Summary
Menopause is a normal life stage, not a disease. Natural menopause is diagnosed retrospectively after 12 consecutive months without menstruation when there is no other physiological, pathological or treatment-related explanation. Perimenopause begins when menstrual pattern changes and related symptoms emerge, and extends to one year after the final menstrual period. Most natural menopause occurs between 45 and 55 years worldwide, but the timing, symptoms and effect on daily life vary substantially. Ovarian follicular depletion and fluctuating, then declining, oestrogen exposure can produce vasomotor symptoms, sleep disturbance, mood or cognitive complaints, musculoskeletal symptoms and genitourinary syndrome of menopause. Some people have few symptoms; others have marked impairment for years.
Clinical care should begin with the person's priorities rather than a hormone result. In an otherwise healthy person aged 45 or older with typical symptoms and menstrual change, the diagnosis is usually clinical. Pregnancy and other causes of amenorrhoea or systemic symptoms must still be considered when the history suggests them. Menopause before 40 raises concern for premature ovarian insufficiency; menopause from 40 to 44 is early menopause. Both warrant more deliberate evaluation because untreated hypo-oestrogenism at a young age affects bone, cardiovascular, sexual and reproductive health.
Management combines explanation, healthy-ageing care and symptom-directed options. Menopausal hormone therapy is the most effective treatment for troublesome vasomotor symptoms and may also help sleep when sleep disruption is driven by night sweats. Genitourinary symptoms may respond to lubricants, moisturisers or local vaginal oestrogen. Treatment must be individualised for uterine status, age, time since menopause, bleeding history, thromboembolic and cardiovascular risk, liver disease, cancer history, medicines, contraception needs and informed preference. How Common Is It?
Menopause is universal among people who retain ovarian function long enough, but symptom prevalence is not a single fixed number. WHO places most natural menopause between 45 and 55 years. The Indian Menopause Society describes substantial variation across Indian studies in age at menopause, symptom clusters, severity, socioeconomic context and methods of measurement. Clinic samples often over-represent people with severe symptoms, while community surveys may use different symptom scales and definitions. A percentage from one city, age band or questionnaire should therefore not be presented as a national prevalence estimate.
Vasomotor symptoms -- hot flushes and night sweats -- are among the most recognised complaints. Sleep disturbance, fatigue, irritability, anxiety or low mood, vaginal dryness, dyspareunia, urinary symptoms, joint or muscle discomfort and reduced sexual well-being are also common in practice, but they are neither inevitable nor specific to menopause. Symptoms can begin while cycles continue, change over time and persist after the final menstrual period. Genitourinary syndrome often becomes more prominent with time unless it is addressed. The transition can affect work, caregiving, relationships and quality of life even when examination and routine investigations are normal.
India's large and diverse midlife population makes access and communication as important as raw prevalence. Cultural silence, normalisation of distress, cost, rural access, comorbidity and limited clinician training can all delay care. The Central Health Education Bureau's WoMAN:2024 initiative explicitly recognised unmet needs and sought appropriate health-promotion material. Clinicians should ask proactively but respectfully about vasomotor, sleep, mood, sexual, urinary and bleeding symptoms. They should not assume that every midlife complaint is hormonal, nor dismiss a symptom because menopause is common. Population burden supports routine capability in primary care, while diagnostic uncertainty, severe symptoms or complex risk requires referral.
Risk Factors
It is useful to separate factors associated with earlier loss of ovarian function from factors that influence symptom burden and from risks that become clinically important after menopause. Earlier menopause can follow bilateral oophorectomy, gonadotoxic chemotherapy or pelvic radiotherapy. Premature ovarian insufficiency may be associated with chromosomal or genetic conditions, autoimmune disease, ovarian surgery or an unknown cause. Smoking is associated with earlier natural menopause in population studies. A family history of early menopause can be informative, but no history or blood test predicts the precise date of an individual's final menstrual period.
The severity of menopausal symptoms cannot be read from oestrogen concentration alone. Baseline anxiety or depression, poor sleep, chronic pain, obesity, tobacco or alcohol use, workplace stress, caregiving, intimate relationships, cultural expectations and access to support may alter how symptoms are experienced. Some medicines and illnesses mimic or amplify flushing, sweating, insomnia or palpitations. Cancer treatment can produce an abrupt transition with a different risk-benefit discussion from gradual natural menopause. People with hysterectomy may have no bleeding marker, while those using hormonal contraception may have masked cycles.
