Summary
Endometrial cancer arises from the lining of the uterine corpus and is distinct from cervical cancer, uterine sarcoma and ovarian malignancy. Endometrioid carcinoma is the commonest histological type, but serous, clear-cell, carcinosarcoma and other aggressive tumours require different risk interpretation. Most patients present with abnormal uterine bleeding, often after menopause, which creates an opportunity for diagnosis while disease is confined to the uterus. A single episode of postmenopausal bleeding is therefore investigated rather than observed as a normal effect of age or medicines.
Diagnosis requires endometrial tissue. Transvaginal ultrasonography can guide the pathway, but an endometrial-thickness measurement neither establishes nor completely excludes malignancy in every context. Hysteroscopy and directed sampling are useful when focal disease, inadequate tissue or persistent bleeding creates uncertainty. Once carcinoma is confirmed, expert pathology defines histotype and grade; mismatch-repair, p53 and POLE-related molecular information increasingly refines prognosis and treatment planning. Imaging and operative findings determine anatomical extent.
For apparently confined disease, total hysterectomy with removal of both tubes and ovaries, with a risk-adapted nodal strategy, is the usual foundation. Adjuvant radiotherapy, chemotherapy, endocrine treatment, immunotherapy or other systemic therapy depends on stage, molecular group, histology, residual disease and patient factors. Fertility-sparing hormonal treatment is exceptional and demands strict eligibility, surveillance and a plan for definitive surgery. This guide uses current European and United States sources for bounded evidence, not as automatic Indian policy. How Common Is It?
Cancer of the uterine corpus is an important gynaecological malignancy whose incidence varies widely between populations. The IARC India country sheet provides modelled estimates from subnational registry inputs and lists corpus uteri among the country's cancer sites, but those estimates are not a complete national case count. The term corpus uteri in population data also does not guarantee that every registered tumour is an endometrial carcinoma, because some uterine sarcomas and incompletely specified cancers may be included. Burden statements must preserve that distinction.
Incidence has risen in many settings alongside ageing and metabolic risk, but trends cannot be attributed to body weight alone. Changes in hysterectomy prevalence, reproductive patterns, hormone exposure, diagnostic access, pathology classification and registration also alter recorded rates. NCI's United States statistics are inappropriate as Indian prevalence estimates. Within India, state, urban-rural, socioeconomic and healthcare-access differences can affect presentation and outcome, while registry coverage and methods limit comparisons.
The disease burden extends beyond incidence. Diagnostic delay can allow anaemia, pain or advanced spread; treatment may produce infertility, surgical menopause, lymphoedema, neuropathy, bowel or bladder effects and financial toxicity. People with Lynch syndrome face risks of other malignancies and implications for relatives. Because bleeding commonly appears early, health systems can improve outcome through rapid evaluation of postmenopausal bleeding and persistent abnormal bleeding rather than population screening of asymptomatic average-risk women. The practical message is not that every bleed is cancer, but that a symptom with a broad benign differential still deserves a reliable diagnostic endpoint.
Risk Factors
Prolonged endometrial exposure to oestrogen without adequate progestogenic opposition promotes hyperplasia and endometrioid carcinogenesis. Clinical associations include obesity, chronic anovulation such as polycystic ovary syndrome, nulliparity, earlier menarche, later menopause and oestrogen-only systemic menopausal therapy in a person with an intact uterus. Tamoxifen can increase endometrial malignancy risk, but it may provide substantial breast-cancer benefit; abnormal bleeding warrants assessment rather than unilateral cessation. Diabetes, metabolic syndrome and advancing age are associated with risk but are not diagnostic tests. Counselling must avoid blame, particularly around weight and fertility history.
Atypical endometrial hyperplasia, also termed endometrial intraepithelial neoplasia in relevant classifications, may coexist with carcinoma or progress to it. Prior pelvic radiation and some ovarian oestrogen-producing tumours are additional contexts. Combined hormonal contraception and pregnancy are associated with lower population risk, but neither confers immunity and neither is prescribed solely as a cancer guarantee. Protective and adverse associations should not override symptoms.
