Summary
Onychomycosis is fungal infection of the nail unit caused most often by dermatophytes, but yeasts and non-dermatophyte moulds can be responsible. Typical changes include distal or lateral onycholysis, yellow-white discoloration, subungual debris, thickening and crumbling. Appearance alone is unreliable because trauma, psoriasis, lichen planus, eczema, bacterial colonisation and tumours can look similar. The central quality rule is therefore to obtain mycological confirmation before exposing a patient to systemic antifungal treatment. Direct microscopy, culture, histopathology with a fungal stain or an appropriately validated molecular test can contribute, with method choice guided by availability and the need to identify an organism. A negative result may reflect sampling error or previous treatment and should be interpreted rather than automatically overruled.
Management depends on the affected digit, proportion of nail involved, matrix disease, number of nails, organism, nail thickness, symptoms, comorbidity, interactions, pregnancy status, cost and patient preference. Limited distal disease without matrix involvement may be suitable for prolonged topical therapy and nail care; extensive, matrix, proximal, multiple-nail or dermatophytoma disease often requires clinician-considered systemic treatment. How Common Is It?
Fungal nail infection is common in adult dermatology and primary care, especially in toenails, but there is no single prevalence figure that should be applied to all of India. Estimates vary with climate, age, occupation, diagnostic method, whether clinically suspected or laboratory-confirmed disease is counted, and the population studied. A survey of people with diabetes, athletes, miners or dermatology-clinic attendees will not represent the general population. Studies also differ on whether Candida and non-dermatophyte mould isolates indicate true nail infection or contamination. Any local figure should therefore name the setting, definition and confirmation method.
Frequency rises with increasing age, repeated nail trauma, occlusive footwear, tinea pedis, peripheral vascular disease, diabetes and immunosuppression. Toenails grow more slowly and experience more trauma than fingernails, which helps explain their greater burden and slower apparent recovery. Infection is often dismissed as cosmetic, yet thick, painful or distorted nails can impair walking, footwear, manual work and self-care. A damaged nail and surrounding skin may add risk in a neuropathic or ischaemic diabetic foot, although onychomycosis alone does not establish an ulcer or bacterial infection. Risk Factors
Local risk factors include tinea pedis, repeated microtrauma, tight or occlusive footwear, sweaty feet, communal wet surfaces, nail damage, slow growth and difficulty trimming thick nails. Athletes, manual workers and people whose feet remain wet for long periods may have greater exposure, but occupation alone does not diagnose infection. A family or household pattern may reflect shared susceptibility, exposure or footwear rather than inevitable transmission. Toenail deformity from an old injury can both mimic infection and create a nail environment vulnerable to secondary fungal invasion. Psoriasis and other inflammatory nail disorders may coexist with fungus, so identifying one does not always explain the entire dystrophy.
Systemic susceptibility includes older age, diabetes, peripheral arterial disease, neuropathy, immunosuppression, HIV, transplantation and some cancer treatments. Ask about recurrent skin infection, poor wound healing and medicines that alter immunity. In diabetes, assess glycaemic context, sensation, pulses, skin breaks, footwear and ability to inspect the feet; a sharp thick nail can cause trauma even if fungal treatment is deferred. Proximal subungual disease, rapid progression or unusual organisms can warrant evaluation for immune compromise according to the wider history. Diagnosis
History
Record which nails are affected, sequence, duration, pain, walking or work limitation, trauma, footwear, wet exposure, tinea pedis, psoriasis and prior tests or treatment. Ask about diabetes, neuropathy, circulation, immune status, pregnancy or breastfeeding, liver and kidney disease, heart failure, alcohol use, allergies and every prescribed, over-the-counter, traditional or supplement product. Clarify whether appearance changed after empirical antifungals and whether family members have skin or nail disease.
Examination
Inspect all fingernails and toenails, surrounding folds, interdigital spaces, soles, palms and relevant skin. Describe distribution and pattern: distal lateral subungual, superficial white, proximal subungual, endonyx or total dystrophic. Estimate percentage involvement, matrix extension, subungual thickness and possible dermatophytoma. Look for pitting, oil-drop change, periungual inflammation, bacterial pigment, longitudinal melanonychia, ulceration and a solitary destructive lesion. In a diabetic or high-risk foot, assess pulses, sensation, deformity, pressure injury and infection.
