Summary
Lymphogranuloma venereum (LGV) is an invasive sexually transmitted infection caused by Chlamydia trachomatis genovars L1, L2 or L3. It behaves differently from the more common non-LGV genital chlamydial infection caused by serovars D to K. After inoculation, a small transient genital, anal or oral papule or ulcer may pass unnoticed. Infection then spreads through lymphatics, producing tender unilateral inguinal or femoral lymphadenopathy and sometimes fluctuant buboes. Rectal exposure can instead produce severe proctocolitis with anorectal pain, tenesmus, constipation, mucoid or bloody discharge, ulceration and fever. This presentation can resemble inflammatory bowel disease and may occur in women or men; recent outbreaks have been concentrated among men who have sex with men, many living with HIV.
Untreated inflammatory disease can cause fistulas, strictures, chronic lymphatic obstruction, genital oedema and destructive scarring. A definitive diagnosis requires LGV-specific molecular genotyping, but this is not widely or rapidly available. In practice, clinicians combine a compatible syndrome and exposure history with a positive C. trachomatis NAAT from the symptomatic site, while excluding syphilis, herpes, chancroid, gonorrhoea, inflammatory bowel disease, malignancy and other causes. Severe compatible illness should be treated presumptively while results are pending because delay can extend tissue injury.
CDC recommends doxycycline 100 mg orally twice daily for 21 days for symptomatic LGV. This duration is supported chiefly by longstanding clinical practice and observational evidence rather than modern randomised trials. Buboes may need carefully performed aspiration or drainage in addition to antibiotics. Test for HIV, syphilis and gonorrhoea, evaluate partners, and follow clinically until symptoms resolve. Pregnancy, complex proctocolitis, fistula, stricture or diagnostic uncertainty requires specialist care. How Common Is It?
LGV is much less common than uncomplicated genital chlamydia, but its frequency is difficult to measure because routine chlamydia NAAT does not identify the L genovar and confirmatory typing is limited. Surveillance therefore captures a selected subset of cases, while unrecognised proctocolitis may be coded as ordinary rectal chlamydia, inflammatory bowel disease or nonspecific proctitis. Conversely, a rectal chlamydia-positive result does not mean every infection is LGV. Prevalence figures must state the population, anatomic site, symptoms and whether genotyping was performed.
Since the early 2000s, sustained outbreaks of rectal LGV have been reported in Europe and North America, especially among gay, bisexual and other men who have sex with men. High proportions of reported patients have HIV or another STI. These outbreak characteristics guide clinical suspicion but must not become a stereotype: women can acquire rectal LGV, heterosexual transmission can produce genital disease and lymphadenopathy, and oral exposure can rarely lead to oral ulceration with cervical nodes. Identity alone neither diagnoses nor excludes infection.
Reliable contemporary population-level Indian incidence data are sparse. NACO includes LGV within the national STI/RTI and genital-ulcer or inguinal-bubo framework, establishing its clinical relevance even where genotyping is unavailable. Published clinic series and laboratory reports cannot be treated as a national denominator. The practical Indian burden is also affected by care-seeking delay, stigma around anal symptoms, limited rectal sampling and empirical antibiotics before specialist assessment.
Because LGV is uncommon yet consequential, indiscriminate screening of all asymptomatic people is not justified by current evidence. The higher-value strategy is anatomical-site testing in patients with proctitis, genital ulcer plus nodes, or a compatible bubo syndrome; confirmatory typing where accessible; presumptive treatment when invasive disease is strongly suspected; and public-health follow-up. A small absolute case count does not make a missed case harmless when progressive inflammation can permanently damage tissue.
Risk Factors
The primary risk is sexual exposure at a genital, rectal or oral site to a partner carrying an LGV genovar. Condomless receptive anal intercourse, traumatic mucosal exposure, multiple or overlapping partners and sexual networks in which LGV is circulating increase probability. Sharing sex toys without cleaning or a new barrier can transfer secretions between sites. Group sex, anonymous partners or use of substances during sex can make partner notification and precise exposure recall difficult, but they should be discussed neutrally and only insofar as they change testing, prevention or safety.
Recent outbreaks have disproportionately affected men who have sex with men and people living with HIV. HIV does not require a different first-line regimen, but coinfection, delayed resolution and other simultaneous STIs increase care complexity. A history of rectal chlamydia, proctitis or an LGV contact raises suspicion. Travel or sexual contact within a known outbreak network matters more than nationality. Women and heterosexual men are sometimes missed when clinicians assume LGV is restricted to one population.
