Summary
Polymyalgia rheumatica (PMR) is an inflammatory syndrome of later life characterised by bilateral shoulder-girdle aching, prolonged morning stiffness and often hip-girdle symptoms. It is a clinical diagnosis supported by raised inflammatory markers after plausible alternatives have been assessed. Symptoms can be dramatic, yet objective muscle power is usually preserved once pain is overcome; true weakness, marked muscle-enzyme elevation or focal neurological findings point elsewhere. The 2012 EULAR/ACR criteria are classification tools for research, not a substitute for diagnostic judgement.
The central safety task is to look deliberately for giant cell arteritis (GCA), which may coexist with PMR and threatens vision and other organs. New headache, scalp tenderness, jaw or tongue claudication, transient or persistent visual symptoms, or limb claudication changes urgency. Current EULAR recommendations call for specialist assessment of suspected PMR and assessment within 24 hours for suspected GCA; strong clinical suspicion of GCA warrants glucocorticoid treatment without waiting for confirmatory testing.
For uncomplicated confirmed PMR, oral glucocorticoids remain initial therapy. The 2025 EULAR update recommends a prednisone-equivalent starting range of 15-25 mg daily, followed by an individualised taper. That international range is not an automatic Indian prescription: comorbidity, diagnostic certainty, drug access, local formularies and follow-up capacity matter. Relapse, toxicity or steroid dependence requires diagnostic review and specialist consideration of a glucocorticoid-sparing strategy. Every plan must include adverse-effect prevention, monitoring and clear escalation instructions.
How Common Is It?
PMR almost exclusively affects adults aged 50 years or older, and incidence rises substantially with advancing age. Women are affected more often than men in most reported cohorts. The disease is best characterised in populations of northern European ancestry, where reported incidence is comparatively high; estimates vary markedly by geography, ancestry, case definition, data source and access to rheumatology. A prevalence figure from one country should therefore not be transferred to India as if it were a measured national rate.
Robust, contemporary population-level Indian incidence and prevalence estimates are not supplied by the cited international recommendations or by the NMC curriculum. Indian clinicians should not infer rarity merely because formal datasets are sparse. An ageing population, fragmented referral pathways and overlap with common shoulder, cervical and metabolic disorders can all affect recognition. Conversely, applying a familiar foreign pre-test probability without reconsidering infection, malignancy and endocrine disease can promote overdiagnosis.
For an individual patient, age and phenotype are more useful than a headline population number. New bilateral shoulder stiffness in a person over 50 deserves structured assessment, but PMR is improbable in a younger adult and never explains every elevation of ESR. Case identification in practice depends on recognising the inflammatory pattern, documenting duration and functional loss, testing for competing disorders, reviewing the course and maintaining surveillance for GCA. The evidence base establishes who was studied; it does not prove how frequently the syndrome occurs in every Indian state, ancestry group or care setting.
Risk Factors
Age is the dominant epidemiological association: PMR should not be casually diagnosed below 50. Female sex and northern European ancestry are associated with higher observed rates, although these are probabilistic features rather than diagnostic requirements. Familial aggregation and immune-genetic associations have been reported, but no genetic test is recommended to confirm routine PMR. Environmental and seasonal signals remain inconsistent and do not justify attributing an individual episode to a particular infection, vaccine or exposure without separate evidence.
Risk assessment must also cover factors that change treatment harm. Older adults may have diabetes, hypertension, dyslipidaemia, chronic kidney disease, peptic ulcer risk, osteoporosis, glaucoma, cataract, infection susceptibility or frailty before glucocorticoids begin. Previous fragility fracture, falls, low body mass, smoking, high alcohol intake and prolonged steroid exposure increase skeletal risk. Concomitant NSAIDs, anticoagulants and other interacting medicines may increase gastrointestinal or bleeding harm. These features do not exclude therapy, but they influence baseline testing, prophylaxis, dose choice and monitoring.
A separate risk screen addresses associated GCA. PMR symptoms, constitutional upset and inflammatory-marker elevation can coexist with cranial or large-vessel vasculitis. The relevant clinical cues are new localised headache, scalp sensitivity, jaw or tongue claudication, visual disturbance, pulse or blood-pressure asymmetry, limb claudication and unexplained systemic inflammation. Their absence at the first visit does not eliminate future risk, so patients and families need symptom-specific safety-netting throughout follow-up. Risk-factor lists should never override the current history and examination.
Diagnosis
PMR has no single confirmatory blood test. Diagnosis combines an appropriate age and symptom pattern, evidence of inflammation, exclusion of credible mimics, and a coherent course under follow-up. A rapid response to glucocorticoids can support but cannot prove the diagnosis; mimics may temporarily improve and not every genuine case responds completely.
