Summary
Renal colic is acute pain most often caused by a ureteric stone, but flank pain alone does not establish the diagnosis. The first task is to identify sepsis, anuria, a solitary functioning kidney, pregnancy, renal impairment, uncontrolled symptoms or a dangerous alternative diagnosis. Those findings change a pain pathway into urgent assessment, imaging and urology care. Give analgesia early. Non-steroidal anti-inflammatory drugs are first choice when safe; paracetamol and monitored opioid rescue are alternatives when they are not. Imaging confirms the stone, obstruction and competing diagnoses. Observation is reasonable only when the patient is stable and symptoms are controlled. Alpha-blocker medical expulsive therapy is selective and off-label, with best evidence for a distal ureteric stone over 5 mm in a person appropriate for conservative care. Sepsis with obstruction, and anuria with obstruction, require antibiotics, cultures and urgent collecting-system drainage; antibiotics alone do not give source control. Stone analysis and recurrence risk determine later prevention. A normal temperature at one assessment does not rule out early infection, especially after antipyretics; repeat observations and the clinical trajectory matter. Conversely, a visible stone should not terminate assessment when pain, shock, pregnancy-related symptoms, peritonism or laboratory findings point to another emergency. Record the time of last urine, baseline kidney disease, single-kidney status and any immunosuppression because these alter the urgency of imaging and drainage. If an observation plan is selected, ensure the person can obtain medicines, maintain oral intake, understand the return instructions and access emergency care; social barriers can make an apparently small stone unsafe to manage at home. Document the agreed review interval and responsible service, because a discharged patient with an obstructed ureter must not be left without ownership of follow-up. Reassess sooner if symptoms change rather than waiting for a routine appointment.
How Common Is It?
Stone disease is common, but frequency varies with climate, diet, occupation, genetics, imaging access and whether a report includes only hospital presentations. A tertiary-centre series cannot be used as an India-wide prevalence estimate. For individual care, separate a first uncomplicated episode from high-risk recurrence. Early onset, family history, bilateral disease, one functioning kidney, infection stones, recurrent urinary infection, bowel disease or surgery, renal tubular acidosis, hyperparathyroidism, obesity, reduced renal reserve and inherited disease change the prevention pathway. Heat and restricted access to safe water can reduce urine volume, but no individual stone should be attributed to climate without assessment. In many Indian settings ultrasound is readily available while CT and endourology require transfer; that changes logistics, not the urgency of fever with obstruction. Relief of pain does not prove stone passage. A written follow-up plan is essential, because silent persistent obstruction can injure the kidney.
Risk Factors
Ask about previous stones, relatives, fluid intake relative to heat and work, urinary infection, gout, bowel disease, bariatric or intestinal surgery, immobilisation, supplements and regular medicines. Low urine volume, hypercalciuria, hypocitraturia, hyperoxaluria and hyperuricosuria are mechanisms to investigate selectively, not diagnoses made from a food history alone. Excess salt can increase urinary calcium in susceptible people, while unnecessary calcium restriction can increase intestinal oxalate absorption; blanket dietary calcium restriction is poor advice for most calcium-oxalate stone formers. Review topiramate, loop diuretics, carbonic anhydrase inhibitors, high-dose vitamin C and relevant antiretrovirals when clinically appropriate. Urease-producing infection may be associated with struvite stones and needs culture-directed treatment and a clearance plan. Pregnancy changes both differential diagnosis and imaging. Fever, rigors, dysuria, vomiting, reduced urine, immunosuppression, diabetes and a solitary kidney lower the threshold for objective assessment and urgent referral.
Diagnosis
History
Typical pain is abrupt, severe and intermittent from flank to groin, often with nausea, vomiting, urinary urgency or haematuria. Ask about fever, rigors, dysuria, anuria, pregnancy, anticoagulants and prior stones, while actively considering abdominal, vascular and gynaecological alternatives.
