Summary
Primary biliary cholangitis (PBC) is a chronic autoimmune cholestatic liver disease characterized by progressive injury and loss of small intrahepatic bile ducts. Continued cholestasis can lead to ductopenia, fibrosis, cirrhosis, portal hypertension and liver failure. Many people are identified through persistently raised alkaline phosphatase before jaundice appears; others present with pruritus, fatigue, sicca symptoms, xanthomata or complications of advanced liver disease. PBC is not an obstruction of the large bile duct and is not the same condition as primary sclerosing cholangitis.
Diagnosis usually requires two of three elements: biochemical cholestasis, PBC-specific antibodies and compatible histology. Antimitochondrial antibody is the hallmark, but AMA-negative disease can be diagnosed when cholestasis and PBC-specific ANA such as anti-gp210 or anti-sp100 are present. Imaging excludes mechanical obstruction. Biopsy is not mandatory for every typical antibody-positive case, but becomes important when tests conflict, another liver disease is suspected or marked hepatitic activity raises concern for AIH features. The term overlap is a clinical shorthand; the dominant biochemical and histological injury should drive specialist treatment.
Ursodeoxycholic acid (UDCA) is lifelong first-line disease-modifying treatment, with biochemical response assessed after about twelve months and prognosis refined by bilirubin, alkaline phosphatase, albumin, platelets and validated risk scores. Persistent cholestasis warrants specialist discussion of second-line therapy. The field changed after obeticholic acid was withdrawn from the United States market in 2025, while newer PPAR-targeted agents obtained approvals in some jurisdictions; current Indian licensing, availability, contraindications and outcome evidence must be checked rather than copied from an old algorithm. Symptoms, bone health and cirrhosis complications require parallel care.
How Common Is It?
PBC is uncommon and its measured frequency varies greatly by geography, ancestry, sex, age, laboratory testing and case ascertainment. EASL reports that roughly one in 1,000 women older than forty may live with PBC across pooled populations, but this is not an India-specific prevalence estimate. Women predominate strongly, although men develop PBC and may present later or with more advanced disease. True paediatric disease is exceptionally unusual; unexplained cholestasis in a child should prompt a broad specialist differential rather than automatic transfer of an adult diagnosis.
Detection has shifted toward earlier biochemical disease because alkaline phosphatase and AMA are more widely tested. This can improve outcomes when UDCA begins before advanced fibrosis, but it also creates a risk of labelling isolated AMA positivity as active PBC. A person with positive AMA and normal liver biochemistry needs follow-up and evaluation for context, not immediate assumption that all diagnostic criteria are met. Conversely, a normal bilirubin does not mean cholestatic injury is harmless; bilirubin often rises later and carries prognostic importance.
There is no complete national Indian PBC registry with uniform antibody assays, biopsy adjudication and longitudinal outcomes. Published tertiary-centre cohorts cannot define population prevalence because referral concentrates advanced and diagnostically difficult cases. Service quality is better measured by timely exclusion of obstruction, correct UDCA exposure, twelve-month response assessment, symptom burden, bone-density surveillance, portal-hypertension care and transplant access. Demographic likelihood should inform suspicion but never serve as a rule-out test for a man, younger adult or person without another autoimmune condition.
Risk Factors
PBC develops through interaction between genetic susceptibility, immune dysregulation and incompletely defined environmental triggers. Family history increases relative risk, particularly among first-degree relatives, but routine screening policy should follow specialist advice and local feasibility. Associations include autoimmune thyroid disease, Sjögren disease, systemic sclerosis and coeliac disease. Their presence can support a broader autoimmune phenotype yet does not establish PBC without the liver-specific pattern. Smoking and recurrent urinary infection have epidemiological associations in some studies, but neither is a diagnostic criterion or a basis for blame.
