Summary
Neonatal jaundice is yellow discolouration caused by bilirubin accumulation during the first 28 days of life. It is common and often physiological, but visual appearance cannot determine the bilirubin concentration or distinguish unconjugated from conjugated hyperbilirubinaemia. Severe unconjugated hyperbilirubinaemia can cause acute bilirubin encephalopathy and permanent kernicterus. Conjugated hyperbilirubinaemia may indicate cholestasis or serious liver disease and is never explained by ordinary physiological jaundice.
Assessment is time-critical. Record gestational age, exact postnatal age in hours, birth and feeding history, weight trajectory, urine and stool colour, maternal and infant blood groups, family or sibling history, bruising, cephalohaematoma, infection risk and medicines. Examine the baby naked in bright light, including sclerae and gums because skin colour change can be harder to see in darker skin, but measure bilirubin whenever jaundice is suspected. Jaundice in the first 24 hours requires urgent serum bilirubin measurement and medical review for pathological causes.
Management is based on measured total serum bilirubin plotted against postnatal age, gestation and neurotoxicity risk using one current validated guideline. Phototherapy, intensified phototherapy, escalation of care and exchange transfusion have different thresholds. Do not mix values from current guidelines, AAP or a local neonatal chart. Support effective feeding, treat the cause and monitor the response. Pale stools, dark urine, poor feeding, lethargy, a high-pitched cry, abnormal tone, arching, apnoea or seizures are red flags. How Common Is It?
Jaundice is extremely common during the first week because newborns produce more bilirubin, have immature hepatic conjugation and undergo enterohepatic circulation. current guidelines reports visible jaundice in approximately 60% of term and 80% of preterm babies during the first week, with some breastfed infants remaining jaundiced at one month. These figures are context estimates rather than an Indian prevalence measurement, and visible jaundice is not synonymous with clinically significant hyperbilirubinaemia.
Most physiological jaundice resolves without injury, but the distribution has a dangerous tail. A small number of babies develop bilirubin concentrations high enough to threaten the brain, often because of haemolysis, prematurity, infection, bruising, inadequate milk transfer, G6PD deficiency or missed follow-up. Severe disease is preventable when maternity and newborn systems measure bilirubin, interpret it by age in hours, communicate the result and arrange timely review after discharge. The AAP therefore treats predischarge bilirubin measurement and risk-linked follow-up as system responsibilities, not optional individual memory.
Frequency also depends on how jaundice is detected. Visual inspection underrecognises some cases and cannot estimate severity, especially across different skin pigmentation and lighting. Transcutaneous screening identifies more babies for confirmation than visual assessment alone. Hospital data overrepresent treated cases, while home births and early discharge can hide disease until neurological signs appear.
This guide does not claim a current Indian national incidence of severe hyperbilirubinaemia or kernicterus. The practical epidemiological lesson is that common benign jaundice and rare catastrophic toxicity coexist; universal observation, objective measurement when suspected and reliable follow-up are required to separate them safely.
Risk Factors
Current guidelines identifies gestation under 38 weeks, a previous sibling who needed phototherapy, intended exclusive breastfeeding and visible jaundice in the first 24 hours as factors associated with significant hyperbilirubinaemia. Breastfeeding itself should be protected. The clinical concern is ineffective milk transfer, delayed lactogenesis, excessive weight loss, dehydration or too few feeds, not a reason to substitute formula automatically. Provide skilled lactation assessment and monitor intake, urine, stool and weight.
Haemolysis can produce a rapid rise. Risks include maternal red-cell antibodies, Rh or ABO incompatibility, a positive direct antiglobulin test interpreted in context, G6PD deficiency, hereditary spherocytosis and other red-cell disorders. Bruising, cephalohaematoma, polycythaemia and swallowed blood increase bilirubin load. Prematurity and lower gestation reduce bilirubin tolerance and change treatment thresholds. Sepsis, clinical instability, hypoxia or asphyxia, acidosis and low albumin may increase neurotoxicity risk.
