Plantar fasciitis is a common clinical label for pain arising around the medial plantar calcaneal attachment of the plantar fascia. Contemporary guidance also uses plantar fasciopathy because persistent symptoms often reflect a load-related tendon-like disorder rather than acute inflammation alone. The familiar presentation is sharp plantar-medial heel pain with the first steps after rest, often easing after moving and recurring after prolonged standing or walking. Symptoms affect activity and participation as well as pain intensity. How Common Is It?
Plantar heel pain is a frequent reason for musculoskeletal consultation and can disrupt work, sleep, mobility and participation in ordinary activity. Prevalence estimates differ by case definition, occupation, sport, age, duration and whether self-managed symptoms are counted. AAOS describes plantar fasciitis as one of the commonest causes of pain under the heel and reports that many people receive treatment each year in the United States. That supports clinical relevance; it does not establish an Indian prevalence, annual incidence, sex ratio or occupational risk estimate.
The condition is often encountered in midlife and in people whose work or recreation repeatedly loads the foot, but it can occur outside those groups. A clinic population cannot be used to infer community burden because referral, imaging access and self-treatment change who presents. Bilateral symptoms deserve a broader history because systemic inflammatory disease, footwear exposure and diffuse pain conditions may alter the probability of alternatives.
For service planning in India, record local denominator, definition, duration, occupation, footwear, metabolic comorbidity and referral source rather than repeating an imported percentage. Risk Factors
Risk is best understood as a load-capacity mismatch, not a fault in the patient. A recent rise in walking, running, dancing, hill work, standing or work on hard surfaces can precede symptoms. Tightness or reduced dorsiflexion at the ankle, calf weakness, a change in footwear, foot posture, higher body mass and prolonged standing may be relevant, but none confirms the diagnosis. Diabetes, peripheral neuropathy, inflammatory arthritis, gout and vascular disease do not cause every heel pain, but they change examination, skin care, differential diagnosis and safety of any intervention. Smoking, sleep disruption and distress can influence recovery and adherence without proving that pain is psychological.
A young person with bilateral heel pain, inflammatory back pain, prolonged morning stiffness, psoriasis, uveitis, bowel symptoms or recent infection needs assessment for enthesitis and spondyloarthritis. Sudden pain with a pop after acceleration or jumping suggests acute tissue injury rather than ordinary gradual fasciopathy. These are diagnostic signals, not routine plantar-fasciitis risk factors.
Diagnosis
History
Locate pain precisely: plantar-medial heel near the calcaneal attachment is more typical than posterior heel, diffuse sole pain or calf pain. Ask onset, injury, first-step pattern, pain after activity, morning stiffness, walking tolerance, work demands, sport, footwear, previous treatment, systemic symptoms and neurological sensations. Establish whether pain is unilateral or bilateral and whether there is fever, night pain, weight loss, skin breakdown, swelling, numbness, back pain, inflammatory symptoms, diabetes, immunosuppression or cancer history.
Examination
Observe gait, shoes, skin, arch posture, calf bulk and swelling. Palpate the plantar-medial calcaneal region and the rest of the heel; assess ankle dorsiflexion, calf flexibility, toe extension and pain with loading. Examine Achilles insertion, midfoot, posterior tibial and peroneal tendons, pulses, sensation and a focused neurological screen when symptoms suggest neuropathy. Calcaneal compression tenderness, focal bony tenderness, erythema, warmth, a mass or sensory change shifts the differential. Compare both sides and document function rather than relying on one provocative sign.
Investigations
Typical stable plantar fasciitis usually needs no routine blood test, ultrasound, MRI or radiograph. Plain radiographs can help when trauma, bony tenderness, arthritis or another bone disorder is plausible; a calcaneal spur is common and does not by itself explain pain. Ultrasound or MRI is selected for persistent uncertainty, suspected stress injury, fascial tear, mass or alternative soft-tissue pathology. Blood tests are directed by suspected inflammatory, infective or metabolic disease, not ordered to confirm ordinary plantar fasciitis.
Differential Diagnosis
Calcaneal stress fracture may follow a change in impact activity and can produce progressive pain, focal bony tenderness or pain on calcaneal compression; early radiographs may be unrevealing. Acute plantar-fascia tear can follow a sudden load and may cause bruising, swelling or an abrupt change in function. Heel fat-pad pain is often more central and load-related. Achilles insertional tendinopathy is posterior, while bursitis and posterior impingement have their own anatomical pattern.
