Summary
Steatotic liver disease describes excess hepatic fat identified by imaging, biomarkers or histology and has several causes. Metabolic dysfunction-associated steatotic liver disease (MASLD), the term that has replaced non-alcoholic fatty liver disease in current international nomenclature, requires hepatic steatosis with at least one cardiometabolic risk factor and no better alternative explanation. Metabolic dysfunction-associated steatohepatitis (MASH) adds hepatocellular injury and inflammation; fibrosis stage, rather than fat alone, is the strongest practical marker of liver-related risk. Older records, Indian programmes and examinations may still use NAFLD and NASH, so clinicians must recognise both naming systems without treating them as exactly interchangeable in every historical dataset.
An incidental bright liver or mildly raised alanine aminotransferase is the start of assessment, not the diagnosis. Quantify alcohol; review prescription, non-prescription and traditional medicines; assess viral hepatitis exposure; and consider autoimmune, genetic, nutritional and pregnancy-related disease when the phenotype is atypical. Normal aminotransferases do not exclude advanced fibrosis, and ultrasound cannot stage inflammation reliably.
Primary care should identify cardiometabolic risk, calculate a validated first-line fibrosis score such as FIB-4 when appropriate, and arrange second-line elastography or specialist assessment for indeterminate or high risk. Care centres sustainable weight loss, physical activity, diabetes, blood pressure, lipids, sleep apnoea and cardiovascular prevention. How Common Is It?
Steatotic liver disease is common globally and in India, but a single prevalence figure can mislead. Estimates change with rural or urban sampling, age, diabetes and obesity prevalence, ultrasound versus controlled attenuation parameter, alcohol definitions and the older NAFLD versus newer MASLD criteria. India's 2024 national operational guideline treats fatty liver as a non-communicable disease priority integrated with metabolic-risk services. It cites a broad Indian range rather than a single definitive national percentage, reflecting heterogeneous studies and populations.
Prevalence is substantially higher among people with type 2 diabetes, central adiposity, dyslipidaemia, hypertension and obstructive sleep apnoea. Lean people can also have MASLD, particularly in Asian populations, so a normal body mass index does not exclude visceral adiposity or metabolic risk. Conversely, obesity does not prove that abnormal liver tests are metabolic; viral hepatitis, alcohol, medicines and other causes can coexist.
Most people with steatosis will not develop liver failure, while a smaller group with steatohepatitis and progressive fibrosis accounts for much of the serious liver burden. Cardiovascular disease and extrahepatic malignancy contribute importantly to outcomes, which is why assessment cannot stop after telling someone to lose weight.
For an individual, the useful questions are whether steatosis is present, what caused it, whether clinically important fibrosis exists, and whether cardiometabolic risk is being treated. Service planning should use locally measured prevalence and referral capacity. Repeating an alarming population statistic without its definition can increase stigma while doing little to identify the person who needs elastography, hepatology follow-up or cirrhosis care.
Risk Factors
MASLD risk rises with type 2 diabetes, central adiposity, insulin resistance, dyslipidaemia, hypertension and multiple cardiometabolic abnormalities. Obstructive sleep apnoea, sedentary behaviour, energy-dense diets and weight gain contribute. South Asian adults may develop diabetes and visceral adiposity at lower body mass index than European populations, so measure waist circumference and metabolic markers rather than using appearance alone. Lean MASLD is real but should prompt particularly careful evaluation for alcohol, medicines and less common causes.
Alcohol must be quantified in standard units, including pattern, binges, duration and recent change. Metabolic risk and alcohol can act together; current nomenclature includes MetALD for steatosis with metabolic dysfunction and a moderate increased alcohol exposure, while higher exposure supports alcohol-related liver disease. A binary question such as 'Do you drink?' is inadequate, and shame-based questioning reduces accuracy.
Medicines and exposures associated with steatosis or steatohepatitis include corticosteroids, amiodarone, tamoxifen, methotrexate, valproate and selected antiretroviral or oncological treatments. Attribution depends on timing, dose, competing causes and recovery; essential treatment must not be stopped from a list alone. Severe malnutrition, rapid weight loss, parenteral nutrition and some endocrine disorders can also produce hepatic fat.