Factors predicting progression are distinct from factors associated with developing disease. Younger age at diagnosis, higher bilirubin or alkaline phosphatase, lower albumin, thrombocytopenia, fibrosis and inadequate biochemical response to UDCA identify higher-risk groups. Men may experience delayed recognition and need the same systematic staging. Symptoms alone do not measure liver risk: severe pruritus can occur without decompensation, while portal hypertension may develop in someone reporting little discomfort. Validated GLOBE and UK-PBC scores support prognosis after treatment but do not replace clinical review.
Coexisting injury can accelerate or confuse the course. Alcohol-related disease, metabolic steatotic liver disease, viral hepatitis and hepatotoxic medicines should be evaluated independently. Markedly raised ALT, elevated IgG or interface hepatitis suggests additional AIH activity; treating such a patient as uncomplicated PBC may leave inflammatory injury uncontrolled. Pregnancy usually does not create PBC, but pruritus and cholestatic tests can change and must be distinguished from intrahepatic cholestasis of pregnancy. Access risks in India include delayed antibody testing, under-dosed UDCA, unregulated ursodeoxycholic products, absent response calculation and loss to specialist follow-up.
Diagnosis
History
Establish when alkaline phosphatase or bilirubin first became abnormal and whether the pattern persisted. Ask about generalized pruritus, sleep disruption, fatigue, dry eyes or mouth, jaundice, dark urine, pale stool, right-upper-quadrant discomfort, weight loss, abdominal swelling, bleeding and confusion. Document autoimmune disease, pregnancy, alcohol, viral risk, surgery and family history. Reconcile prescription medicines, hormones, antibiotics, antitubercular agents, supplements and traditional products. Clarify whether itching predates pregnancy or medicine exposure and how it affects function.
Examination
Record vital signs, weight, muscle mass and mental state. Look for scratch marks, hyperpigmentation, jaundice, xanthelasma, hepatosplenomegaly, ascites, oedema, bruising, spider naevi and palmar erythema. Examine for sicca-related oral or ocular damage and signs of thyroid or systemic autoimmune disease when relevant. Findings may be normal in early PBC. Stigmata of chronic liver disease, asterixis, fever or hypotension indicate a different level of urgency than uncomplicated pruritus.
Investigations
Confirm hepatic cholestasis using alkaline phosphatase and GGT alongside ALT, AST, bilirubin, albumin, INR and full blood count. Ultrasound is the initial test to exclude extrahepatic obstruction and assess liver, spleen and portal features; MRCP follows when ducts are abnormal, PSC is plausible or obstruction remains unexplained. Test AMA using a validated assay; if negative despite sustained cholestasis, request PBC-specific anti-gp210 or anti-sp100 where available. Check immunoglobulins, especially IgM and IgG, but neither is diagnostic alone. Exclude viral, drug-related, infiltrative and metabolic causes. Biopsy is indicated for unresolved seronegative disease, suspected AIH features, competing steatohepatitis or staging questions that will change care. Non-invasive fibrosis assessment and baseline ultrasound support risk stratification.
Differential Diagnosis
Extrahepatic obstruction must be excluded before attributing cholestasis to PBC. Choledocholithiasis, benign or malignant stricture, pancreatic or ampullary tumour and postoperative biliary injury may require urgent imaging and intervention. PSC produces multifocal intrahepatic or extrahepatic duct strictures and is associated with inflammatory bowel disease; small-duct PSC can have normal cholangiography but compatible histology. IgG4-related sclerosing cholangitis may accompany autoimmune pancreatitis and needs different treatment. Secondary sclerosing cholangitis follows critical illness, infection, ischaemia or duct injury.
Drug-induced cholestasis can follow antibiotics, anabolic agents, antitubercular therapy and many other exposures. Sepsis-associated cholestasis, congestive hepatopathy, sarcoidosis, lymphoma and other infiltrative disease enter the differential according to presentation. Metabolic steatotic liver disease usually has a different biochemical emphasis but may coexist. Viral hepatitis and alcohol-related injury must be assessed rather than dismissed because AMA is positive. Isolated AMA positivity with normal alkaline phosphatase does not by itself prove active PBC; follow-up should be individualized.