Investigations
Confirm fungus before systemic therapy. Obtain an adequate sample from the most proximal active diseased area after cleaning: subungual debris or nail clipping according to phenotype, not merely the crumbly free edge. Potassium hydroxide microscopy can show fungal elements quickly; culture may identify genus or species and guide unusual cases but is slower and can be falsely negative or contaminated. Periodic acid-Schiff histology of clippings is sensitive for invasion but may not identify the organism. Molecular tests can be useful where validated and affordable. Repeat correctly targeted sampling or seek dermatology/mycology input when suspicion remains high after a negative result. [ONY-1]
Differential Diagnosis
Traumatic onycholysis and onychodystrophy are common mimics, particularly when one repeatedly impacted great toenail is affected. Nail psoriasis may produce pitting, oil-drop or salmon-patch change, subungual hyperkeratosis and onycholysis; skin or joint findings help but can be absent. Lichen planus can cause thinning, longitudinal ridging, splitting and pterygium with permanent scarring. Chronic dermatitis, alopecia areata, pachyonychia, age-related thickening and medication-related nail change broaden the differential. Chronic paronychia with Candida colonisation is not automatically Candida invasion of the nail plate, and a positive culture must fit the phenotype.
Green or dark pigment can reflect bacterial colonisation, blood, exogenous stain or melanin. A new or changing longitudinal pigmented band, periungual pigment, nail-plate destruction, bleeding mass or persistent single-nail dystrophy raises concern for nail-unit melanoma or squamous cell carcinoma and needs urgent specialist assessment rather than repeated antifungals. Painful focal lesions may represent glomus tumour, wart or other nail-unit pathology. Yellow nail syndrome and systemic disease produce different multi-nail patterns. Non-dermatophyte moulds are difficult because environmental contamination is common; repeated isolation, compatible microscopy or histology and clinical correlation may be required before assigning causality. Mixed infection is possible. Failure of terbinafine is not proof of mould infection or resistance. A diagnostic label should state the evidence: clinically suspected, microscopy-positive, culture-confirmed or histology-supported, along with organism where known. Management
First decide whether treatment is necessary now. Mild asymptomatic disease may be observed with nail care after discussing progression, transmission uncertainty and risk, whereas pain, functional impairment, spreading disease or a high-risk foot favours active management. Correct tinea pedis, reduce moisture and trauma, keep nails safely trimmed, and debride excessive thickness when skilled care is available. Do not share clippers, and clean instruments between users. Mechanical or chemical approaches can reduce load or improve penetration but are usually adjuncts rather than reliable stand-alone cures. Complete nail avulsion is not routine and can create wound risk, especially with diabetes or poor circulation.
Topical lacquer or solution is most rational for limited superficial or distal disease without matrix involvement, when systemic treatment is unsuitable, or as part of selected combination or recurrence-prevention strategies. Adherence is difficult because treatment lasts for months and requires correct preparation and application. Systemic therapy is more effective for extensive dermatophyte disease, multiple nails, matrix involvement, marked thickening, proximal disease or dermatophytoma, but only after confirmation and safety assessment. Nail Society of India guidance generally favours terbinafine for dermatophyte disease and itraconazole for selected non-dermatophyte or Candida patterns, with organism, contraindications and interactions determining the actual choice. Combination topical-systemic therapy is not automatically superior for every patient. Laser, photodynamic, device and home remedies have variable or limited evidence; visible clearing without mycological assessment is not necessarily cure. Agree whether the goal is symptom relief, mycological cure, clinical clear growth or prevention of diabetic-foot trauma, then schedule follow-up around nail growth rather than expecting rapid normalization. Prescribing Information
Systemic antifungals require a confirmed indication, medication reconciliation and current Indian product information. Terbinafine can cause gastrointestinal symptoms, rash, headache, taste or smell disturbance and uncommon serious hepatic or severe cutaneous reactions. Azoles, especially itraconazole, create clinically important cytochrome-mediated interactions and require caution or avoidance in heart failure and liver disease. The precise interaction depends on the co-medication and formulation; check anticoagulants, cardiovascular drugs, lipid-lowering medicines, antiepileptics, immunosuppressants and other agents rather than relying on memory. Baseline liver assessment is recommended in Indian nail guidance for oral treatment, with subsequent monitoring driven by agent, duration, symptoms, comorbidity and local label. Stop-and-assess symptoms include jaundice, dark urine, persistent nausea, severe fatigue, pale stool, widespread rash, blistering or mucosal lesions.