After menopause, age and established risk factors remain central. Bone risk increases with prior fragility fracture, low body weight, parental hip fracture, long-term glucocorticoids, smoking, excess alcohol, rheumatoid disease, malabsorption, thyroid or parathyroid disease, renal or liver disease and premature ovarian insufficiency. The ICMR-DHR fragility-fracture workflow specifically flags postmenopausal status and premature ovarian failure as reasons to assess bone health in context. Cardiovascular assessment should still address blood pressure, diabetes, lipids, tobacco, adiposity, activity and family history rather than treating menopause as the sole cause. Before systemic hormone therapy, identify unexplained vaginal bleeding, previous venous thromboembolism, thrombophilia, stroke, myocardial infarction, active liver disease and hormone-sensitive cancer history because these may change route, regimen or suitability and often require specialist input.
Diagnosis
Menopause is a clinical diagnosis in the usual age group, but safe diagnosis requires confirmation that the pattern fits and that dangerous alternatives are not being overlooked. Establish whether the person has a uterus and ovaries, whether cycles are visible, whether contraception or hormonal treatment alters bleeding, and whether menopause could be induced by surgery, chemotherapy or radiotherapy.
History
Document the last normal menstrual period and the previous one to two years of cycle length, flow, intermenstrual or postcoital bleeding. Characterise hot flushes, night sweats, sleep, mood, concentration, headaches, palpitations, musculoskeletal symptoms, vaginal dryness, dyspareunia, libido, dysuria, urgency, recurrent urinary symptoms and their functional impact. Ask about pregnancy possibility, contraception, medicines, thyroid symptoms, galactorrhoea, weight change and systemic illness. Record personal and family history of breast, ovarian and endometrial cancer; venous thromboembolism; thrombophilia; migraine; cardiovascular disease; liver disease; osteoporosis and fragility fracture. Ask sensitively about sexual health, mental health and the person's goals.
Examination
Examination is guided by the presentation, not performed as a ritual barrier to care. Record blood pressure, pulse and clinically relevant weight or body-mass context. Examine the thyroid, cardiovascular system or breasts only when history or routine preventive care indicates. Abdominal and pelvic examination is appropriate for abnormal bleeding, pelvic pain, a mass, prolapse, discharge or persistent urinary or vulvovaginal symptoms. Vulval and vaginal examination may distinguish atrophy from dermatosis, infection, trauma or neoplasia. Assess gait, height loss or spinal tenderness when fracture risk is raised.
Investigations
For a person aged 45 or older with typical symptoms and cycle change, current guidelines recommends clinical identification without routine anti-Mullerian hormone, inhibin, oestradiol, antral-follicle count or ovarian-volume testing. Consider FSH when the person is aged 40 to 45 with symptoms and cycle change, or under 40 when premature ovarian insufficiency is suspected; interpret it in the correct clinical and hormonal-treatment context. Check pregnancy testing, full blood count, TSH, prolactin, glucose, liver or renal tests only when symptoms, bleeding, medicines or comorbidity justify them. Investigate abnormal uterine bleeding according to age and risk rather than labelling it menopausal. Bone-density testing and cardiovascular investigations are risk-based. Cervical and breast screening should follow current Indian programmes and individual risk; they are not diagnostic tests for menopause.
Differential Diagnosis
A menopausal transition may coexist with another disorder. First exclude pregnancy whenever ovulation remains possible and pregnancy would change management; irregular cycles do not provide contraception. Thyroid disease can cause cycle change, sweating, palpitations, anxiety, weight change and sleep disturbance. Hyperprolactinaemia, hypothalamic amenorrhoea, polycystic ovary syndrome and other endocrine disorders may produce amenorrhoea, especially outside the typical age range. Anaemia can explain fatigue, dyspnoea or palpitations, particularly when bleeding is heavy. Medication effects -- including stimulants, corticosteroids, opioids, some psychotropics and cancer therapies -- should be reviewed rather than assumed.
Hot flushes and night sweats are not pathognomonic. Infection, malignancy, hypoglycaemia, alcohol withdrawal and, rarely, neuroendocrine conditions can cause episodic flushing or sweats. Features such as fever, objective weight loss, lymphadenopathy, persistent diarrhoea, wheeze, severe hypertension or a new murmur should redirect the work-up. Primary sleep disorders, depression, anxiety, trauma and chronic pain may be more important than hormonal change when night sweats are absent. New focal neurological symptoms, exertional chest symptoms or sustained arrhythmia require their own evaluation.