AIH-PBC features require particular care. Marked aminotransferase elevation, high IgG, smooth-muscle antibody and moderate or severe interface hepatitis point toward additional hepatitic activity. The Paris framework requires specified combinations of PBC and AIH features and performs best for classic cases, but may miss less pronounced variants. Histology guides whether UDCA alone is insufficient and immunosuppression is needed. In pregnancy, new pruritus and raised bile acids may represent intrahepatic cholestasis of pregnancy rather than PBC progression. Persistent itch can also arise from kidney disease, thyroid disease, iron deficiency, skin disease, lymphoma or medicines, so symptom severity never substitutes for biochemical and anatomical diagnosis.
Management
Start UDCA for people with PBC and abnormal liver tests regardless of histological stage, unless a true contraindication or intolerance is established. Use a verified weight-based dose and review adherence, formulation and weight change. It improves transplant-free outcomes most clearly when instituted early and continued. Assess alkaline phosphatase and bilirubin response after approximately twelve months; calculate a validated risk score when the necessary data are available. A person with an inadequate response needs escalation of risk assessment, not abandonment of UDCA. Continue fibrosis, platelet and synthetic-function monitoring because biochemical improvement does not erase established cirrhosis.
Second-line treatment is evolving. APASL guidance discusses fibrates as add-on therapy in selected UDCA inadequate responders, with renal, hepatic and muscle monitoring and avoidance in decompensated disease. Newer PPAR agonists such as elafibranor and seladelpar have regulatory approval in some countries based largely on biochemical endpoints; availability and authorization in India, pregnancy safety, drug interactions and long-term clinical-outcome evidence must be verified at the time of prescribing. Obeticholic acid should not be presented as a stable universal option after its 2025 US market withdrawal. Specialist hepatology should reconcile jurisdiction-specific product status and individual cirrhosis risk.
Treat symptoms in parallel. Stepwise pruritus care may include cholestyramine, rifampicin, opioid antagonists or sertraline under monitoring; refractory itch itself can justify transplant evaluation. Fatigue requires assessment for anaemia, thyroid dysfunction, sleep disorder, depression and decompensation, because there is no reliable PBC-specific drug cure. Manage dry eyes and mouth, hyperlipidaemia according to cardiovascular risk, osteoporosis prevention and fat-soluble vitamin deficiency when cholestasis is advanced. Suspected AIH activity needs biopsy-informed immunosuppression in addition to UDCA. Decompensation triggers standard cirrhosis care and timely transplant referral.
Prescribing Information
UDCA is generally prescribed at 13 to 15 mg/kg/day. Recalculate after meaningful weight change and confirm the dispensed strength, because an apparently adherent patient may be under-dosed. It can be taken in divided doses according to formulation and tolerance. Bile-acid sequestrants impair absorption, so separate cholestyramine from UDCA and other oral medicines by the product-specific interval. Common adverse effects are usually mild gastrointestinal symptoms; persistent intolerance merits pharmacist and hepatologist review rather than substitution with an unverified supplement. UDCA is not an adequate treatment for AIH interface activity.
Cholestyramine is commonly first-line for itch but can cause constipation, bloating and multiple absorption interactions. Rifampicin may reduce pruritus but can cause hepatitis, cytopenia, renal injury and major drug interactions; obtain baseline tests and monitor closely. Opioid antagonists can precipitate an opioid-withdrawal reaction and require careful initiation. Sertraline has limited supportive evidence and its psychiatric, bleeding and interaction profile still applies. Antihistamines may aid sleep but do not correct cholestasis and can worsen sedation or encephalopathy.