Examination
Record observations, hydration, abdominal tenderness, guarding, costovertebral-angle tenderness and bladder distension. Assess for shock, peritonism, a pulsatile mass and genital pathology where appropriate. Restlessness can fit colic but does not exclude sepsis or ectopic pregnancy.
Investigations
Use urinalysis and culture when infection is suspected, and obtain renal function, electrolytes, blood count and pregnancy testing according to presentation. EAU recommends ultrasound first, then non-contrast CT to confirm an acute suspected stone when needed; CT is more accurate and may identify another cause. Image urgently with pyrexia, solitary kidney or diagnostic doubt. In pregnancy and children ultrasound is first line; MRI may help when needed. Do not delay analgesia or resuscitation for imaging.
Differential Diagnosis
Appendicitis, diverticulitis, cholecystitis, pancreatitis, bowel obstruction and perforated viscus may cause acute abdominal or flank pain. A leaking or ruptured abdominal aortic aneurysm is a time-critical mimic, particularly with hypotension, collapse, back pain or a pulsatile mass. Renal infarction, pyelonephritis, papillary necrosis and retention have different mechanisms but may produce similar symptoms. In anyone with pregnancy potential, obtain a pregnancy test and consider ectopic pregnancy, ovarian torsion and a complicated ovarian cyst. Acute scrotal pain requires assessment for testicular torsion. Musculoskeletal pain may coexist but lacks the classic visceral pattern. Haematuria supports neither stone nor safety: it may be absent in stones and present in infection, malignancy or glomerular disease. Fever with obstruction must be treated as infected hydronephrosis until excluded. Cross-sectional imaging matters partly because it finds stones and partly because it prevents premature closure in a dangerous atypical presentation.
Management
Assess and resuscitate an unwell person using ABCDE principles, establish access and give early analgesia. Offer an NSAID first if renal function, bleeding, gastrointestinal, cardiovascular, allergy, anticoagulant and pregnancy context permit. EAU evidence finds NSAIDs and paracetamol effective and NSAIDs superior to opioids for acute colic. Use paracetamol when NSAIDs are unsuitable and an opioid other than pethidine as monitored second-line or rescue treatment. Antispasmodics do not add useful analgesia to an NSAID. Do not force intravenous fluids in an otherwise euvolaemic person to flush a stone: it does not speed passage and can increase distension pain. Analgesia-refractory colic needs urological decompression or ureteroscopic removal. Observation requires a return plan and review of symptoms, renal function and stone position. Fever, sepsis, anuria, worsening renal function and solitary-kidney obstruction are never routine medical-expulsive-therapy cases. Definitive treatment depends on stone size, site, density, anatomy, infection and local expertise.
Prescribing Information
Do not use this guide as a prescription. EAU gives diclofenac 100–150 mg per day by oral or suppository route for 3–10 days as an example when spontaneous passage is expected; use the lowest effective dose for the shortest duration and current local product information. Avoid or seek senior advice with acute kidney injury, reduced GFR, dehydration, active ulcer or bleeding, heart failure, high cardiovascular risk, NSAID-sensitive asthma, anticoagulation risk and pregnancy. NSAIDs are generally avoided later in pregnancy, requiring obstetric input. Opioids require observation for sedation, respiratory depression and vomiting; avoid pethidine because vomiting is more common. Tamsulosin is not an analgesic. It is an off-label option for a selected distal ureteric stone over 5 mm when conservative care is safe; explain dizziness, postural hypotension, syncope risk and ejaculatory adverse effects. Do not use it to defer drainage. Potassium citrate and other prevention drugs require a stone phenotype, serum or urine assessment and monitoring rather than empirical alkalinisation.
