Summary
Hypertension is persistently raised arterial blood pressure and a leading modifiable risk factor for stroke, myocardial infarction, heart failure, chronic kidney disease and premature death. It is usually asymptomatic; headache, dizziness or anxiety do not diagnose it. Accurate measurement, confirmation outside the clinic when appropriate, cardiovascular-risk assessment and repeated review matter because a single stressed, painful or poorly measured clinic reading can misclassify a patient.
Current guidelines confirms hypertension when clinic blood pressure is at least 140/90 mmHg and daytime ambulatory or home average is at least 135/85 mmHg. Its pathway offers ambulatory monitoring for clinic readings from 140/90 to below 180/120, using home monitoring if ambulatory monitoring is unsuitable. These thresholds are a United Kingdom guideline framework, not an automatic Indian legal or programme definition. WHO's pharmacological guideline addresses appropriately diagnosed, non-pregnant adults and supports country-specific protocols.
Care combines tobacco avoidance, healthy diet, salt reduction, physical activity, weight management, sleep and alcohol assessment with medicines selected for age, pregnancy potential, kidney disease, diabetes, cardiovascular disease, electrolyte status and adverse effects. The aim is lower cardiovascular risk without orthostatic symptoms, kidney injury, dangerous electrolyte disturbance or inappropriate treatment in pregnancy. How Common Is It?
Hypertension is common worldwide and its burden rises with age, urbanisation, obesity, dietary sodium, alcohol, inactivity, diabetes and longer survival. WHO reported that the number of adults aged 30 to 79 years with hypertension increased from 650 million to 1.28 billion over three decades in a global analysis released in 2021. That estimate is useful for public-health scale, but it does not describe the prevalence, treatment rate or control rate in a particular Indian state, district or clinic.
Local prevalence depends on the age range, pregnancy exclusion, device validation, cuff size, number of visits, use of ambulatory or home confirmation and chosen threshold. Opportunistic screening can over-represent people who seek care; workplace surveys can miss people without employment; hospital data over-represent comorbidity. A single elevated reading is not prevalence and a pharmacy reading is not interchangeable with a standardised clinical or ambulatory average.
The important clinical implication is that most patients feel well until complications occur. Service measures should track adults screened with validated devices, confirmation after raised readings, treatment initiation, follow-up, blood-pressure control, medicine continuity and stroke or kidney outcomes. India has important public-health programmes and diverse care settings, but this guide does not assert a current national prevalence or treatment target without a current Indian primary source. Data must be dated and geographically specified, especially when used for procurement or programme claims.
Risk Factors
Most hypertension is primary and reflects combined genetic, environmental and life-course influences. Risk rises with age, family history, excess adiposity, high dietary sodium, low fruit and vegetable intake, physical inactivity, alcohol excess, smoking, poor sleep and diabetes. Obstructive sleep apnoea is common enough to ask about when there is snoring, witnessed apnoea, sleepiness or resistant hypertension. Stress can transiently raise blood pressure but should not be used to dismiss persistently elevated measurements.
Secondary hypertension should be considered with young onset, abrupt or severe worsening, resistant blood pressure, hypokalaemia, renal dysfunction, episodic headache-palpitations-sweating, endocrine features, renal bruits or medication triggers. Renal parenchymal disease, renal artery disease, primary aldosteronism, thyroid disease, Cushing syndrome, phaeochromocytoma, coarctation and obstructive sleep apnoea are examples. Pregnancy-related hypertension is a separate urgent pathway.
Review medicines and substances: non-steroidal anti-inflammatory drugs, oral contraceptives, corticosteroids, decongestants, stimulants, calcineurin inhibitors, erythropoietin, alcohol, cocaine and some herbal or traditional products can raise pressure or interfere with treatment. A family history does not prove primary hypertension and a normal body mass does not exclude it. Risk stratification also needs kidney disease, diabetes, dyslipidaemia, smoking, cardiovascular disease, target-organ damage and social barriers to follow-up, because the decision to start or intensify treatment is not based on a pressure number alone.