Fibrosis progression is more likely with diabetes, older age, greater metabolic burden, continuing alcohol exposure and established steatohepatitis or fibrosis. Genetic variants influence susceptibility but routine consumer genetic testing does not replace clinical stratification. Chronic hepatitis B or C, autoimmune liver disease, haemochromatosis, Wilson disease and drug injury may coexist with metabolic steatosis and alter prognosis and treatment.
Diagnosis
History
Clarify why the liver was assessed: incidental imaging, abnormal chemistry, metabolic screening or symptoms. Ask about weight trajectory, waist gain, diabetes, blood pressure, lipids, sleep apnoea, diet, activity and family liver disease. Quantify alcohol by drink type, volume, frequency and binges, using a validated tool when possible. Build a timeline of prescribed, over-the-counter, herbal, Ayurvedic, bodybuilding and weight-loss products. Assess transfusion, injection, sexual and household risks for viral hepatitis without moral judgement. Fatigue or right-upper-quadrant discomfort is nonspecific; pruritus, jaundice, swelling, bleeding or confusion suggests another or advanced process.
Examination
Measure weight, height, waist circumference and blood pressure; look for acanthosis, lipodystrophy and cardiovascular risk. Examine for hepatomegaly and features of advanced disease: firm or nodular liver, splenomegaly, ascites, oedema, muscle loss, bruising, spider angiomas, palmar erythema and encephalopathy. Absence of stigmata does not exclude fibrosis. Check for alternative clues such as Kayser-Fleischer rings in an appropriate young patient, autoimmune disease, thyroid dysfunction or heart failure.
Investigations
Initial testing usually includes liver chemistry, full blood count with platelets, glucose or HbA1c, lipid profile, renal assessment and targeted hepatitis B and C testing. Select autoimmune serology, ferritin with transferrin saturation, ceruloplasmin, alpha-1 antitrypsin assessment or coeliac testing when age, pattern or phenotype warrants. Ultrasound detects moderate steatosis but cannot reliably exclude mild fat, MASH or fibrosis. Calculate FIB-4 only in an appropriate stable adult and interpret age limitations; indeterminate or elevated risk leads to transient elastography, ELF or another validated second-line test. MRI-based tests and liver biopsy are reserved for unresolved diagnosis or staging questions rather than routine confirmation.
Differential Diagnosis
Alcohol-related liver disease can coexist with metabolic dysfunction. Establish grams or units rather than relying on labels, and consider periods of heavier historical intake. MetALD recognises an overlap range, but thresholds and terminology continue to evolve; local guidance and a detailed history matter more than arguing over the name. Viral hepatitis B and C remain important in India and may produce steatosis, inflammation or fibrosis independently. Test according to national pathways and risk rather than assuming every bright liver is metabolic.
Drug-induced steatosis or steatohepatitis is considered when the timing fits corticosteroids, amiodarone, tamoxifen, methotrexate, valproate or other implicated agents. Traditional and gym supplements can cause hepatocellular or cholestatic injury that is not necessarily fatty liver. Autoimmune hepatitis, haemochromatosis, Wilson disease, alpha-1 antitrypsin deficiency and coeliac disease are selected by age, biochemical pattern, family history and associated features. Rapidly progressive or disproportionate abnormalities deserve broader investigation.
Secondary fat accumulation occurs with severe malnutrition, rapid weight loss, intestinal failure or parenteral nutrition, lipodystrophy and selected endocrine disorders. Acute fatty liver of pregnancy is a third-trimester emergency with systemic illness, hypoglycaemia and coagulopathy, not a form of routine MASLD. Reye syndrome and inherited metabolic disorders belong to paediatric pathways.
Congestive hepatopathy, biliary obstruction and infiltrative disease can mimic chronic liver disease clinically or biochemically. Ultrasound echogenicity may be affected by technical factors, while focal fatty sparing can resemble a lesion. MASH cannot be diagnosed from aminotransferase elevation alone, and normal enzymes do not rule out advanced fibrosis. When tests disagree, revisit the causal history and fibrosis probability rather than repeatedly relabelling the same incidental scan.
