Summary
Gout is an inflammatory arthritis caused by deposition of monosodium urate crystals in joints and other tissues after sustained hyperuricaemia. A typical flare begins rapidly, often overnight, with severe pain, swelling, heat and erythema; the first metatarsophalangeal joint is classic, but ankles, knees, wrists, fingers and elbows are also affected. Recurrent untreated disease can produce tophi, chronic destructive arthritis and substantial disability. Serum urate supports assessment but does not by itself diagnose gout, and it can fall during an acute flare. When the presentation is uncertain, synovial-fluid demonstration of needle-shaped, strongly negatively birefringent crystals provides definitive confirmation.
The immediate safety task in every acutely hot joint is to consider septic arthritis. Infection and crystals can coexist, so finding urate crystals does not justify discarding culture when sepsis is plausible. Acute treatment uses an NSAID, colchicine or a corticosteroid selected around kidney function, cardiovascular and gastrointestinal risk, diabetes, infection probability and interacting medicines. Long-term care is separate: indicated urate-lowering therapy is started low, titrated using serial serum urate, and usually continued lifelong.
A treat-to-target strategy aims below 6 mg/dL, with a lower target considered for tophaceous or severe disease. Anti-inflammatory prophylaxis reduces mobilisation flares while urate lowering is initiated and titrated. Weight, alcohol and diet should be discussed without blame, but lifestyle change alone rarely dissolves a clinically important crystal burden and no specific diet has strong evidence to prevent flares. How Common Is It?
Gout is common worldwide and becomes more frequent with age, chronic kidney disease, cardiometabolic disease and exposure to certain medicines. Reliable contemporary prevalence estimates vary because studies use different definitions: self-report, a diagnostic code, serum urate, clinician diagnosis or crystal confirmation. Hyperuricaemia is more common than clinical gout and must not be counted as equivalent disease. India lacks a single national, population-representative crystal-confirmed registry from which one current prevalence can be quoted. Urban hospital cohorts, insurance databases and regional surveys are informative for their own populations but should not be presented as the rate for the whole country.
Men are affected more often through much of adult life; risk in women rises after menopause. The apparent burden is also shaped by survival with kidney and cardiovascular disease, changing dietary environments, obesity, alcohol exposure, diuretic use and access to diagnosis. Gout can occur in younger adults, where strong family history, renal disease, transplant medicines, enzymatic disorders or unusually high urate warrant closer investigation. Children with suspected gout need specialist assessment because routine primary gout is uncommon.
Service data underestimate patients who self-treat repeated flares with over-the-counter analgesics or receive episodic steroids without a long-term plan. Conversely, labelling every high urate result as gout overestimates disease and exposes people to unnecessary lifelong treatment. A useful clinical audit records diagnostic basis, comorbidities, flare frequency, tophi, kidney stones, serum-urate trajectory, urate-lowering dose, prophylaxis and adverse effects. Epidemiological statements should always specify population, year, test and case definition, and should not imply that ethnicity or a dietary stereotype determines an individual's diagnosis.
Risk Factors
Sustained serum urate above its saturation threshold permits crystal formation, but the relationship between a measured value and a flare is not linear. Important risk factors include chronic kidney disease, reduced renal urate excretion, older age, male sex before menopause, family history, obesity, hypertension, insulin resistance, dyslipidaemia and cardiovascular disease. Dehydration, acute illness, surgery or rapid changes in urate can precipitate a flare in someone with an existing crystal burden. A past flare, tophi, chronic gouty arthritis and uric-acid nephrolithiasis signal established disease rather than merely theoretical risk.
Medicines matter. Thiazide and loop diuretics, ciclosporin, tacrolimus and some cancer therapies can raise urate or trigger flares. Low-dose aspirin may modestly affect urate, but cardioprotective aspirin should not be stopped casually. Losartan has a uricosuric effect and some calcium-channel blockers may be urate-neutral or favourable, yet antihypertensive changes require an overall cardiovascular and renal decision. Rapid tumour lysis causes a separate emergency and is not managed as ordinary gout. Review over-the-counter products, supplements and recent antibiotic or antifungal use because these may interact with flare medicines.
