Summary
Rhinosinusitis is inflammation of the nasal cavity and paranasal sinuses, usually after a viral upper-respiratory infection. Congestion, nasal discharge, facial pressure and reduced smell are common, but purulent mucus alone does not prove bacterial disease. The key management decision is whether the illness is uncomplicated and self-limiting, severe, persistent without improvement, or worsening after initial recovery. Most acute episodes settle over about two to three weeks; antibiotics offer little benefit for uncomplicated early illness and cause adverse effects and resistance. Assessment must also identify dental disease, allergic rhinitis, migraine, orbital infection, invasive fungal disease and intracranial complications. Ask the patient to describe the side, duration and trajectory of symptoms rather than relying on colour of discharge. A safe consultation ends with explicit eye, neurological and systemic warning signs.
Sinus anatomy matters because the ostia are narrow drainage pathways and obstruction can produce pressure without bacterial invasion. The maxillary, frontal, ethmoid and sphenoid compartments have different relationships to the orbit, teeth, skull base and cavernous sinus. Symptoms alone cannot map a compartment reliably. Do not infer a scan finding from a facial-pressure description.
In teaching, emphasise that anatomical proximity explains the red flags but does not make every headache a sinus complication. A careful examination, clear time course and reliable safety net are more valuable than indiscriminate imaging or repeated antimicrobial exposure. Seek help early when danger signs develop. Clinical judgement should remain proportionate, documented and responsive to change, especially where specialist access, transport or follow-up is limited.
How Common Is It?
Acute sinus symptoms are common in primary care and community pharmacies, particularly during viral respiratory seasons, pollution episodes and periods of crowding. Estimates vary because studies use different symptom definitions and many people self-manage without consultation. Chronic rhinosinusitis is a different inflammatory syndrome, usually lasting at least 12 weeks and often associated with polyps, asthma, allergy, dental disease or structural obstruction. In India, smoke, particulate pollution, occupational dust, biomass fuel exposure, crowded living conditions and variable access to ENT review influence symptoms and follow-up. A coloured discharge or facial pain does not by itself establish bacterial infection. Ask about duration, trajectory, unilateral symptoms, dental pain, smell, allergy, asthma, prior surgery, medicines, pregnancy, immune status and ability to return if the condition worsens.
Ask about the patient’s ability to sleep, eat, work and care for dependants, but do not use functional disruption as a substitute for severity assessment. A person with modest pain but new diplopia is urgent; a person with severe pressure but normal vision and improving symptoms may need supportive care and review.
Risk Factors
Viral exposure, allergic rhinitis, asthma, nasal polyps, smoking, air pollution, dental infection, impaired mucociliary clearance and recent upper-respiratory infection increase symptom burden. Structural obstruction or prior surgery may predispose to recurrent or chronic disease. Immunosuppression, uncontrolled diabetes, haematological disease, chemotherapy or prolonged corticosteroid exposure raises concern for invasive fungal infection when severe facial pain, necrosis, fever or orbital signs occur. Children have age-specific anatomy and medicine risks; older adults may present atypically. Pregnancy changes the acceptable medicine list, not the need to treat orbital or intracranial danger. Recurrent unilateral disease, foul smell, epistaxis or a mass needs examination rather than repeated empiric antibiotics. Additional context includes recurrent viral episodes, poor nasal airflow, dehydration and inability to clear secretions. Ask about prior endoscopic surgery, septal deviation, retained dental material, recent extraction, swimming or contaminated irrigation equipment, and exposure to construction dust. These factors do not establish bacterial infection; they help explain recurrence and guide examination. Chronic disease should be confirmed with appropriate objective findings rather than a symptom label alone.
Use a trajectory diary when the history is uncertain: note the first day, peak severity, fever, discharge, analgesic use, improvement and any second deterioration. This is often more informative than a single examination after several days of self-treatment. Explain that a delayed prescription is conditional, not a routine course.
Diagnosis
History
History should establish the exact day of onset, initial viral symptoms, improvement or “double worsening”, fever severity, nasal blockage, discharge, facial or dental pain, cough, smell, headache, visual symptoms and neurologic symptoms. Examine vital signs, hydration, face, eyes, vision and ocular movements; inspect the nose when equipment and skill permit and examine the teeth and throat. Severe unilateral pain may be dental or another diagnosis. Acute uncomplicated rhinosinusitis is clinical; routine sinus radiographs or CT are not indicated. CT is reserved for suspected complication, alternative diagnosis, chronic/recurrent disease or pre-operative planning under ENT direction. Nasal swabs do not reliably identify the cause of uncomplicated disease. Consider COVID-19 or influenza pathways when the epidemiology and systemic symptoms fit.