When to Refer
Obtain immediate emergency or urology input for suspected infected obstruction, sepsis, anuria, bilateral obstruction, obstruction of a solitary kidney, uncontrolled pain or vomiting, acute kidney injury, pregnancy with concerning symptoms or diagnostic uncertainty. Refer urgently when imaging shows persistent obstruction or a stone unlikely to pass, renal function deteriorates or emergency attendance recurs. Elective urology review is appropriate for recurrent stones, high-risk composition, complex anatomy, residual burden, recurrent infection or a preference for active treatment after informed discussion. Ureteroscopy has a higher chance of a single-procedure stone-free outcome than shock-wave lithotripsy but has different anaesthetic and complication considerations. Children, recurrent formers and people with bilateral, infection, uric-acid or cystine stones need metabolic and specialist evaluation. Where capacity is limited, arrange transfer rather than repeating analgesic injections if red flags persist. Discharge advice should state the imaging result, expected course, medicine limits, stone-capture advice, follow-up and exactly when to return. Absence of pain does not independently prove resolution.
Red Flags
Fever, rigors, hypotension, tachycardia, confusion or inflammatory concern with obstructing stone disease indicates possible urosepsis. Take urine and blood cultures when feasible, start antimicrobials promptly and arrange urgent drainage with a ureteric stent or percutaneous nephrostomy; both are effective. Definitive stone treatment waits until infection resolves. Anuria, bilateral obstruction or a stone in a solitary functioning kidney threatens renal function and needs urgent decompression assessment. Persistent pain despite appropriate medication or vomiting that prevents hydration also requires escalation. Consider aneurysm with shock or collapse, ectopic pregnancy with relevant symptoms, testicular torsion with acute scrotal pain and peritonitis with guarding. Avoid the shorthand that 'fever plus renal colic' is a final diagnosis: it signals an emergency evaluation. Safety-net for fever, reduced urine, inability to drink, escalating pain, syncope, new bleeding or failure to improve. Use a lower threshold for review in pregnancy, immunosuppression and chronic kidney disease.
Indian Clinical Context
Hydration advice must be feasible. Heat exposure, outdoor work, long travel and limited toilet access make regular intake difficult; workplace planning and safe water access may be more useful than a generic extreme-volume target. Dietary counselling should retain normal calcium intake and reduce excess salt or animal protein where relevant, rather than blaming staple foods or endorsing unvalidated stone-dissolving remedies. Uric-acid stones can dissolve with supervised alkalinisation, but most stones do not dissolve with herbal products. Generic medicines, CT access, microbiology and endourology availability vary, so document the local escalation route. A negative ultrasound does not safely exclude a ureteric stone or alternative diagnosis when clinical risk remains. Antibiotic selection should follow local resistance patterns, renal function, cultures and policy; this guide intentionally provides no empirical regimen. Stone analysis and a basic metabolic screen are worthwhile after recurrence or high-risk disease only when results will guide prevention. Unsubstantiated regional prevalence, procedure-price and universal stone-belt claims are avoided because they do not improve safe care.

NSAIDs are first-line when safe and superior to opioids; antispasmodics add no benefit. Do not force fluids. Alpha blockers are off-label MET with best evidence for conservatively managed distal ureteric stones over 5 mm, particularly 5–10 mm. Frequently Asked Questions
Why are NSAIDs usually preferred for acute renal colic?
Guideline evidence supports NSAIDs as first-choice analgesia because they relieve pain effectively and reduce prostaglandin-related ureteric pressure; they are generally more effective than opioids. That does not make them safe for everyone. Kidney injury or dehydration, low baseline GFR, active bleeding, heart failure, cardiovascular risk, allergy, anticoagulants and pregnancy can change the balance. Check the person, not only the pain score. Paracetamol or a monitored opioid may be needed when an NSAID is unsuitable. Persistent pain despite appropriate medicines is not a reason to stack unmonitored doses; it can indicate a need for drainage or stone removal. Before discharge, make the analgesic plan concrete: explain maximum dose and duration from the product label or prescription, avoid duplicate cold-and-flu or combination products, avoid alcohol and driving if an opioid causes sedation, and seek review instead of taking extra tablets when pain changes. A person with vomiting may not absorb oral medicine reliably and may need reassessment. In chronic kidney disease, dehydration after vomiting or diarrhoea can magnify NSAID harm; renal function and fluid status require particular attention. The goal is enough pain relief to permit assessment and safe observation, not simply a lower pain score. Pain may recur as a stone moves, but recurrent pain without a safety review should not be assumed harmless.