Entrapment of a plantar nerve or tarsal-tunnel pathology can cause burning, tingling, altered sensation or night symptoms. Lumbar radicular pain, peripheral neuropathy and complex regional pain syndrome require a neurological and vascular assessment rather than repeated local treatment. Plantar fibromatosis presents with a palpable nodule in the fascia, not simply insertional tenderness. A painful skin lesion, foreign body or ulcer is a different pathway.
Inflammatory enthesitis from spondyloarthritis, gout, rheumatoid disease or sarcoidosis can mimic local plantar pain. Fever, warmth, rapidly increasing swelling, wound, immunosuppression or severe pain raises infection. Persistent rest or night pain, a mass, constitutional symptoms or previous malignancy needs urgent evaluation for bone or soft-tissue disease. Diagnosis should be revised when the history, examination or response is inconsistent with a mechanical plantar pattern.
Management
Agree a functional goal: tolerating first steps, a work shift, walking, prayer, sport or a commute. Explain that recovery can be slow and that a short-lived pain reduction from passive treatment is not the only outcome. Reduce the specific aggravating load temporarily, then rebuild it rather than prescribing complete inactivity indefinitely. Low-impact activity may maintain fitness when tolerated. A physiotherapist can tailor plantar-fascia-specific stretching, calf flexibility work, progressive calf and foot strengthening, balance and return-to-activity progression.
The 2023 heel-pain guideline supports manual therapy, plantar-fascia and gastrocnemius/soleus stretching, resistance exercise, taping for short-term benefit and patient education as parts of a multimodal plan. Foot orthoses may be used with other interventions but should not be framed as a guaranteed stand-alone cure. Night splints can be considered for a first-step-pain pattern. Choose a programme the person can perform safely and review technique, symptoms and adherence rather than simply issuing devices.
If meaningful disability persists after an adequate, supervised conservative programme, reassess diagnosis, load, comorbidity and access barriers before escalating. Shockwave therapy, injection procedures and surgery require informed specialist discussion of uncertain benefit, alternatives and harms. A procedure is not a substitute for correcting an unsafe return-to-load plan.
Prescribing Information
Analgesia may help a person participate in rehabilitation, but it does not repair the underlying load-capacity problem. Before recommending paracetamol, a topical agent or an NSAID, reconcile all products and check allergy, pregnancy or conception plans, renal disease, liver disease, peptic-ulcer or bleeding history, cardiovascular risk, anticoagulants, antiplatelets and interacting medicines. The product, dose, duration and maximum exposure must come from the current Indian formulary, labelled preparation and treating clinician; this guide intentionally gives no universal regimen.
Avoid treating persistent pain by repeatedly extending self-purchased analgesics. Duplicate ingredients are common in combination cold, pain and traditional products. A new need for escalating analgesia should trigger reassessment of diagnosis, work demands, footwear and red flags. Opioids are not routine treatment for uncomplicated plantar heel pain because adverse effects and dependence can impair function without addressing the condition.
Corticosteroid injection is a specialist decision, not a benign next step. AAOS notes that injection may weaken fascia and can contribute to rupture; risk, technique, diabetes, infection risk, skin condition and competing diagnoses need individual review. Platelet-rich plasma, botulinum toxin and other injections have variable evidence and cost. Do not promise availability, benefit or safety without a local specialist and a documented indication.
When to Refer
Arrange urgent assessment for inability to bear weight after injury, suspected fracture or acute rupture, rapidly increasing pain or swelling, fever, an infected wound, neurovascular compromise, new objective sensory or motor deficit, or severe unexplained rest pain. A person with diabetes, neuropathy, vascular disease or immunosuppression needs a lower threshold for foot examination and appropriate referral when skin or infection concerns arise. Emergency transfer follows local capability rather than waiting for a routine orthopaedic appointment.
Refer to orthopaedics, sports medicine, rehabilitation or podiatry when diagnosis remains uncertain, symptoms remain function-limiting despite a structured conservative plan, imaging suggests an alternative pathology, recurrent injection is being considered, or specialist footwear and rehabilitation assessment is needed. Rheumatology is appropriate for suspected inflammatory enthesitis or multisystem disease. Neurology, spine or pain services may be relevant for a neuropathic pattern.
A useful referral records duration, exact site, inciting load, first-step and activity pattern, work and sport effect, footwear, comorbidities, skin and neurovascular findings, range of motion, focal tests, investigations, treatment tried and response. State any red flags and the reason for urgency. Referral should not be used to evade basic assessment or to promise a procedure.
Red Flags
Urgent reassessment is required for fever, spreading redness, warmth, wound discharge, systemic illness or a rapidly swollen painful foot because infection or another acute process may be present. Sudden heel pain with bruising, a pop, marked loss of function or inability to bear weight after loading may indicate fascial tear or fracture. Severe focal bony tenderness after a training change, especially if pain progresses despite rest, raises stress fracture.
