs.
How Common Is It?
Trigeminal neuralgia is uncommon compared with dental and temporomandibular pain, and a clinic series cannot be converted into an India-wide incidence claim. It is more frequent in older adults, but age does not establish the diagnosis and a young person may have multiple sclerosis or another structural cause. The right educational question is whether the attack phenotype fits and whether the examination is safe, not whether a patient resembles a textbook demographic.
Record the affected side and divisions, attack frequency, triggers, pain-free intervals, background pain, sleep and eating impact, weight change, dental treatment and all medicines. Some people avoid brushing, drinking or eating because touch triggers attacks; dehydration and malnutrition can then become secondary problems. Distinguish a first episode from a changed pattern in established disease. In India, delayed access to MRI or neurology may result in repeated dental procedures, so a clear referral note and focused neurological examination are valuable.
Incidence estimates vary with diagnostic criteria, referral patterns and whether secondary neuralgias are counted separately. A person who has had several dental consultations may not appear in neurological statistics, while a tertiary series may over-represent refractory disease. This uncertainty is a reason to avoid unsupported prevalence claims, not a reason to dismiss symptoms. Explore practical access: distance to a neurologist, ability to obtain a quality MRI, cost of sodium testing, transport during a severe attack and whether a family member can help with meals. A culturally respectful explanation can prevent stigma and the assumption that severe facial pain is exaggerated. Document the language used for the pain and the patient's own priorities, since functional impact often determines urgency more than an attack count alone.
Risk Factors
Age, family history, demyelinating disease and neurovascular contact are associated with trigeminal neuralgia, but a vascular loop on MRI is not automatically causal. Ask about multiple-sclerosis symptoms, prior facial trauma or surgery, shingles, malignancy, HIV or other immunosuppression, and symptoms affecting both sides. Dental infection, cracked teeth, sinus disease, temporomandibular disorders, migraine, cluster headache and post-herpetic neuralgia are frequent competing explanations.
Medication risk factors matter from the first consultation. Carbamazepine and oxcarbazepine can cause hyponatraemia, dizziness, diplopia, ataxia and serious cutaneous reactions. Carbamazepine induces hepatic enzymes and can reduce exposure to oral contraceptives and many other medicines; oxcarbazepine also interacts and may lower sodium. Ask about anticoagulants, psychotropics, antiretrovirals, antifungals, macrolides, hormonal contraception, alcohol and over-the-counter products. Pregnancy potential, breastfeeding, liver disease, kidney disease, falls, driving and blood dyscrasia change the safety plan.
Diagnosis
History
Ask the person to describe one attack in their own words: sudden onset, duration of seconds to about two minutes, electric or stabbing quality, exact distribution, triggers and return to baseline. Establish whether attacks are unilateral, whether pain is in V1, V2 or V3, and whether there is persistent ache, numbness, weakness, hearing or visual change. Ask about rash, fever, dental symptoms, jaw limitation, headache syndrome and neurological episodes elsewhere.
Examination
Inspect the face and mouth, assess teeth and temporomandibular movement, map light touch and pinprick in all trigeminal divisions, test corneal reflexes when appropriate and examine other cranial nerves, limbs, gait and coordination. Look for sensory loss, weakness, optic symptoms, long-tract signs, ataxia, Horner syndrome, vesicles, proptosis or a painful red eye. A normal examination supports classical disease but never excuses ignoring a changed history.
Investigations
MRI brain with a dedicated trigeminal-nerve/posterior-fossa protocol is generally recommended to assess secondary pathology and possible neurovascular contact; local radiology capability determines protocol. Contrast and additional imaging are selected for the clinical question. Do not delay emergency assessment for imaging when there is acute visual loss, ophthalmic zoster, stroke-like deficit, meningism or rapidly progressive neurological disease. Baseline blood counts, liver tests, renal profile and sodium are needed before and during anticonvulsant treatment according to the medicine and local protocol.
Differential Diagnosis
Dental pulpitis, dental abscess, cracked tooth, temporomandibular pain and sinus disease can produce facial pain but usually have local findings or a different temporal pattern. Persistent burning pain after shingles suggests post-herpetic neuralgia. Migraine, short-lasting unilateral neuralgiform headache attacks, cluster headache and other trigeminal autonomic cephalalgias have characteristic headache or autonomic features. Glossopharyngeal neuralgia causes pain in the throat, tonsillar fossa, angle of the jaw or ear rather than a typical trigeminal map.
