Summary
Acute diffuse otitis externa is inflammation of the external auditory canal, usually with rapid-onset otalgia, tenderness of the tragus or pinna, canal oedema or erythema, and sometimes otorrhoea, itch, fullness or conductive hearing reduction. Moisture, canal trauma and skin-barrier disruption commonly precede it. It is not synonymous with every discharging ear: acute otitis media with perforation, chronic suppurative middle-ear disease, a foreign body, dermatitis, furunculosis, otomycosis, herpes zoster oticus and malignant otitis externa have different implications.
For uncomplicated diffuse disease, assess pain, confirm the canal-based syndrome, check whether the tympanic membrane is intact or visible, and identify factors that alter management: diabetes, immunocompromise, previous radiotherapy, a tympanostomy tube, known or suspected perforation, severe canal obstruction and inability to use drops. Topical therapy is preferred initial treatment because it delivers high local concentration. Aural toilet or a wick may be needed to make topical treatment reach the canal; neither is a casual home procedure when anatomy is uncertain.

How Common Is It?
Otitis externa is a common primary-care and emergency presentation, particularly in warm humid weather, after swimming or water exposure, and where people clean the ear canal aggressively. Incidence estimates vary with season, age, access to care and how clinicians distinguish diffuse disease from otomycosis, eczema and middle-ear discharge. A precise national Indian prevalence cannot be inferred from selected clinic series. The useful service measure is not a promotional count but the proportion correctly assessed, given appropriate topical access, reviewed when failing to improve, and protected from inappropriate systemic antimicrobial exposure.
Pain may be severe despite a small area of visible inflammation. Canal swelling can impair hearing and prevent a view of the membrane, which makes certainty about perforation impossible. That uncertainty changes topical safety; it should be documented rather than guessed away. Most uncomplicated cases improve with correct canal-directed care, but people with diabetes, immunocompromise or severe unremitting pain need a lower threshold for reassessment because invasive skull-base infection is a different disease.
Community practice is affected by self-medication, over-the-counter combination drops, oils, herbal products, ear buds, ear candles and unsterile probing. These can worsen trauma, obscure culture, delay diagnosis or introduce ototoxic risk when the membrane is not intact. Prevention and counselling are therefore core clinical interventions, not filler after a prescription.
Risk Factors
Water trapped in the canal, frequent swimming, sweating, humid climate, occlusive hearing aids or earphones, and repeated irrigation can macerate skin and alter the canal environment. Trauma from cotton buds, fingernails, matchsticks, scratching or attempted wax removal creates a portal for inflammation and infection. Ask what was inserted, whether a previous perforation, ear surgery or tube exists, and which drops or oils have already been used.
Dermatitis, psoriasis, seborrhoeic disease, allergy and chronic discharge predispose through barrier dysfunction. Diabetes, immunosuppression, chemotherapy, HIV with immune compromise and previous radiotherapy are management modifiers rather than automatic evidence of malignant disease. Severe pain out of proportion, nocturnal pain, granulation tissue, cranial-nerve symptoms or failure to improve are the features that demand urgent invasive-disease assessment. Avoid calling every diabetic patient malignant otitis externa and avoid dismissing a high-risk patient with topical drops alone.
Furunculosis is a localised follicular process, often with a focal tender swelling, whereas diffuse disease involves the canal skin. Thick itch-predominant debris may suggest otomycosis. A perforated middle ear, chronic mastoid cavity or tube can cause otorrhoea without primary canal infection. Correct syndrome recognition prevents both resistance-driving oral antibiotics and dangerous topical selection.
Diagnosis
History
Establish onset, pain severity, itch, hearing change, discharge, fever, swimming, trauma, ear cleaning, hearing-aid use, eczema and prior episodes. Ask about perforation, tympanoplasty, mastoid surgery or tympanostomy tube, because an unseen membrane cannot be presumed intact. Record diabetes control, immunosuppression, renal disease, radiotherapy and medications. Screen specifically for persistent night pain, headache, facial weakness, dysphagia, hoarseness, vertigo, severe malaise and treatment failure.
