Summary
Gilbert syndrome is a common inherited reduction in bilirubin conjugation caused by decreased activity of uridine diphosphate glucuronosyltransferase 1A1, or UGT1A1. It produces intermittent, mild, predominantly unconjugated hyperbilirubinaemia without haemolysis, hepatocellular injury, cholestasis or impaired hepatic synthetic function. Many people are identified incidentally. Some notice scleral icterus during fasting, dehydration, febrile illness, sleep deprivation, strenuous exercise, menstruation or other physiological stress. The condition does not scar the liver, progress to cirrhosis or shorten life, and it ordinarily needs explanation rather than treatment.
The safety challenge is diagnostic discipline. Gilbert syndrome should not become a label for every raised bilirubin. Fractionate bilirubin; examine the pattern; review previous results, medicines and alcohol; and exclude clinically relevant haemolysis and liver disease. A typical profile has otherwise normal liver biochemistry and a normal blood count, reticulocyte count, lactate dehydrogenase, haptoglobin and blood film when haemolysis is assessed. Dark urine, pale stools, pruritus, fever, abdominal pain, anaemia, weight loss, abnormal aminotransferases or alkaline phosphatase, coagulopathy or a substantial conjugated fraction point elsewhere.
Routine imaging, liver biopsy, fasting-provocation tests and UGT1A1 genotyping are not required for a straightforward presentation. Genotyping can be useful when uncertainty remains, a more severe inherited unconjugated hyperbilirubinaemia is possible, family counselling has a defined purpose, or a medicine with a validated UGT1A1 association is being considered. Important examples include atazanavir-associated jaundice and irinotecan toxicity risk. The finding should be recorded accurately without telling patients to avoid long lists of ordinary medicines unsupported by drug-specific guidance.
How Common Is It?
Published estimates vary widely because studies use different bilirubin cutoffs, genotypes, ethnic populations and sampling conditions. The Journal of Hepatology review describes an overall reported range of roughly 2% to 12%, while professional patient resources give population-specific estimates. These figures are not interchangeable. A genotype may have incomplete penetrance, bilirubin fluctuates with fasting and illness, and laboratory reference intervals differ. Many people are never tested or are diagnosed only when a routine panel shows an isolated bilirubin rise.
Sex differences are reported, with clinical recognition more frequent in males, but this may reflect hormonal, biological and testing differences rather than a simple Mendelian sex effect. Presentation commonly becomes apparent after puberty. UGT1A1 variants also differ by ancestry: the promoter repeat often called *28 is not the only causal pattern, and coding variants such as *6 matter in some Asian populations. A limited assay that tests one variant can therefore miss clinically relevant reduced-function alleles and should not be marketed as a universal yes-or-no test for every Indian ancestry.
No reliable contemporary all-India prevalence estimate should be inferred from a single hospital series or a foreign genotype frequency. India is genetically diverse and has heterogeneous access to laboratory testing. From a clinical perspective, the high frequency of benign Gilbert physiology makes recognition useful because it prevents repeated imaging, “liver detox” treatments and anxiety. Frequency must never substitute for exclusion of haemolysis, viral or metabolic liver disease, biliary obstruction or medicine injury when history, examination or laboratory pattern is atypical.
Risk Factors
Gilbert syndrome is inherited and reflects reduced UGT1A1-mediated glucuronidation rather than an acquired lifestyle injury. Family history of intermittent mild jaundice or isolated bilirubin elevation supports the possibility, but absence of family history does not exclude it. The relationship between genotype and phenotype is complex: multiple reduced-function variants exist, allele frequencies vary across populations, and not everyone with a variant has the same bilirubin level. It is therefore more accurate to describe genetic susceptibility plus a compatible biochemical phenotype than to attribute every case to one promoter repeat.
Triggers increase bilirubin transiently rather than causing permanent hepatic damage. Reduced caloric intake or prolonged fasting, dehydration, intercurrent infection, strenuous exertion, sleep disruption, menstruation and major physiological stress are commonly reported. A person may first become visibly jaundiced during gastroenteritis, religious fasting, examination stress, surgery or a long journey with poor intake. These associations support a known diagnosis but are not safe provocation tests; deliberately fasting a patient to raise bilirubin is obsolete. Normal hydration and regular nutrition can reduce fluctuations without requiring a restrictive “liver diet.”
