Summary
Superficial-vein thrombosis (SVT), traditionally called superficial thrombophlebitis, is thrombus with inflammation in a superficial vein. It is not automatically benign: concurrent deep-vein thrombosis (DVT) or pulmonary embolism (PE) can be present, and clot close to a deep-vein junction can extend. Start with stability and PE symptoms, then document the symptomatic vein, its distance from the saphenofemoral or saphenopopliteal junction, and the measured length. A tender cord over varicose veins has a different risk profile from a long, non-varicose thrombus.
Compression duplex ultrasound is the useful test when disease is extensive, above the knee, near a junction, recurrent, non-varicose, associated with DVT risk, or when the diagnosis is uncertain. Management combines analgesia, mobility and local measures with anticoagulation for selected higher-risk SVT. A short course of a prophylactic-dose anticoagulant is commonly used for symptomatic SVT at least 5 cm long and away from a junction; therapeutic treatment is needed when DVT is found or the clot is very close to the deep system. How Common Is It?
SVT is encountered frequently in primary care, emergency departments and vascular clinics, particularly in people with varicose veins or recent venous instrumentation. Exact incidence is uncertain because many episodes are treated without imaging and studies use different definitions, lengths and anatomical sites. Therefore a local count of visible thrombosed veins cannot be converted into a reliable national Indian rate. The clinically important epidemiology is the overlap with DVT and PE, not the superficial label alone.
A first episode along a varicose tributary is often self-limited, while non-varicose, extensive, proximal or recurrent SVT carries more concern for an underlying systemic risk. Hospitalisation, surgery, pregnancy, cancer and inflammatory disease alter the threshold for imaging and follow-up. In India, access to same-day duplex varies between district hospitals and tertiary centres; missed proximal extension is a service risk. Record whether ultrasound was performed, the vein and length involved, the junction distance, DVT status and treatment start date. Auditing these fields is more useful than claiming a single incidence figure.
SVT may recur even after symptoms settle. Persistent redness does not by itself mean treatment failure, and residual ultrasound abnormality may remain. Risk Factors
Local venous stasis and inflammation are common with varicose veins, prior SVT, venous cannulation, sclerotherapy, trauma and prolonged immobility. Systemic risks include active cancer, previous VTE, pregnancy and puerperium, oestrogen exposure, obesity, increasing age, recent surgery, hospitalisation, infection and inflammatory or autoimmune disease. Thrombophilia is not the explanation for every episode; consider it selectively when SVT is recurrent, migratory, extensive, non-varicose, unusually early, or accompanied by a strong family history.
Ask whether the event followed an IV line or injection, and examine for a palpable cord in a known varicosity. A spontaneous, long segment in a non-varicose vein deserves more attention than a short cannula-associated segment. Risk rises with proximity to the deep venous junction, length of at least 5 cm, above-knee location, severe symptoms, bilateral or recurrent disease and simultaneous DVT. Cancer may be occult but routine exhaustive imaging is not justified solely by SVT; take a careful history, examine, and ensure age-appropriate screening.
Pregnancy changes medication choice and imaging thresholds. Anticoagulant history, kidney function, platelet count, bleeding, antiplatelets and NSAIDs must be reviewed before treatment. Diagnosis
History
Ask when pain, redness, swelling and the cord began; whether they are spreading; and whether a cannula, injury, operation, travel, immobility, pregnancy, cancer or oestrogen exposure preceded them. Ask about prior SVT or DVT, bleeding, medicines, renal disease and allergies. Screen explicitly for PE symptoms including new breathlessness, pleuritic chest pain, haemoptysis, syncope or unexplained tachycardia. Clarify fever, purulent discharge and skin trauma because infection can coexist.
Examination
Record observations and cardiopulmonary stability. Trace the indurated tender vein and measure the involved length; note whether it is above the knee, on the great or small saphenous pathway, and estimate the distance from a deep junction. Compare limb circumference, look for whole-limb oedema, collateral veins, varicosities, cellulitis, abscess and arterial compromise. A tender superficial cord does not exclude DVT. Document pregnancy and postpartum status and assess injection sites.
Investigations
Use compression duplex ultrasound to define extent and exclude DVT when SVT is extensive, above the knee, near a junction, non-varicose, recurrent, atypical or clinically uncertain. Ultrasound should report vein, length, occlusion, junction distance and deep-system findings. Do not use a negative D-dimer to replace imaging in a high-risk or anatomically proximal presentation. Before anticoagulation check blood count, renal function, liver function, pregnancy status where relevant and bleeding risk. Investigate PE according to the acute PE pathway, not by treating a leg ultrasound as reassurance. Differential Diagnosis
Cellulitis causes diffuse warmth, erythema, tenderness and sometimes fever, whereas SVT more often follows a linear palpable vein; both may occur together. A ruptured Baker cyst, muscle injury, haematoma, abscess, lymphoedema, chronic venous insufficiency and contact dermatitis can mimic swelling or redness. DVT may coexist or present with nonspecific pain, so a superficial cord should not end the assessment when swelling is disproportionate or risk is high. Lymphangitis and infected thrombophlebitis are considerations with systemic illness, purulence or a recent intravenous device.
