Summary
Hepatitis A is an acute liver infection caused by hepatitis A virus (HAV). It is transmitted mainly by the faecal-oral route: an uninfected person ingests food or water contaminated with virus, or acquires infection through close contact, including sexual contact involving oral-anal exposure. The illness ranges from asymptomatic infection to fever, anorexia, nausea, abdominal discomfort, dark urine and jaundice. Young children are often minimally symptomatic; adults more often have recognisable illness and severe outcomes are more likely with increasing age or underlying liver disease.
HAV does not establish chronic hepatitis, cirrhosis or hepatocellular carcinoma. Most patients recover completely and develop lasting immunity, but acute liver failure is a rare, life-threatening complication. Cholestatic, prolonged or relapsing courses can occur and must be distinguished from a second diagnosis or ongoing toxin exposure. A patient with jaundice therefore needs assessment for severity and for alternative infectious, drug-induced, obstructive, autoimmune, ischaemic and pregnancy-related causes, not an assumption that every outbreak-associated illness is HAV.
Specific antiviral therapy is not available. Care is supportive, with attention to hydration, nutrition, vomiting, pruritus, safe use of medicines and timely referral for coagulopathy or encephalopathy. Case recognition also starts public-health action: identify contacts, assess exposure settings, notify through the applicable local system and arrange time-sensitive post-exposure prophylaxis where indicated. This quarantined educational guide cannot replace local laboratory, vaccine, immunoglobulin or outbreak-control policy.
How Common Is It?
HAV occurs worldwide, both as sporadic disease and outbreaks. Epidemiology follows sanitation, access to safe water, crowding, food handling, population immunity and travel patterns. In high-endemicity settings, many infections occur in childhood and may be asymptomatic; as childhood exposure falls, a larger susceptible adolescent and adult population can experience more clinically apparent disease. A low number of reported jaundice cases can therefore mean low transmission, under-detection, or early-age infection rather than safety.
WHO reports that hepatitis A can cause explosive food- or water-associated outbreaks and prolonged person-to-person transmission. Its 2026 fact sheet estimated 35,569 deaths globally in 2023, but that is a modelled global estimate, not an Indian annual count. India-wide incidence varies by state, season, testing availability, outbreak reporting and whether clinicians test for both HAV and HEV. Hospital series should not be treated as community incidence because they select patients with jaundice or complications.
The useful local measures are laboratory-confirmed cases, age distribution, onset dates, shared water or food exposures, school or hostel clusters, travel history and household contacts. Denominators matter: a positive anti-HAV IgM proportion among jaundiced admissions is not population prevalence. Surveillance can miss mild childhood infections and cases managed outside hospitals. For students, the key epidemiological lesson is the relationship between sanitation, age at infection and clinical severity, while avoiding unsupported claims that any particular Indian district is uniformly high or low endemicity.
Risk Factors
Risk follows exposure and lack of immunity. Important exposures include unsafe drinking water, inadequate sewage disposal, food handled by an infectious person, household contact, shared childcare settings, travel to an area with substantial transmission, homelessness or congregate living in an outbreak, and oral-anal sexual contact. People who inject or use drugs can be affected in person-to-person outbreaks, but risk communication must describe practices and access barriers rather than stigmatise an identity. Casual contact without faecal-oral exposure is much less relevant than shared food preparation or poor hand hygiene.
Severe illness is more likely in older adults and in people with chronic liver disease; chronic hepatitis B or C, cirrhosis and other liver conditions therefore change urgency. Immunocompromised people may have altered vaccine or post-exposure needs. Pregnancy does not make every hepatitis A illness severe, but vomiting, dehydration, coagulopathy or diagnostic uncertainty require obstetric and medical assessment. Infants and young children may transmit infection with little or no jaundice, which is important when investigating a household or school cluster.
A medication history is essential because paracetamol toxicity, herbal and traditional products, antitubercular therapy, antiepileptics, antibiotics and other medicines can cause acute hepatitis or complicate a viral illness. Alcohol and fasting can worsen nutritional risk but do not prove HAV causation. Prior documented vaccination or prior infection changes susceptibility; a vague memory of childhood jaundice does not reliably establish immunity. Assess vaccine records, date and type of exposure, co-travellers, food and water source, sexual contacts where appropriate, and concurrent hepatitis risks before offering a preventive product.