Medicines can expose or amplify the phenotype. Atazanavir inhibits UGT1A1 and can produce unconjugated hyperbilirubinaemia; reduced-function genotypes increase the chance that cosmetic jaundice leads to discontinuation. Irinotecan's active metabolite is inactivated partly by UGT1A1, so poor metabolizer genotypes can increase severe neutropenia and diarrhoea risk. These are drug-specific pharmacogenetic problems, not evidence that every common analgesic, statin or antibiotic is dangerous in all people with Gilbert syndrome. Coexisting haemolysis or liver disease can raise bilirubin through a separate mechanism and must not be folded into the benign label.
Diagnosis
History
Establish whether jaundice is new, intermittent or progressive and retrieve previous total and direct bilirubin values. Ask about fasting, fluid intake, fever, exertion, menstruation and recent surgery. Seek dark urine, pale stool, pruritus, right-upper-quadrant pain, fever, vomiting, weight loss, bleeding, confusion and constitutional symptoms. Review alcohol, prescribed and nonprescribed medicines, herbal products, transfusion, hepatitis exposures, travel, tattoos and family history. Ask about anaemia, gallstones, splenomegaly, inherited red-cell disorders and prior chemotherapy or antiretroviral therapy. Symptoms such as fatigue are nonspecific and should not automatically be assigned to bilirubin.
Examination
Look for scleral icterus in natural light; it can be more reliable than skin colour across diverse pigmentation. A well person with no hepatosplenomegaly or chronic-liver-disease signs fits Gilbert syndrome. Pallor, fever, tachycardia, splenomegaly or dark urine suggests haemolysis or systemic illness. Hepatomegaly, ascites, oedema, bruising, scratch marks, xanthomata, abdominal tenderness, a palpable gallbladder, encephalopathy or stigmata of chronic liver disease require another pathway. Examination may be normal in important early liver disease, so it complements rather than replaces laboratory pattern recognition.
Investigations
Start with total and direct bilirubin, ALT, AST, alkaline phosphatase, usually GGT, albumin and an assessment of coagulation when clinically indicated. For suspected unconjugated elevation, obtain full blood count, reticulocyte count, LDH, haptoglobin and blood film; direct antiglobulin testing follows the haemolysis question. Typical Gilbert syndrome has isolated mild indirect bilirubin with otherwise normal results. Repeat testing under ordinary fed, hydrated conditions can confirm intermittence. Imaging, biopsy and broad chronic-liver panels are selected only for atypical findings. Genetic testing is optional, not routine, and fasting or rifampicin provocation is obsolete.
Differential Diagnosis
First separate unconjugated from conjugated hyperbilirubinaemia. Increased unconjugated bilirubin results from excess production, impaired hepatic uptake or impaired conjugation. Haemolysis may be immune, enzymatic, membrane-related, haemoglobinopathic, mechanical or infection-associated; anaemia may be absent early, so reticulocytes, LDH, haptoglobin and smear matter. Ineffective erythropoiesis, a resolving large haematoma and recent transfusion can also increase bilirubin production. Medicine-related uptake or conjugation effects must be identified from the actual medicine and timing.
Crigler-Najjar syndromes produce more severe inherited unconjugated hyperbilirubinaemia, usually evident earlier in life; a persistently much higher level, neonatal history or family pattern makes a casual Gilbert label unsafe. Liver disease can sometimes begin with bilirubin change, but hepatocellular injury more often raises ALT or AST and cholestasis raises alkaline phosphatase or GGT with a larger conjugated fraction. Viral hepatitis, alcohol-related injury, metabolic dysfunction-associated steatotic liver disease, autoimmune hepatitis, Wilson disease and drug-induced liver injury are pursued according to age, history and abnormal tests rather than through one indiscriminate panel.
Predominantly conjugated hyperbilirubinaemia suggests hepatocellular excretory failure, intrahepatic cholestasis, extrahepatic obstruction or inherited transport disorders such as Dubin-Johnson or Rotor syndrome. Dark urine occurs because conjugated bilirubin is water soluble; pale stool and pruritus suggest impaired bile flow. Gallstone disease can coexist with Gilbert syndrome, so biliary pain, fever or ductal features are not “a Gilbert flare.” Sepsis, heart failure and pregnancy-related liver disorders also enter the differential in the right clinical context. A benign prior diagnosis does not protect a patient from acquiring new disease.
Management
Once the biochemical pattern is secure and alternatives have been reasonably excluded, management is clear explanation and documentation. Tell the person that bilirubin is produced during normal haem breakdown, that UGT1A1 conjugation is modestly reduced, and that levels can fluctuate without liver injury. State what was normal: blood count and haemolysis assessment, other liver enzymes and relevant clinical findings. This specific reassurance is more credible than saying merely “all tests are fine.” Give a copy or accessible record of the diagnosis to prevent repeated emergency investigations when jaundice recurs.
