Summary
Peripheral arterial disease (PAD) is atherosclerotic obstruction of arteries outside the heart and brain, most often in the legs. It may cause exertional calf, thigh or buttock discomfort relieved by rest, but many people have atypical symptoms or no recognised limitation. PAD is not merely a walking problem: it marks high cardiovascular risk and can progress to tissue loss or acute limb ischaemia. Assessment therefore asks two questions together: is limb perfusion threatened now, and is cardiovascular risk being treated comprehensively?
Examination, ankle-brachial index (ABI) and selective imaging establish the diagnosis and its anatomical relevance. A normal appearance at rest does not exclude functional claudication, and an abnormal pulse does not explain every painful leg. Distinguish stable chronic symptoms from chronic limb-threatening ischaemia (CLTI), characterised by rest pain, non-healing wound or gangrene attributable to PAD, and from acute limb ischaemia, where a sudden perfusion change threatens tissue over hours.
Guideline-directed care includes antiplatelet therapy when indicated, high-intensity statin therapy, blood-pressure and diabetes management, smoking cessation, preventive foot care and structured exercise. Revascularisation is for CLTI to preserve tissue or for persistent functionally limiting claudication despite best medical therapy and structured exercise. How Common Is It?
PAD becomes more common with age and with tobacco exposure, diabetes, kidney disease, hypertension and dyslipidaemia. Prevalence figures vary because surveys use different ABI thresholds, symptom questionnaires and populations; many people with objectively abnormal perfusion do not report textbook claudication. Consequently, a vascular clinic series over-represents severe disease, while community screening may identify asymptomatic disease. Neither setting gives a safe shortcut for an individual with a wound or walking limitation.
The burden is amplified by coronary and cerebrovascular atherosclerosis. A person with PAD may present first with myocardial infarction, stroke, heart failure or a diabetic foot wound rather than calf pain. Functional effects include loss of walking confidence, work limitation, falls and social isolation. In advanced disease, tissue loss and amputation carry high mortality as well as disability.
No contemporary nationally representative Indian PAD prevalence estimate was verified for this draft. India-specific care must not simply apply a high-income screening statistic: diabetes burden, tobacco products, rural walking demands, foot-care access, renal disease, late presentation and availability of vascular services alter both detection and outcome. A careful pulse and foot examination remains useful precisely because disease can be silent until the stakes are high.
Risk Factors
Cigarette smoking and other tobacco exposure are potent, modifiable PAD risks. Diabetes, hypertension, elevated LDL cholesterol, chronic kidney disease, older age and established coronary or cerebrovascular disease also raise risk. Ask about walking pattern, rest pain, prior ulcer or amputation, vascular procedure, renal function, neuropathy, visual impairment, housing and footwear. These factors influence both disease probability and the safety of a home-based exercise or wound plan.
Symptoms do not rank risk reliably. A sedentary patient may have little claudication because they never walk far enough to provoke it, while a person with neuropathy may not feel ischaemic injury. Diabetes can combine neuropathy, infection and arterial insufficiency; absence of pain is not reassurance in a warm, swollen or ulcerated foot. Tobacco cessation should include smokeless products and second-hand exposure where relevant, not a narrow cigarette question.
Non-atherosclerotic causes of exertional leg symptoms include spinal stenosis, hip or knee osteoarthritis, venous disease, exertional compartment syndrome and peripheral neuropathy. Premature or unusual disease should prompt consideration of inflammatory, embolic, traumatic, radiation-related or anatomic causes. Treating every reduced walking distance as PAD risks missing a neurological or musculoskeletal diagnosis and may lead to inappropriate vascular intervention.
Diagnosis
History
Clarify site, trigger, distance, reproducibility and relief of pain. Classical claudication is reproducible exertional muscle discomfort relieved within minutes by rest; ischaemic rest pain often affects the forefoot, worsens on elevation and may improve when dependent. Ask about wounds, colour change, coldness, sudden deterioration, prior revascularisation, coronary or cerebrovascular events and all vascular-risk medicines. Establish smoking exposure, diabetes history, renal disease and functional goals. A wound history needs duration, trauma, infection features, drainage and off-loading practices.