Examination
Examine the patient in a structured order: general appearance and hydration first, then temperature, pulse, respiratory rate and oxygen saturation when indicated. Inspect facial symmetry and eyelids; assess visual acuity, pupils and ocular movements if symptoms are significant. Examine the nose, throat and teeth with appropriate lighting. Document focal tenderness without treating it as diagnostic alone.
Investigations
Examine vital signs, hydration, face, eyes, vision and ocular movements; inspect the nose when equipment and skill permit and examine the teeth and throat. For recurrent symptoms, look for a pattern rather than labelling every episode acute bacterial sinusitis. Seasonality, allergy, polyps, asthma, dental disease and occupational exposures may be more important than antimicrobial treatment. ENT assessment can identify anatomy and chronic inflammation when objective confirmation is needed.
Differential Diagnosis
A viral upper-respiratory infection is most likely when symptoms are within ten days and gradually improve. Acute bacterial rhinosinusitis is considered when symptoms persist for around ten days without improvement, are severe for several days with high fever and purulent discharge or facial pain, or worsen after an initial improvement; no single symptom is definitive. Allergic rhinitis causes itching, sneezing and fluctuating watery discharge; migraine can mimic facial pressure. Dental infection causes focal tooth pain and unilateral maxillary symptoms. Chronic rhinosinusitis requires persistent symptoms plus objective inflammatory evidence and may coexist with polyps or asthma. Unilateral foul discharge or bleeding suggests foreign body, tumour or dental disease. Invasive fungal disease, orbital cellulitis, meningitis and cavernous-sinus complications are emergencies, not routine sinusitis.
Caregivers should be shown how to prepare and use saline safely and how to clean the irrigation device. Forceful pressure, unsafe water and shared devices can cause harm. Young children may need age-appropriate alternatives and should not receive an adult combination product. Written instructions reduce dosing and technique errors.
Management
For uncomplicated early illness, explain the expected two-to-three-week course and offer self-care: fluids as tolerated, rest, saline nasal irrigation with safe water and clean equipment, and simple analgesia when appropriate. A nasal corticosteroid can be considered for troublesome symptoms in adults and children aged 12 years or more according to local guidance and product instructions; teach correct technique. Evidence for oral decongestants, antihistamines, mucolytics and steam is limited or context-dependent, and topical decongestants should not be prolonged because rebound congestion can occur. Reassess rather than repeatedly adding medicines if symptoms persist. When bacterial criteria are met, use current local antibiotic guidance, consider delayed or immediate prescribing according to severity and follow-up, and document why. Do not use antibiotics for colour alone.
When access is limited, prioritise the examination that changes urgency: visual function, eye movements, mental state, hydration and systemic appearance. Arrange review by a defined time rather than advising indefinite observation. If referral is delayed, document the escalation route and the symptoms that should prompt immediate transfer.
Prescribing Information
When an antibiotic is selected, confirm the working diagnosis, age, weight, allergy history, pregnancy or breastfeeding, renal function, interacting medicines and local resistance guidance. Do not copy a regimen from a foreign guideline without checking the current Indian formulary. Delayed prescriptions require a defined start rule and reliable follow-up. Analgesics should be single-ingredient where possible, with label and contraindication checks. Nasal steroid choice and duration should follow the product label and diagnosis. Do not use oral steroids, antifungals or prolonged decongestants without specialist rationale. Review prior antibiotic exposure and avoid routine macrolides when resistance makes them poor choices. Explain expected benefit in absolute terms and record shared decisions. Check whether the patient can return if worse before offering a delayed prescription. Avoid sharing antibiotics between family members, saving leftovers or using a pharmacy substitution without confirming the active ingredient and strength. In children, calculate from a current paediatric reference and never extrapolate an adult tablet. In pregnancy, breastfeeding and renal impairment, obtain current specialist or formulary advice before prescribing. Reassess treatment failure instead of automatically broadening coverage.
Good stewardship is compatible with good symptom care. Explain why antibiotics do not shorten uncomplicated early disease, offer measures with a plausible comfort benefit, and make a review plan. This avoids the false choice between an unnecessary antibiotic and no care at all.