Family origin and history can guide G6PD assessment, but ancestry is an imperfect proxy and testing during acute haemolysis may be falsely reassuring. Ask about a sibling with neonatal jaundice, anaemia, gallstones, splenectomy or unexplained neonatal death. Review maternal medicines and neonatal illness.
Risk continues after discharge. Birth before 38 weeks, bilirubin close to the treatment threshold, discharge at a young postnatal age, feeding difficulty and inability to obtain a scheduled test or clinical review require earlier follow-up. Social, geographic and financial barriers are safety factors. A treatment chart is only safe if the correct gestation, age, units and risk category are entered and the family can return when planned.
Diagnosis
Diagnosis requires objective bilirubin measurement and interpretation, followed by cause assessment when timing, concentration or course is atypical. Record the result with postnatal age in completed hours, gestational age, units, specimen type and the exact chart used. Never rely on the cephalocaudal extent of yellow skin to prescribe treatment.
History
Ask onset and progression, gestation, birth weight, delivery trauma, bruising, cephalohaematoma, delayed cord clamping, feeding frequency and effectiveness, maternal milk coming in, supplement use, urine output, stool transition, current weight and percentage loss. Record maternal and infant blood groups, antenatal antibody screen, anti-D administration, previous affected siblings, family red-cell disease, G6PD risk, infection or sepsis risks and neonatal medicines. Ask specifically about dark urine staining the nappy and pale, chalky or white stools.
Examination
Assess temperature, heart rate, respiration, perfusion, alertness, tone and hydration. Examine the naked baby in bright natural light where possible, including sclerae and gums and blanched skin. Look for pallor, petechiae, bruising, cephalohaematoma, hepatosplenomegaly, dysmorphism, infection and weight loss. Neurological examination looks for poor suck, lethargy, hypotonia, irritability, high-pitched cry, hypertonia, retrocollis, opisthotonus, apnoea or seizures. Check stool and urine colour directly when possible.
Investigations
Use serum bilirubin for jaundice in the first 24 hours, gestation under 35 weeks, transcutaneous values above the device’s confirmatory range, values near or above treatment thresholds, and all subsequent treatment decisions. Otherwise, transcutaneous bilirubin can screen babies at least 35 weeks and over 24 hours under current guidelines. For significant hyperbilirubinaemia, assess packed cell volume, maternal and infant groups and DAT, with CBC, film, reticulocytes, G6PD and cultures as indicated. Measure direct or conjugated bilirubin in prolonged jaundice and whenever cholestasis is suspected.
Differential Diagnosis
Physiological unconjugated jaundice appears after the first 24 hours, rises and then falls in a well baby without haemolysis or other disease. Suboptimal-intake jaundice occurs in the first week when milk transfer is inadequate and is accompanied by excess weight loss, infrequent stool or urine and feeding difficulty. Breast-milk jaundice is a later, prolonged unconjugated pattern in a thriving infant after other causes are excluded. Neither label should be used before assessing feeding and measuring bilirubin.
Pathological unconjugated causes include Rh, ABO or minor blood-group haemolysis, G6PD deficiency, hereditary spherocytosis, bruising or cephalohaematoma, polycythaemia, sepsis, prematurity, hypothyroidism and rare conjugation disorders such as Crigler-Najjar syndrome. Jaundice within 24 hours, a rapid bilirubin rise, anaemia, reticulocytosis, hepatosplenomegaly or a family history makes haemolysis or disease more likely.
Conjugated hyperbilirubinaemia suggests cholestasis. Differential diagnoses include biliary atresia, neonatal hepatitis, sepsis, urinary infection when clinically suspected, metabolic disease, galactosaemia, tyrosinaemia, alpha-1 antitrypsin deficiency, hypothyroidism and structural obstruction. Pale or acholic stools and dark urine are urgent clues. Early biliary-atresia referral matters because treatment is time-sensitive; a well appearance does not make pale stools benign.
Extravasated blood, swallowing maternal blood and laboratory or unit errors may complicate interpretation. Direct bilirubin is not subtracted from total when deciding phototherapy or exchange thresholds under current guidelines. If measured values and clinical appearance disagree, verify patient identity, sampling, units and assay, repeat appropriately, and seek neonatal advice rather than choosing the more reassuring result.