Examination
Inspect both legs and feet for ulcers, gangrene, dependent rubor, pallor on elevation, hair loss, temperature asymmetry and tissue loss. Palpate femoral, popliteal, dorsalis pedis and posterior tibial pulses, but record that pulse examination alone does not quantify disease. Assess capillary refill in context, examine the skin between toes, test neuropathy when diabetes is present and look for oedema, cellulitis, deformity or pressure points. In suspected acute ischaemia, rapidly assess pain, pallor, pulselessness, paraesthesia, paralysis and poikilothermia while arranging emergency vascular care.
Investigations
Resting ABI is a key initial test in compatible symptoms or signs; toe pressures, toe-brachial index or other vascular laboratory testing are useful when ABI is non-compressible or discordant, particularly in diabetes or kidney disease. Duplex ultrasound is first-line imaging when revascularisation is contemplated. CTA or MRA answers a defined anatomical question after renal function, contrast and local availability are assessed. Do not delay urgent CLTI or acute-ischaemia referral for outpatient imaging.
Differential Diagnosis
Neurogenic claudication from lumbar spinal stenosis often relates to standing and extension, may improve with flexion or sitting, and has a different pulse and ABI pattern from arterial claudication. Hip, knee and ankle disease can limit walking at a less predictable distance and produce joint-specific findings. Peripheral neuropathy can cause burning, numbness or nocturnal pain; venous disease can cause heaviness and swelling that worsens with prolonged standing. These conditions may coexist with PAD.
A diabetic foot ulcer may be neuropathic, arterial, infected or mixed. Infection can produce pain, erythema, warmth and systemic illness, yet ischaemia can mute inflammatory signs. Do not label a wound 'vascular' or 'diabetic' without documenting perfusion, neuropathy, pressure exposure and infection assessment. Deep-vein thrombosis and acute limb ischaemia require different urgent pathways.
Acute embolic occlusion, thrombosis of a diseased artery, dissection, graft occlusion and compartment syndrome can produce a sudden painful limb. Acute limb ischaemia is a time-critical emergency; chronic claudication advice is unsafe in that setting. Aortic aneurysm, pelvic arterial disease and cardiac embolic sources may alter the anatomic and cardiac work-up under specialist direction.
Management
Every confirmed PAD plan should address cardiovascular and limb risk: tobacco cessation support, high-intensity statin therapy unless contraindicated, blood-pressure treatment, diabetes care, diet, physical activity, vaccination where relevant and foot protection. Medication choice must consider kidney function, frailty, pregnancy possibility, interactions, affordability and adherence. Regularly reconcile medicines and set a concrete smoking-cessation plan rather than simply advising a person to stop.
For chronic symptomatic PAD, supervised exercise therapy is recommended to improve walking performance, function and quality of life. Structured community or home-based programmes with behavioural support are an alternative when supervised services are unavailable; casual advice to walk without structure has not shown the same benefit. Exercise should be withheld from a limb with suspected CLTI, acute ischaemia, uncontrolled infection or another condition making walking unsafe until assessed.
Revascularisation is not the first response to every ABI abnormality or claudication complaint. For functionally limiting claudication, optimise medical therapy and structured exercise, define the patient's goals, confirm anatomy and discuss durability, restenosis and procedural risk. In CLTI, timely multidisciplinary vascular assessment, wound care, infection control, off-loading and revascularisation planning may be limb-saving. The procedure type is an anatomy-, conduit-, comorbidity- and centre-specific decision.
Prescribing Information
For symptomatic PAD, single antiplatelet therapy is standard secondary prevention unless a contraindication exists; choose the agent and dose from the current local cardiovascular formulary. High-intensity statin therapy is recommended to reduce cardiovascular and limb events, with lipid response, adverse effects and interacting medicines reviewed rather than abandoning treatment after vague muscle symptoms. Blood-pressure treatment should follow current hypertension guidance, and diabetes medicines should be chosen for glycaemic and cardiovascular benefit while monitoring renal function and foot status.
