Summary
A transient ischaemic attack (TIA) is an acute episode of focal brain, spinal-cord or retinal dysfunction caused by ischaemia without established infarction on appropriate imaging under the tissue-based definition. Older evidence and some guidelines use symptom resolution within 24 hours, so terminology differs. The safe operational rule is unchanged: sudden focal neurological loss is a stroke emergency even when it improves. A patient with resolved symptoms but diffusion-restricted infarction has a minor ischaemic stroke, not a reassuring TIA, and both require rapid cause-directed prevention.
Typical symptoms are abrupt unilateral weakness or numbness, dysphasia, dysarthria, monocular visual loss, hemianopia, diplopia or ataxia fitting an arterial territory. Isolated dizziness, syncope, confusion, amnesia or bilateral tingling is less specific, but no symptom is dismissed without clinical reconstruction. Migraine aura, focal seizure, hypoglycaemia and functional neurological disorder are common mimics. Ongoing, recurrent or fluctuating deficit must remain on an acute stroke pathway because thrombolysis or thrombectomy may still be relevant; the label TIA must never be assigned before recovery and imaging assessment.
Immediate care combines brain imaging, cervicocephalic vascular imaging, ECG and targeted laboratory tests with early antithrombotic treatment after haemorrhage and contraindications are considered. Aspirin is started promptly in suspected TIA in guideline pathways unless contraindicated. Short-course aspirin plus clopidogrel is for selected high-risk non-cardioembolic TIA or minor stroke patients presenting early, not every transient event. Atrial fibrillation generally calls for anticoagulation rather than long-term antiplatelet substitution. Symptomatic carotid stenosis requires urgent imaging and surgical review. Prediction scores cannot safely replace specialist judgement, vascular imaging or a reliable follow-up pathway.
How Common Is It?
TIA incidence is difficult to measure because symptoms resolve, patients do not always seek care and definitions vary between time-based and tissue-based systems. The 2023 American Heart Association statement estimated at least 240,000 events annually in the United States and cited a 90-day stroke risk as high as 17.8%, with almost half of subsequent strokes occurring within two days. Those figures communicate urgency but must not be presented as Indian incidence. Modern diffusion MRI reclassifies a proportion of apparently transient episodes as ischaemic stroke, while specialist review reclassifies many referrals as mimics.
Risk is concentrated early and is not uniform. Crescendo attacks, motor or speech symptoms, infarction on imaging, symptomatic extracranial or intracranial stenosis and a major cardiac embolic source identify mechanisms needing rapid action. Yet apparently low-risk presentations can still conceal carotid disease or atrial fibrillation. The ESO guideline therefore recommends specialist review within 24 hours and advises against using ABCD2 alone for triage. Improved rapid-access pathways reduce recurrence compared with historical cohorts, although the evidence does not prove one admission model fits every health system.
India carries a large stroke burden, but a reliable current national TIA incidence is not available because community recognition, coding, imaging access and referral differ widely. Rural and low-income patients may present only after a completed stroke, while tertiary datasets enrich for people who reached imaging. Service audits should measure symptom-to-first-contact time, completion of brain and vascular imaging, antithrombotic safety, atrial-fibrillation detection, carotid referral and recurrent stroke. Counts based on a clinic register alone should not be marketed as population prevalence. Public education should avoid the phrase mini-stroke, which can falsely imply that resolved symptoms are harmless.
Risk Factors
TIA shares vascular mechanisms with ischaemic stroke. Age, hypertension, diabetes, tobacco exposure, dyslipidaemia, chronic kidney disease, obesity, physical inactivity and established coronary or peripheral arterial disease increase risk, but their absence does not exclude an event. Extracranial carotid atherosclerosis can embolise to the ipsilateral retina or hemisphere; intracranial atherosclerotic disease is also important in Asian populations. Small-vessel lipohyalinosis can produce lacunar symptoms. A sudden pressure-like fall in perfusion may cause watershed events when critical stenosis is present, but most TIAs should not be attributed to low blood pressure without vascular evidence.
Cardioembolism must be sought. Persistent or paroxysmal atrial fibrillation, recent myocardial infarction, left-ventricular thrombus, mechanical valve disease, severe cardiomyopathy, atrial myxoma and selected valvular lesions change prevention from antiplatelet therapy toward anticoagulation or structural treatment. A normal single ECG does not exclude intermittent atrial fibrillation. Patent foramen ovale is common and should not be assumed causal without age, infarct pattern, venous-thromboembolism and competing mechanism assessment. Cervical-artery dissection is important after neck pain, trauma or in younger patients.
