Summary
Fungal skin infection is a broad phrase; this guide concentrates on dermatophytosis, or tinea, affecting keratinised skin, hair and nails. Dermatophytes invade keratin rather than living tissue in uncomplicated disease. The site names matter because management differs: tinea corporis affects trunk or limbs, cruris the groin, pedis the feet, manuum the hand, faciei the face, capitis the scalp, barbae the beard and unguium the nail. Classic glabrous-skin tinea forms an itchy, enlarging, scaly plaque with a more active edge and relative central clearing, but morphology is often incomplete.
India has a substantial burden of recurrent, extensive and corticosteroid-modified dermatophytosis. Potent steroid-antifungal combination creams can suppress scale and redness without eradicating fungi, producing tinea incognito, wider spread, striae and diagnostic delay. Severe or refractory disease may involve Trichophyton indotineae, which has emerged in South Asia and often carries terbinafine resistance. Clinical non-response is not synonymous with resistance: wrong diagnosis, wrong site, poor application, inadequate duration, reinfection, drug interaction and immunosuppression must be assessed first.
Localized skin disease can often be managed with an appropriate topical antifungal and moisture control. Scalp, beard and nail infection, hair-follicle invasion, widespread disease or confirmed failure usually needs clinician-directed systemic therapy. Before oral treatment, verify indication, pregnancy status, liver and renal risk and interactions. Diabetes, immunosuppression, secondary bacterial infection and inability to access testing lower the threshold for review. How Common Is It?
Dermatophytosis is common worldwide, but a single reliable national prevalence for India cannot be stated from clinic series. Estimates depend on climate, season, crowding, occupation, sampled population, body site, laboratory confirmation and whether recurrent attenders are counted repeatedly. Hot, humid conditions, occlusive footwear and close household contact support transmission. Tinea pedis and onychomycosis become more frequent with age and diabetes; tinea capitis mainly affects children, while extensive glabrous tinea now occurs in otherwise healthy adults as well as immunocompromised people.
Indian dermatology services have reported a marked shift toward chronic, recurrent, multisite and steroid-modified tinea. The 2020 IADVL task-force consensus explicitly described an epidemic of difficult-to-treat glabrous tinea while acknowledging limited high-quality epidemiological and therapeutic data. That limitation is clinically important: referral-centre experience must not be converted into a claim that every Indian infection is resistant or needs prolonged oral combination therapy. Most localized, correctly diagnosed infections still deserve site-appropriate standard care and follow-up.
Burden includes itching, sleep disturbance, embarrassment, school or work absence, repeated medicine costs, household spread and adverse effects from unsupervised mixtures. Surveillance of antifungal resistance is incomplete because culture, species identification and susceptibility testing are not uniformly available, and routine laboratories may misidentify T. indotineae within the T. mentagrophytes/interdigitale complex. A useful local audit records body site, microscopy or culture, steroid exposure, previous drugs and duration, adherence, household clustering, diabetes or immunosuppression and outcome rather than relying on an unsupported prevalence percentage.
Risk Factors
Transmission occurs through infected people, animals, contaminated floors, clothing, towels, bedding, combs and footwear. Risk rises with heat, sweating, maceration, tight or occlusive clothing, communal bathing, contact sport and prolonged wearing of damp shoes. Shared sleeping space and delayed treatment can sustain household cycles. Exposure to a kitten or puppy supports a zoophilic source, while inflammatory scalp disease may spread among children through combs and close contact. Ask about similar lesions in family members without assuming that every contact requires medicine.
Host factors include diabetes, obesity with moist skin folds, peripheral vascular disease, older age, chronic oedema, hyperhidrosis and skin-barrier disruption. HIV, haematological malignancy, transplantation, systemic corticosteroids, biologic therapy and other immunosuppression can produce extensive, atypical or invasive follicular disease. Atopy, psoriasis and chronic eczema increase diagnostic confusion. Repeated antibiotic use may promote candidal intertrigo rather than dermatophytosis. Pregnancy does not itself prove fungal infection but materially changes systemic-drug decisions.
