Summary
Systemic lupus erythematosus, or SLE, is a chronic autoimmune disease in which loss of immune tolerance, autoantibodies and inflammatory pathways can injure almost any organ. Manifestations fluctuate over time and may include constitutional symptoms, inflammatory arthritis, photosensitive skin disease, oral or nasal ulcers, alopecia, cytopenias, serositis, kidney disease, neurological syndromes and thrombosis related to antiphospholipid antibodies. No single symptom or blood test establishes the diagnosis. The clinician must assemble a compatible pattern, exclude mimics and continually distinguish active lupus from infection, treatment toxicity and irreversible damage.
The 2019 EULAR/ACR criteria use a positive antinuclear antibody at least once as an entry requirement, followed by weighted clinical and immunological domains; ten or more points classify SLE in an appropriate research context. These are classification criteria, not a substitute for diagnosis. A person may require treatment before satisfying a score, while an ANA-positive person with nonspecific fatigue does not thereby have lupus. Renal, central nervous system, pulmonary, cardiac, haematological or thrombotic involvement can be urgent even when skin and joint symptoms are modest.
Care is multidisciplinary and aims for remission or low disease activity, prevention of flares and organ damage, minimal glucocorticoid exposure and preservation of fertility, function and quality of life. Hydroxychloroquine is recommended for most patients unless contraindicated, but all prescribing is individualized. How Common Is It?
SLE occurs worldwide, but measured incidence and prevalence differ markedly by sex, age, ancestry, case definition, access to specialists and completeness of health records. Women are affected much more often than men, particularly during reproductive years, although children, men and older adults can develop severe disease. Population estimates from one country should not be transferred uncritically to India because clinic referral patterns, survival, laboratory access and recognition of mild disease alter the denominator. Tertiary-centre cohorts also overrepresent nephritis and other complex manifestations.
The practical burden extends beyond case counts. Lupus can interrupt education, employment, pregnancy plans and family roles; chronic pain and fatigue may persist even when measurable inflammation is controlled. Kidney failure, accelerated cardiovascular disease, infection and treatment toxicity contribute substantially to morbidity. Childhood-onset disease often has greater organ involvement and requires paediatric rheumatology and transition planning. Men with SLE are fewer in number but should not be treated as having a benign variant.
India does not have one complete, continuously updated national SLE registry that can provide a uniform state-by-state prevalence. Local service planning should therefore report how cases were identified and should track delayed diagnosis, renal screening, biopsy access, pregnancy outcomes, vaccination, infection, damage and loss to follow-up. For an individual, demographic probability is never a rule-out test. Atypical age or sex should prompt careful differential diagnosis rather than dismissal when objective multisystem findings support lupus.
Risk Factors
Susceptibility reflects interaction between many genetic variants, sex-related biology, immune regulation and environmental exposures; SLE is not caused by one behaviour and is not contagious. Family history of lupus or another autoimmune condition increases probability but most relatives do not develop SLE. Risk varies across ancestral populations, yet ancestry must not be used as a biological shortcut or to delay investigation. Ultraviolet exposure can trigger cutaneous activity and sometimes systemic flares. Smoking may worsen cardiovascular and cutaneous risk and can undermine treatment response.
Medicines can produce drug-induced lupus syndromes, with the implicated agents and clinical pattern differing from idiopathic SLE. A careful timeline should include antihypertensives, antiarrhythmics, antimicrobials, antiseizure medicines and biologic therapies without encouraging unsupervised discontinuation. Oestrogen exposure, pregnancy and the postpartum period influence thrombosis or flare risk in selected patients, particularly when disease is active, antiphospholipid antibodies are present or nephritis has occurred. Infection may precipitate symptoms and can resemble a flare.
Once SLE exists, risks for poor outcome include persistent disease activity, hypertension, proteinuria, impaired kidney function, delayed nephritis recognition, recurrent thrombosis, high cumulative glucocorticoid exposure, smoking and inability to access monitoring. Immunosuppression increases vulnerability to tuberculosis and other infections relevant in India, but exposure history or a screening test alone does not diagnose active disease. Risk assessment should include pregnancy intentions, vaccination, bone and cardiovascular health, adherence barriers, medicine affordability and travel distance. These factors guide prevention and intensity of follow-up; they do not replace organ-specific assessment.
Diagnosis
History
Establish a dated timeline rather than accepting a list of positive tests. Ask about fever, weight change, photosensitive or scarring rash, painless mouth or nasal ulcers, hair loss, pleuritic or positional chest pain, breathlessness, Raynaud phenomenon, inflammatory joint symptoms, oedema, foamy or bloody urine, headache, seizure, focal neurological change and cognitive or psychiatric symptoms. Record pregnancy history, fetal loss, thrombosis, infections, medicines and family history. Determine which manifestations occurred together and whether glucocorticoids altered them.