Genitourinary symptoms have a broad differential: candidiasis or bacterial infection, urinary tract infection, sexually transmitted infection, lichen sclerosus or other vulval dermatoses, contact irritation, pelvic-floor dysfunction, interstitial bladder pain, prolapse and malignancy. Visible lesions, persistent discharge, haematuria or non-response to an initial reasonable treatment should prompt examination and targeted testing. Postmenopausal bleeding is never a normal diagnostic feature; causes include atrophy, polyps, endometrial hyperplasia or cancer, cervical disease, trauma and treatment-related bleeding, and it requires a defined assessment pathway.
In a person younger than 40, distinguish premature ovarian insufficiency from hypothalamic, pituitary, thyroid, pregnancy-related and genetic causes. In someone with hysterectomy or amenorrhoea from hormonal contraception, symptoms, treatment history and age carry more weight because the final menstrual period cannot be observed. The discipline is simple: use menopause to explain a coherent syndrome, not as a shortcut that closes diagnostic reasoning.
Management
Begin with validation and shared decisions. Explain the transition, expected variability, fertility and contraception, and which symptoms require separate assessment. Agree a small number of outcomes that matter to the person -- for example fewer night sweats, restored sleep, comfortable intercourse or improved ability to work -- and review against them. Offer support for tobacco cessation, alcohol moderation, regular weight-bearing and resistance exercise, adequate dietary protein and calcium, vitamin D sufficiency, sleep routines and management of blood pressure, diabetes and lipids. These measures support healthy ageing but should not be sold as guaranteed cures for severe vasomotor symptoms. Menopause-specific cognitive behavioural therapy can help some people manage vasomotor symptoms, sleep problems or associated distress.
For troublesome vasomotor symptoms, systemic menopausal hormone therapy is the most effective option for many suitable patients. Discuss absolute benefits and harms in relation to age, time since menopause, uterine status and individual vascular, cancer and liver risk. People with a uterus need endometrial protection with a progestogen when using systemic oestrogen; oestrogen-only therapy is generally reserved for those after total hysterectomy. A transdermal route can be preferable when oral first-pass effects or venous-thromboembolic and metabolic risk are concerns. Menopausal hormone therapy is not contraception and should not be initiated solely to prevent cardiovascular disease, dementia or ageing. Premature ovarian insufficiency and early menopause require a distinct discussion because replacement until around the usual age of natural menopause may protect bone and other health outcomes when not contraindicated.
For genitourinary syndrome, start with regular vaginal moisturiser and lubricant according to need, and offer local vaginal oestrogen when appropriate. Systemic absorption is low but not zero; people with current or previous breast cancer, unexplained bleeding or complex treatment histories need coordinated advice. Pelvic-floor treatment, infection management and sexual counselling may be more relevant for selected patients. Non-hormonal medicines for vasomotor symptoms can be considered when hormone therapy is unsuitable or declined, but the drug should match comorbidity, interactions and adverse-effect tolerance; antidepressants should not be presented as a routine first-line substitute when the only indication is vasomotor symptoms.
Review effectiveness, adverse effects, adherence and bleeding at about three months, then at least annually, or sooner after a material change. Reconsider rather than automatically continuing or stopping at a fixed age. Any prescribing recommendation must be checked against the Indian Menopause Society 2026 guideline together with its published corrigendum, current Indian product information and the patient's complete clinical record.
Prescribing Information
This section teaches regimen logic and safety checks; it is not a prescription. Before systemic therapy, confirm the indication, menstrual stage, uterus and ovary status, pregnancy possibility, contraception need, bleeding history, blood pressure, relevant cancer history, venous-thromboembolic and cardiovascular history, migraine, liver disease, medicines and patient preference. Explain likely benefit, common adverse effects, uncertainty and alternatives. Record informed consent and a review plan. Unexplained bleeding must be assessed rather than suppressed with hormones.
For systemic oestrogen, use the lowest effective dose and individualise the route. Examples identified as low-dose options in the Indian Menopause Society 2026 tables include oral 17-beta-estradiol 0.5 mg daily, transdermal 17-beta-estradiol 0.025 mg (25 micrograms) per 24 hours and oral estradiol valerate 1 mg daily. These are examples for clinician verification, not interchangeable instructions: formulations, patch schedules, absorption and licensed indications differ. If a uterus is present, do not give unopposed systemic oestrogen. A commonly guideline-based endometrial-protection pattern with oral micronised progesterone is 200 mg nightly for 12 to 14 days in each 28-day cycle for a sequential regimen, or 100 mg nightly continuously for a continuous combined regimen. The selected progestogen, duration and dose must be adequate for the oestrogen dose and checked against the current label and the 2026 guideline plus corrigendum.