Examination
Assess systemic illness and pain before instrumentation. Inspect pinna, tragus, mastoid, surrounding skin and cranial nerves. Otoscopy should identify diffuse canal oedema, debris, focal boil, foreign body, vesicles, fungal material, granulation tissue and membrane status where safely visible. Do not force an instrument through a swollen canal or label a membrane normal when it cannot be seen. Mastoid tenderness, post-auricular swelling, pinna displacement or a toxic appearance suggests another pathway.
Investigations
Uncomplicated diffuse disease is usually clinical. Culture is useful for recurrent, chronic, severe, immunocompromised, treatment-failure or suspected invasive infection contexts, ideally before systemic treatment when that does not delay emergency care. Check glucose in undiagnosed or poorly controlled diabetes and use renal tests, inflammatory markers, imaging and microbiology selectively for suspected malignant otitis externa. CT and MRI define different aspects of extension and must be directed by ENT, infectious-disease and radiology pathways. Imaging is not a reason to delay urgent referral.
Differential Diagnosis
Acute otitis media with perforation often follows respiratory symptoms and may have middle-ear findings rather than primary tragal tenderness. Chronic suppurative otitis media, cholesteatoma and a postoperative cavity can produce persistent otorrhoea and need otological assessment. A painful swollen mastoid, displaced pinna or systemic illness must not be called simple otitis externa. Foreign body, retained wick, wax with secondary inflammation and contact dermatitis may be visible causes.
Otomycosis commonly causes itch and characteristic debris, but clinical appearance alone can be misleading; repeated antibacterial-steroid combinations may worsen fungal overgrowth or obscure diagnosis. Eczematous otitis externa requires a skin-barrier and trigger approach, not repeated empiric systemic antibiotics. Herpes zoster oticus may bring vesicles, facial weakness or vertigo, requiring urgent specialist assessment. Referred otalgia from dental, temporomandibular, pharyngeal or head-and-neck disease can occur with a normal canal.
Malignant otitis externa, also called necrotising otitis externa, is invasive infection extending beyond the canal, often in older people with diabetes or immunocompromise. It is not diagnosed by a high glucose result alone. Severe persistent pain, granulation tissue, cranial-neuropathy symptoms, refractory disease or systemic concern should trigger urgent ENT and hospital assessment, culture and appropriately directed imaging. Neoplasm can mimic invasive infection; failure to respond may require tissue diagnosis under specialist care.
Management
Give analgesia appropriate to severity and assess response; pain control is an explicit part of treatment. For uncomplicated diffuse disease, select a topical preparation consistent with membrane status and local protocol, explain technique, and arrange reassessment if not improving within 48 to 72 hours. Do not begin systemic antimicrobial therapy merely because pain is dramatic, the canal looks inflamed or a topical medicine is unfamiliar. AAO-HNS guidance reserves systemic treatment for extension outside the canal or host factors that justify it.
Enhance topical delivery when debris or oedema blocks the canal. Aural toilet is removal of obstructing material by a clinician with appropriate equipment and expertise; avoid traumatic probing, routine syringing where perforation is possible, and blind deep instrumentation. A wick can hold the canal open and conduct drops across marked oedema, but it needs review or removal according to the local protocol. Explain that a wick is a delivery device, not proof of severe invasive infection.
Known or suspected perforation, a tube or an unvisualised membrane requires a non-ototoxic topical preparation. Do not use a potentially ototoxic product because it is cheap or familiar. Review technique: warm drops in the hand, position the affected ear upward, instil without contaminating the tip, gently move the outer ear where appropriate, and keep the position briefly. Keep the ear dry during active symptoms and stop inserting objects. Reassess diagnosis, adherence, obstruction and invasive-disease features if the expected early response does not occur.
Prescribing Information
Topical selection follows syndrome and membrane safety. Antiseptic, antibiotic, corticosteroid and combination preparations exist, but product names, concentrations, approved ages, contraindications and availability vary. A corticosteroid component may reduce inflammatory symptoms in selected topical products; it does not convert unrecognised fungal disease or malignant otitis externa into routine diffuse disease. Prescribe the exact medicine and frequency only from a current local label, hospital protocol or ENT direction. Do not copy a foreign dose or use ophthalmic, dermatologic or oral formulations as interchangeable ear treatment.