Examination
Measure temperature, blood pressure, weight and oxygen saturation. Examine skin and scalp, oral and nasal mucosa, lymph nodes, joints and muscles. Seek oedema, serosal signs, cardiac murmurs or rub, lung abnormalities, focal neurology and evidence of digital ischaemia. Document active inflammation separately from fixed damage. A normal examination between flares does not erase credible prior findings, but nonspecific tenderness without synovitis should not be automatically attributed to lupus.
Investigations
Initial testing commonly includes full blood count, blood film when cytopenia or haemolysis is possible, creatinine and estimated filtration rate, liver profile, urinalysis with sediment, quantified urine protein, inflammatory markers and pregnancy testing when relevant. Immunology is staged: ANA is sensitive but nonspecific; anti-double-stranded DNA, anti-Sm, complement and antiphospholipid antibodies refine probability and phenotype. Direct antiglobulin testing supports selected haemolysis evaluations. Persistent proteinuria, active sediment or unexplained renal dysfunction needs urgent nephrology-rheumatology assessment and often kidney biopsy because clinical findings do not reliably reveal histological class. Tests are interpreted longitudinally and against infection, medicines and alternative diagnoses.
Differential Diagnosis
Infection is the most consequential mimic. Tuberculosis, HIV, viral hepatitis, bacterial endocarditis and other acute or chronic infections can produce fever, weight loss, cytopenias, rash, renal abnormalities or autoantibodies. Immunosuppressed patients may have muted inflammatory signs. Cultures, imaging and targeted microbiology should precede escalation when infection is credible. Sepsis and a lupus flare can coexist. Malignancy, particularly haematological disease, may cause constitutional symptoms, nodes and blood-count abnormalities and must not be concealed beneath an autoimmune label.
Other systemic autoimmune diseases overlap clinically. Rheumatoid arthritis causes inflammatory polyarthritis; Sjögren disease causes sicca and systemic features; systemic sclerosis brings Raynaud phenomenon, skin thickening and organ fibrosis; inflammatory myopathy causes objective proximal weakness; vasculitis can affect kidney, skin, lung and nerves. Mixed connective tissue disease and overlap syndromes require phenotype-based reasoning rather than selection by one antibody. Antiphospholipid syndrome may occur with or without SLE and explains thrombosis or pregnancy morbidity but not every inflammatory manifestation. Adult-onset Still disease, sarcoidosis and Behçet disease enter selected differentials.
Common problems also coexist: iron deficiency, thyroid disease, fibromyalgia, osteoarthritis, depression, sleep disorder and medicine adverse effects can drive fatigue or pain after inflammation is controlled. Drug-induced lupus is suggested by a temporal exposure and characteristic pattern, though serology varies by drug. Pre-eclampsia, thrombotic microangiopathy and lupus nephritis may be difficult to distinguish in pregnancy. A diagnosis should explain objective findings, timing and exclusions. New organ dysfunction in established SLE deserves a fresh differential, not reflex classification as a flare.
Management
Confirm which organs are active, grade severity and agree measurable targets. Current EULAR and BSR guidance emphasizes early diagnosis, regular organ screening and treatment toward remission or low disease activity. Education covers sun protection, smoking cessation, physical activity adapted to symptoms, adherence, contraception and pregnancy planning, vaccination, cardiovascular and bone health and prompt infection assessment. Hydroxychloroquine is generally recommended for SLE unless contraindicated. Glucocorticoids, when required for active disease, are bridging treatment; long-term maintenance exposure should be minimized and withdrawal pursued when feasible.
Organ-threatening disease needs urgent specialist therapy. Immunosuppressive choices include methotrexate, azathioprine, mycophenolate and cyclophosphamide, with biologic options such as belimumab and anifrolumab for selected phenotypes; rituximab may be considered for refractory or selected severe disease. Drug hierarchy depends on manifestation, previous response, infection risk, fertility, pregnancy, kidney function, cost and availability. Neuropsychiatric syndromes must be attributed carefully because inflammation, thrombosis, infection, metabolic disturbance and primary psychiatric illness require different treatment.
Active lupus nephritis is jointly managed with nephrology. Kidney biopsy guides class-specific therapy. KDIGO 2024 lists glucocorticoid-containing regimens with mycophenolic acid analogues, low-dose intravenous cyclophosphamide or selected combinations for proliferative disease. The newer BSR 2026 guideline more strongly prioritizes combination immunosuppression for new or relapsing class III or IV nephritis, including mycophenolic acid paired with obinutuzumab, a calcineurin inhibitor or belimumab. These are not interchangeable universal orders: licensing, phenotype, fertility, infection, kidney function and Indian access determine selection. Supportive care includes blood-pressure and proteinuria control, while follow-up measures renal response, systemic activity, toxicity and damage. Stable symptoms alone do not prove renal remission.