A sequential combined regimen commonly suits someone still having periods or within the earlier postmenopause, where a predictable withdrawal bleed is acceptable. Continuous combined therapy aims for amenorrhoea after menopause but irregular bleeding can occur during establishment. Current guidelines advises prompt help when unscheduled bleeding persists beyond the expected early treatment window or starts after stability. Dose escalation should follow an effectiveness and adherence review, not a reflex response to every symptom. Transdermal oestrogen may be preferred in obesity, diabetes, hypertriglyceridaemia, gallbladder risk, migraine or raised venous-thromboembolic risk, but a patch does not erase contraindications.
For isolated vaginal dryness, dyspareunia or urinary symptoms, use vaginal therapy rather than exposing the whole body without another indication. Product-specific vaginal oestrogen schedules vary, so verify the local label. Moisturisers and lubricants can be used alone or alongside it. Review at roughly three months for symptom response, adverse effects and bleeding, then annually. Advise urgent assessment for chest pain, acute breathlessness, unilateral leg swelling, focal neurological deficit, jaundice or significant new bleeding. Never use a brand claim as evidence of superiority, and never imply that hormone therapy is risk-free or universally required.
When to Refer
Refer urgently through an appropriate gynaecology or suspected-cancer pathway for any postmenopausal bleeding, a suspicious cervical or vulval lesion, a pelvic mass, persistent unexplained discharge or bleeding that meets the current abnormal-bleeding threshold during hormone therapy. Postmenopausal bleeding is an NMC learning objective because the differential includes malignancy; atrophy may be common, but it is a diagnosis made after appropriate assessment. Refer acutely for suspected venous thromboembolism, stroke, acute coronary syndrome, severe liver injury or another emergency rather than managing it as a menopause adverse effect in routine clinic.
Seek menopause or gynaecology expertise when symptoms are severe or persist despite a correctly used first option, the diagnosis is uncertain, treatment causes difficult bleeding, multiple conditions complicate the choice, or the patient wants a therapy outside the clinician's competence. Specialist advice is usually appropriate with previous or active breast or another hormone-sensitive cancer, high inherited cancer risk, previous venous thromboembolism or thrombophilia, established cardiovascular or cerebrovascular disease, active significant liver disease, complex migraine, severe endometriosis history or an uncertain uterine status. Coordination with oncology, haematology, cardiology, hepatology or psychiatry may be more useful than a menopause-only referral.
Suspected premature ovarian insufficiency before 40 requires confirmation and aetiological, fertility, bone, cardiovascular and psychosocial planning. Early menopause from 40 to 44 also deserves a fuller discussion than routine natural menopause. Refer for fertility counselling promptly when future pregnancy is desired because menopausal hormone therapy does not restore fertility. Bone specialist input is appropriate for fragility fracture, very low bone density, recurrent fracture, secondary osteoporosis or treatment complexity. Refer to pelvic health, urogynaecology, dermatology or sexual medicine when prolapse, incontinence, persistent recurrent urinary symptoms, vulval dermatosis, pain or sexual dysfunction is the dominant problem. Mental-health referral should be prompt for severe depression, mania, psychosis, self-harm risk or symptoms that cannot safely wait for routine review.
Red Flags
Bleeding is the highest-yield red flag. Any vaginal bleeding occurring after 12 months of natural amenorrhoea needs assessment, even when it is a single small episode. Also escalate heavy or prolonged bleeding, postcoital bleeding, bleeding with pelvic pain, bleeding after a previously stable hormone regimen, or unscheduled bleeding outside the expected initiation or dose-change window. Do not reassure solely because the person uses hormone therapy or vaginal oestrogen. Associated abdominal distension, early satiety, unexplained weight loss, a pelvic mass, offensive discharge, vulval ulceration or a new breast mass strengthens the need for rapid evaluation.