Pregnancy and the puerperium create a special differential. Hypertensive disorders, arterial dissection, cardiomyopathy, antiphospholipid syndrome and venous thrombosis can cause neurological events, while migraine aura is common. Pregnancy does not justify delaying emergency neuroimaging. In younger adults, phenotype directs testing for antiphospholipid syndrome, inherited or acquired thrombophilia, vasculitis, sickle-cell disease, HIV or other infection and substance exposure; indiscriminate panels generate false positives. Medication adherence, cost, uncontrolled blood pressure and delayed access are modifiable systems risks in India alongside biological factors.
Diagnosis
History
Reconstruct the exact first second of the event, maximum deficit, sequence, duration and recovery using patient and witness accounts. Negative loss of function, such as weakness, numbness, aphasia or visual field loss, is more typical than spreading positive scintillation or tingling, although overlap exists. Ask whether monocular visual loss was truly in one eye. Document recurrent stereotyped attacks, headache, loss of awareness, automatisms, jerking, post-event confusion, neck pain, trauma and vascular risk. Record antiplatelets, anticoagulants, missed doses, bleeding, pregnancy, migraine and seizure history and the last-known-well time in case deficits recur.
Examination
Perform and document a full neurological examination even after recovery: language, neglect, visual fields, pupils and fundi, eye movements, face, power, sensation, coordination and gait. Measure glucose immediately, vital signs and cardiac rhythm; examine pulses and listen for cardiac or carotid clues without allowing a bruit to determine stenosis. Persistent subtle aphasia, visual field loss, ataxia or hand weakness converts the problem into acute stroke. Examine for temporal arteritis where relevant, dissection signs, endocarditis, trauma, intoxication and pregnancy-related hypertension. A zero NIH Stroke Scale score does not exclude disabling posterior-circulation or retinal ischaemia.
Investigations
Urgent imaging must answer both tissue and mechanism questions. Non-contrast CT rapidly excludes haemorrhage and major mimics and is mandatory when symptoms persist or acute stroke treatment is possible; CT angiography identifies large-vessel occlusion and stenosis. For a fully resolved suspected TIA, current guidelines discourages routine CT solely to confirm TIA and recommends specialist-led MRI with diffusion- and blood-sensitive sequences when appropriate, ideally the same day. Local availability may make CT plus angiography the practical first study. Add ECG, blood count, electrolytes, renal function, glucose or HbA1c and lipids; obtain prolonged rhythm monitoring when embolic suspicion remains. Echocardiography is mechanism-directed. Carotid ultrasound requires CTA or MRA clarification when intervention is contemplated. Imaging never substitutes for history.
Differential Diagnosis
Migraine aura usually evolves over minutes, often with spreading positive visual or sensory phenomena followed by headache, whereas TIA classically starts abruptly with maximal negative deficit. However, older patients can have aura without headache and ischaemia can cause headache, so first or atypical aura still requires assessment. Focal seizure may cause positive motor or sensory activity, forced head or eye deviation, automatisms and postictal weakness or aphasia. Recurrent identical brief episodes can occur with either seizure or haemodynamic ischaemia; EEG is supportive only when the history indicates it.
Hypoglycaemia must be checked immediately because correction is time critical. Syncope produces global loss of consciousness and postural tone from cerebral hypoperfusion rather than a focal arterial syndrome; arrhythmia remains clinically important. Peripheral vestibular disorders cause vertigo but central posterior-circulation ischaemia can present with acute vestibular syndrome, especially with gait inability, direction-changing nystagmus, diplopia or limb ataxia. Bell palsy, radiculopathy, mononeuropathy and musculoskeletal pain can mimic focal weakness or numbness. Transient global amnesia predominantly impairs new memory without focal deficit.
Functional neurological symptoms are diagnosed through positive inconsistency or incongruity, not normal CT or psychiatric history. Subdural haematoma, tumour, cerebral amyloid-related transient focal episodes, demyelination and inflammatory or infectious lesions can fluctuate. Monocular visual loss also requires retinal detachment, vitreous pathology, acute glaucoma and giant-cell arteritis consideration. In pregnancy, pre-eclampsia, posterior reversible encephalopathy, cerebral venous thrombosis and migraine expand the differential. Minor stroke is not a mimic: persistent deficit or imaging infarction places the patient in the stroke category even if symptoms were initially mild or subsequently resolved.