Prescribing Information
Hydroxychloroquine dosing is based on actual body weight and individual retinal risk; EULAR gives a target of 5 mg/kg/day while allowing adjustment for flare risk and toxicity. Baseline ophthalmic assessment and risk-based retinal screening are essential, with extra caution in kidney impairment, high cumulative exposure or interacting medicines. Adherence should be explored before declaring treatment failure. Rare cardiomyopathy, conduction disturbance, myopathy, hypoglycaemia and severe skin reactions require clinical judgment. Chloroquine is not an interchangeable default because toxicity and availability differ.
Glucocorticoids may rapidly control inflammation but cumulative harm includes infection, diabetes, hypertension, osteoporosis, avascular necrosis, cataract, glaucoma and adrenal suppression. Never stop prolonged systemic therapy abruptly. Conventional immunosuppressants require agent-specific blood counts, liver or renal monitoring, infection screening, vaccination planning and reproductive counselling. Methotrexate, mycophenolate and cyclophosphamide are teratogenic or embryo-fetotoxic in relevant settings; cyclophosphamide can impair fertility. Azathioprine has different pregnancy considerations but still requires specialist supervision.
Biologic and targeted therapies require phenotype confirmation, screening for tuberculosis and hepatitis according to the agent and local protocol, attention to vaccination timing and monitoring for serious infection. Belimumab, anifrolumab and rituximab are not equivalent and access in India is variable. Antiplatelet or anticoagulant treatment depends on antiphospholipid profile and clinical events; a positive antibody alone does not justify unsupervised anticoagulation. This guide intentionally avoids a universal dose table because organ severity, body size, kidney function, pregnancy, infection and local formulations alter safe prescribing.
When to Refer
Refer promptly to rheumatology when objective multisystem disease, SLE-specific antibodies with compatible manifestations, persistent cytopenias, inflammatory arthritis, serositis or unexplained complement consumption makes lupus plausible. Do not wait for every classification point if an organ is threatened. Nephrology referral is urgent for rising creatinine, significant or increasing proteinuria, haematuria with active sediment, cellular casts, hypertension with renal findings or suspected thrombotic microangiopathy. Kidney biopsy and induction therapy belong in an experienced multidisciplinary pathway.
Same-day hospital assessment is appropriate for severe breathlessness, chest pain with haemodynamic concern, hypoxia, seizure, focal deficit, acute confusion, meningism, major bleeding, rapidly worsening renal function, severe hypertension, suspected sepsis or a new thrombosis. Haematology, neurology, respiratory medicine, cardiology or intensive care may be required according to presentation. Dermatology can clarify scarring or treatment-resistant cutaneous disease. Ophthalmology is needed for hydroxychloroquine retinal monitoring and any visual symptom.
Pregnancy should be planned with rheumatology and high-risk obstetric care, ideally during stable low activity and after medication review. Antiphospholipid antibodies, nephritis, pulmonary hypertension or prior major organ disease increase complexity. Children need paediatric rheumatology rather than simple transfer of adult regimens. In India, referrals should specify the capability required, current medicines, pregnancy status, infection risk, renal trends and urgency. The sending team remains responsible for safety-netting and interim monitoring until specialist care is actually established.
Red Flags
Possible lupus nephritis may be clinically quiet. New proteinuria, haematuria, cellular casts, oedema, hypertension or declining kidney function demands urgent reassessment even if joint pain and rash are controlled. Neurological red flags include seizure, psychosis with altered cognition, focal deficit, myelopathy, severe new headache or reduced consciousness. Chest pain, haemoptysis, hypoxia, rapidly increasing breathlessness, pulmonary haemorrhage, myocarditis, tamponade or pulmonary embolism can deteriorate quickly. Digital ischaemia, catastrophic thrombosis and severe abdominal pain with vasculitic or thrombotic concern require emergency evaluation.
Fever in an immunosuppressed person is infection until carefully evaluated; absence of a high white count or C-reactive protein response does not guarantee safety. Tuberculosis may be extrapulmonary or disseminated. Neutropenia, severe thrombocytopenia, haemolysis, pancytopenia or fragmented red cells can signal marrow disease, immune destruction, drug toxicity or thrombotic microangiopathy. Escalating immunosuppression before obtaining appropriate cultures and imaging may be dangerous, but delaying treatment of genuine organ-threatening lupus is also harmful, so parallel investigation and senior multidisciplinary decisions are often necessary.
Treatment red flags include visual change on hydroxychloroquine, severe rash, pregnancy exposure to teratogenic therapy, major infection after biologic treatment, unexplained bruising, jaundice, cytopenia or liver injury. A patient taking chronic glucocorticoid who becomes hypotensive, vomiting or unable to take medication may be at risk of adrenal crisis. Suicidal thoughts, inability to function and severe fatigue deserve active assessment rather than dismissal as ordinary lupus. Safety-netting should name the symptom, destination and timescale.