Does tamsulosin make every ureteric stone pass?
No. Alpha blockers are off-label medical expulsive therapy, not universal stone-dissolving treatment. Current EAU guidance describes contradictory evidence overall, with clearest benefit in selected people with a distal ureteric stone over 5 mm who are otherwise suitable for observation; recent studies particularly support 5–10 mm stones. Weigh benefit against dizziness and postural hypotension and never use it to manage fever, sepsis, anuria, a solitary-kidney obstruction, uncontrolled pain, acute kidney injury or uncertainty. Follow-up imaging and review, not disappearance of pain alone, establish whether observation remains safe. The decision also incorporates location, symptom trajectory, work and travel circumstances, ability to return quickly and the person's preference after discussion of procedural options. A distal stone has less distance to pass than a proximal stone, but size and location never replace assessment of infection or renal risk. Ask about antihypertensives, falls, fainting and occupational risks before prescribing an alpha blocker. A patient should know that postural dizziness is a medicine adverse effect, whereas fever, inability to pass urine or escalating pain are possible complications requiring medical review. Straining urine may permit stone analysis, but inability to capture a stone should not delay follow-up imaging. Alpha blockers do not treat infection, dissolve calcium stones, correct dehydration or replace analgesia. They are one time-limited component of an observation plan, with a defined endpoint and a route to urological intervention if passage fails or complications occur.
What happens when a stone causes infection and blockage together?
An infected obstructed collecting system is an emergency because pressure and infection can progress rapidly to sepsis and renal damage. The pathway is resuscitation where needed, cultures when feasible, prompt antimicrobials tailored to local policy and renal function, and urgent drainage by ureteric stent or percutaneous nephrostomy. Both routes are accepted effective options; choice depends on anatomy, severity and local expertise. Antibiotics alone do not drain infected urine behind obstruction. Definitive ureteroscopy or lithotripsy is usually delayed until sepsis and infection resolve. Fever, rigors, collapse, confusion, vomiting or reduced urine with possible stone symptoms requires emergency assessment.
How can recurrent kidney stones be prevented safely?
Confirm what was passed or removed. Stone analysis, imaging review and targeted blood and urine testing distinguish a low-risk first event from recurrence mechanisms such as low urine volume, hypercalciuria, hypocitraturia, uric-acid disease, infection stone or inherited disorder. General measures often include steady fluid intake to avoid concentrated urine, normal dietary calcium, lower excess salt and moderation of excessive animal protein; exact goals depend on climate, work and metabolic results. Do not self-start citrate, allopurinol, thiazides or high-dose supplements: each has indications, interactions and monitoring needs. Prevention includes treating recurrent infection, reviewing medicines and arranging specialist follow-up for high-risk or recurrent disease. A prevention appointment should review the original CT or ultrasound, whether the stone was analysed, weight and blood-pressure context, occupation and travel, urine infections, bowel symptoms and every prescription or supplement. Two 24-hour urine collections may be used in high-risk evaluation according to the treating team and laboratory practice; one result can be distorted by an unusual workday, illness or diet. Interpret results with serum creatinine, calcium, uric acid and composition. For calcium stones, preserve dietary calcium with meals unless a clinician identifies another indication. For enteric hyperoxaluria, gastrointestinal disease and meal-time calcium may matter more than indiscriminate food bans. For uric-acid disease, urine pH and metabolic-syndrome management are relevant. For infection stones, eradication of infection and complete stone treatment are central. Children and people with cystinuria require specialist plans. Ask about barriers such as heat, cost, toilet access and shift work, then agree an achievable pattern rather than issuing a vague instruction to drink more.