Night pain, rest pain that is escalating, a palpable mass, unexplained weight loss, prior cancer or pain disproportionate to examination needs a cause-directed pathway. Burning, numbness, weakness, colour change, coldness, absent pulses, unilateral calf swelling or severe allodynia is not routine plantar fasciitis. Bilateral symptoms with inflammatory back pain, psoriasis, eye pain or photophobia, bowel symptoms or prolonged inflammatory stiffness warrant consideration of systemic inflammatory disease.
Safety-net advice should name action: seek urgent care for fever, spreading redness, a new wound, sudden inability to bear weight, marked swelling, new numbness or weakness, blue or cold toes, or severe pain at rest. Do not continue a home programme through an evolving red-flag presentation. A normal prior X-ray or a heel spur report does not remove the need for re-examination when symptoms change.
Indian Clinical Context
This guide is framed for Indian practice but relies on international orthopaedic and rehabilitation guidance; it is not an Indian government formulary, insurance rule or occupational standard. Access to physiotherapy, podiatry, imaging, work modification, supportive shoes and shockwave treatment differs by district and payment setting. A safe plan identifies what is realistically available, gives a low-cost home programme only after assessment, and states where to return if symptoms worsen.
Ask about barefoot walking, sandals, safety footwear, hard work surfaces, standing shifts, long commutes, prayer practices, caregiving and the ability to rest or change duties. These questions should guide shared adaptation, not stereotype a patient or imply that footwear alone caused the condition. Inspect shoes rather than assuming a brand, price or style is appropriate. Explain that a radiographic spur does not automatically require removal and that any purchase should be reviewed for comfort, safety and actual function.
Where imaging or specialist care is distant, red flags still require escalation. Written instructions in the patient's preferred language, a simple activity log and a photograph of existing footwear can improve follow-up. Local medicine supply, referral wait times, rehabilitation quality and regional epidemiology are uncertain in this source set and must be verified by the treating institution.

Key Exam Pearls for NEET PG
The usual examination pattern is plantar-medial heel pain that is worst on the first steps after rest and may improve after brief walking. The fascia spans from the calcaneal region toward the forefoot and helps support the arch. Pain provoked by toe extension can support a plantar-fascia pattern, but it is not a stand-alone diagnostic test. A calcaneal spur on radiography is not proof that the spur is the pain generator and should not dictate surgery.
Differentiate location and tempo. Posterior heel pain suggests Achilles insertional pathology; central heel pain may be fat-pad related; burning or paraesthesia suggests nerve involvement; a sudden painful pop with bruising suggests tear; and progressive focal bony pain after increased impact suggests stress fracture. Bilateral heel pain with inflammatory back symptoms, uveitis, psoriasis or bowel symptoms suggests spondyloarthritis-related enthesitis rather than ordinary isolated fasciopathy.
Remember that ordinary plantar fasciitis is a clinical diagnosis. Routine laboratory testing does not confirm it, and routine advanced imaging is not the best answer in a stable characteristic presentation. Select radiography when a bony differential is plausible and select ultrasound or MRI when a specific soft-tissue, stress, tear or mass question remains. The scan result must be interpreted with history and examination.
First-line management is a structured conservative programme: education, temporary load adjustment, plantar-fascia and calf stretching, progressive strengthening and appropriate footwear review. Orthoses, taping and night splints may be adjuncts in selected patterns. Avoid choosing an injection or operation before reconsidering diagnosis and an adequate rehabilitation trial. In a stem with fever, wound, neurovascular change, night pain or inability to bear weight, prioritise cause-directed urgent assessment rather than a routine heel-pain plan.
Frequently Asked Questions
Does a heel spur on an X-ray prove that plantar fasciitis is causing pain?
No. A spur can reflect longstanding traction at the attachment and can be present without symptoms. The diagnosis depends on the history, examination and exclusion of important alternatives. A changed or atypical pattern still needs reassessment even when an X-ray report mentions a spur.
Should every person with first-step heel pain have an ultrasound or MRI scan?
No. Typical stable symptoms are commonly diagnosed clinically. Imaging is selected when trauma, stress fracture, rupture, mass, infection, nerve symptoms or persistent diagnostic uncertainty changes the question. A scan should answer a defined clinical problem rather than simply confirm a familiar label.
Can a steroid injection be the first treatment for plantar fasciitis?
It should not be assumed to be first-line. Conservative load management and rehabilitation are central, and injection decisions require assessment of diagnosis, risks, diabetes, skin and infection issues and alternatives. Steroid can weaken fascia and needs specialist discussion rather than repeated self-directed treatment.
