Investigations
Baseline testing is often directed at glucose or HbA1c, full blood count, renal and liver function, thyroid function, vitamin B12 with a clarifying metabolite test when appropriate, and serum protein electrophoresis or immunofixation when a paraprotein is plausible. Add HIV, syphilis, coeliac, copper or autoimmune testing only when history or examination supports it. Alcohol-related and nutritional neuropathies may coexist. Entrapment neuropathy, radiculopathy and plexopathy are usually focal or dermatomal rather than a symmetric stocking pattern. Spinal cord disease is suggested by a sensory level, brisk reflexes, sphincter symptoms or upper motor-neuron signs. Guillain–Barré syndrome and other inflammatory neuropathies can progress rapidly with weakness, areflexia or autonomic instability. Consider erythromelalgia, restless legs, peripheral arterial disease, small-fibre disorders, functional symptoms and musculoskeletal pain when examination does not fit. Burning feet with normal large-fibre studies need a coherent reassessment, not dismissal. A sudden unilateral deficit, acute ataxia, cauda-equina symptoms or a rapidly ascending illness is an urgent neurological problem, not routine chronic neuropathy.
Management
Treat the cause where identified. Optimise diabetes collaboratively without promising that glucose normalisation will reverse established nerve injury; review B12 deficiency promptly and address malabsorption or the responsible medicine. Reduce alcohol, correct nutritional deficiency, and discuss toxic or neurotoxic exposures with the relevant prescriber or occupational service. Never discontinue insulin, antimicrobials, anticonvulsants or cancer therapy independently. Review at a defined interval with symptoms, examination and function rather than a single laboratory number.
Protect feet every day: inspect soles and between toes, wash and dry carefully, moisturise dry skin but not between toes, avoid walking barefoot and test bath water away from an insensitive foot. Use properly fitting footwear and avoid direct heat, hot-water bottles and chemical corn removers. A clinician or foot-care service should assess callus, deformity, ulcers, infection and pulses; diabetes may require a risk-stratified foot pathway. For painful neuropathy, first discuss sleep, pacing, activity, psychological support and realistic goals. Correct B12 deficiency with a clinician-selected preparation and route, while investigating why deficiency occurred; do not give folate alone when B12 deficiency has not been considered. For neuropathic pain, commonly used classes include a serotonin–noradrenaline reuptake inhibitor, a gabapentinoid or a tricyclic antidepressant in selected patients. Urgent or same-day assessment is required for new weakness with breathing or swallowing difficulty, rapidly ascending symptoms, acute severe ataxia, bowel or bladder dysfunction, a sensory level, severe autonomic instability, acute foot infection with systemic illness, or sudden focal neurological deficit. A painful swollen or discoloured foot, ulcer, spreading cellulitis, gangrene or suspected Charcot change requires urgent foot or vascular assessment.
Refer to diabetes or foot services for recurrent ulcers, high-risk diabetes, deformity, callus, poor vision or inability to inspect the feet. Consider haematology, infectious disease, oncology, toxicology, dietetics or pain services when testing identifies a paraprotein, infection, treatment toxicity, nutritional disorder or complex pain. Referral should coordinate care, not produce parallel unsupported prescriptions.
Red Flags
Red flags for acute neurological disease include hours-to-days onset, rapid progression, ascending numbness, new weakness, facial or bulbar symptoms, breathlessness, severe gait ataxia, urinary retention or incontinence, a sensory level, marked neck or back pain, fever, meningism or autonomic instability. These features need urgent assessment for spinal, inflammatory, infectious, vascular or toxic causes.
Foot emergencies include an ulcer, black or pale tissue, rapidly spreading redness, pus, fever, disproportionate pain, new warmth and swelling, suspected foreign body or inability to bear weight. Neuropathy can mask pain; visual inspection and palpation matter. Do not wait for routine blood tests when tissue is threatened.
Cancer, weight loss, night sweats, monoclonal protein, vasculitic rash, renal decline or recent neurotoxic exposure raise concern for systemic disease. Severe hypoglycaemia, intoxication, medication overdose or abrupt withdrawal also requires immediate care.
Safety red flags from medicines include new confusion, falls, suicidal thinking, severe sedation, breathing difficulty, allergic reaction or escalating unsupervised doses. A normal HbA1c, normal nerve-conduction study or absence of visible foot injury does not neutralise a concerning clinical pattern.

Screen for diabetes using current local practice, ask about vegetarian or restrictive diets and gastrointestinal disease when B12 deficiency is plausible, and review alcohol, occupational solvents, agricultural chemicals, unregulated supplements and traditional remedies without judgement. Foot-care advice must fit footwear, household floors, work demands, climate and access to podiatry.