Fibrates can cause myalgia, creatine-kinase elevation, creatinine change, transaminase abnormalities and gallstone-related problems; avoid routine use in decompensated cirrhosis and assess interacting statins. For elafibranor, seladelpar or any other newer agent, consult current Indian authorization and full product information rather than extrapolating from foreign approval. Obeticholic acid carried serious advanced-cirrhosis restrictions before withdrawal in the United States; old prescriptions or teaching slides require active reconciliation. During pregnancy and breastfeeding, continue or change disease and itch treatments only after specialist risk-benefit review. Every plan should name biochemical targets, laboratory timing, symptom measures and the clinician responsible for action.
When to Refer
Refer persistent unexplained cholestasis, positive AMA with abnormal liver tests, AMA-negative suspected PBC or discordant serology to gastroenterology or hepatology. Provide serial ALP, GGT, aminotransferases, bilirubin, albumin, INR, platelets, immunoglobulins, antibody method, ultrasound findings, medicines and pregnancy status. Marked ALT or IgG elevation, smooth-muscle antibody or biopsy concern for interface hepatitis warrants a centre able to evaluate AIH-PBC features. Abnormal ducts or inflammatory bowel disease warrants MRCP and possible PSC expertise.
Higher-risk disease needs expedited specialist care: rising bilirubin, falling albumin or platelets, increasing liver stiffness, inadequate UDCA response, suspected portal hypertension or severe refractory pruritus. Refer for transplant assessment before terminal deterioration when decompensation, recurrent variceal bleeding, ascites, encephalopathy, high prognostic risk or intractable itch is present. PBC can recur after transplantation, but transplant remains effective for appropriate candidates. A MELD-based allocation score may underrepresent severe itch, so specialist documentation and applicable policy matter.
Coordinate ophthalmology or dentistry for severe sicca complications, endocrinology or bone services for osteoporosis, and high-risk obstetric care for pregnancy. A lipid specialist is rarely needed solely because cholesterol is high; first assess the atherogenic profile and overall cardiovascular risk. If newer second-line therapy is considered, referral should establish regulatory status, affordability, monitoring and evidence uncertainty. In geographically fragmented Indian care, specify the exact clinical question and ensure interim UDCA, tests and emergency advice continue. A patient should not lose effective first-line therapy while waiting for a tertiary appointment.
Red Flags
Fever with right-upper-quadrant pain, rigors, hypotension or rapidly worsening jaundice suggests obstruction or cholangitis rather than uncomplicated PBC and requires emergency assessment. New confusion, asterixis, haematemesis, melaena, tense ascites, oliguria, severe hyponatraemia or sepsis indicates decompensated cirrhosis. Falling aminotransferases do not reassure when bilirubin and INR are worsening. Ultrasound or cross-sectional imaging should not be delayed by a positive AMA when an obstructing stone or tumour is clinically plausible.
A marked hepatitic shift—substantial ALT elevation, rising IgG or loss of synthetic function—raises AIH, drug injury or viral hepatitis and should trigger urgent reassessment. Immunosuppression must not be started simply because an antibody is present, yet untreated significant interface hepatitis can progress. A new dominant duct stricture, weight loss, progressive pain or mass needs malignancy and PSC evaluation. Men and people with cirrhosis require appropriate hepatocellular-carcinoma surveillance; a new lesion enters a dedicated diagnostic pathway.
Therapy red flags include severe myalgia or dark urine on a fibrate, jaundice or systemic illness on rifampicin, bowel obstruction or disabling constipation with cholestyramine, and any evidence of decompensation while receiving a drug contraindicated in advanced disease. Pregnancy while taking a medicine with uncertain or harmful reproductive profile requires prompt specialist reconciliation, not abrupt cessation of all liver therapy. Itch associated with suicidal thoughts, total sleep loss or extensive skin injury is a serious complication. Progressive malnutrition, fractures or inability to afford monitoring should prompt active service intervention rather than being recorded as non-adherence.