History
Ask when bilateral shoulder aching and morning stiffness began, whether onset was abrupt, how long stiffness lasts, and whether dressing, rising from bed, washing hair or turning in bed is impaired. Explore hip-girdle pain, constitutional symptoms, peripheral joint swelling, true weakness, rash, psoriasis, diarrhoea, urinary or genital symptoms, drug exposure and cancer or infection clues. Ask specifically about every GCA symptom: new headache, scalp tenderness, jaw or tongue fatigue while chewing, diplopia, transient visual obscuration, visual loss and limb claudication.
Examination
Record temperature, weight, blood pressure in both arms when vasculitis is plausible, pulses and vascular bruits. Assess active and passive shoulder and hip movement, tenderness, synovitis, enthesitis and cervical range. Test power carefully after accounting for pain. Examine temporal arteries without treating a normal examination as exclusion. Look for lymphadenopathy, rash, muscle wasting, focal neurological signs, thyroid features and infection or malignancy clues.
Investigations
Obtain full blood count, CRP and/or ESR, renal and liver profiles, glucose and urinalysis; consider calcium, alkaline phosphatase, thyroid function, creatine kinase, serum protein electrophoresis, RF/anti-CCP, chest imaging or infection testing when indicated by the differential. Shoulder and hip ultrasound may show bursitis, biceps tenosynovitis or synovitis but does not reliably separate PMR from rheumatoid arthritis. Suspected GCA needs an urgent pathway for vascular ultrasound and/or other appropriate imaging or biopsy without delaying necessary treatment.
Differential Diagnosis
Late-onset rheumatoid arthritis is a major inflammatory mimic, especially when there is persistent peripheral synovitis, positive anti-CCP antibody or an erosive pattern. Seronegativity does not exclude it, and PMR-type shoulder symptoms can precede clearer arthritis. Other inflammatory possibilities include late-onset spondyloarthritis, psoriatic arthritis, remitting seronegative symmetrical synovitis with pitting oedema, crystal arthritis, connective-tissue disease and inflammatory myopathy. GCA is not merely a differential: it may be associated and must be actively assessed.
Mechanical and periarticular disorders include bilateral rotator-cuff disease, adhesive capsulitis, glenohumeral or cervical osteoarthritis and cervical radiculopathy. Passive restriction that remains prominent, local provocative signs or a focal neurological distribution can help. Endocrine, metabolic and drug-related mimics include hypothyroidism, hyperparathyroidism, osteomalacia, statin-associated symptoms and electrolyte disturbance. True proximal weakness, substantially raised creatine kinase or characteristic skin changes should shift attention toward inflammatory or toxic myopathy.
Systemic infection, endocarditis, tuberculosis and occult malignancy can cause pain with high inflammatory markers and constitutional symptoms. Myeloma, lymphoma and solid tumours deserve consideration when weight loss, anaemia, hypercalcaemia, lymphadenopathy, bone pain or an atypical course is present. Parkinsonism, depression and deconditioning may resemble stiffness or impaired function. Red flags for an alternative diagnosis include age below 50, normal inflammatory markers with an unconvincing phenotype, marked asymmetry, neurological deficit, persistent fever, prominent nocturnal focal pain, poor steroid response or repeated relapse without a plausible taper-related pattern. Reopen the diagnosis rather than escalating treatment reflexively.
Management
First establish whether the presentation is uncomplicated PMR, possible GCA or an alternative emergency. Current EULAR recommendations support specialist assessment for suspected PMR and urgent assessment for suspected GCA. In most suspected PMR, glucocorticoids can wait until diagnostic evaluation is sufficiently complete; in strong suspected GCA they should not be delayed for tests. This distinction prevents both avoidable visual harm and indiscriminate steroid trials.
For confirmed PMR, discuss objectives: restore function, suppress inflammation, minimise cumulative glucocorticoid exposure and detect relapse or evolving alternative disease. The 2025 EULAR recommendation gives an initial prednisone-equivalent range of 15-25 mg/day. Select within a local protocol according to severity, comorbidity and follow-up, then taper according to clinical response rather than ESR alone. Arrange early review after initiation, reassess symptoms and function, and document adverse effects. A rising marker without compatible symptoms should prompt evaluation for infection or another cause before dose escalation.
Supportive care includes graded activity, resistance and balance work when pain settles, falls assessment, adequate protein and bone-health measures, vaccination review and management of cardiovascular risk. Educate the patient not to stop prolonged glucocorticoids abruptly. Relapses require confirmation of the phenotype and adherence, followed by a supervised dose adjustment and renewed taper. Recurrent relapse, inability to taper, high predicted toxicity or inadequate response warrants rheumatology review. EULAR identifies IL-6 receptor inhibition for selected PMR and methotrexate as an alternative glucocorticoid-sparing option; these are specialist decisions requiring screening, monitoring and access planning, not routine first-visit additions.