Lynch syndrome is the most important inherited predisposition. A personal history of colorectal or another Lynch-associated cancer, endometrial cancer at a young age, synchronous cancers, or relatives with early colorectal, endometrial, ovarian, gastric, urinary-tract or other related tumours should trigger a structured pedigree and genetics pathway. Tumour mismatch-repair testing can help identify hereditary risk but is not identical to germline diagnosis. Cowden syndrome is uncommon but relevant. A family history without a recognised syndrome can also increase risk. Risk-reducing surgery and surveillance decisions for pathogenic-variant carriers require specialist genetics and gynaecology counselling, not a generic web schedule.
Diagnosis
History
Clarify postmenopausal, intermenstrual, prolonged, heavy or otherwise changed bleeding; watery or blood-stained discharge; pelvic pressure or pain; abdominal distension; early satiety; urinary or bowel symptoms; weight change and functional decline. Establish the last menstrual period, pregnancy possibility, contraception, hormone therapy, tamoxifen, prior hyperplasia or polyps, previous sampling, cervical screening, parity and fertility wishes. Record diabetes, hypertension, metabolic disease, anticoagulation, anaemia symptoms, cancer history and a three-generation Lynch-focused family history. Distinguish vaginal bleeding from haematuria or rectal blood.
Examination
Assess pulse, blood pressure, pallor, body habitus relevant to procedural risk, abdomen, pelvic masses, ascites, lymph nodes and leg swelling. With consent and a chaperone, inspect the vulva, vagina and cervix to identify another bleeding source. Bimanual examination may assess uterine size, mobility, adnexa and tenderness but cannot exclude endometrial disease. A normal pelvic examination does not close the pathway. Active major bleeding or severe pain changes priority from routine assessment to stabilisation.
Investigations
Exclude pregnancy when possible and relevant; obtain blood count and organ-function tests according to bleeding and planned procedures. Transvaginal ultrasound assesses endometrial thickness and morphology, uterus and adnexa, but thresholds depend on menopausal and treatment context. Definitive diagnosis requires endometrial sampling by office biopsy, hysteroscopy-directed biopsy or curettage when indicated. Persistent bleeding after a reassuring or insufficient sample requires reassessment. Histopathology reports histotype, grade and relevant invasion after surgery. Mismatch-repair proteins or microsatellite status, p53 and POLE testing support molecular classification where available. Pelvic MRI and CT of chest, abdomen or pelvis are selected for local or distant staging and treatment planning rather than ordered mechanically for every pre-biopsy bleed.
Differential Diagnosis
Postmenopausal bleeding is commonly caused by vulvovaginal or endometrial atrophy, but cancer must be excluded through the correct pathway. Endometrial polyps and hyperplasia can produce intermittent bleeding. Cervical cancer, a cervical polyp, cervicitis, vaginal or vulval malignancy, trauma and infection may be identified on examination. Haematuria from infection, stone or urinary cancer and rectal bleeding from haemorrhoids or colorectal disease can be mistaken for vaginal blood. Anticoagulants may amplify bleeding from an underlying lesion and do not constitute a complete diagnosis.
Before menopause, pregnancy, miscarriage and ectopic pregnancy take priority where conception is possible. Ovulatory dysfunction, fibroids, adenomyosis, polyps, thyroid disease, coagulopathy and contraceptive-related bleeding are frequent alternatives. Persistent intermenstrual bleeding, failed treatment or risk of unopposed oestrogen changes the threshold for sampling. Endometrial thickness varies across the menstrual cycle, so a postmenopausal ultrasound rule cannot be transferred to a cycling patient.
Within malignant disease, distinguish endometrial carcinoma from uterine leiomyosarcoma or endometrial stromal sarcoma, cervical adenocarcinoma extending upward, ovarian or tubal cancer involving the uterus, and metastasis. Carcinosarcoma is managed as an aggressive endometrial carcinoma in modern oncological frameworks despite its biphasic appearance. Histological mimics and mixed tumours warrant expert pathology review when morphology, immunohistochemistry and clinical findings conflict.