Management
Explain that treatment targets both liver progression and the more immediate cardiometabolic risk. Agree on changes the person can sustain: a culturally appropriate energy deficit when weight loss is needed, less sugar-sweetened drink and ultra-processed food, more vegetables, pulses, whole grains and unsaturated fats, and portions compatible with diabetes and cardiovascular care. No single branded diet is mandatory. A weight reduction of more than 5% can reduce liver fat, while approximately 7-10% or more may be needed for greater effects on steatohepatitis and fibrosis; response varies, and weight cycling or malnutrition should be avoided. Physical activity benefits liver and cardiovascular health even without large weight loss.
Treat diabetes, hypertension and dyslipidaemia to evidence-based targets individualized for comorbidity. Statins should not be withheld merely because MASLD is present; cardiovascular indication and standard safety monitoring govern use. Assess sleep apnoea, smoking, kidney disease and cardiovascular symptoms. Discourage alcohol, particularly when fibrosis is present; abstinence is appropriate in cirrhosis. Vaccination and liver-specific preventive care follow serology and chronic-liver-disease guidance.
Use a documented fibrosis pathway. Low-risk adults can generally remain in primary care with metabolic treatment and periodic reassessment, while indeterminate or high-risk tests require elastography or hepatology input. Exact intervals and cut-offs depend on age, assay and national pathway.
Bariatric or metabolic surgery may improve MASLD in eligible people with obesity, but it is selected for the whole obesity indication after multidisciplinary assessment, not offered simply because ultrasound shows fat. Decompensated cirrhosis and portal hypertension materially change operative risk. Advanced disease requires cirrhosis care, portal-hypertension assessment, hepatocellular carcinoma surveillance and transplant referral when indicated.
Prescribing Information
There is no universal tablet for every person with fatty liver. Prescribe diabetes, obesity, lipid and blood-pressure therapies for their approved indications and individual benefits. GLP-1 receptor agonists and dual incretin therapies can produce weight loss and improve metabolic control; their liver effects do not remove the need for fibrosis assessment, and access, contraindications, adverse effects and current Indian authorization must be checked. SGLT2 inhibitors may benefit diabetes, heart failure or kidney disease but are not a stand-alone treatment for fibrosis.
Pioglitazone and vitamin E have evidence in selected non-cirrhotic steatohepatitis populations, yet harms and uncertain generalizability limit routine use. Pioglitazone can cause weight gain, oedema, worsen heart failure and increase fracture risk. High-dose vitamin E is not an innocuous supplement and requires specialist discussion of bleeding and long-term uncertainty. Do not prescribe omega-3 products as a liver-specific cure, although standard lipid indications may differ. Statins are used for cardiovascular risk and should not be stopped solely for stable MASLD.
Resmetirom has evidence for selected adults with non-cirrhotic MASH and significant fibrosis and is included in recent specialist guidance where locally approved. It is not treatment for simple steatosis, unconfirmed risk or decompensated cirrhosis. Verify national regulatory status, label, thyroid and hepatic considerations, interactions and specialist monitoring at the time of prescribing; availability changes. Current evidence does not support a MASH-targeted medicine for cirrhotic-stage disease as a substitute for cirrhosis care.
Review alcohol and every medicine or supplement before attributing injury. Never abruptly stop corticosteroids, antiepileptics, anticoagulants or cancer therapy without the responsible prescriber. This section defines safety boundaries, not a patient-specific regimen.
When to Refer
Refer to hepatology or gastroenterology when FIB-4 or another first-line score is indeterminate or high and second-line testing is unavailable, abnormal or discordant with clinical suspicion. Persistently abnormal liver chemistry, uncertain cause, suspected autoimmune or genetic disease, possible drug-induced injury, imaging suggesting a focal lesion, thrombocytopenia, splenomegaly or other evidence of portal hypertension also merits specialist evaluation. A low score should not overrule clear clinical signs, and FIB-4 is unreliable in some ages and during acute illness.
Advanced fibrosis or cirrhosis requires structured specialist care: aetiology confirmation, portal-hypertension assessment, vaccination and nutrition review, medication adjustment, variceal and HCC surveillance according to local protocol, and transplant consideration when decompensation or prognostic thresholds arise. Suspected MASH-targeted pharmacotherapy should be handled by a service able to confirm eligibility, discuss uncertain long-term benefit and monitor toxicity. Bariatric surgery referral requires an obesity multidisciplinary team and liver-risk assessment; decompensated cirrhosis is not routine bariatric territory.