Management
Activate an emergency stroke pathway for ongoing or recurrent deficit, record last-known-well time and obtain immediate brain and vascular imaging. A patient with a disabling minor stroke may still qualify for reperfusion despite a low NIHSS; a resolved TIA is not treated with thrombolysis. For suspected TIA with no aspirin contraindication, current guidelines recommends immediate aspirin 300 mg and specialist assessment within 24 hours. Local teams must account for current anticoagulation, active bleeding, allergy and a plausible haemorrhagic or alternative diagnosis. Do not delay urgent evaluation while calculating ABCD2.
After specialist confirmation, antithrombotic choice follows mechanism. Short-term aspirin plus clopidogrel started early reduces recurrence in trial-defined high-risk non-cardioembolic TIA or minor stroke populations, but bleeding rises with prolonged combination; it is not a default for low-certainty mimic or patients needing anticoagulation. Continue a single antiplatelet thereafter when non-cardioembolic. Treat atrial fibrillation with an appropriate oral anticoagulant after imaging and timing assessment; antiplatelets are not equivalent stroke prevention. Manage blood pressure safely, start high-intensity lipid lowering where indicated, support tobacco cessation, diabetes control, activity, diet and adherence, and treat sleep apnoea when relevant.
Image extracranial and intracranial vessels urgently. Stable patients with symptomatic ipsilateral carotid stenosis of 50% to 99% by NASCET criteria require urgent vascular or stroke-surgical referral alongside best medical therapy; benefit is greatest when endarterectomy is performed early in suitable patients. Degree of stenosis, sex, age, disability, anatomy, comorbidity and procedural risk affect benefit. Stenting is not interchangeable for everyone. Intracranial stenosis generally begins with intensive medical management. Before discharge, define who owns pending imaging, rhythm monitoring and blood results, provide recurrence instructions and secure a follow-up route that can actually occur.
Prescribing Information
Aspirin given immediately in suspected TIA is a clinical decision, not a self-treatment instruction. Check active bleeding, true hypersensitivity, severe thrombocytopenia, concurrent antithrombotics and whether intracranial haemorrhage or another diagnosis is plausible. Dyspepsia alone is not an automatic reason to withhold it, but gastrointestinal protection is individualised. Clopidogrel requires attention to bleeding, drug interactions and planned surgery. Short-course dual antiplatelet therapy should use a defined start and stop date; continuing two agents beyond the evidence-based window without another indication exposes patients to avoidable haemorrhage.
Anticoagulation is indicated for many patients with atrial fibrillation or another high-risk cardiac source, but the agent and start time depend on renal function, age, weight, valve status, infarct burden, haemorrhagic transformation risk, adherence and affordability. Direct oral anticoagulants are not used for mechanical heart valves. Warfarin demands reliable INR monitoring and interaction counselling. Combining an anticoagulant with antiplatelet therapy is not routine after cardioembolic TIA and needs a separate coronary or vascular indication. Pregnancy changes drug choice; direct oral anticoagulants are generally avoided and specialist obstetric-haematology guidance is required.
Statins can cause muscle symptoms, liver-enzyme abnormalities and interactions, but these risks should be evaluated rather than permitting uncontrolled atherosclerotic risk. Blood pressure treatment is essential long term, while aggressive immediate reduction can be unsafe when cerebral perfusion is pressure-dependent; acute decisions belong to the stroke team. Review NSAIDs, traditional medicines and over-the-counter aspirin that increase bleeding. Provide written medicine names, purpose, duration and missed-dose advice in the patient's language. Before carotid surgery, the operating service must reconcile antithrombotics. No guide can specify one prescription for every TIA because the correct regimen is inseparable from mechanism and imaging.
When to Refer
Every suspected TIA needs specialist stroke assessment within 24 hours of symptom onset, not a routine clinic appointment weeks later. Refer directly to emergency services when symptoms are present, recur, crescendo, involve motor or speech function, suggest posterior circulation disease, occur during anticoagulation or accompany severe headache, neck pain, arrhythmia or pregnancy. The referral must state last-known-well, exact deficit and duration, current examination, glucose, blood pressure, antithrombotic exposure and bleeding history. A low ABCD2 score must not be used to refuse assessment.
