Summary
Peptic ulcer disease is a mucosal break in the stomach or duodenum, most often associated with Helicobacter pylori infection or non-steroidal anti-inflammatory drug exposure. It can present with epigastric pain, nausea or anaemia, but symptoms do not reliably predict ulcer location or cause. Some ulcers are silent until bleeding, perforation or gastric-outlet obstruction occurs. The first decision is whether the person is haemodynamically stable and free of peritonism; emergency complications outrank outpatient dyspepsia algorithms.
For stable patients, identify alarm features, medicine exposure, bleeding risk and the indication for acid suppression. Test for H. pylori only with a plan to treat a confirmed infection and to confirm eradication using an appropriate non-serological method. Empiric antibiotic combinations, repeated unverified courses and copy-pasted foreign regimens are unsafe in a setting with variable resistance and product availability. Current local susceptibility guidance, allergy history, prior macrolide or quinolone exposure and current Indian labels determine an eradication prescription.
Ulcer care is cause directed: stop or minimise an avoidable ulcerogenic medicine, protect a necessary antiplatelet or NSAID user through coordinated risk management, eradicate confirmed H. pylori with an appropriate regimen, and confirm healing or exclude malignancy when clinically indicated. This educational draft is quarantined and contains no universal antibiotic dose or duration.
The prescription record must distinguish ulcer healing, prevention of NSAID-related injury, post-endoscopic bleed management and H. pylori eradication; a PPI is not one interchangeable treatment for all four. How Common Is It?
Peptic ulcer disease is common enough that dyspepsia, anaemia and gastrointestinal bleeding pathways must recognise it, but frequency estimates differ by H. pylori prevalence, NSAID use, smoking, testing practice, endoscopy access and whether uncomplicated symptoms are counted. Hospital endoscopy series overrepresent complicated disease, while community symptom surveys cannot establish an ulcer diagnosis. A single national Indian percentage would not guide an individual decision.
The burden is increasingly shaped by medicines. Aspirin, conventional NSAIDs, anticoagulants, corticosteroids when combined with other risks, selective serotonin reuptake inhibitors and concurrent antithrombotics can alter bleeding risk. Risk rises with age, previous ulcer or bleed, H. pylori, high-risk NSAID exposure, renal or liver disease and multiple interacting medicines. A patient may require antiplatelet therapy for a compelling cardiovascular indication; the answer is not automatic permanent cessation without the prescribing team.
Complications determine urgency. Haematemesis, coffee-ground vomit, melaena, haematochezia with shock, syncope, acute abdomen, persistent vomiting and weight loss need assessment beyond routine dyspepsia care. Access to endoscopy, transfusion, surgery, interventional radiology and follow-up varies, so a safe referral system and early resuscitation matter as much as an outpatient testing strategy.
Risk Factors
Ask specifically about every NSAID, including over-the-counter pain tablets, combination cold products, topical or oral preparations, intermittent use, traditional products and medicines obtained from several clinics. Record aspirin, other antiplatelets, anticoagulants, corticosteroids, SSRIs, bisphosphonates, potassium products and alcohol. Do not assume a gastroprotective medicine eliminates risk or that a selective NSAID is risk free. Establish why each medicine is prescribed before changing it.
H. pylori is a major cause of duodenal and gastric ulcers. Testing choice depends on whether endoscopy is indicated and whether acid suppression, antibiotics or bismuth may create a false-negative non-invasive result. Serology cannot establish current eradication. Previous treatment, penicillin allergy, macrolide or quinolone exposure, documented susceptibility and prior culture results influence antibiotic selection and should be obtained before an eradication plan is chosen.
Smoking, heavy alcohol use, serious illness and poor nutrition may impair healing or increase complication risk, but counselling must not blame patients for infection or prescribed therapies. Prior ulcer bleeding, advanced age, comorbidity, renal impairment and combined antithrombotic therapy should prompt coordinated gastroenterology, cardiology or primary-care planning. Pregnancy, breast-feeding, liver disease and kidney disease change medicine safety and require current local product information.
Diagnosis
History
Ask about epigastric pain, relation to meals, nocturnal symptoms, vomiting, early satiety, appetite, weight loss, melaena, haematemesis, iron deficiency symptoms and syncope. Record prior ulcer, endoscopy, H. pylori testing and treatment, family history of upper gastrointestinal cancer, travel and all medicines. Ask about chest pain, dyspnoea and exertional symptoms because cardiac disease can mimic epigastric discomfort. Establish pregnancy possibility, alcohol and smoking without judgment.
Examination
Assess airway, breathing, circulation, mental status, capillary refill and orthostatic symptoms if bleeding is possible. Examine for pallor, dehydration, abdominal tenderness, guarding, rigidity, distension, masses, hepatosplenomegaly and stigmata of chronic liver disease. A soft abdomen does not exclude an ulcer; a rigid abdomen, rebound tenderness, shock or altered consciousness is an emergency. Rectal examination and stool assessment are context dependent and should be performed with consent and dignity.