Alcohol, especially heavy consumption and beer or spirits, sugar-sweetened drinks, excess energy intake and some high-purine dietary patterns are associated with gout, while the effect size for any single food is modest compared with genetic and renal determinants. Advice must avoid blaming cultural food choices. Crash dieting and fasting may precipitate attacks. A family history at young age or very high urate suggests inherited transport variants or rare metabolic disorders; broad genetic testing is not routine. Risk factors help decide prevention and comorbidity care, but neither obesity nor a high urate value proves that an acutely swollen joint is gout.
Diagnosis
History
Establish onset, speed to peak pain, joint distribution, previous similar episodes and complete resolution between attacks. Ask about fever, rigors, wounds, recent procedures, immunosuppression and antibiotics because infection is the dangerous mimic. Record tophi, kidney stones, renal function, psoriasis, transplant status, alcohol, fasting or dehydration, family history and precipitating illness. Review diuretics, aspirin, ciclosporin, tacrolimus, urate-lowering therapy and flare medicines. Clarify cardiovascular, gastrointestinal, diabetic and anticoagulant risks before treatment.
Examination
Assess vital signs and systemic appearance before the joint. Document swelling, warmth, erythema, exquisite tenderness, effusion and active and passive movement. Inspect the first MTP joints, ankles, knees, hands, wrists and elbows, and look for tophi at the pinna, olecranon, fingers, toes and tendons. Examine skin for cellulitis or entry wounds and look for another infection source. Multiple joints, fever or immune compromise does not rule out gout, but should lower the threshold for cultures and urgent assessment.
Investigations
If diagnosis is uncertain, infection is possible or the pattern is atypical, aspirate synovial fluid for polarised microscopy, Gram stain and bacterial culture; send cell count where useful. Monosodium urate crystals are needle-shaped and strongly negatively birefringent. Crystals and high leukocyte counts do not exclude simultaneous sepsis. Measure serum urate, remembering that a value below 6 mg/dL during a flare does not exclude gout; repeat at least two weeks after resolution when suspicion remains. Check blood count, CRP, creatinine/eGFR, liver tests and metabolic comorbidity as clinically indicated. Plain radiographs show late erosive disease, not early confirmation. Ultrasound double-contour sign or tophi and dual-energy CT can support diagnosis when aspiration is unavailable or inconclusive, but artefact and access limit interpretation.
Differential Diagnosis
Septic arthritis is the first exclusion in an acutely hot, swollen joint. Fever may be absent, particularly in older or immunosuppressed patients, and gout can coexist with bacterial infection. A previous gout diagnosis, serum urate elevation, response to colchicine or visible tophi does not make culture unnecessary when clinical suspicion exists. Aspirate before antibiotics if the patient is stable; if sepsis or shock is present, cultures must not delay resuscitation and empiric antimicrobial therapy. Prosthetic-joint infection belongs in an implant-specific pathway.
Calcium pyrophosphate deposition disease often affects the knee or wrist and shows rhomboid, weakly positively birefringent crystals. Basic calcium phosphate disease, palindromic rheumatism, reactive arthritis, psoriatic or rheumatoid arthritis and an osteoarthritis flare can overlap. Cellulitis, septic bursitis, flexor tenosynovitis, trauma, fracture, haemarthrosis, Charcot arthropathy and avascular necrosis are anatomical alternatives. Dactylitis should prompt consideration of psoriatic arthritis, infection and sickle-cell disease. A painful first MTP can also arise from bunion inflammation, osteoarthritis or injury.