Diagnosis
History
Ask about previous readings, method and cuff size, home device validation, symptoms of target-organ damage, chest pain, breathlessness, neurological symptoms, visual change, claudication, sleep, diet, alcohol, tobacco, pregnancy possibility, medicines and adherence. Ask about renal, endocrine and cardiovascular history and family history of early cardiovascular disease. Most uncomplicated hypertension causes no symptoms; severe headache or dizziness should trigger assessment for emergency causes rather than reassurance from a prior diagnosis.
Examination
Measure blood pressure with a validated, maintained device, appropriate cuff and standardised seated setting, after checking for pulse irregularity. Measure both arms initially; Current guidelines advises repeating an inter-arm difference above 15 mmHg and then using the higher-reading arm if it persists. Check pulse, weight, waist context, cardiovascular examination, peripheral pulses, renal bruits, fundoscopy where trained, neurological examination and signs of endocrine disease. Measure lying or seated and standing pressure in people with falls, postural dizziness, diabetes or older age.
Investigations
For clinic blood pressure 140/90 to below 180/120 mmHg, current guidelines offers ambulatory monitoring; home monitoring is the alternative if ambulatory monitoring is unsuitable. Confirm with clinic at least 140/90 and daytime ambulatory or home average at least 135/85. Screen for target-organ damage and cardiovascular risk with urine albumin or haematuria assessment, creatinine or eGFR, electrolytes, HbA1c, lipids, ECG and eye assessment according to local practice. Test for secondary causes selectively; an indiscriminate endocrine panel creates false positives and delay.
Differential Diagnosis
First exclude measurement error and temporary elevation. Wrong cuff size, unsupported arm, talking, recent caffeine, nicotine, exercise, pain, full bladder, anxiety and an unvalidated device can raise readings. White-coat hypertension has higher clinic than out-of-clinic pressure; masked hypertension has lower clinic than home or ambulatory pressure. Neither is settled by repeating a rushed measurement. Ambulatory or structured home data clarify the pattern.
Differentiate primary hypertension from secondary causes when the presentation is atypical. Kidney disease may be a cause and consequence; proteinuria, haematuria, rapidly reduced eGFR or renal asymmetry prompt renal evaluation. Hypokalaemia can suggest primary aldosteronism but normal potassium does not exclude it. Episodic adrenergic symptoms, endocrine stigmata, resistant hypertension or abrupt onset require targeted specialist assessment. Pregnancy, postpartum hypertension and pre-eclampsia are outside ordinary adult algorithms and require obstetric pathways.
Hypertensive emergency means severe blood pressure with acute target-organ injury, not a number alone. Acute pulmonary oedema, myocardial ischaemia, aortic syndrome, encephalopathy, stroke symptoms, retinal haemorrhage or papilloedema, acute kidney injury and pregnancy complications change urgency. Panic, pain or isolated headache may coexist with high pressure but should not be assumed to be the cause. Conversely, an asymptomatic high reading still needs same-day assessment at extreme levels and structured follow-up if acute injury is excluded.
Management
Agree a plan that combines accurate diagnosis, cardiovascular-risk reduction and achievable follow-up. Advise reduced dietary sodium, a varied diet rich in vegetables and fruit, regular physical activity, weight reduction when appropriate, smoking cessation, moderated alcohol, sleep assessment and avoidance of pressor substances. Advice should be culturally and financially realistic: a generic instruction to buy special food or a home monitor can widen inequity. Use a written action plan in the patient's language and address medicine cost and refill continuity.
WHO recommends pharmacological treatment in appropriately diagnosed adults based on blood pressure and cardiovascular-risk context, using one of three classes as initial therapy: thiazide or thiazide-like diuretic, ACE inhibitor or ARB, or long-acting dihydropyridine calcium-channel blocker. Current guidelines uses an age, ethnicity and clinical-context stepwise pathway and has detailed sequences for combinations. These approaches are not contradictory; local protocol, comorbidity, pregnancy potential and individual tolerability determine selection. Never combine an ACE inhibitor with an ARB routinely.