If the membrane is known or suspected non-intact, choose a non-ototoxic topical preparation. When it cannot be seen, manage it as uncertain rather than assuming safety. Check allergy, prior adverse reactions, pregnancy or breast-feeding context, and concurrent otic products. Repeated topical antibiotic exposure without re-examination can select resistant organisms, delay fungal diagnosis and create contact dermatitis. Culture-directed changes are more defensible in recurrent, refractory, high-risk or invasive contexts than serial empirical switches.
Systemic antimicrobials are not routine initial treatment for uncomplicated diffuse otitis externa. If extension outside the canal or a host factor indicates systemic therapy, choice, route, dose, duration and renal adjustment need current local susceptibility data and specialist or institutional guidance. Suspected malignant disease requires urgent culture, imaging and multidisciplinary care; it is not a setting for a casual oral prescription. Document review timing, explicit safety-netting and the reason for every systemic agent.
When to Refer
Refer urgently to ENT or emergency care for suspected malignant otitis externa, cranial-nerve symptoms, severe persistent nocturnal pain, granulation tissue in a high-risk patient, systemic illness, cellulitis extending beyond the canal, mastoid signs, post-auricular swelling, facial weakness, vertigo, rapidly worsening hearing loss or failure to improve with correctly delivered initial treatment. Referral urgency rises with diabetes, immunocompromise, prior skull-base radiotherapy or inability to ensure follow-up.
ENT assessment is also appropriate when the canal is fully obstructed, aural toilet or wick placement is needed beyond available expertise, the membrane cannot be assessed in a symptomatic high-risk patient, fungal disease is recurrent, a foreign body is difficult to remove, perforation persists, chronic otorrhoea suggests middle-ear disease, or pain is unexplained by findings. Audiology may be useful after resolution if hearing does not return to baseline.
Send a focused handover: onset, pain trajectory, diabetes and immune status, prior ear surgery or tube, membrane visibility, cranial-nerve findings, treatments with exact names, culture obtained, allergy, renal status if systemic treatment is being considered, and images or imaging reports. Do not delay a transfer to obtain a superficial culture if the patient is unstable or invasive disease is likely. A referral labelled only as an ear infection loses the risk information the receiving team needs.
Red Flags
Severe pain that is persistent, disproportionate, nocturnal or worsening despite correctly delivered topical treatment is a red flag, especially with diabetes or immune compromise. Granulation tissue, deep canal tenderness, cranial-nerve palsy, dysphagia, hoarseness, facial asymmetry, vertigo, headache or altered mental status raises concern for skull-base extension and requires urgent escalation. Fever or raised inflammatory markers may support serious disease but their absence does not exclude it.
Post-auricular swelling, protruding pinna, mastoid tenderness, toxic appearance, meningism, facial weakness, sudden sensorineural hearing symptoms or severe vertigo should be assessed on an emergency pathway; these are not routine diffuse otitis externa findings. A young child with severe ear pain, foreign body concern or inability to permit safe examination may need specialist examination rather than repeated restraint and instrumentation.
Medication red flags are allergic reaction, rapidly worsening dermatitis, significant dizziness after an otic product, suspected ototoxic exposure with a non-intact membrane, or unplanned systemic antimicrobial use without review. A person who has used several drops, oils or traditional products may have both dermatitis and infection; bring containers or photographs. Failure at 48 to 72 hours is a prompt to reassess diagnosis, delivery and complications, not a reflex to add oral antibiotics.
Indian Clinical Context
Indian settings combine humid climates, water exposure, variable access to otoscopy, high use of self-medication, mixed topical products and variable access to ENT aural toilet. A national-facing guide must resist the false choice between doing nothing and issuing broad oral antibiotics. The ICMR/DHR otorrhoea workflow describes external-ear scenarios and topical options, but it predates current local resistance patterns and cannot replace a hospital antibiogram, current Indian product label or specialist assessment of an invasive presentation.

Uncomplicated diffuse disease is usually managed with topical therapy; routine oral therapy exposes patients to adverse effects and resistance while achieving lower canal concentrations. In recurrent, severe, hospital-acquired, immunocompromised or suspected malignant disease, obtain culture where feasible and tailor systemic treatment to current local susceptibility and the multidisciplinary plan. Do not present ciprofloxacin susceptibility, route or dose as universal: resistance, contraindications, renal function and local access vary.