Systemic emergencies remain emergencies in midlife. Chest pressure, acute breathlessness, haemoptysis, unilateral leg swelling, sudden facial weakness, speech disturbance, focal weakness, collapse or severe new headache require immediate assessment for thromboembolism, stroke or cardiovascular disease. Jaundice, dark urine or marked right-upper-quadrant pain can indicate hepatobiliary disease. Fever, drenching sweats with objective weight loss, persistent tachycardia or progressive lymphadenopathy should not be attributed to a hot flush. New fracture after minor trauma, acute severe back pain, height loss or repeated falls may reveal fragility fracture.
Psychiatric red flags include suicidal thoughts, self-harm, psychosis, mania, severe functional collapse or risk from domestic or sexual violence. These need direct, compassionate questions and urgent support; menopause may contribute to distress but does not replace a safety assessment. Severe headache with visual symptoms, galactorrhoea, or amenorrhoea far outside the usual age range may signal endocrine or neurological disease. In perimenopause, severe lower abdominal pain, syncope or bleeding with pregnancy possibility requires urgent pregnancy testing and ectopic-pregnancy assessment.
Finally, a treatment red flag is any regimen that gives systemic oestrogen to a person with a uterus without adequate endometrial protection, or a prescription issued without investigating unexplained bleeding. Stop, verify the anatomy and regimen, and obtain expert advice rather than perpetuating unsafe exposure.
Indian Clinical Context
Indian practice spans tertiary menopause clinics, private gynaecology, district hospitals, primary health centres and teleconsultation. A high-quality plan must work within the patient's access, literacy, cost and follow-up realities. The Indian Menopause Society 2026 guideline is the principal India-specific professional source for assessment and treatment; it should be read with its 2026 corrigendum. The ICMR-DHR fragility-fracture workflow supplies an India-specific pathway for recognising risk, selecting basic investigations and referring within different health-system levels. The Central Health Education Bureau's WoMAN:2024 work confirms a public-health need for clearer menopause awareness and navigation. None of these sources supports a one-size-fits-all package or a commercial brand preference.
Use generic names, document affordability and explain exactly which outcomes a medicine is intended to improve. Confirm that the formulation and strength are currently licensed and available in India before prescribing; availability can differ by state, sector and time. When regular follow-up is uncertain, choose a plan the patient can understand, obtain and monitor safely. A written bleeding-safety message and follow-up date are more valuable than an elaborate regimen that cannot be reviewed. Teleconsultation can support counselling and monitoring but should not delay physical examination for bleeding, a mass, a vulval lesion, breast symptoms or serious systemic signs.
Preventive care should be integrated without turning menopause into a compulsory test bundle. Assess blood pressure, diabetes and cardiovascular risk according to age and risk; address tobacco, activity, nutrition and fall prevention. Use fracture history and clinical risk to decide on bone-density testing rather than ordering it universally. Offer cervical and breast screening according to current Indian national or professional guidance and individual risk, while making clear that normal screening does not evaluate postmenopausal bleeding. Contraception remains relevant during perimenopause, and hormone therapy is not contraceptive. Ask about sexual health and urinary symptoms without assuming marital status or heterosexual activity.
Respect local language, family context and the patient's autonomy. Some people prefer a family member present; others need private consultation. Validate symptoms without medicalising normal ageing, and offer treatment when symptoms impair well-being. Where a guideline from another country fills an evidence gap, adapt it explicitly to Indian licensing, contraindications, access and referral pathways rather than copying it uncritically.
NMC Competency Mapping
This guide directly supports the Competency Based Medical Education Curriculum 2024. Physiology competency PY9.9 requires the learner to discuss hormonal changes and their effects during perimenopause and menopause. The core mechanism is progressive depletion of ovarian follicles, variable ovarian function during transition and eventual sustained reduction in ovarian oestrogen, with loss of predictable hypothalamic-pituitary-ovarian feedback. A competent answer should define perimenopause and natural menopause; classify natural, premature, early, surgical and treatment-induced menopause; organise symptoms into vasomotor, menstrual, psychological or sleep, musculoskeletal and genitourinary domains; and distinguish health promotion from symptom treatment. It should explain why systemic oestrogen treats vasomotor symptoms, why a progestogen is needed to protect the endometrium when the uterus is present, why route and dose are individualised, and why hormone therapy is not contraception or a universal anti-ageing intervention.
OG32.2 requires enumeration of causes and management of postmenopausal bleeding. Learners should name atrophy, polyps, endometrial hyperplasia or cancer, cervical pathology, trauma and treatment-related bleeding, then state the governing principle: every episode requires appropriate evaluation and cancer exclusion. A reassuring explanation is not a substitute for examination and investigation.
