Summary
Rubella is an acute infection caused by rubella virus, an enveloped RNA virus in the genus Rubivirus. It spreads mainly through respiratory droplets and is often mild or asymptomatic, but viraemia during early pregnancy can infect the placenta and fetus. Congenital rubella syndrome (CRS) may include sensorineural hearing loss, cataract or other eye disease, congenital heart disease, microcephaly, developmental impairment, hepatosplenomegaly and thrombocytopenic purpura. The clinical priority is therefore not simply treating a rash; it is recognising possible exposure or infection in pregnancy and interrupting transmission.
Incubation is usually 14–21 days. A person can transmit virus from about seven days before until seven days after rash onset; an infant with CRS may shed virus for much longer. Postnatal illness commonly presents with low-grade fever, malaise, a fine maculopapular rash beginning on the face and tender posterior auricular, posterior cervical or suboccipital nodes. Arthralgia is especially common in adult women. Clinical appearance alone is unreliable because many infections are subclinical and several exanthems resemble rubella.
This guide supports supervised assessment and public-health action. A pregnant person with rash or exposure should not be reassured from appearance alone, and a newborn with compatible defects needs coordinated paediatric, ophthalmic, cardiac and audiological evaluation.
How Common Is It?
Rubella incidence changes with vaccine coverage, age at vaccination, immunity gaps and surveillance quality. Before widespread immunisation, most people acquired infection in childhood and periodic outbreaks exposed susceptible pregnant people. High coverage with a rubella-containing vaccine reduces both infection and CRS, but pockets of susceptibility can sustain outbreaks. Serosurveys are useful for planning, yet a single local percentage should not be assumed to represent every Indian district or antenatal population.
India introduced measles-rubella (MR) vaccine into the Universal Immunisation Programme (UIP) in 2017 and conducted a large catch-up campaign for children. CRS surveillance and laboratory confirmation remain important because mild maternal infection may be missed and congenital deafness or cataract may be labelled without an etiological diagnosis. WHO describes elimination as interruption of endemic transmission, not merely fewer visible rashes; documentation and molecular epidemiology matter.
In an Indian clinic, ask about childhood vaccination records, household children, school or hostel outbreaks, travel, and contact with a person with a febrile rash. Do not use ethnicity, residence or absence of a vaccine card as proof of susceptibility. The practical denominator is the individual’s validated immunisation and laboratory history.
Risk Factors
The principal risk for CRS is rubella infection during pregnancy, particularly in the first trimester, when organ development is active. Susceptibility may follow no vaccination, incomplete vaccination, uncertain records, failure to seroconvert or waning documented immunity in a setting with ongoing transmission. Crowded schools, hostels, childcare, outpatient waiting rooms and households facilitate spread. A person can transmit infection before recognising a rash.
Ask about gestational age, date and nature of exposure, vaccination, previous laboratory results, travel, contact with children and current rash or joint symptoms. Immunosuppression may alter presentation and antibody responses. Infants with CRS are an important reservoir because prolonged shedding can expose caregivers and clinical staff. Pregnancy is not a reason to avoid routine clinical care; it is a reason to use a clear testing and infection-control pathway.
Avoid preventable risk by vaccinating eligible children and non-pregnant susceptible adults according to current national or institutional guidance, improving ventilation and keeping a suspected case away from pregnant contacts during the infectious period. Do not recommend live vaccine during pregnancy, and do not delay urgent evaluation while searching for a vaccine certificate.
Diagnosis
Rubella is a clinical and laboratory diagnosis. The combination of a rapidly spreading, non-confluent fine rash, low-grade fever and posterior auricular or posterior cervical lymphadenopathy is suggestive but not specific. Measles, parvovirus B19, dengue, enterovirus infection, scarlet fever, roseola and drug eruption may look similar. Confirmation is especially important in pregnancy, outbreak investigation and suspected CRS.