Indian Clinical Context
Availability of AMA testing, PBC-specific ANA, transient elastography, MRCP and expert hepatopathology varies across Indian districts and sectors. A safe resource-conscious sequence confirms persistent hepatic cholestasis, obtains ultrasound to exclude obstruction, tests AMA with a validated assay and refers unresolved cases. Where anti-gp210 or anti-sp100 is unavailable, that limitation should be documented; it does not justify inventing a seronegative diagnosis. Biopsy samples should be adequate and interpreted by a pathologist familiar with bile-duct lesions and interface hepatitis when the result will change treatment.
UDCA is available from multiple manufacturers, so clinicians should verify strength, weight-based exposure, adherence and storage rather than assuming all treatment failure is biological. Second-line access is much less predictable. Fibrates are off-label for PBC in many jurisdictions, and newer PPAR agents may be unaffordable, unavailable or not locally approved. The 2025 US withdrawal of obeticholic acid makes old international pathways unsafe if reproduced without a date and jurisdiction check. No medicine should be imported or prescribed solely because a foreign website calls it approved.
Indian patients may travel long distances for tests and transplantation. Combine visits when possible, provide written monitoring schedules, and explain urgent symptoms in the patient's preferred language. Evaluate tuberculosis, viral hepatitis, diabetes, nutrition and pregnancy before immunosuppression for AIH features. Regional APASL guidance includes Asian expertise, including Indian contributors, but much efficacy evidence still comes from non-Indian cohorts and biochemical surrogate endpoints. Local services should audit twelve-month UDCA response, fractures, variceal screening, symptom burden and transplant referral. The absence of comprehensive Indian prevalence and long-term comparative-treatment data must remain explicit.

Key Exam Pearls for NEET PG
PBC is a small intrahepatic bile-duct disease with cholestatic biochemistry, AMA positivity and histological nonsuppurative destructive cholangitis or florid duct lesions. The classic patient is a middle-aged woman with pruritus, fatigue or incidental alkaline-phosphatase elevation, but sex and symptoms cannot confirm or exclude it. Diagnosis generally needs two of three: ALP-defined cholestasis, AMA or PBC-specific antibodies, and compatible biopsy. Anti-gp210 or anti-sp100 supports AMA-negative PBC. Imaging excludes extrahepatic obstruction; MRCP is not a routine substitute for serology but is important when PSC is suspected.
UDCA at 13 to 15 mg/kg/day is lifelong first-line therapy. Assess biochemical response at twelve months; persistent ALP and bilirubin abnormalities signal higher risk and prompt second-line review. Obeticholic acid is no longer a timeless second-line answer after its 2025 US withdrawal; newer PPAR agonists and fibrates require up-to-date jurisdictional context. Pregnancy-related pruritus is not automatically PBC progression. Decompensation, recurrent variceal bleeding, encephalopathy or refractory pruritus can lead to liver-transplant assessment even when a single laboratory value seems modest.
Frequently Asked Questions
Does a positive antimitochondrial antibody test always mean that a person has primary biliary cholangitis?
No. AMA is highly characteristic, but diagnosis ordinarily also requires persistent cholestatic liver biochemistry or compatible histology. Isolated positivity with normal alkaline phosphatase should be assessed and followed rather than automatically treated as active disease. Viral, drug-related and metabolic liver conditions can coexist and still need independent evaluation.
When is liver biopsy needed in suspected primary biliary cholangitis?
Biopsy is often unnecessary when persistent cholestasis and characteristic antibodies agree. It is valuable when AMA and PBC-specific ANA are absent, results conflict, another liver disease is plausible, or ALT and IgG suggest AIH features. Histology assesses duct injury, interface activity and fibrosis but is interpreted with serology and imaging.

Can primary biliary cholangitis be managed safely during pregnancy?
Many patients can be managed through pregnancy, but conception should ideally follow hepatology and obstetric review. UDCA has substantial clinical experience, while safety evidence for several itch and second-line medicines is limited or unfavourable. New pruritus also requires assessment for pregnancy-specific cholestasis. Medicines should be reconciled, not stopped indiscriminately.