Indian Clinical Context
Indian care spans government medical colleges, district facilities and private rheumatology services with uneven access to autoantibody panels, kidney pathology, retinal screening and advanced medicines. A safe pathway starts with tests that change immediate decisions: blood pressure, blood counts, creatinine, urinalysis and quantified protein should not be delayed while awaiting extensive immunology. Samples and trends should be documented so that a patient moving between sectors does not repeatedly restart the diagnostic process. Cost-sensitive care still requires truthful discussion of expected benefit, toxicity and monitoring.
Tuberculosis, hepatitis and other infections require deliberate assessment before substantial immunosuppression, but screening results must be interpreted under current National Tuberculosis Elimination Programme and institutional protocols. A positive latent-infection test is not evidence that every fever or radiographic lesion is active tuberculosis. Conversely, empiric glucocorticoid can mask infection. Medicine availability, biosimilar quality, cold-chain reliability, pharmacovigilance and laboratory access vary; substitutions should be made by the treating specialist rather than inferred from international algorithms.
Pregnancy counselling must respect the patient's goals and include contraception only as informed shared care, not coercion. Reliable medicine lists in regional languages, warning cards for long-term steroids and clear review intervals can reduce preventable harm. The EULAR, KDIGO and EULAR/ACR sources used here are international recommendations and classification standards, not Indian statutory prescribing rules. NMC competencies define undergraduate learning outcomes, not independent management authority. State rules, hospital formularies, infection-control guidance and specialist judgment take precedence where they differ.

A graduating learner should distinguish classification from diagnosis; recognize that ANA is sensitive but nonspecific; integrate anti-double-stranded DNA, anti-Sm, complement and antiphospholipid testing with phenotype; and screen urine, protein excretion, blood pressure and renal function even when symptoms are mild. Clinical teaching should cover inflammatory arthritis, mucocutaneous findings, cytopenias, serositis, nephritis, neurological disease, infection and thrombosis. Learners should construct competing explanations before labelling fever or organ dysfunction as a flare.
Therapeutic knowledge includes hydroxychloroquine principles, minimization of glucocorticoid exposure, indication-dependent immunosuppression, reproductive safety and monitoring. Communication skills include shared decisions, adherence barriers, fertility, pregnancy and emergency safety-netting. Practical prescribing or adjustment of high-risk therapy requires supervised workplace training, local privileges and specialist oversight. PA8.5 and PE21.4 do not by themselves cover every adult organ-management decision, so the broader GM competencies and postgraduate guidance are necessary context. Key Exam Pearls for NEET PG
The classic mechanism is loss of self-tolerance with autoantibody formation, immune-complex injury, complement activation and type I interferon signalling. ANA is the sensitive entry test but has low specificity. In the 2019 EULAR/ACR framework, ANA positivity at least once is obligatory; weighted criteria are counted only when SLE is the most likely explanation, only the highest item in a domain counts and ten or more points classifies disease after at least one clinical criterion. Anti-Sm and anti-double-stranded DNA are SLE-specific antibodies; low C3 and C4 support immune activity but normal complement does not exclude lupus.
Renal disease may be asymptomatic, so urinalysis, quantified protein, creatinine and blood pressure are recurrent high-yield answers. Active sediment and proteinuria prompt biopsy consideration; histological class directs treatment. Neuropsychiatric symptoms require exclusion of infection, metabolic causes, medicines and antiphospholipid thrombosis. Antiphospholipid antibodies predispose to thrombosis and pregnancy morbidity, but laboratory positivity is not identical to clinical antiphospholipid syndrome. Anti-Ro/SSA is relevant to neonatal lupus and congenital heart block risk.
Hydroxychloroquine is foundational for most patients, with weight- and retinal-risk-aware use. Glucocorticoids control activity but should be bridged and minimized. Mycophenolate or cyclophosphamide are anchor options for proliferative nephritis, with newer combination strategies selected by specialists. Pregnancy must be planned during controlled disease and after medication reconciliation. In every vignette, first decide whether the immediate threat is infection, nephritis, thrombosis, cytopenia, neurological disease or cardiopulmonary involvement before selecting a long-term regimen.
Frequently Asked Questions
Does a positive antinuclear antibody test mean that a patient has systemic lupus erythematosus?
No. ANA is sensitive for SLE but occurs in other autoimmune diseases, infections, medicine-related states and healthy people. Diagnosis requires a compatible clinical pattern and exclusion of better explanations. Severe illness warrants urgent parallel investigation and senior decisions before simply increasing or withdrawing immunosuppression.