History
Record rash onset, sequence of spread, fever, cough, coryza, conjunctivitis, node tenderness, arthralgia, medication exposure and vaccination. Establish last menstrual period or gestational age, contact dates and whether the contact had laboratory-confirmed rubella. Ask about hearing, vision and cardiac concerns in a child. A “rubella contact” based only on a neighbour’s rash is not equivalent to a confirmed exposure.
Examination
Assess temperature, hydration, rash morphology and distribution, posterior auricular/cervical/suboccipital nodes, joints and respiratory symptoms. Look for measles clues such as high fever, the three Cs and Koplik spots, while remembering that absence does not exclude it. In a newborn, examine for cataract or abnormal red reflex, murmurs, hepatosplenomegaly, jaundice, petechiae, microcephaly and growth restriction. Use standard precautions and local infection-control advice.
Investigations
Collect serum for rubella IgM and IgG at the time advised by the laboratory; IgM timing affects sensitivity and false-positive results occur. Demonstrating seroconversion or a significant rise in IgG, or low-avidity IgG in the right context, can support recent infection. Confirm unexpected positive results through a reference laboratory, particularly during pregnancy. CRS evaluation may include neonatal IgM, PCR or virus detection according to national laboratory protocol, plus hearing, eye and cardiac assessment. Do not diagnose immunity from IgM, and do not interpret an isolated result without dates and assay context.
Differential Diagnosis
Measles is usually more toxic, with higher fever, cough, coryza, conjunctivitis, Koplik spots and a coalescing rash that progresses over several days; it requires its own urgent surveillance pathway. Parvovirus B19 can cause a slapped-cheek or lacy rash and arthralgia, while dengue may produce fever, myalgia, thrombocytopenia and warning signs. Roseola classically causes high fever followed by rash as fever settles in a young child. Scarlet fever has a sandpaper texture and streptococcal pharyngitis. Drug eruption, enterovirus, secondary syphilis and autoimmune disease remain context-dependent alternatives.
Rubella lymphadenopathy is helpful but not definitive. A maculopapular rash in pregnancy should be approached as an undifferentiated rash illness until testing and exposure history clarify it. Avoid using the presence or absence of arthralgia as a gatekeeper. In endemic settings, dengue, chikungunya and measles may be more immediately likely than rubella, but competing diagnoses do not remove the need to protect a fetus.
For CRS, consider genetic syndromes, cytomegalovirus, toxoplasmosis, congenital syphilis and other causes of cataract, deafness, heart disease, purpura or growth restriction. A newborn’s phenotype may be incomplete; one feature does not prove or exclude CRS. Coordinate targeted testing rather than ordering an indiscriminate congenital-infection panel.
Management
There is no specific antiviral treatment for uncomplicated postnatal rubella. Provide fluids, rest, and age-appropriate antipyretic or analgesic advice under a clinician’s plan. Avoid aspirin in children. Use respiratory precautions and exclude a suspected case from school, childcare or work according to the local public-health protocol, generally through seven days after rash onset. Notify or discuss with surveillance services when required; rubella is a vaccine-preventable outbreak-sensitive disease.
The management priority after maternal exposure is accurate immune and infection assessment, not empiric antibiotics or immediate termination of pregnancy. Arrange obstetric or infectious-disease review, interpret serial serology with a reference laboratory and offer ultrasound surveillance when infection is confirmed or remains plausible. Counselling must explain that fetal risk varies with gestational age and that ultrasound cannot exclude later hearing impairment. Decisions about continuation or termination belong to the pregnant person with specialist counselling and the legal framework of India, including the MTP Act and institutional safeguards.
A child with suspected CRS needs multidisciplinary follow-up: paediatrics, audiology, ophthalmology, cardiology, developmental services and infection control. Assess feeding, growth and developmental milestones, and support the family without stigma. Antibiotics do not shorten rubella infection unless a separate bacterial diagnosis exists.