Advanced endometrial cancer may present as ascites, omental or nodal disease, lung symptoms or bone pain rather than bleeding. Conversely, a benign biopsy does not explain persistent symptoms if the sample was scant, the cavity contains a focal lesion or the bleeding source was never observed. The diagnostic endpoint is concordance among symptom pattern, imaging, visual assessment and adequate pathology.
Management
A gynaecological-oncology multidisciplinary team should review confirmed cancer with pathology, molecular results where available, imaging, operative fitness, fertility goals and patient priorities. For disease thought confined to the uterus, minimally invasive total hysterectomy with bilateral salpingo-oophorectomy is commonly preferred when oncologically suitable. Sentinel-node mapping or lymph-node assessment is risk-adapted; systematic dissection is not a ritual for every low-risk tumour. Avoid uncontained uterine morcellation when malignancy is known or suspected. Final pathology must document myometrial invasion, cervical stromal involvement, lymphovascular space invasion, adnexal or serosal spread, nodes, margins, histotype, grade and molecular features.
Adjuvant decisions now integrate anatomical stage with biology. Some low-risk cancers need no further treatment after complete surgery. Increasing risk may justify vaginal brachytherapy, external-beam radiotherapy, chemotherapy, combined treatment or a trial. The 2025 ESGO-ESTRO-ESP framework is European and assumes pathology, molecular testing, radiotherapy and systemic options that may not be uniformly accessible in India; it supports principles and risk dialogue, not automatic copying. Advanced disease may require cytoreductive surgery when complete macroscopic resection appears feasible, systemic therapy, radiotherapy, endocrine treatment in selected hormone-sensitive disease, immunotherapy according to molecular status, or symptom-directed palliation.
Fertility-sparing management is restricted to rigorously selected, usually low-grade endometrioid disease without myometrial invasion or extrauterine spread after expert pathology and imaging. It uses progestin-based therapy with scheduled hysteroscopic or endometrial reassessment; non-response, progression or completion of childbearing triggers definitive surgery. Across all pathways, optimise diabetes, blood pressure, thrombosis risk and nutrition without delaying necessary cancer care. Address menopause, bone and cardiovascular health, sexual function, lymphoedema, obesity stigma, mental health and family-genetic implications. Follow-up is symptom- and risk-informed, with rapid evaluation of recurrent bleeding, pain, cough, weight loss, swelling or neurological symptoms.
Prescribing Information
Systemic anti-cancer medicines and fertility-sparing hormonal treatment require protocol-level prescribing, pharmacy verification and specialist surveillance. Before therapy, confirm pathology, stage, molecular and receptor information relevant to the proposed drug, treatment intent, previous exposure, performance status, pregnancy status or potential, allergies, body measurements where required, full blood count, renal and hepatic function, comorbidities and interactions. Document cycle, route, dose calculation, supportive medicines, laboratory limits and criteria for delay, reduction, cessation or emergency review. This educational summary provides no patient-specific dose.
Platinum-taxane chemotherapy is a common systemic backbone in higher-risk, advanced or recurrent disease, but regimen, sequence and combination with radiation or immunotherapy depend on the current specialist protocol. Immune checkpoint inhibitors have molecularly and line-specific roles and can cause inflammatory toxicity in endocrine organs, bowel, liver, lung, skin, nervous system or heart. Endocrine treatments may be considered for selected receptor-positive, low-volume or indolent disease; thrombotic, metabolic and hormonal adverse effects still require review. Targeted agents have product-specific cardiovascular, bleeding, wound-healing and interaction risks.
Progestin-based fertility-sparing treatment is not ordinary contraception. Verify the exact oral or intrauterine product, dose, adherence and surveillance timetable, and explain that treatment may fail or conceal progression. Oestrogen-only systemic therapy in a person with an intact uterus requires immediate regimen verification, but abrupt changes should follow clinical assessment. During cytotoxic treatment, fever or deterioration activates the local neutropenic-sepsis pathway. Urgent symptoms include dyspnoea, chest pain, unilateral swelling, uncontrolled vomiting, severe diarrhoea, jaundice, reduced urine, confusion, heavy bleeding or infusion reaction. Provide a 24-hour contact route and review all supplements or traditional medicines for interactions rather than assuming they are harmless.