Refer urgently for new jaundice with systemic illness, ascites, gastrointestinal bleeding, confusion, severe coagulopathy, rapidly rising enzymes, hypoglycaemia, sepsis or suspected acute liver failure. A new liver mass or rising HCC concern requires expedited diagnostic imaging, not serial routine ultrasound alone.
India's 2024 operational guideline describes stepped care from community and primary services to higher centres. Referral should name the actual test or decision needed, such as transient elastography, causal work-up, cirrhosis care or lesion characterization. Record distance, affordability and follow-up rather than assuming tertiary access.
Red Flags
Jaundice, dark urine with pale stools, rapidly increasing abdominal girth, haematemesis, melaena, confusion, marked drowsiness, fever with ascites, hypotension or reduced urine output may indicate decompensation, variceal bleeding, spontaneous bacterial peritonitis, acute-on-chronic liver failure or sepsis. These are emergency presentations. A person previously told they have 'only fatty liver' can still develop an unrelated acute hepatitis, biliary obstruction, drug injury or infection; the old label must not delay assessment.
Very high or rapidly rising aminotransferases, prolonged INR, hypoglycaemia, metabolic acidosis or encephalopathy suggests severe acute injury and possible acute liver failure. Ask immediately about paracetamol, antituberculous drugs, antibiotics, antiepileptics, mushrooms and herbal or traditional products. Pregnancy with vomiting, abdominal pain, jaundice, hypertension, low platelets, hypoglycaemia or coagulopathy requires urgent obstetric and liver assessment for acute fatty liver, HELLP or another emergency.
A firm liver, splenomegaly, falling platelet count, ascites or imaging signs of portal hypertension signal advanced disease even if aminotransferases are normal. Unexplained weight loss, early satiety, persistent right-upper-quadrant pain, worsening liver function, a focal lesion or constitutional decline raises hepatocellular carcinoma or another malignancy and warrants expedited imaging and specialist review.
Cardiometabolic emergencies remain important: chest pain, focal neurological deficit, severe hyperglycaemia, ketoacidosis and hypertensive crisis should not be redirected to a routine liver clinic. Severe restrictive dieting, purging or rapid unsupervised weight loss can produce malnutrition and liver injury. Assess alcohol dependence safely because abrupt unsupervised cessation in a dependent person can cause withdrawal.
Indian Clinical Context
India's National Programme for Prevention and Control of Non-Communicable Diseases incorporates NAFLD prevention, early detection, risk stratification and referral. The September 2024 operational guideline supports a tiered pathway involving community health workers, primary care medical officers and secondary or higher services. This is valuable because metabolic liver disease intersects with diabetes and hypertension programmes, but implementation and access to ultrasound, platelet counts, elastography and hepatology vary. Use the current state pathway rather than claiming that every district provides identical tests.
Risk assessment should account for South Asian metabolic disease at lower body mass index, but avoid the stereotype that every Indian with diabetes has MASLD. Measure waist, glucose, lipids and blood pressure; quantify alcohol privately; test for viral hepatitis when indicated; and review tuberculosis medicines, antiepileptics, traditional products and supplements. India's burdens of hepatitis B and C and drug-related liver injury make an exclusion-based history essential.
Counselling should use affordable local foods and household realities. Portion size, cooking fat, refined grains, sweets and sugar-sweetened drinks can be discussed without banning rice or culturally important meals. Walking, cycling, household activity and resistance work are options according to safety, disability, heat and air quality. Weight targets must not become humiliation; explain that fibrosis risk, blood pressure and glucose matter even when the scale changes slowly.
INASL guidance offers India-specific risk and resource considerations, while EASL and AASLD add newer nomenclature and evidence. Where recommendations differ, document the chosen pathway and its jurisdiction. Medicines marketed as liver detoxifiers, unverified supplements and commercial packages that repeat scans without fibrosis stratification can waste resources or cause harm.
NMC Competency Mapping
The 2024 NMC curriculum maps this topic principally to General Medicine Topic 5 on liver disease. GM5.2 covers aetiology and pathophysiology of liver diseases; GM5.4 addresses alcohol-related disease; GM5.5 covers cirrhosis, portal hypertension and hepatocellular carcinoma; and GM5.6 covers drug-induced liver injury. GM5.8 through GM5.13 require cause-focused history, systematic examination, differential diagnosis, interpretation of liver and viral tests, appropriate imaging and a structured diagnostic approach. GM5.15 and GM5.17 support management of cirrhosis complications and transplant indications.