Multiple sclerosis, cerebellopontine-angle tumour, meningioma, epidermoid cyst, infarction, inflammatory disease and dental or skull-base malignancy are important secondary causes. Bilateral pain, onset at a young age, prominent continuous pain, numbness, weakness, hearing loss, abnormal corneal reflex, optic symptoms or poor response to treatment increases concern. A painful red eye, visual loss, proptosis or fever is not routine trigeminal neuralgia and needs urgent eye, emergency or infectious assessment. The working diagnosis may remain provisional until imaging and examination are complete.
Management
Explain the diagnosis as a pain syndrome under investigation, identify triggers without advising harmful avoidance of food or hygiene, and make a plan that protects nutrition and oral care. Carbamazepine is usually offered first line; oxcarbazepine may be selected after considering interactions, sodium risk, formulation, cost and availability. Start low and titrate under supervision using the current Indian product information and neurology protocol. A medicine that reduces attacks but causes disabling ataxia or hyponatraemia is not a successful plan.
Review response, attack diary, function, adverse effects, sodium and other required laboratory tests. Do not stop carbamazepine or oxcarbazepine abruptly after prolonged use without advice. Neuropathic-pain medicines, botulinum toxin or combination treatment may be specialist options when first-line therapy is inadequate. Persistent pain despite adequate, tolerated treatment, intolerable adverse effects, diagnostic uncertainty or a structural lesion requires neurology review. Procedures may include microvascular decompression, stereotactic radiosurgery or percutaneous radiofrequency, balloon compression or glycerol techniques; selection depends on imaging, age, anaesthetic risk, sensory-risk discussion and expertise.
Prescribing Information
This is not a dosing chart. A prescriber should document baseline sodium, full blood count, liver function, kidney function, pregnancy potential, allergy history, other medicines and a follow-up date. Carbamazepine can cause leukopenia, thrombocytopenia, hepatic injury, hyponatraemia, sedation and serious skin reactions; oxcarbazepine particularly increases hyponatraemia risk. Check sodium promptly if confusion, headache, nausea, falls, seizures or worsening fatigue develop. Review blood counts, liver tests and sodium at intervals defined by the current product information and local policy.
Carbamazepine is a potent enzyme inducer: it can reduce hormonal-contraceptive effectiveness and interact with anticoagulants, antiretrovirals, psychotropics, macrolides, azoles and many other drugs. Some medicines raise carbamazepine concentrations and toxicity. Oxcarbazepine has fewer enzyme-induction effects but is not interaction-free. HLA-B*1502 is associated with carbamazepine-related Stevens–Johnson syndrome/toxic epidermal necrolysis in people with relevant Asian ancestry; consider locally available genetic guidance before starting. Pregnancy and breastfeeding require coordinated neurology and obstetric advice; do not switch or stop treatment independently.
When to Refer
Arrange urgent assessment for facial pain with visual loss, painful red eye, proptosis, fever, vesicular rash involving the eye, new weakness, ataxia, altered consciousness, meningism or rapidly progressive numbness. Refer to neurology for every suspected new case when possible, especially if the phenotype is atypical, the patient is young, symptoms are bilateral, the examination is abnormal or MRI is unavailable locally. Dental review is appropriate when oral findings suggest a dental source, but repeated procedures without a coherent diagnosis can cause harm.
Refer to a specialist pain or neurosurgical service when attacks remain disabling despite an adequate tolerated medicine trial, adverse effects prevent treatment, medication interactions are complex, MRI shows a lesion or neurovascular compression, or the patient wants a procedural discussion. Include attack description, trigger map, examination, MRI report and images where available, laboratory results, treatment doses and dates, adverse effects, contraception/pregnancy status and the patient's goals. In India, state referral pathways and access to MRI, epilepsy/neurology expertise and procedures vary; transfer early when local monitoring cannot be provided.
Red Flags
Do not reassure a patient with trigeminal neuralgia when there is persistent or progressive facial numbness, weakness of mastication, bilateral symptoms, hearing loss, optic symptoms, ataxia, other long-tract signs, a first severe headache, cancer history, immunosuppression or a new symptom after trauma. These findings can signal multiple sclerosis, tumour, infection, stroke or another secondary cause. Ophthalmic shingles, a painful red eye or reduced vision needs same-day eye or emergency assessment.