Diagnosis
History
Establish onset of fever, malaise, anorexia, nausea, vomiting, right-upper-quadrant discomfort, dark urine, pale stool, pruritus and jaundice. Ask about water and food exposure, travel, household or school illness, food handling, contact with a confirmed case, sexual exposure, drug use, vaccination, pregnancy, chronic liver disease, alcohol and all prescribed, over-the-counter, herbal and traditional medicines. Record when symptoms began because contact tracing and post-exposure prophylaxis depend on exposure timing, not merely the laboratory result.
Examination
Assess hydration, mental state, vital signs, jaundice, hepatomegaly, right-upper-quadrant tenderness, bruising or bleeding, rash, lymph nodes and signs of chronic liver disease. Look for encephalopathy, asterixis, sepsis, hypotension, severe abdominal pain or an enlarged tender gallbladder, which may indicate acute liver failure, alternative diagnosis or biliary obstruction. In pregnant patients assess obstetric symptoms and fetal concerns with the relevant service. A normal examination does not exclude acute hepatitis, especially before jaundice develops.
Investigations
Acute HAV is specifically diagnosed with serum anti-HAV IgM; total anti-HAV or IgG alone indicates past infection or vaccine-associated immunity and does not prove current illness. WHO notes that RT-PCR for HAV RNA can be useful early or for specialised investigation, but it is not required for routine diagnosis. Obtain bilirubin, alanine and aspartate aminotransferases, alkaline phosphatase, albumin, glucose, full blood count, renal function and prothrombin time or INR to assess severity and alternatives. Test for other acute viral hepatitis, pregnancy-related disease, sepsis, drug injury or obstruction according to clinical context; ultrasound is targeted when biliary disease is plausible.
Differential Diagnosis
Acute viral hepatitis from HEV, HBV, HCV or less commonly other infections can resemble HAV. In India, HEV is a particularly important alternative in acute jaundice and pregnancy; it cannot be distinguished from HAV by symptoms alone. Acute HBV may include arthralgia or rash, while chronic HBV or HCV can coexist with an acute HAV infection and increase concern for severe disease. Serology must be interpreted with the history, timing and local laboratory quality rather than ordered as an unexplained panel.
Drug-induced liver injury is a central differential. Ask specifically about overdose or duplicate paracetamol products, antitubercular drugs, antibiotics, antiepileptics, supplements, herbal preparations and recreational substances. Ischaemic hepatitis, sepsis, leptospirosis, malaria and dengue may be relevant depending on fever pattern, travel, shock and local epidemiology. Autoimmune hepatitis and Wilson disease are considered in selected patients. A very high aminotransferase concentration is not by itself proof of viral hepatitis.
Obstructive jaundice from gallstones, cholangitis, pancreatic or biliary disease often has pain, fever, cholestatic tests or duct dilatation, but overlap exists. Haemolysis causes unconjugated hyperbilirubinaemia without the same hepatic injury pattern. Acute fatty liver of pregnancy, pre-eclampsia with liver involvement and HELLP syndrome require urgent obstetric differentiation. Fulminant Wilson disease, acute liver failure from toxins and severe viral hepatitis share coagulopathy and encephalopathy; emergency referral should never wait for a final serological label.
Management
Most confirmed, uncomplicated hepatitis A is managed with rest as needed, oral fluids, small tolerated meals and treatment of nausea or pruritus under clinician guidance. Hospitalisation is unnecessary solely because jaundice is present, but patients need safety-netting for dehydration, persistent vomiting, bleeding, confusion, reduced urine, severe pain or worsening jaundice. There is no antiviral that shortens routine illness. WHO advises supportive care with hydration and nutrition and cautions against unnecessary medicines that may adversely affect the liver.
Review the medication list immediately. Avoid alcohol and unnecessary hepatotoxic medicines; never use the diagnosis as a reason to stop essential treatment without discussing it with the responsible clinician. Paracetamol may be dangerous in overdose or liver injury, so total daily exposure from all products must be checked rather than offering casual reassurance. Give clear advice on food preparation, handwashing after toilet use and before preparing food, avoiding work in food handling or other high-risk settings according to public-health direction, and avoiding oral-anal sexual exposure while infectious.