Risk of severe outcome is related to delay and site. Untreated proctocolitis can progress to strictures and fistulas. Suppurative lymph nodes can rupture and scar. Immunosuppression, extensive ulceration, secondary bacterial infection and an already formed fistula or bubo may prolong recovery. Prior incomplete short-course chlamydia treatment can reduce organism load without resolving established invasive inflammation, potentially obscuring the history.
A positive rectal chlamydia NAAT in an asymptomatic person does not automatically establish symptomatic LGV, and the optimal management of asymptomatic genotype-confirmed infection has evidence gaps. Similarly, swollen groin nodes after genital exposure have many infectious and malignant causes. Risk factors change pre-test probability; they are not diagnostic criteria. Clinicians should ask about anatomy and symptoms directly, offer self-collected specimens where validated, protect confidentiality and avoid letting stigma delay examination of anorectal disease.
Diagnosis
Consider LGV in three connected syndromes: a transient inoculation lesion, tender inguinal or femoral lymphadenopathy with or without a bubo, and invasive proctocolitis after rectal exposure. Definitive typing is ideal, but treatment decisions often precede it. Collect specimens before antibiotics when this does not delay necessary treatment.
History
Ask about a recent genital, anal or oral papule or ulcer that may already have healed; onset and laterality of groin swelling; pain, fever and drainage; and rectal pain, bleeding, mucoid discharge, tenesmus, constipation or altered bowel habit. Record weight loss, night sweats, abdominal symptoms, joint symptoms and systemic illness. Take a private site-specific sexual history, including receptive anal exposure, condom and sex-toy practices, recent partners, known chlamydia or LGV contact, travel and previous STI treatment. Ask about HIV status and PrEP, pregnancy possibility, allergies and antibiotics. Do not infer practices from sex, gender or identity.
Examination
Record observations and inspect oral, genital, perianal and inguinal regions with consent. Look for a small ulcer or scar, tender unilateral nodes, fluctuation, overlying inflammation, sinus formation and the classic groove between inguinal and femoral node groups, recognising that textbook signs are not always present. Examine the abdomen. For proctitis, perform external inspection and, when safe and consented, anoscopy or proctoscopy to assess discharge, bleeding, ulceration and mucosal inflammation. Defer traumatic instrumentation in severe pain or suspected perforation and seek specialist input.
Investigations
Obtain a C. trachomatis NAAT from the symptomatic site: rectal swab, lesion swab or bubo aspirate as appropriate and validated. Request LGV-specific PCR genotyping where available. Test rectal specimens for gonorrhoea and evaluate syphilis, herpes and other ulcer causes. Offer HIV testing and other STI testing. Bubo aspirate is preferred over indiscriminate incision when diagnostic material is needed. Full blood count and inflammatory markers support severity but are nonspecific. Imaging, endoscopy and biopsy may be required for deep abscess, fistula, stricture, inflammatory bowel disease, tuberculosis or malignancy. Chlamydia serology is not routinely diagnostic because methods and interpretation are insufficiently standardised.
Differential Diagnosis
Primary syphilis commonly causes a painless indurated chancre with regional nodes, but lesions can be atypical, multiple or painful and serology may be negative early. Genital herpes usually causes painful grouped vesicles or ulcers and tender nodes; PCR from a fresh lesion is more reliable than appearance. Chancroid can cause painful ragged ulcers and tender suppurative nodes where Haemophilus ducreyi circulates. Granuloma inguinale produces progressive beefy-red ulcers that bleed easily, typically without true lymph-node enlargement. Coinfection is possible, so a single classic adjective cannot safely choose treatment.
Inguinal lymphadenopathy also occurs with bacterial skin infection of the leg or genitalia, tuberculosis, cat-scratch disease, plague in the relevant epidemiological context, lymphoma and metastatic genital or skin cancer. Hidradenitis, an abscess or an incarcerated hernia may be mistaken for a bubo. Ultrasound can distinguish a node from a collection or hernia but cannot identify the chlamydial genovar. A draining sinus in India requires consideration of tuberculosis and other chronic infections rather than automatic repeat doxycycline.