Prescribing Information
MR vaccine is a live attenuated rubella-containing vaccine used in India’s UIP; schedule details must be checked against the current MoHFW operational guidance and local programme. MMR may be used where indicated in private or institutional programmes. A live rubella-containing vaccine is contraindicated during pregnancy and in people with a previous severe vaccine reaction or specified severe immunodeficiency. Accidental vaccination shortly before recognising pregnancy is not an indication for termination, but the pregnancy should be documented and counselled through the appropriate service.
A non-immune postpartum patient can generally receive rubella-containing vaccine before discharge when clinically eligible; breastfeeding is not a contraindication. Advise avoidance of conception for the interval specified in current national guidance, commonly 28 days after a live vaccine. Do not use immunoglobulin or antibiotics as routine post-exposure substitutes for vaccination.
Paracetamol dosing, if used, must be age-, weight- and formulation-specific and verified from current product information. Do not provide a fixed public prescription in an educational page. Check pregnancy, liver disease, duplicate combination products and allergies. Serology is not a reason to delay urgent clinical review, and vaccine history should be recorded in the individual’s immunisation record.
When to Refer
Refer urgently to obstetrics or a specialist infectious-disease service when a pregnant person has a compatible rash, a credible exposure with uncertain immunity, positive or equivocal rubella serology, or an ultrasound abnormality that may fit congenital infection. Laboratory results should be interpreted with gestational age, timing of exposure and assay characteristics. A general practitioner should not make fetal-risk promises from one IgM result.
Refer a child or newborn to paediatrics when CRS is suspected, especially with congenital heart disease, cataract or abnormal red reflex, sensorineural hearing loss, microcephaly, purpura, hepatosplenomegaly or developmental concern. Arrange cardiology, ophthalmology and audiology assessment rather than waiting for all three classic manifestations.
Escalate any severely unwell rash patient, neurological syndrome, respiratory compromise, bleeding or dehydration through emergency pathways. Coordinate public-health notification and contact tracing according to state and national surveillance instructions. Referral is part of containment and family protection, not a sign that routine supportive care has failed.
Red Flags
A rash illness in pregnancy is a red flag because clinical rubella cannot be reliably confirmed or excluded at the bedside. Urgent testing and specialist contact are warranted after a compatible exposure or illness, particularly before 16 weeks’ gestation. Newborn red flags include an absent or abnormal red reflex, murmur or heart failure, petechiae or thrombocytopenia, jaundice, hepatosplenomegaly, microcephaly, poor growth and failed hearing screening.
High fever, severe toxicity, breathlessness, altered consciousness, seizures, neck stiffness, shock, bleeding or dehydration suggests a complication or another diagnosis and requires emergency care. A unilateral painful eye, severe headache or focal neurological sign is not routine rubella.
Do not delay measles or dengue assessment while waiting for rubella results. Isolate according to the suspected infection pathway, document dates and contacts, and avoid exposing pregnant patients in waiting areas. A positive screening result should trigger confirmation and counselling, not panic or an unsupervised medication decision.
Indian Clinical Context
India’s UIP uses MR vaccine to protect children against measles and rubella, with campaign and routine schedules updated by MoHFW. Programme details, age windows and catch-up instructions can change; verify the latest state and national operational guidance rather than reproducing a fixed schedule from memory. Private MMR schedules may differ from UIP delivery, so document the product and date.
In antenatal care, record rubella vaccination history and prior serology, but do not promise that routine screening is identical across every facility. Where laboratory access is limited, establish a referral route and preserve the serum sample and dates. A government medical college, district hospital or accredited reference laboratory may be needed for confirmatory testing and fetal assessment. Costs, travel and confidentiality affect whether a plan is workable.
CRS prevention is a systems task: high routine immunisation coverage, catch-up vaccination, rash surveillance, safe specimen transport, congenital-anomaly recognition and postpartum vaccination of eligible non-immune women. Counsel families in clear language and avoid blaming a mother for an infection acquired before she knew she was pregnant. Use interpreters when needed and protect patient privacy during contact tracing.