When to Refer
Refer any postmenopausal bleeding into a defined gynaecology pathway, even when it is a single episode, scant spotting or occurs during hormone or tamoxifen treatment. Prompt referral is also appropriate for persistent intermenstrual bleeding, abnormal discharge, treatment-resistant abnormal uterine bleeding, an endometrial lesion, inadequate sampling with continuing symptoms, atypical hyperplasia or a suspicious pelvic finding. The referral should state menstrual and menopausal status, pregnancy result where relevant, exact bleeding pattern, examination, haemoglobin, ultrasound report, hormone or tamoxifen exposure, previous pathology, metabolic and anaesthetic risks, family history and fertility priorities.
After cancer is confirmed, refer to a gynaecological-oncology service before non-oncological hysterectomy or morcellation. Transfer pathology slides or blocks and the original report for expert review when needed. Stage-dependent care may require radiology, specialist surgery, radiation oncology, medical oncology, genetics, fertility, palliative care and rehabilitation. Mismatch-repair loss, young presentation or a strong Lynch-pattern pedigree warrants genetics referral; tumour testing is not itself a germline result and relatives should not be labelled carriers without appropriate confirmation.
Emergency assessment is necessary for haemodynamic instability, major ongoing bleeding, sepsis, bowel obstruction, acute urinary obstruction, venous thromboembolism, severe uncontrolled pain, spinal-cord features or life-threatening treatment toxicity. In India, referral must be operational: confirm the receiving centre's pathology, molecular, surgical, radiotherapy and systemic-treatment capability; avoid duplicating unaffordable tests; and provide navigation for transport, accommodation and financial schemes. A written safety net and named contact are particularly important when appointments require long-distance travel.
Red Flags
Heavy vaginal bleeding with syncope, tachycardia, hypotension, angina, dyspnoea or confusion requires resuscitation and urgent gynaecological care. Obtain venous access, full blood count, cross-match and pregnancy testing when relevant while activating the local haemorrhage pathway. Severe pelvic pain, fever, offensive discharge, peritonism or haemodynamic compromise raises concern for infection, necrosis, perforation or another acute abdomen. Do not wait for outpatient biopsy when the patient is unstable.
New unilateral leg swelling, pleuritic pain, haemoptysis or acute breathlessness may indicate venous thromboembolism, a risk increased by malignancy, surgery, immobility and some systemic treatments. Progressive abdominal distension, vomiting, constipation and obstipation can signal bowel obstruction. Reduced urine output, flank pain or rising creatinine requires assessment for urinary obstruction or treatment-related injury. Severe back pain with weakness, saddle numbness or bladder and bowel dysfunction demands emergency spinal or cauda equina evaluation. Neurological deficit, seizure or altered consciousness may represent metastasis, metabolic disturbance, thrombosis or toxicity.
During chemotherapy or immune therapy, fever, rigors, hypotension, severe diarrhoea, jaundice, chest pain, new cough, hypoxia, endocrine collapse or rapidly progressive rash must follow the oncology emergency plan. Wound dehiscence, increasing postoperative pain, redness, discharge or calf swelling needs urgent review. Recurrent post-treatment bleeding, new pelvic pain, weight loss, cough, bone pain or lymphoedema should not be deferred to a routine appointment. Psychosocial danger also matters: suicidal thoughts, intimate-partner violence, inability to obtain food or medicines, and anticipated abandonment of treatment for cost need direct support and escalation.
Indian Clinical Context
India does not have a single nationwide population-screening pathway for endometrial cancer comparable with the public cervical-screening framework. In average-risk people without symptoms, routine ultrasound or endometrial biopsy is not an evidence-based substitute for symptom awareness. The high-value public message is to seek assessment for postmenopausal bleeding and persistent abnormal bleeding. Clinics must then provide timely transvaginal ultrasound, hysteroscopy or adequate sampling rather than leaving a positive symptom without diagnostic completion.