Assess contacts promptly. Time-sensitive post-exposure prophylaxis may prevent illness when administered within two weeks of exposure, but eligibility depends on age, immune status, chronic liver disease, prior vaccination and product availability. Report and investigate suspected clusters through local public-health channels so that water, food, sanitation and contact measures are evidence-led. Follow clinically until symptoms and biochemical abnormality resolve, and reassess a prolonged or relapsing course for cholestasis, alternate diagnosis, drug injury or complications rather than repeatedly prescribing symptomatic medicines.
Prescribing Information
There is no routine HAV antiviral prescription. Prescribing is mainly supportive and must account for vomiting, dehydration, pregnancy, renal function and liver injury. Do not prescribe or recommend a fixed paracetamol dose without reconciling all products and assessing hepatic risk; WHO specifically warns against unnecessary medicines that may adversely affect the liver. Antiemetics, antipruritics, intravenous fluids or admission are chosen according to severity and local protocols, not because an online guide lists them. Corticosteroids, antibiotics and liver tonics have no routine role in uncomplicated HAV.
Hepatitis A vaccination and immune globulin are preventive biologics, not treatment for established symptomatic infection. CDC recommends a single-antigen hepatitis A vaccine as soon as possible within two weeks for unvaccinated exposed people aged 12 months or older, with immune globulin added for certain age and health-risk groups. Its product-specific immune-globulin dose and United States schedule are not an Indian prescribing instruction. Verify the exact vaccine formulation, age indication, dose, interval, contraindication, cold chain and availability in the Indian product information and local public-health protocol.
Immunocompromised patients and people with chronic liver disease exposed within the preventive window need urgent specialist or public-health advice because combined vaccine and immune globulin may be indicated in CDC guidance. Do not use a combined hepatitis A/B product as post-exposure prophylaxis where evidence or local guidance does not support it. Record exposure date, vaccine history, lot details and site. A patient with acute liver failure needs referral, not an outpatient 'hepatitis prescription'.
When to Refer
Refer or discuss urgently if there is confusion, sleep reversal, asterixis, rising INR or prothrombin time, hypoglycaemia, rapidly worsening jaundice, bleeding, severe persistent vomiting, dehydration, renal injury, sepsis, severe abdominal pain, hypotension or concern for acute liver failure. These patients require hospital monitoring and early referral to a centre with gastroenterology, critical-care and transplant capability where feasible. Acute liver failure is defined clinically by coagulopathy and encephalopathy in a person without known cirrhosis; waiting for bilirubin to become extreme is unsafe.
Seek specialist input for pregnancy, chronic liver disease, immunosuppression, diagnostic uncertainty, suspected drug-induced injury, biliary obstruction, prolonged cholestasis, relapse, significant pruritus, recurrent admission or inability to maintain nutrition. Public-health referral is also clinical care: notify the appropriate district surveillance or health authority of confirmed or suspected outbreak-associated cases, identify food handlers and contacts through official processes, and arrange access to prevention. Do not personally promise exclusion periods or mass vaccination rules without local authority guidance.
In India, testing, immunoglobulin, isolation facilities and transplant referral are unevenly available. The plan should identify the nearest emergency hospital, testing laboratory, public-health contact and referral centre before deterioration. Family members may face cost, transport and language barriers; give written return precautions and a realistic follow-up plan. A district outbreak team decides source investigation and community interventions. Individual clinicians contribute accurate onset, exposure and laboratory data, while avoiding disclosure of sensitive sexual, substance-use or household information beyond what public-health law and patient safety require.
Red Flags
Acute liver failure is the critical red flag: new confusion, drowsiness, personality change, asterixis, disorientation, seizures or coma with jaundice or acute liver injury requires emergency admission. Bleeding, easy bruising, severe vomiting, hypoglycaemia, reduced urine, hypotension, marked dehydration, severe weakness or rapidly progressive jaundice similarly need urgent assessment. Children can deteriorate with dehydration even without dramatic jaundice. A patient with chronic liver disease, older age or immunosuppression merits a lower threshold for review.