The most important modifiable Indian risk is unsupervised use of fixed-dose topical combinations containing a potent corticosteroid, antifungal and sometimes antibiotic. Steroids alter the border, reduce scale, permit spread and can cause atrophy, striae, telangiectasia or infection. Incomplete courses, intermittent application, sharing prescriptions and using an antifungal for an unconfirmed mimic also contribute to apparent failure. Emerging T. indotineae is linked with extensive, inflammatory and refractory tinea, and CDC highlights misuse of steroid combinations, inappropriate antifungal use and poor adherence as resistance pressures. Nevertheless, resistance should be investigated, not inferred from one persistent patch.
Diagnosis
History
Record onset, first site, centrifugal spread, itch, pain, scale, hair loss, nail change, recurrence after stopping therapy and affected contacts. Ask about animals, sport, communal floors, shared towels, travel, footwear, sweating and occupation. Obtain every previous tube or tablet, dose if known, duration, adherence and response, specifically identifying steroid combinations. Document pregnancy, liver or kidney disease, heart failure, diabetes, HIV risk, immunosuppressants and interacting medicines. Pain, fever, purulence or rapidly progressive inflammation suggests complication or another diagnosis.
Examination
Inspect the entire relevant skin surface, scalp, hair, nails, palms, soles and mucosa with consent rather than treating the most visible plaque in isolation. Note site, number, size, border, scale, central change, follicular papules, pustules, maceration, fissures, alopecia and lymph nodes. Scrape the active scaly edge, not an exhausted centre. Tinea cruris often spares the scrotum, whereas candidal intertrigo may involve it with satellite pustules. Steroid-modified tinea may be poorly demarcated, unusually extensive or eczematous.
Investigations
Typical localized disease may be diagnosed clinically, but potassium-hydroxide microscopy is valuable when morphology is atypical, previous treatment has failed or systemic therapy is contemplated. Collect scales, plucked affected hairs or nail material correctly; a negative mount does not exclude infection if sampling or prior treatment was suboptimal. Culture is appropriate for scalp or nail disease and chronic, recurrent, widespread or refractory infection. Species identification and antifungal susceptibility testing require a reference laboratory when resistant T. indotineae is suspected. Consider glucose or HbA1c and targeted immune assessment when severity or recurrence is disproportionate. Biopsy with fungal stains is reserved for unresolved mimics or follicular invasion.
Differential Diagnosis
Annular plaques are not automatically fungal. Nummular eczema is usually uniformly inflamed rather than advancing at a scaly edge. Plaque psoriasis has sharper margins and thicker silvery scale and may involve extensor sites or nails. Pityriasis rosea follows cleavage lines after a herald patch; granuloma annulare has a smooth, non-scaly rim; annular urticaria is transient. Subacute cutaneous lupus, erythema annulare centrifugum, leprosy and fixed-drug eruption need consideration when sensation, photosensitivity, persistence or distribution does not fit tinea.
In folds, candidiasis commonly involves opposing moist surfaces with satellite papules or pustules, while erythrasma is brown-red with fine scale and coral-red fluorescence under a Wood lamp. Irritant or allergic contact dermatitis, inverse psoriasis and intertrigo from friction are frequent groin or submammary mimics. Scaly feet may represent eczema, psoriasis, pitted keratolysis or juvenile plantar dermatosis. A unilateral scaly palm with bilateral tinea pedis supports the two-feet-one-hand pattern, but laboratory confirmation helps before prolonged treatment.
Scalp alopecia requires distinction from alopecia areata, trichotillomania, bacterial folliculitis, seborrhoeic dermatitis and scarring alopecia. Kerion is an inflammatory tinea capitis presentation and should not be incised as an abscess without careful assessment. Thick dystrophic nails may result from trauma, psoriasis, lichen planus or ageing; confirm fungus before oral therapy. Malassezia-associated pityriasis versicolor and Candida are fungal but are not dermatophytes and require different reasoning. Failure after treatment should restart the differential, look for follicular Majocchi granuloma and examine all reservoirs before escalating antifungals.