For bedside or viva performance, demonstrate respectful history-taking, symptom-impact assessment, red-flag recognition, risk assessment, shared decision-making and safety-netting. The expected graduate is not required to memorise every commercial preparation. The safer competence is to choose between non-drug care, local therapy and systemic therapy; match oestrogen-only versus combined therapy to uterine status; recognise contraindications; arrange follow-up; and refer uncertainty. This also integrates physiology, pharmacology, pathology, community medicine, psychiatry and preventive care.
Key Exam Pearls for NEET PG
Natural menopause is retrospective: 12 consecutive months of amenorrhoea with no other physiological or pathological cause and no intervention explaining it. Perimenopause extends from the first recognised transition changes to one year after the final menstrual period. Menopause before 40 suggests premature ovarian insufficiency; 40 to 44 is early menopause. Pregnancy remains possible during perimenopause, and menopausal hormone therapy is not contraception.
In a typical symptomatic patient aged at least 45 with cycle change, diagnosis is usually clinical. Do not choose FSH, AMH, inhibin, oestradiol, ovarian volume or antral-follicle count as routine confirmation. Hot flushes and night sweats are vasomotor symptoms. Vaginal dryness, dyspareunia, vulvovaginal discomfort and urinary symptoms fit genitourinary syndrome of menopause, which may persist or progress. Postmenopausal bleeding is not a normal menopausal symptom: investigate it. Remember endocrine, pregnancy, medication, infection, malignancy, psychiatric and primary sleep differentials when the presentation is atypical.
Systemic menopausal hormone therapy is the most effective treatment for bothersome vasomotor symptoms in an appropriately selected patient. Uterus present means systemic oestrogen requires adequate progestogen protection; after total hysterectomy, oestrogen-only therapy is generally used. Use the lowest effective dose and individualise the route. Transdermal oestrogen avoids hepatic first-pass exposure and can be favoured when venous-thromboembolic or metabolic considerations make oral therapy less attractive, but it is not free of all contraindications. Local vaginal oestrogen is targeted to genitourinary symptoms and is not a treatment for hot flushes.
Review after approximately three months and at least annually. Escalate persistent or late-onset unscheduled bleeding, thromboembolic or neurological symptoms and diagnostic uncertainty. Do not use hormone therapy solely to prevent cardiovascular disease or dementia. In examinations, the safest sequence is define, classify, exclude red flags and mimics, assess uterus and risks, individualise treatment, provide endometrial protection, and plan monitoring.

Does every person need a blood test to diagnose menopause?
No. In an otherwise healthy person aged 45 or older who has typical menopausal symptoms and menstrual-cycle change, the diagnosis is usually clinical. Routine FSH, AMH, inhibin, oestradiol, ovarian-volume or antral-follicle testing adds little. FSH may be considered from age 40 to 45 when the diagnosis is uncertain, and is part of evaluating suspected premature ovarian insufficiency under 40. Pregnancy, thyroid disease or other tests are ordered when the history indicates them, not as a universal panel.
Is menopausal hormone therapy safe for everyone with hot flushes?
No treatment is safe for everyone. Systemic menopausal hormone therapy is highly effective for vasomotor symptoms, but suitability depends on age, time since menopause, uterine status, unexplained bleeding, thromboembolic and cardiovascular history, liver disease, cancer history, medicines and informed preference. A person with a uterus needs adequate progestogen with systemic oestrogen. Route and dose are individualised, and complex histories often need specialist advice. Can pregnancy still occur during the menopausal transition?
Yes. Ovulation becomes less predictable during perimenopause but can still occur, so pregnancy remains possible until menopause is established. Menopausal hormone therapy does not provide contraception. A pregnancy test belongs in the assessment whenever pregnancy is possible and would change management, particularly with amenorrhoea, pain or bleeding. Contraceptive choice and the time at which contraception can stop depend on age, menstrual history and the method used, and should be discussed using current India-appropriate guidance.
When does bleeding around menopause need urgent assessment?
Any bleeding after 12 months of natural amenorrhoea requires assessment, even if it is light or happens once. Heavy or prolonged bleeding, postcoital bleeding, bleeding with pelvic pain, or bleeding after a hormone regimen had become stable also needs prompt review. Early unscheduled bleeding can occur after starting or changing systemic hormone therapy, but persistence beyond the expected adjustment window must not be normalised. Emergency assessment is needed when bleeding is accompanied by collapse, severe pain or possible pregnancy.