Do not equate antiplatelet therapy with full-dose anticoagulation. Full-intensity oral anticoagulation should not be used solely to reduce PAD cardiovascular or limb events when there is no other indication, because bleeding harm outweighs benefit. Low-dose rivaroxaban combined with low-dose aspirin is a specific, guideline-supported option for selected symptomatic PAD or post-revascularisation patients who do not have increased bleeding risk; it is not an automatic substitute for assessing haemorrhage risk, renal function, drug interactions, cost or an indication for therapeutic anticoagulation. Patients who need anticoagulation for atrial fibrillation or venous thromboembolism require coordinated specialist decisions about any added antiplatelet.
Do not start antibiotics for an ischaemic ulcer without clinical evidence of infection, and do not prescribe cilostazol or a vasodilator as a substitute for revascularisation assessment in CLTI. Prescribe a clear foot-care and wound-follow-up plan; medicines alone do not restore perfusion.
When to Refer
Refer urgently to vascular services for suspected CLTI: ischaemic rest pain, non-healing ulcer, gangrene, tissue loss, rapidly worsening wound or a diabetic foot with possible arterial insufficiency. Same-day emergency escalation is needed for acute limb ischaemia, new motor or sensory deficit, a suddenly cold or pale limb, severe rest pain with pulse loss, or infection plus systemic illness. Transfer should not await a trial of exercise, compression or a routine clinic ABI.
Refer patients with lifestyle-limiting claudication that persists despite best medical therapy and a structured exercise attempt, after confirming that symptoms and anatomy are consistent. Vascular review is also appropriate for uncertain diagnosis, non-compressible ABI with concerning symptoms, recurrent symptoms after revascularisation, aneurysm suspicion, complex kidney disease or tissue loss. Podiatry, diabetes, wound, rehabilitation and smoking-cessation services are partners, not replacements for vascular assessment.
A good referral gives duration and impact of symptoms, pulse and wound findings, ABI or toe-pressure results, diabetes control, kidney function, current antithrombotics, statin status, smoking exposure, prior imaging/procedures and the patient's mobility goals. In India, identify a reachable vascular-capable centre and arrange transport and handover rather than sending an unstable person on an unstructured journey.
Red Flags
Acute limb ischaemia is characterised by sudden pain, pallor, absent pulses, coldness, paraesthesia or paralysis. Neurological deficit indicates threatened tissue and requires immediate vascular or emergency-team involvement. Do not use a normal oxygen saturation, normal blood pressure or a brief symptom improvement to rule it out. Anticoagulant and revascularisation decisions in acute ischaemia are hospital protocols requiring urgent examination and imaging strategy.
CLTI warning signs are rest pain, ulceration, gangrene, black eschar, worsening toe discolouration or a wound that fails to heal. In diabetes, neuropathy may reduce pain; a painless ulcer can still be limb-threatening. Fever, spreading erythema, crepitus, malodour, systemic upset or rapidly progressive tissue damage suggests infection requiring urgent multidisciplinary care. Do not apply compression until significant arterial insufficiency has been assessed.
Chest pain, transient neurological deficit, new dyspnoea or syncope in a person with PAD may represent systemic atherosclerotic disease and needs its own emergency pathway. Antithrombotic red flags include major bleeding, melaena, haematemesis, head injury, collapse, severe unexplained anaemia or a new neurological deficit while taking an anticoagulant. Do not advise unsupervised cessation of antithrombotics; seek urgent clinical advice.
Indian Clinical Context
PAD in India is often detected late, especially when diabetes, neuropathy, renal disease, tobacco exposure and limited foot-care access coexist. A busy clinic can mistake a wound for infection alone or call all leg pain 'neuropathy'. A simple documented pulse and foot examination, ABI or toe-pressure referral where available, and explicit return precautions can prevent these omissions. Where vascular laboratories are unavailable, uncertainty should accelerate referral rather than be recorded as normal perfusion.