Management
Define site and extent before choosing treatment. For a small area of tinea corporis, cruris or pedis without hair or nail involvement, apply a proven topical antifungal to the lesion and a margin of surrounding skin for the product-specific course. Allylamines such as terbinafine and azoles such as clotrimazole are common options; relative duration and frequency differ, so the label and local formulary matter. Continue for the directed period rather than stopping at the first improvement. Keep folds and feet dry, change damp clothing and treat coexisting reservoirs.
Scalp, beard and most clinically important nail dermatophytosis require oral therapy because topical cream does not reach infected hair shafts or nail adequately. Oral treatment may also be needed for extensive, follicular, recurrent or topical-refractory glabrous disease. Confirm the diagnosis where feasible and select agent from site, likely organism, previous exposure, comorbidity, interactions, pregnancy and local resistance. IADVL consensus informs difficult Indian disease but includes expert-opinion schedules and predates some resistance evidence; it is not permission for automatic high-dose or multi-drug treatment.
At follow-up, distinguish clinical improvement from mycological cure and residual post-inflammatory pigmentation. If response is poor, check application, duration, ongoing steroid use, diagnosis, untreated feet or nails, household or animal exposure and medicine absorption before assuming resistance. Re-sample and seek dermatology or mycology input for severe or recalcitrant infection. Do not combine systemic antifungals empirically, extend them indefinitely or use oral ketoconazole for routine tinea. Treat secondary bacterial infection only when clinically present. Prevention and education are part of treatment, but indiscriminate treatment of asymptomatic contacts is not evidence-based.
Prescribing Information
Topical antifungals can cause burning, irritation or contact dermatitis. Match formulation to site: creams suit many skin lesions, while macerated interdigital areas may need drying measures and powders may reduce moisture but are not a substitute for treatment. Avoid potent topical corticosteroids, alone or hidden within fixed-dose combinations, on suspected tinea. If intense inflammation creates diagnostic uncertainty, confirm fungus and obtain dermatology advice rather than masking the border. Do not apply irritant home remedies to broken skin.
Before systemic therapy, document all medicines, allergy, pregnancy or breastfeeding, alcohol intake and hepatic, renal and cardiac disease. Terbinafine can cause liver injury, rash, taste or smell disturbance and clinically important CYP2D6 interactions. Azoles, particularly itraconazole, have numerous CYP-mediated interactions; absorption varies by formulation and gastric acidity, and itraconazole can worsen heart failure. Fluconazole requires renal-dose consideration and also prolongs QT in susceptible patients. Griseofulvin has interactions and pregnancy restrictions. Exact baseline and interval liver testing depends on agent, duration, risk and Indian product information; symptoms of hepatitis require prompt cessation and assessment.
Topical therapy is generally preferred in pregnancy for localized disease. Systemic treatment during pregnancy or breastfeeding requires a specific risk-benefit decision using current product information and specialist advice; older letter categories are not a safe prescribing shortcut. In children, doses are weight-based and tinea capitis regimens depend partly on organism. Check authenticity and formulation of medicines obtained outside regulated supply chains. Give the patient the generic name, formulation, site, amount, frequency, duration and missed-dose advice in writing. This guide supports verification and safety-netting, not an individual prescription.
When to Refer
Refer urgently when rapidly spreading erythema, severe pain, fever, toxicity, blistering, necrosis, facial or orbital swelling, extensive bacterial superinfection or sepsis is possible. An immunocompromised person with nodules, ulceration, deep follicular disease or systemic symptoms needs prompt specialist assessment because uncomplicated superficial tinea may not explain the presentation. Diabetic foot fissuring with cellulitis, ulceration or ischaemia follows a diabetic-foot pathway, not routine over-the-counter management.
Routine or expedited dermatology referral is appropriate for uncertain diagnosis, extensive or highly inflammatory disease, scalp or beard infection, kerion, scarring risk, multiple nails, recurrent disease, treatment failure after a verified adequate course, significant steroid damage, pregnancy when oral therapy is being considered, or important liver, renal, cardiac or interaction constraints. Refer for microscopy, culture and specialist species identification when T. indotineae or another resistant dermatophyte is suspected. CDC notes that ordinary culture methods may not reliably distinguish T. indotineae, so a generic laboratory name does not settle resistance.