Access to gynaecological oncologists, minimally invasive surgery, sentinel-node mapping, expert pathology, molecular classification, radiotherapy and newer systemic medicines varies widely. Where the full molecular classifier is unavailable, pathology should honestly record what was and was not tested; treatment should use the best available validated risk information rather than invent a molecular category. Tissue conservation and referral for mismatch-repair testing can have both treatment and hereditary implications. Foreign recommendations may assume equipment, medicines, reimbursement and follow-up capacity that differ in Indian public and private systems.
Comorbidity is particularly important. Diabetes, hypertension, obesity, anaemia and thrombotic risk can complicate surgery and systemic therapy, yet should trigger optimisation and appropriate facility selection rather than denial or blame. Many patients must travel repeatedly and may prioritise fertility, caregiving, wages and out-of-pocket cost differently from a guideline panel. Confirm the feasible plan, pathology transfer, treatment sequence and emergency contact before discharge. None alone constitutes an Indian national prescription, and all require reconciliation with current Indian expert practice and local resources.
NMC Competency Mapping
NMC Obstetrics and Gynaecology competency OG34.1 anchors undergraduate learning on carcinoma of the endometrium. It requires learners to describe and discuss aetiology, pathology, clinical features, investigations, staging and principles of management. OG32.2, which addresses causes and management of postmenopausal bleeding, is an important clinical bridge: the learner must recognise the symptom, construct a differential and arrange investigation rather than diagnosing cancer from age alone.

Biopsy, hysteroscopy, cancer surgery and anti-cancer prescribing require credentialled supervision. Key Exam Pearls for NEET PG
Postmenopausal bleeding is the classic presentation of endometrial cancer and must be evaluated even after one episode. Endometrioid adenocarcinoma is the commonest histology and is linked to prolonged unopposed-oestrogen states such as obesity, chronic anovulation, nulliparity, late menopause and oestrogen-only therapy with an intact uterus. Tamoxifen increases risk but should not be stopped reflexively. Lynch syndrome links endometrial cancer with colorectal and several other malignancies. Atypical hyperplasia or endometrial intraepithelial neoplasia can coexist with carcinoma and is a precursor requiring specialist management.
Transvaginal ultrasound is a triage investigation; histological sampling establishes diagnosis. Standard surgery for operable confined disease is total hysterectomy with bilateral salpingo-oophorectomy, with a risk-adapted nodal approach. Adjuvant treatment ranges from observation through vaginal brachytherapy, external-beam radiation, chemotherapy or combined treatment according to risk. Advanced or recurrent disease may use systemic, endocrine, immune, radiation, surgical or palliative strategies. Fertility preservation is exceptional, generally restricted to carefully selected low-grade endometrioid disease without myometrial invasion, and requires progestin therapy plus repeated tissue surveillance. Major bleeding, thrombosis, obstruction, neutropenic sepsis and immune toxicity are emergencies.
Frequently Asked Questions
Does every episode of postmenopausal bleeding mean endometrial cancer?
No. Atrophy, polyps and other benign causes are common, but the symptom cannot be safely classified by appearance or amount alone. Even one episode should enter a defined assessment pathway so that cervical, vaginal, endometrial and non-gynaecological sources are considered and malignancy is excluded.
Can a thin endometrium on ultrasound completely rule out endometrial cancer?
A reassuring measurement can make cancer less likely in the correct postmenopausal context, but it is not absolute. Persistent or recurrent bleeding, poor visualisation, tamoxifen exposure, a focal lesion or discordant clinical findings can require hysteroscopy or tissue sampling despite the measurement.
Why is molecular testing increasingly important after endometrial cancer is diagnosed?
POLE mutation, mismatch-repair status and p53 findings help classify tumours into biologically meaningful groups that refine prognosis and may alter adjuvant or systemic treatment. Testing availability varies, and a partial panel must be reported honestly; mismatch-repair loss can also prompt evaluation for Lynch syndrome.
Can endometrial cancer ever be treated while preserving the uterus?
Only in selected patients, usually with low-grade endometrioid carcinoma confined to the endometrium, after expert pathology, imaging and fertility counselling. Progestin-based treatment requires scheduled repeat tissue assessment, and hysterectomy is advised for non-response, progression or after childbearing because this is not equivalent to standard definitive surgery.