Do not attribute severe right-upper-quadrant pain, high fever with rigors, persistent hypotension, a palpable gallbladder, peritonism or disproportionate cholestasis automatically to HAV. Cholangitis, gallstone obstruction, hepatic abscess, sepsis, pancreatitis and surgical disease may require imaging, antibiotics or intervention. Pregnancy with jaundice, headache, hypertension, abdominal pain, bleeding or reduced fetal movement needs urgent obstetric assessment because HEV and pregnancy-specific liver disease are important alternatives.
Medication and public-health red flags include suspected paracetamol overdose, new herbal product exposure, alcohol withdrawal, unlabelled medicine use, a food-handler cluster, contaminated shared water, cases in a school or hostel, or a vulnerable contact within the two-week prevention window. Report promptly through local systems. Stigma is also a safety issue: fear of job loss or discrimination may conceal illness and exposures. Explain that HAV spreads through faecal-oral exposure, not through ordinary social proximity alone, while supporting hygienic precautions and confidential contact assessment.
Indian Clinical Context
India has heterogeneous water, sanitation, food-safety and laboratory infrastructure. HAV and HEV may both circulate, and a positive local outbreak report does not prove that an individual has HAV without appropriate testing. The National Centre for Disease Control's viral-hepatitis guidance lists contaminated food and water, close contact, sanitation, vaccination and immunoglobulin among HAV prevention measures, and highlights surveillance of acute hepatitis A and contacts. Its older document should be read with current state and district directions rather than treated as a complete current vaccine policy.
Availability of anti-HAV IgM, PCR, ultrasound, INR testing, vaccines, immune globulin and liver-failure referral differs across settings. A clinician should arrange the tests and escalation that are realistically accessible, then document uncertainty and transfer criteria. Public-health teams, water and sanitation departments, food-safety authorities, schools and employers have distinct roles; medical advice alone cannot correct a contaminated water source or food-production breach. Do not advise boiling as a universal answer without considering water safety, storage and the actual outbreak source.
India-specific claims need restraint. National and state immunisation schedules, private vaccine availability, product brands and outbreak response protocols change; verify current MoHFW, state health and local product information before prescribing or promising post-exposure prophylaxis. Patient counselling should be language-concordant and non-stigmatising. NMC training supports acute-jaundice assessment, rational prescribing, hygiene counselling, notification and referral, but institutions must map the exact competency code locally. Frequently Asked Questions
Does a positive anti-HAV IgG test prove current hepatitis A?
No. Acute hepatitis A is identified by anti-HAV IgM in the appropriate clinical setting. Total anti-HAV or IgG alone usually reflects past infection or vaccination and does not establish that today’s jaundice is due to HAV. A patient with jaundice still needs severity assessment and testing for alternatives such as HEV, drug-induced liver injury or obstruction when clinically indicated.
Can hepatitis A become chronic or cause cirrhosis years later?
HAV does not cause chronic infection, cirrhosis or hepatocellular carcinoma. Most people recover fully and acquire lasting immunity. However, an acute illness can be prolonged or relapse, and rare acute liver failure can be fatal. People with pre-existing chronic liver disease need a lower threshold for medical review because an acute HAV infection can be more dangerous in that setting.
Should every close contact receive vaccine or immune globulin immediately?
Contacts need prompt public-health or clinician assessment of the exposure date, prior immunity, age, pregnancy, immune status, chronic liver disease and locally available products. CDC guidance uses a two-week window for post-exposure prevention, but Indian vaccine schedules and immunoglobulin access must be verified locally. Do not delay urgent evaluation of a sick contact while waiting to resolve every administrative detail.
Is jaundice alone a reason for hospital admission with hepatitis A?
Not always. Many uncomplicated cases are managed with fluids, nutrition, medication review and reliable follow-up. Admission or urgent assessment is required for confusion, bleeding, dehydration, persistent vomiting, reduced urine, hypoglycaemia, coagulopathy, severe pain, sepsis, chronic liver disease or concern for acute liver failure. The decision depends on severity and access to safe monitoring, not bilirubin colour alone.