Include body sites, morphology, photographs with consent, microscopy and culture results, diabetes or immune status and a complete treatment timeline. In India, access to a dermatology department or mycology reference service may require travel; while waiting, stop unprescribed steroid mixtures, provide safe skin care and avoid repeated empiric oral courses. Veterinary review may be relevant when an animal source remains symptomatic. Children with suspected tinea capitis should be reviewed promptly because oral therapy and household or school advice are needed and delay can lead to inflammatory scarring.
Red Flags
A painful hot rash with fever, rapid expansion, purulence, lymphangitis, hypotension or altered mental state suggests bacterial cellulitis, abscess or sepsis rather than simple tinea. Severe groin or perineal pain, crepitus, skin discoloration or systemic toxicity raises concern for necrotising infection and needs emergency surgical assessment. Vesicles with mucosal involvement, target lesions or widespread epidermal tenderness can signal a severe drug reaction. Do not reassure these presentations as fungal because scale or a prior tinea diagnosis is present.
Eye pain, reduced vision or periocular swelling requires urgent ocular assessment. Scalp bogginess, pustules and alopecia suggest kerion; cervical lymphadenopathy is common, but fever, fluctuance or diagnostic uncertainty requires prompt clinical review. Nodules following follicles, ulceration or extension beyond superficial keratin in an immunosuppressed patient may represent Majocchi granuloma or deeper fungal disease. Failure to thrive in a child, recurrent unusual infection or widespread candidiasis should prompt evaluation beyond dermatophytosis.
Treatment red flags include jaundice, dark urine, pale stool, persistent vomiting, severe fatigue or right-upper-quadrant pain during an oral antifungal course; stop the suspected drug and assess liver injury urgently. Facial swelling, wheeze or generalized urticaria suggests hypersensitivity. Palpitations or syncope may reflect an azole interaction or QT problem. New taste loss, severe rash, cytopenic symptoms or cardiac breathlessness also requires review. Worsening after corticosteroid application, repeated relapse after apparently adequate courses, or spread among household members is not usually an emergency, but it is a signal to confirm diagnosis and stop reflex escalation.
Indian Clinical Context
Indian clinicians face a distinctive combination of warm humid conditions, crowded households, high diabetes prevalence, easy access to mixed creams and an established epidemic of chronic or recalcitrant glabrous tinea. The IADVL INTACT statement is valuable because it addresses corticosteroid modification, adherence, laboratory diagnosis and special populations in this setting. It also openly states that high-quality evidence was limited and that some dose and duration practices were not evidence based. Its more aggressive schedules should therefore be interpreted by dermatologists against current resistance data and product safety, not copied as a national protocol.
T. indotineae emerged in South Asia and may produce extensive plaques involving groin, trunk and face with terbinafine resistance. Yet advanced identification and susceptibility testing are concentrated in specialist laboratories. When testing is unavailable, record that limitation and use structured clinical follow-up; do not invent susceptibility. Access barriers can make patients buy repeated low-cost combinations, share tubes or stop when itching improves. Written advice in the preferred language and review of the actual packaging are high-value interventions.
Practical prevention must fit local life: loose breathable clothing where feasible, complete drying of folds and feet, separate clean towels, regular laundering and sun or heat drying where appropriate, footwear in communal wet areas, cleaning of combs, and treatment of confirmed disease in affected household members. Advice should not stigmatize hygiene or sexual contact. People with diabetes need foot inspection and glycaemic care, while immunosuppressed patients need lower thresholds for confirmation and referral. International CDC advice supports resistance awareness but US reporting pathways and drug availability cannot be assumed to apply in India.

Assessment should reward diagnostic confirmation and antimicrobial stewardship rather than memorization of one regimen in a rapidly changing resistance environment.

Why can a steroid combination cream make ringworm difficult to recognise and treat?