Review after starting or changing therapy, checking blood pressure, symptoms, adherence, creatinine or eGFR and electrolytes according to drug class. Use home monitoring only with validated device training and a clinician-agreed response plan. Treat lipids, diabetes, atrial fibrillation, kidney disease and smoking in parallel. Resistant hypertension demands confirmation of adherence, technique, white-coat effect, interacting medicines and secondary causes before adding drugs. Prescribing Information
ACE inhibitors and ARBs can cause hyperkalaemia, creatinine rise, hypotension and rarely angioedema; they are contraindicated in pregnancy and should be avoided in people planning pregnancy unless the specialist determines otherwise. Check renal function and potassium before and after initiation or dose change, and review dehydration, diarrhoea, NSAIDs, potassium supplements and salt substitutes. Do not use ACE inhibitor and ARB together routinely because combined renin-angiotensin blockade increases harm without routine benefit.
Thiazide or thiazide-like diuretics can cause hyponatraemia, hypokalaemia, gout and glucose changes; their effectiveness and safety depend on renal function and the specific agent. Dihydropyridine calcium-channel blockers can cause ankle oedema, flushing, headache and gingival enlargement. Beta-blockers are not a universal first-line choice for uncomplicated hypertension but are appropriate in selected indications such as angina, arrhythmia, previous myocardial infarction or heart failure; avoid abrupt withdrawal. Alpha-blockers, centrally acting agents and mineralocorticoid receptor antagonists have specialised roles and need individual monitoring.
Pregnancy and breastfeeding need obstetric prescribing, not extrapolation from general adult guidance. Frailty, orthostatic symptoms, older age and multimorbidity may require standing-pressure targets and slower titration. In India, verify current CDSCO labels, fixed-dose combinations, supply and laboratory access; do not assume an imported dose or step sequence is authorised or affordable. Give the patient a medicine list, expected adverse effects, sick-day advice where locally adopted, and a date for blood tests and review. Prescribing is successful only when monitoring is feasible.
When to Refer
Refer immediately to emergency care for suspected hypertensive emergency: severe pressure with chest pain, acute breathlessness, neurological deficit, confusion, seizures, acute visual loss, papilloedema, retinal haemorrhage, aortic pain, acute kidney injury or pregnancy-related symptoms. A clinic blood pressure of 180/120 mmHg or higher needs same-day assessment for target-organ damage under current guidelines guidance; emergency transfer depends on symptoms and signs. Do not lower pressure rapidly in an outpatient setting without a diagnosis and monitored plan.
Refer to a hypertension, renal, cardiology or endocrine service for resistant hypertension after adherence and measurement have been checked, suspected secondary hypertension, chronic kidney disease with complex medication decisions, recurrent electrolyte disturbance, young-onset disease, suspected primary aldosteronism, renal artery disease, phaeochromocytoma or coarctation. Pregnancy, planning pregnancy or postpartum hypertension needs obstetric or physician review. Consider ophthalmology for acute visual symptoms and neurology or stroke services for focal deficits.
In India, ambulatory monitoring, renal imaging, renin and aldosterone assays, sleep studies and specialist clinics may be unevenly available. Referral should identify the nearest feasible public or private service and should not postpone basic ECG, renal tests, urine assessment or emergency transfer. Document readings, device, cuff size, medicines, pregnancy status, symptoms, kidney function and reason for referral. Explain access limits honestly rather than promising a particular test, procedure or branded fixed-dose combination.
Red Flags
Severe blood pressure with acute target-organ injury is an emergency. Call for urgent assessment with chest pain, tearing back pain, severe breathlessness, pulmonary oedema, new weakness or speech disturbance, altered consciousness, seizure, confusion, acute visual loss, papilloedema, retinal haemorrhage, reduced urine or pregnancy-related headache, visual symptoms, epigastric pain or swelling. The blood-pressure number supports but does not replace clinical assessment. A person with 190/120 mmHg and stroke symptoms is different from an asymptomatic person with the same reading.