Prescribing Information
Prescribe systemic glucocorticoids only after recording indication, preparation, prednisone-equivalent dose, timing, intended review and taper responsibility. Prednisolone is commonly available in India, but brand strength and fixed combinations can cause errors. Use one clear daily schedule, preferably in the morning when clinically appropriate, and provide a written taper rather than verbal fractions of tablets. Never promise a fixed duration. Missed doses, intercurrent illness, surgery and vomiting need individual sick-day advice because hypothalamic-pituitary-adrenal suppression becomes relevant with prolonged therapy.
Before and during treatment, check blood pressure, weight, glucose or HbA1c, renal and hepatic context, infection risk, eye history, fracture history and interacting medicines. Bone protection must follow verified local guidance and individual fracture risk; calcium and vitamin D alone do not replace formal assessment when pharmacological osteoporosis prevention is indicated. Consider gastroprotection only when risk factors or concomitant treatments justify it. Monitor for hyperglycaemia, infection, mood or sleep disturbance, dyspepsia, bruising, proximal myopathy, cataract, glaucoma and osteoporosis. A new fever must not be concealed by automatic steroid escalation.
NSAIDs are not substitutes for glucocorticoids in PMR and may add renal, gastrointestinal and cardiovascular harm in older adults. Methotrexate requires pregnancy exclusion where relevant, baseline blood count, liver and renal assessment, interaction review, folate planning and serial monitoring. IL-6 receptor inhibitors can suppress CRP and require specialist infection screening, blood counts, liver tests and lipid monitoring; clinical assessment becomes especially important. Imported guideline doses are evidence references, not authority to bypass Indian labelling, formulary rules or specialist governance.
When to Refer
Refer suspected PMR for specialist assessment, particularly when diagnostic certainty is limited or the presentation is atypical. EULAR's current overarching recommendation favours specialist assessment for all suspected PMR; implementation in India may require a physician-led pathway where rheumatology is not immediately available. The referral should describe symptom onset, morning-stiffness duration, functional impairment, examination, inflammatory-marker trend, relevant negative findings, comorbidities, current drugs and any glucocorticoid exposure with response.
Arrange urgent same-day assessment if GCA is suspected, with a pathway capable of vascular imaging and ophthalmic or rheumatology input. EULAR specifies assessment within 24 hours for suspected GCA and treatment without delay when clinical suspicion is strong. Do not wait for ESR, CRP, ultrasound or biopsy if that delay exposes vision to risk. Sudden visual loss is an emergency, not a routine clinic referral.
Expedite rheumatology review for age below 50, prominent peripheral synovitis, true weakness, normal inflammatory markers with a doubtful phenotype, systemic illness, very high or unexplained inflammatory markers, poor initial response, repeated relapse, steroid toxicity or inability to taper. Consider endocrinology, infectious disease, oncology, neurology or orthopaedic referral when the alternative diagnosis directs it. Specialist review is also appropriate before methotrexate or biologic therapy. Where travel and cost constrain access, communicate the precise clinical question and arrange interim monitoring rather than giving an open-ended steroid supply. A referral is complete only when urgency, receiving service and safety plan are explicit.
Red Flags
Treat visual symptoms in a person with possible PMR as time-critical. Transient monocular visual loss, diplopia, new persistent visual blurring or sudden blindness may represent GCA-related ischaemia. New headache, scalp tenderness, jaw or tongue claudication, abnormal temporal artery, limb claudication or constitutional illness strengthens concern, but no single normal finding excludes GCA. Strong clinical suspicion requires immediate glucocorticoid treatment and urgent specialist confirmation; the risk of contralateral visual loss makes delay dangerous.
Fever, rigors, a new murmur, focal infection, severe night pain, unexplained weight loss, lymphadenopathy or cytopenias can indicate infection or malignancy. A hot swollen single joint requires urgent aspiration for septic and crystal arthritis rather than being labelled a PMR flare. New objective weakness, dysphagia, respiratory weakness, dark urine or a major creatine-kinase rise suggests myopathy. Focal neurological deficit, cord symptoms or rapidly progressive functional loss requires an alternative emergency pathway.