Urgently involve a stroke-capable imaging centre when local CT angiography or MRI is unavailable and the result would change treatment. Transfer decisions consider clinical stability, distance and whether a recurrent deficit could need thrombolysis or thrombectomy. Suspected retinal TIA requires the same vascular urgency plus ophthalmic examination when an ocular mimic remains possible. Detecting atrial fibrillation prompts cardiology or stroke input for anticoagulation and structural evaluation. Younger patients, recurrent unexplained events and suspected dissection, vasculitis or thrombophilia need targeted specialist work-up rather than a blanket test panel.
Symptomatic ipsilateral carotid stenosis requires rapid referral to a service able to confirm NASCET-equivalent severity and assess endarterectomy, because delay erodes benefit. Do not await completion of every outpatient test before initiating the surgical pathway. Pregnancy needs coordinated stroke neurology, maternal medicine, radiology and haematology or cardiology input as indicated. If outpatient management is chosen, it is safe only when same-day or next-day investigations, medicines, responsible clinician and return precautions are genuinely secured. A paper referral without confirmed access is not an adequate pathway in a resource-constrained setting.
Red Flags
Any persistent or recurrent focal deficit is acute stroke until proved otherwise. Face or limb weakness, aphasia, neglect, hemianopia, diplopia, severe gait ataxia or monocular blindness warrants emergency activation even if symptoms are improving. Do not wait for 24 hours to distinguish TIA from stroke. Fluctuating deficits may signal large-vessel occlusion or critical stenosis. A low NIHSS score can miss disabling hand, language, visual or posterior-circulation deficits. Sudden severe headache, vomiting, depressed consciousness or meningism raises haemorrhage and requires immediate imaging before an antithrombotic pathway.
Crescendo attacks over hours or days, infarction on diffusion MRI, symptomatic carotid or intracranial stenosis, atrial fibrillation, recurrent emboli and symptoms during therapeutic anticoagulation all increase concern for early recurrence or an untreated source. Chest pain, shock, new murmur or systemic emboli may indicate myocardial infarction, dissection or endocarditis. Neck pain with partial Horner syndrome suggests carotid dissection. Fever, weight loss and a new murmur broaden the diagnosis beyond atherosclerosis and make empirical anticoagulation hazardous.
Treatment red flags include haematemesis, melaena, intracranial bleeding symptoms, severe thrombocytopenia or expanding bruising after antithrombotics; anticoagulant dosing errors in renal impairment; and combination therapy without a documented indication or stop date. Pregnancy with hypertension, headache, seizures or visual symptoms needs immediate assessment for pre-eclampsia, posterior reversible encephalopathy and cerebral venous thrombosis as well as arterial ischaemia. Syncope with exertion or arrhythmia, persistent hypoglycaemia and first seizure need their own emergency pathways. Symptom resolution never cancels these warnings.
Indian Clinical Context
India's stroke pathway is constrained by geography, ambulance delays, out-of-pocket imaging cost and uneven access to MRI, CT angiography, carotid surgery and prolonged rhythm monitoring. The minimum safe response is not to relabel a resolved deficit as non-urgent. Check glucose, document the syndrome and last-known-well, give aspirin only when appropriate, and arrange brain plus vascular assessment through the fastest credible route. Tele-stroke discussion can support district decisions, but responsibility for pending tests and transfer must be explicit. If MRI is unavailable, CT-based pathways and clinical specialist review are preferable to waiting days for an ideal scan.
The 2022 Government of India stroke guidance addresses prevention, acute care, secondary prevention and rehabilitation across levels of care. Implementation still varies. Hospitals should map where patients can receive 24-hour imaging, thrombolysis, thrombectomy transfer, carotid intervention and anticoagulation follow-up. Essential medicine affordability matters: warfarin may be cheaper than a direct oral anticoagulant but unsafe without dependable INR access, while irregularly purchased antiplatelets or statins do not provide continuous prevention. Bilingual discharge information and family education improve recognition if symptoms recur.