The proctocolitis differential is broad. Non-LGV rectal chlamydia, gonorrhoea, herpes, syphilis, mpox, enteric bacterial infection and amoebiasis can cause pain, discharge or bleeding. Ulcerative colitis and Crohn disease may be clinically and endoscopically similar; inappropriate immunosuppression before excluding infection can worsen harm. Colorectal malignancy, radiation injury, ischaemic proctitis, traumatic injury and solitary rectal ulcer are additional possibilities.
Haemorrhoids and fissures cause bleeding or pain but do not explain fever, purulent discharge and invasive mucosal disease. Perianal fistula may be the complication rather than the original diagnosis. Reactive arthritis can follow chlamydial infection but does not establish LGV. The safe sequence is syndrome recognition, site-specific microbiology, exclusion of immediately dangerous and common alternatives, and genotyping when accessible—not reliance on the groove sign, patient identity or a generic urine chlamydia result.
Management
Start presumptive LGV treatment at the first visit when the clinical syndrome is strongly compatible: severe proctocolitis with bloody discharge, tenesmus or mucosal ulceration; severe inguinal or femoral lymphadenopathy with a bubo, especially after a genital ulcer; or an ulcer when other causes have been excluded and epidemiology supports LGV. Obtain diagnostic samples first where feasible, but do not wait for delayed genotyping while invasive inflammation progresses. Revise the diagnosis when results and clinical response do not fit.
Use the recommended extended doxycycline course after checking pregnancy, allergy, interactions and the patient's ability to adhere. Explain that the goal is microbiological cure and prevention of further tissue damage; established scarring may not reverse immediately. Provide analgesia and local wound care. A fluctuant bubo may require needle aspiration through intact skin or carefully selected incision and drainage to prevent spontaneous rupture; this is a trained procedure, not self-care. Proctocolitis with fistula, stricture, abscess, severe bleeding or inability to maintain hydration needs specialist or inpatient management.
Test for HIV, syphilis and gonorrhoea and manage any coinfection. Offer hepatitis assessment or vaccination and HIV PrEP referral according to exposure. Patients should avoid sexual contact until they and treated partners have completed therapy and symptoms have resolved. Partners with sexual contact during the 60 days before symptom onset should be evaluated, examined and tested at exposed sites; asymptomatic partners receive presumptive standard chlamydia treatment under the applicable local pathway.
Follow clinically until signs and symptoms resolve. Retest for chlamydia at about three months, or at the next visit within 12 months if three-month testing is not possible. Persistent nodes or rectal symptoms require reassessment for adherence, re-exposure, an unrecognised complication or alternative diagnosis. Partner notification must be confidential and adapted when violence, coercion or disclosure risk is present.
Prescribing Information
The CDC 2021 recommended regimen for symptomatic LGV is doxycycline 100 mg orally twice daily for 21 days. The 21-day duration is based on longstanding practice and observational results rather than trials that precisely define the shortest effective course. Shorter doxycycline courses have reported success in selected cohorts, but they are not the standard regimen for symptomatic invasive disease. Severe disease with buboes, fistulas or other tissue complications may need longer clinical management, source control and specialist review rather than an automatically extended online prescription.
Doxycycline should be swallowed with plenty of water while upright and not immediately before lying down, because oesophagitis and ulceration can occur. Nausea and photosensitivity are common practical concerns. Review tetracycline hypersensitivity, pregnancy, breastfeeding context, severe hepatic disease and interactions. Separate dosing from antacids and supplements containing aluminium, magnesium, calcium or iron according to current product instructions. Do not combine multiple chlamydia regimens without a defined reason.
CDC lists azithromycin 1 g orally once weekly for three weeks and erythromycin base 500 mg orally four times daily for 21 days as alternatives. The weekly azithromycin approach has not been validated to the same standard, so a test of cure four weeks after completion can be considered. Erythromycin causes frequent gastrointestinal intolerance. In pregnancy, doxycycline decisions require specialist assessment; CDC notes erythromycin can be used and that azithromycin may be useful, but acknowledges absent published evidence for an effective pregnancy dose and duration. Pregnant patients treated for LGV need test-of-cure planning.
These are US recommendations and must be reconciled with current NACO guidance, Indian product labelling and local specialist advice. The evidence base does not support inventing a regimen from ordinary seven-day chlamydia therapy. Persistent symptoms do not automatically mean antimicrobial resistance: adherence, absorption, partner exposure, deep complications, inflammatory lag and an incorrect diagnosis should be reviewed first.