Do not reproduce a one-size dosing schedule without organism, site, age, liver and kidney context. Fingernails and toenails have different treatment and regrowth times, and pulse versus continuous regimens are not interchangeable across drugs. Oral antifungal treatment is generally deferred in pregnancy because onychomycosis is slowly progressive and fetal safety evidence is limited; breastfeeding decisions also need current product data. Children, older adults, transplant recipients and people with HIV, diabetes, cardiac disease or polypharmacy warrant tailored review. Refer urgently for suspected nail-unit melanoma or squamous cell carcinoma: a new or evolving pigmented band with concerning features, pigment extending onto surrounding skin, unexplained bleeding, ulceration, a mass, progressive plate destruction or persistent solitary dystrophy without a coherent benign explanation. Acute spreading cellulitis, fever, a diabetic-foot wound, ischaemic features, rapidly progressive tissue damage or severe immunosuppression with invasive infection concern needs prompt medical assessment rather than routine nail treatment. A painful nail with drainage may require evaluation for bacterial infection or an ingrowing nail in addition to possible fungus.
Refer routinely to dermatology when diagnosis remains uncertain after properly collected testing, samples repeatedly disagree, a non-dermatophyte mould or mixed infection is suspected, many nails or the matrix are involved, disease relapses despite confirmed appropriate therapy, or psoriasis, lichen planus and fungus may coexist. Mycology advice is valuable for unusual isolates and possible resistance. High-risk patients with diabetes, neuropathy, peripheral arterial disease, transplantation, HIV, chemotherapy or inability to perform foot care may need coordinated dermatology, physician, podiatry or surgical services according to local resources. Pregnancy with severe symptomatic disease and significant liver, kidney, cardiac or interaction complexity also justify specialist planning before systemic treatment. The referral should state site and pattern, percentage and matrix involvement, photographs where consented, sampling method and results, organism, prior treatments and adherence, liver or renal data, medicines, comorbidity, pregnancy status, neurovascular foot findings and the clinical question. Red Flags
The most important diagnostic red flag is a solitary, progressive nail abnormality that does not behave like ordinary distal fungal disease. Longitudinal pigmentation that is widening or irregular, extension onto the proximal or lateral fold, nail destruction, unexplained bleeding, ulceration, a subungual mass or persistent pain requires assessment for melanoma, squamous malignancy or another tumour. Do not repeatedly sample only for fungus while a lesion evolves. Black colour can be blood, organism, stain or melanin; history and dermoscopic or biopsy expertise may be needed. Severe periungual inflammation, pus, lymphangitis, fever or rapidly spreading erythema suggests bacterial complication.
Foot red flags include ulceration, necrosis, rest pain, absent pulses, acute colour or temperature change, deep infection, loss of protective sensation with injury, and inability to bear weight. These are especially important in diabetes and immunosuppression and should not wait for a fungal culture. Medicine red flags after an oral antifungal include jaundice, dark urine, marked anorexia, persistent vomiting, severe fatigue, widespread rash, blistering, facial swelling, breathing difficulty or mucosal involvement. A major interaction, pregnancy discovered during therapy or new heart-failure symptoms warrants prompt prescriber review. Proximal subungual disease, extensive unusual infection or rapid spread may be a clue to immune compromise, though it is not diagnostic by itself. Repeated negative tests are a reason to revisit sampling and alternatives, not to escalate treatment indefinitely. Indian practice spans advanced molecular laboratories and settings where only microscopy or no immediate mycology is available. The safety standard remains confirmation before systemic therapy; when testing is inaccessible, it is safer to improve sampling access, use conservative nail care or refer than to expose repeated unconfirmed dystrophy to oral antifungals. Potassium hydroxide microscopy can be a pragmatic first test when performed by trained staff, while culture or histology becomes important for atypical, failed or mould-suspected disease. Explain that a result may take time and that a thick nail will not look normal immediately. Costs of laboratory tests, prolonged topical products, monitoring and travel should be discussed before a plan is chosen.
The Nail Society of India recommendations provide locally relevant pharmacologic and special-population guidance, including medicine availability, organism patterns and comorbidity. They remain professional recommendations rather than substitutes for an individual prescription, current CDSCO-approved product information or institutional policy. Why should a nail sample be tested before starting oral antifungal medicine?

How long does it take for an infected nail to look normal after treatment?
A successful antifungal course does not instantly remove damaged nail. Improvement appears as healthy proximal growth while old dystrophic plate moves outward, so fingernails take months and toenails often take considerably longer. Some residual deformity may persist after infection clears because trauma or another nail disorder caused permanent change. Follow-up should distinguish mycological cure from complete clinical clearing and should allow an appropriate growth interval. Early appearance alone should not trigger repeated oral courses without checking adherence, diagnosis, sampling and reinfection.
What makes fungal nail disease more urgent in someone with diabetes or immunosuppression?