After starting treatment, fever, sore throat, mouth ulcers, unusual bruising, jaundice, severe rash, skin peeling, facial swelling, marked drowsiness, confusion, falls, diplopia or unsteady walking requires urgent clinical advice. Do not wait for a routine blood test when Stevens–Johnson syndrome, DRESS, marrow suppression, liver injury or severe hyponatraemia is possible. Ask directly about self-harm thoughts because chronic facial pain and anticonvulsant treatment can affect mood. A response to a tablet does not prove the diagnosis and should not delay investigation of red flags.
Indian Clinical Context
Use locally licensed carbamazepine and oxcarbazepine products, the current Indian package insert, hospital formulary and neurology protocol. Availability of dedicated trigeminal MRI, sodium monitoring, HLA testing, radiosurgery and microvascular decompression differs across states and between public and private services. Where MRI is delayed, document the neurological examination, escalation plan and symptoms that require emergency transfer; do not compensate for absent imaging by repeating empirical dental treatment indefinitely.
Ask about non-prescription analgesics, Ayurvedic or herbal products, alcohol, travel to a monitoring centre, cost, transport and food access. Explain the medicine plan in the patient's language, including what to do for rash, fever, severe dizziness or missed monitoring. Pregnancy and contraception counselling must be individualised; enzyme induction may make some hormonal methods less reliable, while pregnancy decisions require specialist coordination. NMC competency teaching should emphasise examination, differential diagnosis, medicine reconciliation, communication and safe referral rather than unsupervised dose selection.

Is an MRI needed for every person with suspected trigeminal neuralgia?
Guidelines generally recommend MRI for people with suspected trigeminal neuralgia because clinical examination cannot reliably exclude multiple sclerosis, tumour or another structural cause. The protocol should be suitable for the trigeminal nerve and posterior fossa, and local radiology expertise matters. A scan is not a substitute for examining the teeth, mouth, cranial nerves and the rest of the neurological system. Urgent symptoms such as visual loss, painful red eye, weakness, fever, altered consciousness or rapidly progressive numbness require emergency assessment rather than waiting for an outpatient scan. If MRI access is delayed in India, document the examination, red flags and responsible referral pathway; do not give indefinite repeat prescriptions without review. A neurovascular contact on MRI may support a classical mechanism but does not by itself prove that the contact causes the pain or guarantee that a procedure will help.
What monitoring is needed with carbamazepine or oxcarbazepine?
Before treatment, the prescriber should review the diagnosis, other medicines, pregnancy potential, allergy history and relevant baseline full blood count, liver function, kidney function and sodium. Monitoring frequency follows the current product information and local neurology protocol, with additional testing after dose changes or if symptoms develop. Sodium is particularly important with oxcarbazepine and can fall with either drug; headache, nausea, confusion, falls, marked fatigue or seizures need prompt assessment. Can trigeminal neuralgia be treated with surgery?
Specialist procedures are options when pain remains disabling despite appropriate tolerated medicine, adverse effects prevent treatment, or a structural mechanism and the person's goals support intervention. Microvascular decompression aims to separate a vessel from the nerve and may provide durable relief in selected patients, but it is an operation with anaesthetic, hearing, neurological and cerebrovascular risks. Stereotactic radiosurgery and percutaneous radiofrequency, balloon-compression or glycerol procedures are less invasive alternatives with different latency, recurrence and facial-numbness risks. The choice depends on MRI findings, age, comorbidity, anaesthetic risk, pain phenotype, local expertise and informed preference. Referral should be to a specialist service able to discuss benefits and harms; a procedure is not an emergency response to an unexamined facial pain syndrome and does not replace investigation for secondary disease.
What should a person do if a rash develops after starting treatment?
Stop and seek urgent medical advice for a new widespread rash, blistering, skin peeling, mouth or eye ulcers, facial swelling, fever or feeling systemically unwell after carbamazepine or oxcarbazepine. These symptoms can signal Stevens–Johnson syndrome, toxic epidermal necrolysis, DRESS or another serious hypersensitivity reaction, and waiting for a routine appointment is unsafe. Do not restart the suspected medicine on your own or substitute another anticonvulsant without specialist advice.