Chronic tophaceous gout can mimic rheumatoid nodules, calcinosis, xanthoma, multicentric reticulohistiocytosis or a soft-tissue tumour. A chalky aspirate should be examined rather than assumed. Kidney colic in a person with gout may be uric-acid or another stone type, and requires standard obstruction and infection assessment. Hyperuricaemia without arthritis is not a differential diagnosis for an acute joint because it is a laboratory state; routine urate-lowering treatment for asymptomatic hyperuricaemia is not generally recommended in the cited gout guidelines. The safest approach integrates tempo, anatomy, host risk and objective samples instead of treating a single number.
Management
Separate flare control from long-term crystal depletion. During a flare, rest the joint, offer an ice pack if tolerated and choose an NSAID, colchicine or a short oral corticosteroid according to comorbidities, co-prescriptions and preference. Intra-articular or intramuscular corticosteroid may be considered when standard oral options are unsuitable, after infection has been reasonably excluded. Suspected septic arthritis overrides a routine gout pathway. Review promptly if pain, systemic features or function worsen. Existing urate-lowering therapy is generally continued during a flare; stopping and restarting destabilises urate exposure.
After the flare, explain that urate crystals can persist between attacks. Current guidelines recommends offering treat-to-target urate-lowering therapy for multiple or troublesome flares, CKD stages 3 to 5, diuretic therapy, tophi or chronic gouty arthritis, and discussing it after a first flare outside those groups. ACR strongly supports ULT for tophaceous disease, radiographic damage or frequent flares and favours allopurinol first line, including in CKD. Start low, titrate against serial urate and adherence, and aim below 6 mg/dL; consider below 5 mg/dL for tophi, chronic arthritis or continued frequent attacks.
Discuss prophylaxis when initiating or escalating ULT, commonly colchicine or an appropriate low-dose anti-inflammatory alternative. ACR recommends at least three to six months, with extension if flares continue; duration is individual, not an automatic stop date. Review kidney and cardiovascular risk, weight, alcohol, hydration and medicines. Advise a balanced diet while being explicit that evidence does not support a special curative diet. Tophi usually shrink with sustained target urate. Surgery is reserved for selected cases with ulceration, infection, nerve compression, severe pain or functional obstruction after multidisciplinary review.
Prescribing Information
For an acute flare, NSAIDs can cause gastrointestinal bleeding, kidney injury, fluid retention and cardiovascular harm; consider renal function, ulcer history, anticoagulation, heart failure and hypertension, and gastroprotection where indicated. Colchicine has a narrow safety margin. Reduce or avoid it in significant renal or hepatic impairment according to current product information, and check strong CYP3A4 or P-glycoprotein inhibitors such as clarithromycin because accumulation can cause myopathy, marrow suppression, multiorgan toxicity and death. Corticosteroids can worsen glucose, mood, blood pressure and infection; do not inject a joint until infection is adequately considered.
Allopurinol is commonly first-line ULT. Begin at a low dose, lower still in CKD, then titrate to serum urate rather than leaving every patient at a nominal starter dose. Rash, fever, facial swelling, mucosal lesions, eosinophilia, hepatitis or kidney injury can indicate allopurinol hypersensitivity and require immediate cessation and urgent assessment. HLA-B*58:01 testing is recommended by ACR for selected higher-prevalence ancestry groups, not universally; evidence and availability for India's diverse populations require local specialist judgement rather than labelling all South Asians alike. Never combine standard-dose azathioprine or mercaptopurine with allopurinol without specialist dose adjustment and intensive monitoring because xanthine-oxidase inhibition can cause profound myelosuppression.
Febuxostat is an alternative xanthine-oxidase inhibitor. Current guidelines favours allopurinol first line in major cardiovascular disease; cardiovascular history and current product warnings must be reviewed. Uricosurics are less effective with impaired renal function and may worsen stone risk. Pegloticase and IL-1 inhibitors belong to specialist refractory-disease pathways. Provide anti-inflammatory prophylaxis when starting or titrating ULT, monitor serum urate monthly during titration where feasible, and consider annual monitoring after a stable target. Document indication, target, baseline kidney and liver function, counselling, prophylaxis, interactions, titration plan and review date.