No medicine, supplement, detoxification regimen or procedure is required. Encourage regular meals and adequate hydration, especially during illness, heat, travel, exercise or fasting. People who choose religious fasting can discuss safer hydration and illness exceptions with their clinician and community advisers; the medical role is to explain likely bilirubin fluctuation, not prohibit beliefs. Ordinary exercise and healthy nutrition should continue. Treat unrelated conditions normally, and investigate symptoms on their merits rather than attributing headache, abdominal pain or chronic fatigue automatically to Gilbert syndrome.
Review is symptom- and pattern-led, not an obligatory lifelong liver-clinic schedule. Recheck if jaundice becomes more marked or persistent, the direct fraction rises, liver enzymes change, anaemia appears or new symptoms develop. Before atazanavir, irinotecan or another medicine with a validated UGT1A1 issue, communicate the phenotype and consider drug-specific guidance, pharmacy input or genotyping. Avoid speculative promises that mild bilirubin protects against cardiovascular disease or cancer: observational associations exist, but they do not create a treatment target or justify inducing hyperbilirubinaemia.
Prescribing Information
Gilbert syndrome itself is not treated pharmacologically. Phenobarbital and other historical approaches can lower bilirubin but have no role in routine benign disease because harms outweigh cosmetic biochemical change. Do not prescribe ursodeoxycholic acid, hepatoprotective mixtures, vitamins or herbal “liver cleanses” simply for isolated unconjugated bilirubin. Such treatment medicalizes a benign phenotype, creates cost and adverse-effect risk, and may obscure investigation of a later genuinely abnormal liver pattern. The safest prescription is an accurate problem-list entry plus drug-specific review.
Atazanavir inhibits UGT1A1 and can cause indirect bilirubin elevation without hepatocellular injury. CPIC guidance applies when genotype is already known and focuses on the likelihood that jaundice will lead to discontinuation; it does not advise stopping effective antiretroviral therapy independently. HIV treatment decisions incorporate virological efficacy, resistance, comorbidity, patient preference and current NACO or specialist protocols. A bilirubin rise on atazanavir must still be fractionated and interpreted alongside liver enzymes because unrelated hepatitis or drug injury can coexist.
Irinotecan exposure is different: reduced UGT1A1 function can increase active-metabolite exposure and the risk of severe neutropenia and diarrhoea. Oncology teams should use the current product label and validated pharmacogenetic guidance for the regimen and genotype; a historical Gilbert diagnosis alone does not supply a dose. Tell patients to mention the condition before chemotherapy and to report fever or severe diarrhoea urgently. For common medicines, do not generate a blanket avoidance list. Check the current label, dose, organ function, interacting medicines and clinical evidence, with pharmacist or specialist input when uncertainty matters.
When to Refer
Refer to gastroenterology or hepatology when the pattern is not convincingly benign: a substantial or rising conjugated fraction, abnormal ALT, AST, alkaline phosphatase, GGT, albumin or coagulation; persistent or unexpectedly high bilirubin; hepatomegaly, splenomegaly or chronic liver signs; constitutional symptoms; suspected viral, autoimmune, metabolic or drug-induced disease; or unexplained imaging findings. Referral is also appropriate when repeated basic assessment cannot distinguish Gilbert syndrome from another inherited hyperbilirubinaemia or when patient anxiety remains severe despite a clear explanation.
Haematology input is indicated for evidence of haemolysis, an abnormal smear, unexplained anaemia, reticulocytosis, low haptoglobin, significant splenomegaly or concern about an inherited red-cell disorder. Neonatal jaundice and marked childhood unconjugated hyperbilirubinaemia follow paediatric pathways rather than an adult Gilbert algorithm. Biliary pain, fever and jaundice may require urgent surgical or gastroenterology assessment for obstruction or cholangitis. Pregnancy with jaundice requires obstetric assessment because several pregnancy-specific disorders can be serious and Gilbert syndrome is a diagnosis of exclusion in that episode.
Refer or seek specialist pharmacy advice before a high-risk UGT1A1 substrate or inhibitor when genotype could change a consequential treatment choice. Oncology should manage irinotecan decisions; HIV specialists should manage antiretroviral selection. Genetic counselling is not routine but may help when bilirubin is unusually high, Crigler-Najjar syndrome is considered or a family-planning question has a defined implication. In India, referral should specify the actual question and attach fractionated bilirubin, liver biochemistry and haemolysis results to reduce duplicated cost and travel.