Investigations
Full blood count, renal function, liver tests, coagulation studies and group-and-save or crossmatch support suspected bleeding care. Endoscopy diagnoses ulcers, permits haemostasis and biopsies gastric ulcers or suspicious lesions when indicated. Stable dyspepsia may be assessed using locally appropriate non-invasive H. pylori testing where no alarm features mandate endoscopy. Test-of-cure must use a validated non-serological test at the correct interval after treatment and after withholding confounding medication according to the chosen test and guideline.
The 2024 ACG H. pylori guideline highlight specifies a test of cure at least four weeks after therapy with urea breath, faecal antigen or biopsy-based testing, and advises withholding PPIs for two weeks and bismuth or antibiotics for four weeks to reduce false negatives. Those timings are source-specific; local laboratory instructions and specialist advice still govern an individual test. A negative result collected during a confounded interval should not be used to declare eradication or to justify reflex salvage antibiotics.
Differential Diagnosis
Functional dyspepsia, reflux disease, biliary colic, pancreatitis, gastroparesis, gastric cancer, oesophageal disease, coeliac disease and medication irritation can resemble ulcer symptoms. Epigastric pain can be cardiac, pulmonary, vascular or metabolic; do not use transient antacid response as diagnostic proof. New dyspepsia with bleeding, progressive dysphagia, persistent vomiting, weight loss, iron deficiency or mass requires an urgent pathway.
Gastritis and H. pylori infection may exist without an ulcer, while an ulcer can be NSAID-related despite a negative H. pylori test. Gastric ulcers need thoughtful follow-up because malignancy can mimic a benign crater. A duodenal ulcer may have different investigation and healing follow-up requirements from a gastric ulcer. Endoscopy interpretation, pathology and local cancer pathways guide the distinction.
Perforated viscus, acute pancreatitis, mesenteric ischaemia, abdominal aortic pathology, bowel obstruction and myocardial infarction are time-critical alternatives. Sudden severe pain, peritonism, shock, raised lactate or pain out of proportion should not be placed in a test-and-treat queue. In patients receiving anticoagulants or antiplatelets, apparent minor bleeding can deteriorate rapidly and needs an emergency risk assessment.
Management
If bleeding, perforation, obstruction or shock is suspected, resuscitate and refer urgently. Establish intravenous access, monitor observations, obtain blood tests and involve senior emergency, gastroenterology, anaesthesia, surgery and transfusion teams under local protocol. Endoscopy, endoscopic haemostasis, radiology or surgery are selected by stability, findings and local capability. Do not delay emergency transfer to obtain outpatient H. pylori testing or wait for oral acid suppression to work.
For stable confirmed or probable ulcer disease, stop an avoidable NSAID and address the reason it was used; alternative analgesia, non-drug measures and a lower-risk plan may be needed. If an antiplatelet, anticoagulant or NSAID is essential, coordinate any interruption or restart with the cardiovascular, rheumatology or pain prescriber. Proton-pump inhibitor treatment is commonly used for ulcer healing and gastroprotection, but product, dose, indication, duration and interaction assessment must follow current local guidance.
Treat confirmed H. pylori with a regimen selected from current local resistance and susceptibility guidance. Do not empirically repeat clarithromycin- or levofloxacin-containing therapy unless susceptibility supports it. Confirm eradication after treatment using a non-serological test at the guideline-specified interval. Persistent symptoms need reassessment for adherence, wrong diagnosis, ongoing NSAID exposure, resistance, complications or malignancy rather than an automatic second antibiotic course.
For overt upper gastrointestinal bleeding, use a bleeding pathway rather than routine outpatient ulcer care. The ACG upper gastrointestinal and ulcer bleeding guideline recommends emergency risk assessment and endoscopy within 24 hours after presentation for admitted patients, following resuscitation. Endoscopic haemostasis, high-dose acid suppression after haemostasis, radiology or surgery are selected by the lesion, response and local capability. It would be unsafe to turn that into a fixed home-management timetable or to delay transfer because an H. pylori result is pending.
Prescribing Information
Before prescribing H. pylori eradication therapy, record the positive test, ulcer context, allergies, pregnancy or breast-feeding status, renal and hepatic function, prior eradication, prior macrolide and quinolone exposure, drug interactions and available local susceptibility information. Select an exact regimen only from current national, state, institutional or susceptibility-guided advice. Antibiotic names, doses and duration are deliberately omitted here because resistance, formulations and contraindications change and a universal Indian regimen cannot be inferred safely from foreign guidance.
Explain adherence, adverse effects, alcohol and food warnings where applicable, and when to seek care for rash, severe diarrhoea, jaundice, bleeding or persistent vomiting. Avoid therapeutic duplication and verify whether a bismuth, PPI or antibiotic product has already been started elsewhere. A negative test obtained while a confounding medicine was continued may need interpretation rather than immediate escalation. Test cure is part of treatment, not an optional administrative step.