Intracranial atherosclerosis, diabetes and hypertension are common clinical realities, but Indian ancestry must not become a shortcut that replaces imaging. Younger patients require directed consideration of dissection, rheumatic or congenital cardiac disease, antiphospholipid syndrome, sickle disorders, pregnancy-related disease and infection according to presentation. Carotid endarterectomy benefit comes from international trials and guideline thresholds; local procedural volume, perioperative outcomes and delay determine whether that evidence transfers safely. Services should audit recurrent stroke rather than advertise raw TIA volumes. Evidence remains limited for the best triage configuration in remote Indian settings, so transparent escalation rules are safer than unvalidated local scores.

Diagnostic reasoning requires separating TIA, minor ischaemic stroke, intracranial haemorrhage, migraine aura, focal seizure, hypoglycaemia, vestibular disease and functional symptoms. The learner should explain the different purposes of non-contrast CT, CT angiography, carotid duplex and diffusion MRI, and why vascular imaging and ECG search for mechanism. Pharmacology competencies encompass immediate aspirin precautions, time-limited dual antiplatelet therapy in selected non-cardioembolic presentations, anticoagulation for atrial fibrillation, statin therapy, blood-pressure treatment and bleeding surveillance. Surgical learning includes recognising symptomatic carotid stenosis that needs urgent endarterectomy assessment.
A staged assessment can begin with ten minutes of aphasia that has resolved, then reveal atrial fibrillation or severe carotid stenosis. The candidate should avoid relying on ABCD2, arrange urgent imaging and alter antithrombotic strategy when the mechanism changes. A second case with spreading scintillation and headache tests mimic analysis without dismissing a first event. A pregnancy scenario tests safe imaging and expanded differential diagnosis. Undergraduate competence means emergency recognition, initial safety, evidence-aware referral and prevention counselling; it does not confer independent authority to select reperfusion, combine antithrombotics, time anticoagulation after infarction or approve carotid intervention.
Key Exam Pearls for NEET PG
TIA is sudden focal neurological or retinal dysfunction from ischaemia with complete clinical recovery; under a tissue-based definition there is no acute infarction. If diffusion MRI shows infarction, diagnose ischaemic stroke even when symptoms resolved. Never wait 24 hours to decide. Negative symptoms such as unilateral weakness, aphasia or monocular visual loss favour ischaemia; spreading positive symptoms favour migraine aura, and jerking with postictal deficit favours focal seizure, but overlap demands clinical judgement. Check glucose immediately. Ongoing deficit enters the acute stroke reperfusion pathway.

Atrial fibrillation usually requires anticoagulation, not substitution with aspirin, after timing and bleeding assessment. Symptomatic ipsilateral carotid stenosis of 50% to 99% by NASCET criteria triggers urgent endarterectomy assessment plus best medical therapy; benefit depends on early treatment and procedural risk. Stenting is not universally equivalent. Frequently Asked Questions
Should resolved stroke-like symptoms still be assessed in an emergency pathway?
Yes. Stroke risk is front-loaded after TIA, and resolved symptoms may conceal diffusion-positive minor stroke, carotid stenosis or atrial fibrillation. Ongoing or recurrent deficit may still qualify for reperfusion treatment. Record the exact onset and last-known-well time, check glucose and seek stroke-capable assessment immediately rather than waiting to see whether symptoms return.
Can an ABCD2 score decide whether a patient with suspected TIA goes home?
No. The score can describe features studied in cohorts but misses some patients with dangerous carotid disease, atrial fibrillation or imaging abnormalities. international and ESO guidance advise against using it alone for triage. Safe discharge requires specialist assessment, relevant brain and vascular imaging, treatment, ownership of pending tests and a follow-up pathway that will occur.
Does every TIA patient need aspirin and clopidogrel together?
No. Immediate aspirin is recommended in suspected TIA when appropriate, while short-term dual therapy is supported for selected early high-risk non-cardioembolic TIA or minor stroke populations. Bleeding risk and diagnostic confidence matter. Patients with atrial fibrillation usually need anticoagulation instead, and combining antiplatelet and anticoagulant medicines requires a separate documented indication.
When is carotid surgery considered after a TIA?
A patient with symptoms in the territory of an ipsilateral carotid stenosis needs urgent confirmatory imaging and surgical assessment. Guideline benefit is established for selected stable patients with 50% to 99% NASCET stenosis, with the greatest benefit when endarterectomy is performed promptly. Anatomy, age, sex, disability, comorbidity and the centre's procedural risk influence the final decision.