Red Flags
New jaundice deserves assessment before reassurance, even when a relative has Gilbert syndrome. Urgent features include fever with right-upper-quadrant pain, hypotension, confusion, persistent vomiting, severe abdominal pain, rapidly deepening jaundice, bleeding, reduced consciousness or suspected sepsis. Dark urine, pale stool and generalized pruritus suggest conjugated hyperbilirubinaemia or cholestasis rather than a typical isolated Gilbert episode. A palpable liver or spleen, ascites, oedema, bruising, marked weight loss or lymphadenopathy also changes the pathway.
Laboratory red flags include a rising direct bilirubin fraction, substantially higher total bilirubin than the person's established pattern, elevated aminotransferases or alkaline phosphatase, prolonged INR, low albumin, falling haemoglobin, reticulocytosis, low haptoglobin, high LDH or an abnormal blood film. Gilbert syndrome can coexist with hepatitis, gallstones, haemolysis and medicine toxicity; an old label must not cancel new objective abnormalities. Persistent jaundice after recovery from fasting or illness needs review rather than repeated self-reassurance.
Medicine-related emergencies are drug specific. Fever or infection symptoms during irinotecan treatment may indicate neutropenia; severe diarrhoea can be life-threatening and needs the oncology emergency pathway. Jaundice with malaise, abdominal pain or abnormal liver enzymes after a new medicine may represent drug-induced liver injury rather than UGT1A1-related cosmetic bilirubin change. Neonates, young children and pregnant people need age- or pregnancy-specific assessment. Any clinician who cannot obtain fractionated bilirubin or basic haemolysis testing should refer rather than diagnose from total bilirubin alone.
Indian Clinical Context
In India, an isolated bilirubin result may be reported on a low-cost “liver function test” panel without automatic fractionation, and access to haptoglobin, reticulocyte review, reliable LDH interpretation or UGT1A1 genotyping varies. The simplest safe pathway is staged: confirm total and direct bilirubin, examine other liver chemistry, obtain a full blood count and targeted haemolysis tests, and use history and examination to decide what comes next. Expensive imaging and broad panels are not substitutes for identifying whether bilirubin is unconjugated.
India's burden of viral hepatitis, haemoglobinopathies, malaria in relevant settings, gallstone disease, alcohol-related harm and metabolic liver disease makes premature reassurance hazardous, but it does not justify testing every asymptomatic person for every condition. Testing should follow exposure, geography, family history, examination and laboratory pattern. Herbal, Ayurvedic and over-the-counter mixtures should be documented without judgment because constituents may be uncertain and some products can cause liver injury. A benign Gilbert result must not be used to endorse an unverified remedy.
Laboratory reference ranges and direct-bilirubin assays differ, so interpret trends from the same laboratory cautiously and retain copies of prior results. Genotyping may be costly and limited to selected variants; a negative narrow panel does not exclude all UGT1A1-related phenotypes across India's diverse ancestries. When referral is needed, send the pattern and clinical question rather than “abnormal LFT.” Clear counselling in the person's language can prevent dietary restriction, marriage stigma, insurance anxiety and repeated testing. The condition is inherited but is not contagious and ordinarily does not impair fertility, work or lifespan.

Atazanavir can inhibit UGT1A1 and cause indirect hyperbilirubinaemia; reduced-function genotypes increase jaundice-related discontinuation risk. Irinotecan's active SN-38 metabolite is glucuronidated by UGT1A1, so poor metabolizers may experience severe neutropenia and diarrhoea. Do not extrapolate these associations into a blanket ban on common medicines. A prior Gilbert diagnosis never explains away fever, pain, pale stool, dark urine, anaemia or abnormal liver enzymes.
Frequently Asked Questions
Does Gilbert syndrome mean that the liver is damaged or will develop cirrhosis?
No. In a correctly established diagnosis, bilirubin conjugation is mildly reduced but the liver is otherwise healthy. Gilbert syndrome does not cause fibrosis, cirrhosis, liver failure or shortened life. That reassurance applies only to the characteristic isolated unconjugated pattern; new abnormal liver enzymes, dark urine, pain or persistent jaundice still need assessment.
Why does jaundice become noticeable during fasting, dehydration or illness?
Reduced UGT1A1 reserve makes bilirubin rise when calorie intake falls or the body is stressed by dehydration, infection, exertion or menstruation. The change is usually temporary and falls as the trigger resolves. Deliberately fasting to prove the diagnosis is unnecessary and obsolete; regular meals and adequate fluids are safer.
Is a UGT1A1 genetic test necessary to confirm every case of Gilbert syndrome?

Must people with Gilbert syndrome avoid all medicines metabolised by the liver?

Dose, genotype, organ function and interacting medicines determine risk; a pharmacist can resolve uncertainty.