When to Refer
Refer immediately for a suspected septic joint, systemic sepsis, rapidly progressive cellulitis, necrotising infection, neurovascular compromise or an acutely painful prosthetic joint. The destination should be able to aspirate, culture, give antimicrobials and obtain surgical source control. If aspiration is unavailable in primary care and infection cannot be confidently excluded, do not treat recurrent gout by telephone; arrange urgent assessment. New monoarthritis in an immunosuppressed patient, severe systemic illness or a child also warrants a low threshold for specialist review.
Current guidelines advises considering rheumatology referral when diagnosis is uncertain, treatment is contraindicated, not tolerated or ineffective, CKD is stage 3b to 5, or the patient has had an organ transplant. Other practical reasons include early-onset or familial disease, very high urate, recurrent attacks despite verified target urate, polyarticular or chronic inflammatory disease, difficult crystal interpretation, severe allergy, and consideration of biologic or uricase therapy. Nephrology input helps with advanced CKD, complex renal dosing, recurrent stones or uncertain renal decline; cardiology or general medicine may be needed when treatment choices intersect with unstable cardiovascular disease.
Refer to orthopaedic, hand or foot surgery only for a defined mechanical or tissue complication: ulcerated or infected tophus, nerve or tendon compression, major functional limitation, threatened skin, destructive deformity or diagnostic uncertainty. Surgery can provoke a flare and does not correct the urate burden, so medical treat-to-target care continues. Dietetic referral is useful for culturally appropriate weight, diabetes, kidney and cardiovascular goals, not for a punitive low-purine list. A good referral states diagnostic evidence, joints, flare history, urate trend, renal function, current and failed medicines, adherence, allergies, comorbidities, tophi and the specific question.
Red Flags
Treat a hot joint with fever, rigors, hypotension, confusion, rapidly spreading erythema, a wound, recent injection or surgery, bacteraemia risk or immune suppression as possible infection. Lack of fever does not make sepsis safe, and crystals do not exclude it. Severe pain out of proportion, bullae, crepitus or anaesthesia raises necrotising soft-tissue infection. A prosthesis, new murmur, intravenous drug exposure or multiple acutely inflamed joints broadens concern to implant infection, endocarditis or disseminated infection. Obtain urgent samples and escalate rather than masking the picture with repeated corticosteroids.
Medication red flags include gastrointestinal bleeding, oliguria, breathlessness or oedema after an NSAID; vomiting, diarrhoea, proximal weakness, neuropathy, bruising or infection after colchicine; and severe hyperglycaemia, psychiatric change or infection on corticosteroids. With allopurinol, any new rash deserves prompt assessment, while blistering, mucosal involvement, facial oedema, fever, eosinophilia, jaundice or renal deterioration suggests a severe hypersensitivity syndrome. Concomitant azathioprine or mercaptopurine without an explicit specialist plan risks life-threatening marrow suppression.
Tophi that ulcerate, discharge, become acutely erythematous, compromise skin, compress a nerve or tendon, or block footwear and function need urgent review. Acute kidney injury, an infected obstructed urinary tract, anuria, a solitary kidney with obstruction, or fever with renal colic requires emergency urological care. A first flare in a young person, unusual distribution, constitutional symptoms, persistent inflammation between attacks or failure despite sustained target urate should trigger diagnostic reconsideration. Do not dismiss cancer, inflammatory arthritis or infection as difficult gout.
Indian Clinical Context
India has no single national gout guideline equivalent to current guidelines, so a transparent approach uses high-quality international recommendations, current Indian product information and local specialist judgement. international and ACR agree on treat-to-target urate lowering but differ in some details, including first-line drug framing and timing. Those differences should be discussed rather than silently merged. The Indian National List of Essential Medicines 2022 includes allopurinol and colchicine, supporting their importance in the health system, but listing does not guarantee stock, correct dose, monitoring or equitable access at every facility.