New hypertension in pregnancy or postpartum is a red flag because pre-eclampsia and related complications require obstetric assessment. In children and adolescents, use paediatric pathways; adult thresholds should not be copied. Sudden onset or rapidly worsening hypertension with hypokalaemia, renal impairment, systemic symptoms, episodic adrenergic attacks or renal bruit warrants a secondary-cause evaluation. Repeated high readings despite multiple drugs may reflect poor adherence, inappropriate measurement, drug interactions or a secondary disorder, each requiring respectful investigation.
Treatment red flags include syncope, falls, severe postural dizziness, bradycardia, facial swelling, severe rash, muscle weakness, palpitations or confusion after medicine changes. These can reflect hypotension, angioedema or electrolyte disturbance. Patients should not stop all medicines independently, but need prompt advice. Give clear instructions on where to go, how to bring their medication packets and when to repeat a reading; do not encourage compulsive repeated measurements that amplify anxiety and obscure a clinical emergency.
Indian Clinical Context
international and WHO guidance are useful clinical sources but do not define Indian diagnosis, public-programme targets, essential-medicine lists, device procurement or legal prescribing. Confirm current national, state, institutional and CDSCO guidance before adopting a threshold, combination or product. India has major variation in validated-device access, ambulatory monitoring, laboratory monitoring, medicine continuity and referral capacity. A trustworthy plan records the method of measurement and uses the resources the patient can genuinely access.
Community and primary-care care may rely on manual or automated measurement, opportunistic screening and fixed-dose combinations. The priorities are trained measurement, appropriate cuff size, validated or maintained device, repeat confirmation, medicine reconciliation and affordable follow-up. Salt advice should fit local cooking and packaged-food patterns; it should not ignore food insecurity. Ask about Ayurvedic, herbal and over-the-counter products non-judgementally because they may contain stimulants, corticosteroids or sodium.
Evidence limits must be explicit. WHO's 2021 adult guideline excludes pregnancy and depends on country implementation; current guidelines' ambulatory thresholds and age-based sequence come from a UK health-system context. Neither proves a unique best pathway for every Indian district. Frequently Asked Questions
Can headache or stress alone diagnose hypertension?
No. Hypertension is often symptom-free, and headache, stress or dizziness are non-specific. Diagnosis requires properly measured repeated blood pressure, usually confirmed by ambulatory or structured home monitoring when the reading is not severely elevated. Severe headache with neurological symptoms, visual change, chest pain or breathlessness requires urgent assessment for target-organ injury rather than a casual blood-pressure check.
Why measure blood pressure outside the clinic?
Ambulatory or home monitoring helps distinguish persistent hypertension from white-coat elevation and detects masked hypertension. Current guidelines uses a lower out-of-clinic confirmation threshold than the clinic threshold. Home monitoring only helps when the device, cuff and technique are appropriate and readings are averaged according to a structured plan. A single home number should not trigger unsupervised medicine changes.
Are ACE inhibitors safe during pregnancy or when planning pregnancy?
No, they are generally contraindicated in pregnancy and require a pre-pregnancy or urgent obstetric medication review. ARBs have similar concerns. A person who may become pregnant needs counselling before starting treatment and a locally appropriate alternative if needed. Do not stop a prescribed medicine without contacting the clinician, because severe uncontrolled hypertension also carries risk.
What does resistant hypertension mean in practice?
It means persistently uncontrolled pressure despite an appropriate multi-drug regimen, but first check measurement technique, cuff size, adherence, white-coat effect, excess sodium, alcohol, NSAIDs and other pressor agents. Then assess secondary causes and refer when indicated. Escalating drugs without this review can cause harm and may miss renal or endocrine disease.