A competent student should identify steatotic liver disease without treating ultrasound brightness as a complete diagnosis. They should quantify alcohol, recognise metabolic risk, build a medication and supplement timeline, test selectively for viral and alternative causes, and identify stigmata of chronic liver disease. They should calculate FIB-4 from age, AST, ALT and platelet count while explaining why age, acute illness and local cut-offs affect interpretation. Supervised clinical teaching, not this guide alone, is required to certify abdominal examination or counselling skills.
Reasoning should separate presence of fat, aetiological category, inflammatory activity and fibrosis stage. Learners should explain the transition from older NAFLD/NASH terms to MASLD/MASH while still understanding historical exam language. They should prioritize cardiovascular and diabetes care, describe evidence-based lifestyle treatment, and state the limits of vitamin E, pioglitazone, incretin therapy, resmetirom and bariatric surgery.

Key Exam Pearls for NEET PG
Steatotic liver disease is the umbrella term. MASLD requires hepatic steatosis plus at least one cardiometabolic risk factor without a better cause; MASH adds hepatocyte injury and inflammation, with or without fibrosis. Older examinations may ask NAFLD and NASH. Alcohol history, viral hepatitis and drug exposure remain obligatory because metabolic and other causes can coexist. Simple steatosis, steatohepatitis, fibrosis and cirrhosis are not synonyms.
Normal aminotransferases do not exclude advanced fibrosis, and their level does not stage disease. Ultrasound detects steatosis imperfectly and cannot prove MASH. FIB-4 uses age, AST, ALT and platelet count; many adult pathways use a low threshold near 1.3 to exclude advanced fibrosis, with a higher age-adjusted threshold in those over 65, followed by transient elastography or ELF when indeterminate or high. Do not apply cut-offs mechanically in acute illness or very young adults. Biopsy is reserved for diagnostic or staging uncertainty, not every fatty liver.
Fibrosis stage drives liver-related prognosis. Management combines sustained weight reduction when indicated, exercise and aggressive cardiometabolic care. More than 5% weight loss can reduce steatosis, while approximately 7-10% or greater loss is often needed for broader histological improvement; individualize and prevent sarcopenia. Statins are used for cardiovascular indications and are not automatically contraindicated. No supplement is a universal cure.
Cirrhosis requires portal-hypertension and HCC surveillance pathways; decompensation changes medicine and surgery decisions. Resmetirom is a specialist option only for selected non-cirrhotic MASH with significant fibrosis where approved. Jaundice, ascites, variceal bleeding, encephalopathy or acute coagulopathy demands urgent evaluation, not routine lifestyle follow-up.
Frequently Asked Questions
Does a fatty liver on ultrasound mean that cirrhosis is present?
No. Ultrasound can suggest steatosis but does not reliably identify steatohepatitis or stage fibrosis. Risk assessment combines history, examination, platelets and liver chemistry with a validated score such as FIB-4, followed when indicated by elastography, ELF or specialist evaluation. Cirrhosis can also exist despite normal aminotransferases.
Can a person with a normal body mass index have MASLD?
Yes. Visceral adiposity, diabetes, dyslipidaemia and genetic susceptibility may occur without a high body mass index, particularly in Asian populations. Lean steatosis still requires waist and metabolic assessment plus careful exclusion of alcohol, viral hepatitis, medicines, malnutrition and uncommon liver disease rather than automatic metabolic attribution.
Are medicines or supplements enough to reverse fatty liver disease?
There is no universal liver-cleansing medicine. Diabetes, obesity, lipid and blood-pressure drugs should be used for their approved indications. Selected MASH therapies require specialist confirmation and local authorization. Vitamin E, pioglitazone and newer agents have population-specific benefits and harms; none replaces sustained lifestyle care or fibrosis assessment.
When does fatty liver require urgent rather than routine review?
Urgent assessment is needed for jaundice with illness, vomiting blood or black stool, new ascites, confusion, fever with abdominal swelling, severe coagulopathy, hypoglycaemia, rapidly worsening liver tests or suspected drug toxicity. A new liver mass or signs of portal hypertension also need expedited specialist investigation.