For PPI treatment, document the indication and review date. Check interaction risk, especially in a person using antiplatelets, anticoagulants, antifungals, antiseizure medicines or multiple over-the-counter products; consult the current label or pharmacist. Do not stop secondary-prevention aspirin or anticoagulation independently after a bleed. Restart timing, gastroprotection and H. pylori management require multidisciplinary balancing of rebleeding and thrombosis risk.
Do not convert a foreign guideline's drug list into a universal Indian prescription. The ACG H. pylori highlight names an optimised bismuth quadruple regimen and specifically says doxycycline is not its recommended substitute for tetracycline; availability, allergy, pregnancy, renal and hepatic safety, susceptibility and current Indian product information still determine whether any regimen is appropriate. Document the exact dispensed products and schedule only after that decision, then arrange test-of-cure. Repeated broad-spectrum exposure without confirmation creates preventable toxicity and resistance pressure.
When to Refer
Send immediately to emergency care for haematemesis, melaena with instability, syncope, shock, severe anaemia symptoms, acute severe abdominal pain, guarding, rigidity, persistent vomiting, suspected obstruction or sepsis. A patient with upper gastrointestinal bleeding may need urgent endoscopy, blood products, reversal or adjustment of antithrombotics, interventional radiology or surgery. Transfer rather than observing at home when monitoring and escalation are unavailable.
Refer urgently for alarm features, gastric ulcer, recurrent ulcer, unexplained iron deficiency, weight loss, dysphagia, abnormal imaging, failed appropriate eradication therapy, suspected malignancy, difficult drug allergy, complex antithrombotic use, renal or hepatic impairment, pregnancy, or persistent symptoms despite a verified plan. Gastroenterology should guide endoscopic biopsy, test selection, eradication failure and follow-up. Surgical input is required for perforation, obstruction or uncontrolled bleeding.
Include haemodynamic observations, bleeding history, all medicines with last doses, comorbidities, laboratory results, transfusion given, H. pylori test and timing, prior regimens, allergies, imaging and pregnancy status. The referral question should identify whether the priority is resuscitation, endoscopy, cancer exclusion, antimicrobial susceptibility, medication-risk balancing or recurrent symptoms.
Red Flags
Haematemesis, coffee-ground vomit, melaena, red rectal bleeding with collapse, syncope, confusion, tachycardia, hypotension, oliguria or pallor can indicate significant haemorrhage. Do not estimate severity from the visible amount alone. Bleeding risk is amplified by anticoagulants, antiplatelets, renal disease, liver disease and comorbidity. Emergency assessment is required even if pain has improved.
Sudden severe epigastric or generalised abdominal pain with guarding, rigidity, fever, shoulder-tip pain, distension or shock suggests perforation or another surgical emergency. Persistent vomiting, inability to tolerate fluids, progressive early satiety, dehydration or electrolyte disturbance can indicate obstruction. Weight loss, persistent dysphagia, iron deficiency, palpable mass or a gastric ulcer requires malignancy-conscious investigation.
Treatment red flags include anaphylaxis, severe rash, jaundice, severe antibiotic-associated diarrhoea, new bleeding on a PPI plan, and symptoms continuing after a supposedly completed eradication course. Never use persistence alone to infer resistance; verify the original diagnosis, medication exposure, adherence, test validity and the appropriate test-of-cure pathway. Do not self-restart NSAIDs or stop prescribed antithrombotics without clinical advice.
Indian Clinical Context
India has high and heterogeneous H. pylori exposure, variable endoscopy access, non-prescription NSAID availability and substantial antimicrobial resistance pressure. These realities make indiscriminate antibiotic treatment a bad idea: it increases adverse effects and resistance without proving infection or cure. ICMR antimicrobial stewardship resources support local, syndrome-based and susceptibility-aware prescribing; they do not authorise one static national H. pylori regimen in every district.
Ask about analgesics bought without prescription, combination products, Ayurveda or other traditional remedies, tobacco, alcohol and delayed access to emergency transport without blame. Test availability may determine whether urea breath, stool antigen or endoscopic biopsy-based testing is feasible. If a reliable test-of-cure or appropriate regimen is unavailable, refer or obtain specialist advice instead of repeating a familiar triple therapy. Use current Indian labels and local hospital antibiograms where available.
A practical safety plan includes clear bleeding and perforation return precautions, a written medicine list, avoidance of duplicate NSAIDs, and coordinated decisions for aspirin or anticoagulation. Cost-effective generic acid suppression may be appropriate when indicated, but its duration should still be reviewed. Public-facing education should state that ulcer symptoms and H. pylori status need assessment; it must not invite self-treatment with antibiotics.