During therapy, urgent review is needed for sepsis, severe hyperglycaemia, gastrointestinal bleeding, acute psychiatric disturbance, adrenal crisis risk, fracture or acute eye symptoms. Patients should seek advice before abruptly stopping glucocorticoids. A relapse that repeatedly appears during taper may be genuine, but escalating treatment without reconsidering adherence, infection, GCA and diagnostic mimics is unsafe. Suppression of CRP by an IL-6 inhibitor can mask inflammatory signals; symptoms and examination retain priority. The red-flag plan should name where the person will go after hours and should be understandable to family or carers.
Indian Clinical Context
India does not have one cited national PMR treatment guideline equivalent to the EULAR document used here. The EULAR recommendations and classification study provide high-quality international evidence, but they were not written as Indian regulatory or formulary policy. Their diagnostic probabilities, access assumptions, imaging pathways and advanced-therapy recommendations must be adapted to local epidemiology, comorbidity, medicine availability and referral capacity. This limitation should remain visible rather than being hidden behind a universal algorithm.
In an Indian differential, clinicians should keep tuberculosis, other chronic infections, haematological or solid-organ malignancy, osteomalacia, thyroid disease and common mechanical shoulder disorders in view when the phenotype warrants them. This does not mean ordering every test for every patient; it means using history, examination and baseline results to avoid an unsafe empirical steroid cycle. Screening and prophylaxis before immunosuppression must follow current Indian and local institutional protocols. Availability of ultrasound, PET-CT, temporal-artery biopsy and rheumatology varies, so the referral plan should specify the attainable next step.
Cost and geography can turn a theoretically safe taper into an unmonitored prescription. Use generic names, confirm tablet strength, write dates and doses, arrange glucose and blood-pressure checks, discuss bone protection, and ensure enough—not unlimited—medicine until review. Provide GCA warnings in the patient's preferred language. The NMC framework supports systematic joint assessment, treatment planning, communication and appropriate consultation; it does not certify unsupervised specialist prescribing. Where a foreign recommendation conflicts with an Indian label, national programme or institutional policy, the verified Indian requirement governs.
NMC Competency Mapping

Think PMR in a person aged at least 50 with bilateral shoulder aching, morning stiffness longer than 45 minutes, elevated CRP and/or ESR and new hip pain or restricted movement. The 2012 EULAR/ACR clinical scoring items are morning stiffness over 45 minutes, hip pain or limited range, absence of RF or ACPA, and absence of other peripheral joint pain. Required entry features include age at least 50, bilateral shoulder aching and an abnormal inflammatory marker. The score is designed for classification and has imperfect sensitivity and specificity; it is not a diagnostic gold standard.
PMR causes pain-limited movement but not primary muscle destruction. True proximal weakness and a raised creatine kinase suggest inflammatory, endocrine or drug-related myopathy. Selected relapsing or steroid-dependent cases may receive IL-6 receptor inhibition or methotrexate under specialist care. In an exam stem, separate uncomplicated PMR, PMR with suspected GCA, late-onset RA, myopathy and septic or malignant mimics before choosing therapy.
Frequently Asked Questions
Can normal muscle enzymes exclude polymyalgia rheumatica or confirm it?
A normal creatine kinase is compatible with PMR because the syndrome primarily affects periarticular structures rather than destroying muscle. It does not confirm PMR. The diagnosis still requires the characteristic age and inflammatory symptom pattern, assessment of inflammatory markers, exclusion of mimics and follow-up. A raised creatine kinase or objective weakness should prompt evaluation for myopathy or another cause.
Does a rapid improvement with prednisolone prove the diagnosis of polymyalgia rheumatica?
No. Improvement can support a carefully constructed diagnosis, but other inflammatory and painful disorders may also improve transiently with glucocorticoids, and some genuine PMR cases do not respond completely. The 2012 classification study did not make steroid response a diagnostic criterion. An incomplete, short-lived or dose-dependent response should trigger review of adherence, GCA, infection, malignancy and rheumatological mimics.
Which symptoms in polymyalgia rheumatica suggest associated giant cell arteritis?
New headache, scalp tenderness, jaw or tongue claudication, transient or persistent visual disturbance, diplopia, sudden visual loss, limb claudication or unexplained systemic inflammation are important warnings. Suspected GCA requires urgent specialist assessment, and strong clinical suspicion warrants immediate glucocorticoids without waiting for confirmatory tests. A normal temporal-artery examination does not reliably exclude it.
Why should glucocorticoids not be stopped suddenly after prolonged treatment?
Prolonged systemic glucocorticoids can suppress the hypothalamic-pituitary-adrenal axis, so abrupt withdrawal may cause adrenal insufficiency as well as a disease flare. Tapering must follow a written, supervised plan. Intercurrent illness, surgery, vomiting or missed medication may require specific advice, and urgent assessment is needed for severe weakness, vomiting, hypotension, confusion or other features suggesting adrenal crisis.