When to Refer
Urgent referral is appropriate for severe proctocolitis with substantial bleeding, dehydration, peritonism, sepsis, urinary retention, suspected perforation or an evolving abscess. Colorectal or infectious-disease assessment is required when endoscopy, imaging or biopsy is needed to distinguish LGV from inflammatory bowel disease, malignancy, tuberculosis or another invasive infection. Do not start immunosuppression for presumed inflammatory bowel disease until an infectious proctitis assessment has been completed in a patient with relevant exposure.
A fluctuant, very painful or spontaneously draining inguinal bubo needs specialist procedural assessment. Aspiration through intact skin may reduce rupture risk and provide a diagnostic specimen; incision is selected case by case. Chronic sinus formation, genital or lower-limb lymphoedema, fistula, stricture, persistent constipation or obstructive symptoms requires surgical or specialist care because antibiotics cannot reverse every established structural complication.
Refer to an STI, dermatology/venereology or infectious-disease service when LGV genotyping is needed but unavailable locally, when a positive rectal chlamydia result conflicts with the clinical picture, when symptoms persist after a documented 21-day course without re-exposure, or when significant coinfection complicates treatment. Pregnancy and breastfeeding warrant specialist regimen selection and test-of-cure planning because alternative-treatment evidence is limited. Significant medicine allergy or intolerance also requires advice rather than abbreviated therapy.
Safeguarding and mental-health needs may require parallel referral after assault, coercion, trafficking, partner violence or severe stigma. In India, NACO-linked STI/RTI services can support testing, HIV and syphilis services and partner care, but molecular genotyping availability varies. A useful referral includes the chronology, anatomical exposures, examination findings, specimen sites, all microbiology, previous antibiotic names and dates, pregnancy status and complications. It should not simply state 'chlamydia treatment failure.'
Red Flags
Fever with tachycardia, hypotension, confusion, rapidly increasing groin swelling or spreading erythema suggests sepsis, secondary bacterial infection or a deep collection. Severe abdominal or rectal pain with guarding, persistent vomiting, inability to pass stool or gas, substantial bleeding or collapse requires emergency evaluation for obstruction, perforation, abscess or another acute abdomen. LGV proctocolitis is inflammatory, but haemodynamic compromise is never routine outpatient disease.
A bubo with threatened skin, spontaneous rupture, necrosis or extensive genital oedema needs prompt specialist care. Chronic draining sinuses, progressive leg or genital lymphoedema, faecal or urinary leakage, narrowing of stool calibre or obstructive constipation suggest fistula, stricture or lymphatic destruction. These complications may persist after organism eradication and require imaging and procedural planning. A hard fixed node, unexplained weight loss or night sweats broadens concern to tuberculosis or malignancy.
Neurological or ocular symptoms, widespread rash, severe headache, pregnancy complications or a destructive ulcer should prompt reassessment for syphilis, herpes and other coinfections. In a person starting doxycycline, facial swelling, breathing difficulty, blistering rash, severe headache with visual disturbance, jaundice or severe persistent vomiting requires urgent medicine review. Significant dysphagia or chest pain can represent pill oesophagitis.
Safeguarding red flags include sexual assault, coercion, trafficking, inability to speak privately and fear that partner notification will cause violence. Follow applicable Indian pathways while minimising repeated examinations. Diagnostic anchoring is itself dangerous: rectal bleeding in a person from a recognised risk network is not automatically LGV, and a normal urine chlamydia test does not exclude rectal disease. Likewise, temporary improvement with doxycycline does not prove the diagnosis because several bacterial infections may respond.
Indian Clinical Context
NACO's National Technical Guidelines on STI and RTI 2024 are the primary programme framework for India. In settings without molecular typing, LGV may enter through genital-ulcer disease or inguinal-bubo syndromic pathways. This enables same-visit care but has limited aetiological specificity: syphilis, herpes, chancroid, donovanosis, bacterial infection and tuberculosis may produce overlapping findings. Clinicians should follow the current national algorithm, collect available tests and reconsider the syndrome when the expected response does not occur.
Anal symptoms need to be asked about directly and respectfully. Rectal NAAT collection, anoscopy and LGV genotyping are not uniformly available outside specialist centres. A urine chlamydia result does not assess the rectum reliably; the specimen must match exposure and symptoms. Where genotyping cannot be obtained quickly, a compatible severe proctocolitis plus rectal chlamydia supports presumptive extended treatment while alternative causes are investigated. This is a clinical inference, not a typed laboratory diagnosis.