Varicose-vein inflammation can be tender without a large thrombus, while a thrombosed non-varicose segment is more concerning. Superficial thrombosis close to the saphenofemoral or saphenopopliteal junction may behave like DVT; the ultrasound report and measured distance matter. An acutely cold, pale or pulseless foot is arterial disease, not uncomplicated SVT. Severe pain out of proportion, tense compartments or neurological deficits require emergency surgical assessment.
Do not label every red line as infection and prescribe antibiotics without examination. Antibiotics are for documented or strongly suspected infection, not sterile SVT. Similarly, do not escalate anticoagulation solely because redness persists if anatomy is stable and symptoms are improving. Re-image when symptoms extend, recur or fail to follow the expected course. Management
For a short, distal, low-risk SVT without DVT, PE, junction proximity or major risk factors, use walking as tolerated, limb elevation, a warm or cool local measure, and analgesia after checking contraindications. Compression may relieve venous symptoms if arterial supply is adequate and the person can apply it safely. Topical or oral NSAIDs can reduce pain and inflammation, but avoid them in significant kidney disease, ulcer or gastrointestinal bleeding risk, anticoagulation, late pregnancy or other product-label contraindications.
For symptomatic SVT at least 5 cm long and at least 3 cm from a deep junction, guideline pathways commonly favour prophylactic-dose fondaparinux for 45 days; prophylactic low-molecular-weight heparin is an alternative where appropriate. Exact agent and dose must follow the current Indian product label, renal function, pregnancy status, availability and local protocol. A clot within 3 cm of a junction, extension into the deep system or confirmed DVT should be managed using a DVT anticoagulation pathway, usually therapeutic anticoagulation for at least three months. Do not routinely excise or strip an acutely inflamed vein, use thrombolysis, or give antibiotics without infection. Prescribing Information
Before an anticoagulant, document ultrasound anatomy, weight, kidney and liver function, platelet count, pregnancy or breastfeeding, active bleeding, recent procedures, medicines and affordability. Fondaparinux and low-molecular-weight heparin are renally cleared and require product-specific precautions; severe renal impairment may require a different specialist plan. Direct oral anticoagulants are not automatically interchangeable with prophylactic SVT regimens, and an evidence-based dose for DVT is not a harmless substitute for a short SVT course. Follow the current Indian label and institutional protocol.
Explain injection technique when heparin is used, adherence, bruising, black stools, haematemesis, severe headache, heavy menstrual bleeding and when to seek emergency care. When to Refer
Send urgently for breathlessness, pleuritic chest pain, haemoptysis, syncope, shock, hypoxaemia or suspected PE. Urgent vascular or emergency assessment is required for whole-limb swelling, cyanosis, rapidly extending clot, severe pain, threatened skin, phlegmasia, suspected DVT, or SVT within 3 cm of a deep junction. Infected thrombophlebitis with fever, rigors, purulence or a central line may need cultures, intravenous antibiotics and source control.
Refer to vascular medicine or surgery for extensive proximal SVT, recurrent or migratory episodes, non-varicose spontaneous SVT, persistent symptoms, uncertain ultrasound anatomy or consideration of intervention. Red Flags
PE symptoms are the most important red flags: sudden breathlessness, pleuritic or central chest pain, haemoptysis, syncope, collapse, new hypoxaemia or unexplained tachycardia. Treat these as an emergency even when the visible vein appears small. Rapidly progressive redness or swelling, oedema of the entire limb, cyanosis, severe pain, blistering, sensory change or weakness may indicate extension, phlegmasia, compartment syndrome or infection. A cold, pale or pulseless foot needs arterial assessment.
During anticoagulation, major bleeding, faintness, falling haemoglobin, vomiting blood, black stool, intracranial symptoms, expanding haematoma or uncontrolled uterine bleeding require immediate review. Fever with rigors, purulent drainage or a tender line track raises infected thrombophlebitis. Worsening symptoms despite apparently correct treatment require repeat examination and usually duplex, not an automatic dose increase.
System red flags include an ultrasound report without junction distance, absent DVT assessment in a proximal or extensive case, no written end date, inability to obtain medicine, or NSAID use without a bleeding discussion. Indian services range from peripheral centres without duplex to tertiary hospitals with vascular ultrasound and interventional expertise. A safe pathway identifies where compression ultrasound can be obtained, how quickly transfer can occur, and who owns review of a pending scan. Bedside signs such as a tender cord or Homans sign cannot define extent or exclude DVT. If imaging is delayed, document the clinical risk, bleeding assessment and specialist discussion rather than creating indefinite empirical treatment.
The National List of Essential Medicines includes heparins and warfarin, but local stock, cold-chain needs, injection training, laboratory access and cost remain practical determinants.