When to Refer
Send urgently to hospital for orbital swelling, eyelid cellulitis, displaced eye, double vision, impaired eye movements, reduced vision, severe frontal headache, meningism, focal neurological signs, frontal-bone swelling, severe systemic infection or rapidly progressive deterioration. Same-day assessment is also appropriate for severe symptoms in an immunocompromised or uncontrolled diabetic patient, suspected invasive fungal disease, dehydration or inability to take treatment. Refer to ENT for recurrent episodes, persistent symptoms despite appropriate care, chronic disease, polyps, unilateral obstruction or bleeding, structural concerns, dental-source uncertainty or need for endoscopy/CT. Children with recurrent disease, atypical symptoms or medicine-safety concerns need paediatric or ENT review. Pregnancy does not itself require referral, but medicine selection should be clinician-led and complications treated without delay.
Clinical uncertainty should be visible in the record. State whether the working diagnosis is viral rhinosinusitis, probable bacterial disease, chronic inflammatory disease, dental disease or an alternative. List the evidence for and against complication, the medicine safety checks, and the agreed follow-up.
Red Flags
Orbital and visual danger signs include periorbital swelling or cellulitis, proptosis, painful or restricted eye movements, diplopia, reduced visual acuity or a severe unilateral headache with eye findings. Intracranial warning signs include meningism, confusion, seizure, focal neurologic deficit, severe frontal headache, vomiting with systemic toxicity and swelling over the frontal bone. Fever with rapidly worsening facial swelling, necrotic nasal tissue, black eschar, severe pain out of proportion or cranial-nerve deficits is concerning for invasive fungal disease, particularly in diabetes or immunosuppression. A toxic appearance, hypotension, dehydration or inability to take fluids requires urgent care. Unilateral persistent obstruction, recurrent epistaxis, facial numbness, a mass or foul discharge needs specialist examination. A normal-looking nose does not exclude a deep complication, and a normal temperature does not reassure in immunosuppression. Ask specifically about eye pain, colour vision, headache pattern, vomiting, drowsiness and new weakness. A patient with rapidly progressive symptoms should not wait for outpatient imaging. Stabilise according to local capability, involve ENT and ophthalmology or emergency services, and communicate the suspected route of spread. Record the time of onset and any prior antibiotic or steroid exposure because these can alter examination findings.
Indian Clinical Context
Antibiotic stewardship is especially important where pharmacy access is easy, cultures and ENT review are uneven, and resistance patterns vary by region and facility. Do not equate a yellow or green discharge with a need for antibiotics; record the illness trajectory and severity. Use safe boiled, sterile or appropriately treated water for nasal irrigation, clean the device and avoid forceful irrigation when completely obstructed. Ask about combination cold medicines, sedating antihistamines, topical decongestant overuse, pregnancy, breastfeeding, diabetes and kidney disease. Pollution and smoke reduction may improve symptoms, but do not promise that masks or steam cure infection. Dental services may be the correct pathway for unilateral maxillary disease. If CT, endoscopy or ENT access is delayed, provide explicit return precautions and a review date; lack of access must not lower the threshold for orbital, intracranial or invasive-fungal emergencies.

Students must know their limits: endoscopy, CT interpretation, drainage decisions and management of invasive fungal disease require supervised specialty practice.

Most acute rhinosinusitis is viral. Symptoms for ten days or less without severe deterioration generally do not justify antibiotics; persistent symptoms without improvement or severe or double-worsening patterns change the discussion. Sinus radiographs are not routine. Orbital cellulitis, proptosis, ophthalmoplegia, diplopia, reduced vision, meningism, severe frontal headache, frontal-bone swelling or focal neurologic signs require hospital assessment. Chronic rhinosinusitis is not the same as repeated acute colds and needs objective assessment. Unilateral foul discharge suggests a dental source, foreign body or mass. How can viral sinusitis be distinguished from bacterial sinusitis?
No single colour of mucus proves bacterial infection. Avoid prolonged topical decongestant use, duplicate paracetamol in combination products and forceful irrigation. Persistent or recurrent symptoms need review rather than an expanding over-the-counter regimen.
Which sinus symptoms require emergency care?
Seek urgent care for eyelid or facial swelling, a displaced or painful eye, double vision, restricted eye movements, reduced vision, severe frontal headache, meningism, confusion, seizure, focal weakness, frontal-bone swelling, severe systemic illness or rapidly progressive deterioration.