India also has a substantial differential for chronic lymphadenopathy and sinuses, including tuberculosis. Empirical repeat antibiotics without aspiration, imaging or biopsy can delay the correct diagnosis. NACO-linked Suraksha Clinics and tertiary dermatology, infectious-disease, gastroenterology or colorectal services can share care. HIV and syphilis testing, condoms, hepatitis prevention, partner services and PrEP discussion should be integrated rather than treated as separate moral judgements.
Use generic prescribing and verify the latest NACO regimen, local formulary, CDSCO-authorised information, pregnancy status and medicine interactions. Do not promise LGV resistance or genotyping facilities that are unavailable locally. Published outbreak data from Europe or North America inform recognition of proctocolitis but do not establish Indian prevalence. Indian services should strengthen anatomical-site testing and surveillance while protecting confidentiality for sexual and gender minorities and for anyone reporting anal exposure.
NMC Competency Mapping
The NMC Competency Based Medical Education Curriculum 2024 does not need an invented standalone LGV code for this guide. LGV is appropriately taught through the curriculum's genitourinary STI, genital-ulcer, microbiology, dermatology, medicine, surgery and communication outcomes. Learners should identify C. trachomatis L1 to L3 as invasive genovars, contrast them with D to K, and explain the progression from a transient inoculation lesion to lymphadenopathy or proctocolitis and late scarring.
At Know and Know How level, a student should distinguish genital-ulcer disease, inguinal-bubo syndrome and proctocolitis; take an anatomical-site sexual history; build a differential including syphilis, herpes, chancroid, donovanosis, rectal gonorrhoea, inflammatory bowel disease, tuberculosis and malignancy; and select specimens from the affected site. Management competence includes starting presumptive therapy when invasive disease is strongly suspected, using an extended rather than uncomplicated chlamydia regimen, identifying when a bubo needs aspiration, testing for coinfections, managing partners and recognising fistula, stricture, sepsis and obstruction. Pharmacology integration covers doxycycline administration, interactions, pregnancy limitations and adverse effects.
At Show How level, learners can practise consented genital and anorectal history-taking, explain self-collected rectal sampling where validated, counsel without assumptions about identity, formulate a referral and discuss partner notification safely. Anoscopy, bubo aspiration, endoscopy and prescribing require supervision and local credentialling. Institutions should map these outcomes to their current approved competency ledger; this clinically focused draft or confer independent practice competence.
Key Exam Pearls for NEET PG
LGV is caused by Chlamydia trachomatis serovars or genovars L1, L2 and L3; ordinary urogenital chlamydia is usually D to K, while A to C cause trachoma. The primary lesion is a small, often painless and transient papule or ulcer that may disappear before presentation. The classic secondary pattern is painful usually unilateral inguinal or femoral lymphadenopathy with a bubo. The groove sign describes separation of enlarged inguinal and femoral nodes by the inguinal ligament, but its absence does not exclude disease.

The duration rests mainly on clinical practice and observational evidence. Weekly azithromycin for three weeks is an incompletely validated alternative, and pregnancy treatment requires specialist judgement. Buboes may need aspiration or drainage; antibiotics alone may not reverse a fistula or stricture.
Test for HIV, syphilis and gonorrhoea, follow until symptoms resolve, and retest for chlamydia at about three months. Evaluate sexual contacts from the 60 days before symptom onset; asymptomatic contacts receive the standard chlamydia regimen under local guidance. How does lymphogranuloma venereum differ from ordinary genital chlamydia?

Why is doxycycline given for twenty-one days in symptomatic LGV?
LGV is an invasive tissue and lymphatic infection rather than uncomplicated surface chlamydia. The 21-day doxycycline regimen has longstanding clinical use and high observed cure, although the exact optimal duration has not been settled by modern randomised trials. Selected shorter-course studies do not justify routine abbreviation. Severe disease, buboes, fistulas or strictures need specialist follow-up and sometimes procedures; simply extending antibiotics cannot repair established structural damage.
Does every positive rectal chlamydia result mean the patient has LGV?
No. Non-LGV D to K strains can infect the rectum, often without severe symptoms. LGV becomes more likely with invasive proctocolitis, bloody discharge, tenesmus, ulceration, systemic symptoms or an epidemiological link, but genotyping provides definitive differentiation. Management may begin presumptively when the syndrome is severe because typing is often delayed or unavailable. An asymptomatic result should not be relabelled LGV solely on the basis of patient identity or rectal site.