Management
Support feeding from the first assessment. Observe a breastfeed, correct attachment and positioning, wake a sleepy baby for frequent feeds when appropriate, record urine and stool output and obtain serial weights. Expressed breast milk or supplementary feeding may be needed for a clinical indication through a shared plan, but routine interruption of breastfeeding is not treatment. Intravenous fluid is reserved for babies who cannot maintain hydration enterally or are clinically unstable.
Plot bilirubin on the current gestation- and hour-specific chart adopted by the facility. Start phototherapy only at the applicable threshold. Use an approved blue-light device with verified irradiance, maximise safe skin exposure, protect the eyes according to equipment policy, maintain thermoregulation and monitor hydration. Continue feeding and parent contact with interruptions only when safe under the chosen protocol. Measure serum bilirubin at specified intervals; visual assessment is unreliable during phototherapy. Intensify phototherapy for rapid rise, failure to fall or proximity to exchange threshold according to the selected guideline.
Escalation of care is an emergency pathway. Notify neonatology, increase phototherapy irradiance and exposed area, obtain urgent laboratory evaluation, prepare compatible blood and transfer early if exchange transfusion capability is absent. Exchange transfusion is used at the relevant threshold or for signs of acute bilirubin encephalopathy; it requires intensive monitoring and skilled staff. Treat sepsis, haemolysis, dehydration or endocrine and metabolic disease concurrently.
Stop phototherapy and plan rebound measurement according to gestation, age at initiation, haemolysis risk and chart-specific criteria. Discharge only with feeding adequacy, a documented bilirubin result, distance below treatment threshold, follow-up time and responsible clinician. Prolonged or conjugated jaundice follows a separate diagnostic and hepatology pathway.
Prescribing Information
Phototherapy is the principal treatment and should be prescribed with the same precision as a medicine: indication, threshold chart, irradiance target, device, start time, monitoring interval and stop criteria. Check equipment output with the appropriate meter and maintain the manufacturer-recommended distance; distance cannot be copied across devices because irradiance and heat differ. Monitor temperature, hydration, weight and skin condition. Eye protection must fit without obstructing the airway. Bronze discolouration can occur in conjugated hyperbilirubinaemia and should prompt review of the diagnosis.
Do not prescribe sunlight as treatment. Irradiance is uncontrolled, effective exposure is unreliable, and overheating, dehydration and ultraviolet injury can occur. Do not use phenobarbital, albumin, clofibrate, cholestyramine, charcoal, herbal products, homeopathy or other unproven remedies for routine neonatal hyperbilirubinaemia. Antibiotics are given only for suspected or proven infection, not for bilirubin elevation itself.
Intravenous immunoglobulin has a narrow, guideline-specific role in isoimmune haemolytic disease when bilirubin reaches escalation criteria despite intensive phototherapy. The 2022 AAP guidance notes uncertain benefit and possible harms; neonatology should decide, not a routine ward protocol detached from the current bilirubin trajectory. Exchange transfusion is a blood procedure, not a last-minute prescription. Arrange cross-matched blood, calcium and glucose monitoring, cardiorespiratory support and experienced staff early.
Avoid prophylactic extra water or dextrose water; it does not improve bilirubin elimination and may undermine milk intake. Any supplementation plan should address inadequate transfer or hydration and protect lactation. Confirm all bilirubin units before action: micromol/L and mg/dL are not interchangeable. Document the exact result and chart rather than writing simply high bilirubin.
When to Refer
Refer every baby with visible jaundice in the first 24 hours for urgent serum measurement and medical review. Transfer or discuss urgently when bilirubin is at or approaching the escalation or exchange threshold, rising rapidly, failing to respond to correctly delivered phototherapy, or accompanied by anaemia, suspected haemolysis, sepsis, hypothermia, poor perfusion or significant dehydration. If the local facility lacks reliable bilirubin measurement, intensive phototherapy or exchange capability, stabilise, start appropriate phototherapy when indicated and arrange equipped transfer without waiting for neurological signs.