Care must be feasible. Supervised exercise programmes, vascular imaging, endovascular intervention, bypass surgery, podiatry and wound supplies are unevenly distributed. A structured, coached community walking plan may be more realistic than an unavailable hospital service for stable claudication, but it is not appropriate for rest pain, tissue loss or acute symptoms. Medicine reconciliation is critical because branded products, self-purchased antiplatelets, traditional tobacco and episodic access to statins or diabetes medicines can produce both undertreatment and bleeding risk.
Antithrombotic policy must follow the local formulary and specialist pathway. The international low-dose rivaroxaban-plus-aspirin evidence cannot be translated into a universal Indian prescription without evaluating renal function, bleeding, drug access and coexisting atrial fibrillation or rheumatic valve disease. Limb salvage requires referral networks linking primary care, diabetes services, vascular teams, imaging, infection control and rehabilitation. This guide supports that reasoning and does not override local protocols.

At show-how level, supervised learners can document pulses, skin and wound findings, deliver foot-care and safety-net advice, and prepare a focused referral.
Catheter intervention, bypass selection, wound debridement, compression decisions, therapeutic anticoagulation and acute-ischaemia drug orders require supervised specialist care. Assessment should use scenarios of claudication with a normal neurologic exam, diabetic ulcer with neuropathy, and sudden cold limb with sensory change. Institutions must map these outcomes to their current NMC curriculum before high-stakes assessment.

Frequently Asked Questions
Does a normal-looking foot exclude clinically important PAD in routine clinical assessment, urgent triage, or a diabetes foot review visit after new tissue change?
No. PAD may be asymptomatic, atypical or limited by low activity. Pulse and skin examination can be normal-looking between episodes and do not quantify perfusion. Exertional symptoms, wound healing, vascular risk, ABI and toe-pressure testing where appropriate must be interpreted together. Diabetes and neuropathy can conceal pain, so a painless ulcer or colour change still requires timely perfusion assessment.
Why is supervised exercise preferred to simply telling a patient to walk?
Structured exercise has defined progression, monitoring, education and behavioural support, and has evidence for improving walking performance and quality of life in chronic symptomatic PAD. Casual walking advice has not shown the same consistent benefit. Exercise is not a trial for a suddenly cold limb, rest pain, tissue loss or an infected foot; those presentations need vascular assessment first. The programme should record baseline walking limitation and be paused for new rest pain, wound deterioration, systemic illness or a clinically unsafe exercise response.
Does every PAD patient need both aspirin and an anticoagulant?
No. Antiplatelet therapy and therapeutic anticoagulation serve different indications and combining them increases bleeding. Selected symptomatic or post-revascularisation patients may benefit from a particular low-dose rivaroxaban-plus-aspirin strategy after formal risk assessment, but this is not the same as full-dose anticoagulation. AF, venous thrombosis, mechanical valves, kidney function, bleeding history and procedures all alter the decision. The decision must state the indication, duration, review date, concurrent ulcer or procedure plan and exact generic products involved. This prevents accidental duplication of antithrombotics when different clinicians treat the leg, heart and diabetes.
When is a leg ulcer an urgent vascular problem?
Urgency is high when there is rest pain, gangrene, black tissue, rapidly worsening ulceration, absent pulses, new neurological deficit or infection with systemic illness. In diabetes, lack of pain does not make an ulcer safe. CLTI and acute limb ischaemia can threaten tissue and require prompt multidisciplinary assessment, not serial topical treatment alone. The assessment should record wound duration, progression, pressure exposure, neuropathy, pulse findings, footwear, renal disease, systemic features and current antithrombotics. This avoids the unsafe shortcut of assuming that every diabetic wound is purely neuropathic or every erythematous foot is simply cellulitis. A rapidly evolving wound, new discolouration, falling function or inability to obtain review changes urgency even if laboratory values are not yet available.