Access to synovial-fluid microscopy, trained crystal identification, ultrasound and dual-energy CT varies widely. Where aspiration is unavailable, a confident classic clinical diagnosis may be reasonable, but sepsis risk requires referral rather than diagnostic convenience. Laboratories need prompt, correctly handled synovial samples for both crystal analysis and culture. Serum urate is inexpensive but frequently misused: a normal value during a flare does not exclude gout, and an incidental high value does not establish it.
CKD, diabetes, hypertension and cardiovascular disease are common co-management issues. Over-the-counter NSAID use and repeated unsupervised steroid injections can cause preventable harm, while undertitrated allopurinol leaves crystals in place. Provide prescriptions with a target and follow-up, not simply a tablet name. Ask about medicines obtained from multiple clinicians and pharmacies, including interacting antibiotics. Drug availability may favour allopurinol, but severe allergy, advanced kidney disease or treatment failure needs specialist support rather than therapeutic inertia.
Counselling should fit local diets and languages. Avoid declaring dal, vegetables or all protein forbidden; such advice is nutritionally crude and unsupported. Focus on healthy weight, limiting excessive alcohol and sugary drinks, hydration when appropriate, and adherence to ULT. Discuss fasting practices, work constraints and travel so prophylaxis and review are feasible. Professional outcomes include avoiding dietary blame, documenting uncertainty, stewarding investigations and arranging accessible follow-up.
Key Exam Pearls for NEET PG
1. Gout results from monosodium urate crystal deposition, not from an isolated serum urate result. The definitive synovial finding is a needle-shaped crystal with strong negative birefringence under compensated polarised light. Calcium pyrophosphate crystals are typically rhomboid and weakly positively birefringent.
2. Always consider septic arthritis in an acutely hot joint. Gout and infection may coexist; crystals, a high synovial leukocyte count, absent fever or prior response to colchicine cannot replace Gram stain and culture when infection is plausible.
3. Serum urate can be below 6 mg/dL during a flare. If clinical suspicion remains, repeat it at least two weeks after the flare settles. Ultrasound and dual-energy CT support selected uncertain diagnoses but do not supersede safe aspiration when sepsis is possible.

Frequently Asked Questions
Can a normal serum urate result during a painful flare rule out gout?
No. Serum urate can fall during acute inflammation, so a value below 6 mg/dL during a flare does not exclude gout. If the clinical pattern remains convincing, repeat serum urate at least two weeks after the flare settles. When diagnosis is uncertain or infection is possible, joint aspiration for crystals, Gram stain and culture is more informative than repeating blood tests alone.
Should long-term urate-lowering therapy be stopped when an acute gout flare occurs?
Usually no. Abruptly stopping established urate-lowering therapy can destabilise serum urate and does not treat the current inflammation. Continue it unless a specific adverse reaction or contraindication requires cessation, and treat the flare separately. A new rash or systemic hypersensitivity after allopurinol is different and requires immediate medical assessment rather than routine continuation.
Why is anti-inflammatory prophylaxis used when urate-lowering treatment is started?
Lowering serum urate mobilises crystals and can temporarily provoke flares even though the long-term treatment is working. Colchicine or an appropriate low-dose anti-inflammatory alternative can reduce this risk during initiation and dose escalation. Selection and duration depend on kidney function, interactions, bleeding and cardiovascular risk, and whether flares continue; prophylaxis does not replace titration to the urate target.
Can diet alone dissolve tophi and prevent future gout attacks?
Usually not. Healthy weight, moderation of excessive alcohol and sugary drinks, and a balanced diet support overall care, but current guidelines found insufficient evidence that any specific diet reliably prevents flares or lowers urate enough to remove a substantial crystal burden. Tophaceous or recurrent gout generally needs sustained treat-to-target urate-lowering therapy, with surgery reserved for selected mechanical, ulcerative or infectious complications.