Neurological abnormality requires emergency neonatology: poor suck, lethargy, abnormal tone, high-pitched cry, arching, retrocollis, opisthotonus, apnoea, seizures or reduced consciousness may indicate acute bilirubin encephalopathy. Exchange transfusion may be required even when a number has not yet crossed the standard line because clinical neurotoxicity changes management.
Refer prolonged jaundice for structured evaluation: beyond 14 days in a baby born at 37 weeks or more, or beyond 21 days when born under 37 weeks under current guidelines. Pale or chalky stools, dark urine, hepatomegaly, poor growth, bleeding or elevated conjugated bilirubin requires urgent paediatric gastroenterology or hepatology assessment. Do not reassure solely because the baby is breastfed.
A high-quality referral includes gestation, birth and current weight, exact age in hours, all bilirubin values with times and units, chart and threshold used, maternal and infant groups and DAT, haemoglobin and reticulocytes when available, feeding and urine or stool history, neurological examination, phototherapy device and start time, response, infection concerns and transport support.
Red Flags
Jaundice visible during the first 24 hours is pathological until evaluated and requires urgent serum bilirubin measurement. At any age, rapidly deepening jaundice, bilirubin rising faster than expected, a value near treatment or exchange threshold, pallor, hepatosplenomegaly, bruising, fever, hypothermia, poor perfusion or marked weight loss needs prompt assessment. Visual extent is not a safe substitute for a number.
Early acute bilirubin encephalopathy may present with sleepiness, poor feeding, weak suck, hypotonia or a high-pitched cry. Progression can produce irritability, hypertonia, backward arching, retrocollis and opisthotonus, then apnoea, seizures, coma or death. These signs require immediate escalation, intensive phototherapy and exchange-transfusion preparation under neonatology. Do not wait for a repeat routine clinic appointment.
Pale, chalky, grey or white stools and dark urine that stains the nappy are cholestasis red flags. They require direct bilirubin measurement and urgent investigation for biliary atresia and other hepatobiliary disease. Ordinary unconjugated physiological jaundice does not make urine dark or stools acholic. Families should be shown how to inspect nappies, not merely told to look for abnormal colour.
Feeding red flags include inability to wake for feeds, ineffective suck, fewer wet nappies, persistent vomiting, dry mucosae and excessive weight loss. Infection may present subtly with temperature instability, lethargy, apnoea or poor feeding. After discharge, parents need the exact return time and emergency contact. Home sunlight, herbal remedies or delay while waiting for skin colour to worsen can allow preventable brain injury.
Indian Clinical Context
India’s National Health Mission currently lists Facility Based Newborn Care Operational Guidelines 2025 for public newborn services. The listing confirms a current national operational framework, but the indexed page does not expose enough text to justify reproducing a particular bilirubin threshold from it here. Facilities should use the full locally issued FBNC document, current neonatal chart, calibrated phototherapy equipment and defined referral network. This guide does not claim that every SNCU, NBSU or delivery point has identical testing or exchange capability.
Where transcutaneous bilirubinometry is unavailable, serum measurement is required when jaundice is suspected. Early discharge, home birth, travel distance and laboratory turnaround should change follow-up planning. ASHA and postnatal home visits can help identify jaundice, poor feeding and abnormal nappies, but visual assessment cannot prescribe phototherapy. A baby with first-day jaundice or neurological signs needs facility care, not serial observation at home.
G6PD deficiency and haemolytic disease must be considered in the relevant family and clinical context. Testing access and variant distribution vary, and a normal result during acute haemolysis may need specialist interpretation. Blood-bank readiness is part of safe referral when exchange transfusion is possible. Do not transfer a severely jaundiced baby without thermal care, feeding or glucose planning and continued effective phototherapy if transport equipment permits.
Commercial devices and apps may display AAP or other charts. The clinician must choose one approved local framework and enter gestation, hours, units and neurotoxicity risks correctly. international and AAP thresholds are not interchangeable. No national prevalence number is asserted. Family counselling should be in the preferred language and explicitly reject sunlight and traditional remedies as substitutes for measurement and treatment.
