Summary
Dyspepsia in pregnancy describes upper gastrointestinal symptoms such as epigastric discomfort, post-meal fullness, heartburn, acid regurgitation, belching and nausea; it is a symptom complex, not a final diagnosis. Progesterone-mediated relaxation of the lower oesophageal sphincter, altered motility and the mechanical effect of the enlarging uterus make reflux particularly common as gestation advances. A reassuring pattern is meal- or posture-related burning or regurgitation without systemic illness, bleeding, dysphagia, persistent vomiting or focal abdominal tenderness. Pregnancy must never be used to explain away new severe epigastric or chest pain. The initial task is to check stability and gestation, identify gastrointestinal bleeding and distinguish uncomplicated reflux from hyperemesis, biliary or pancreatic disease, hepatitis, acute coronary syndrome, pulmonary embolism and, after 20 weeks, pre-eclampsia or HELLP syndrome. Management is stepwise: individualised dietary and positional measures, then a pregnancy-compatible antacid or alginate, followed when necessary by clinician-selected acid suppression such as an H2-receptor antagonist or proton-pump inhibitor. Omeprazole has the largest pregnancy safety experience among PPIs, but prescribing should account for indication, interactions and local formulary. Separate antacids from iron and folic acid. How Common Is It?
Heartburn and indigestion are among the common symptoms of pregnancy, although a single reliable Indian prevalence figure cannot be inferred because studies use different definitions, trimesters and populations. Symptoms may occur at any stage and become more frequent after the first trimester as hormonal effects are joined by increasing intra-abdominal pressure. Previous reflux, later gestation and prior pregnancy make symptoms more likely. The high background frequency creates a diagnostic trap: prevalence increases the probability of benign reflux, but does not reduce the danger of an alternative diagnosis in a patient with abrupt, severe or atypical pain. The burden is also clinically meaningful even when no structural disease exists. Night-time reflux may impair sleep; food avoidance can worsen nutritional intake; nausea can interfere with iron, folate or other essential medicines; and repeated self-treatment may cause interactions or excessive sodium exposure. In India, many preparations are bought without prescription and brand names may hide different salts or combinations, so asking to see the packet is more reliable than recording only an antacid brand. Service access ranges from community pharmacy and primary health centre to obstetric triage and tertiary gastroenterology. Clinicians should therefore describe frequency cautiously, assess severity by effect on eating, hydration, sleep and function, and avoid presenting commonness as proof that a symptom is harmless.
Risk Factors
Physiological risk increases with advancing gestation because progesterone reduces lower oesophageal sphincter tone, oesophageal clearance may slow and the enlarging uterus raises gastric pressure. A history of reflux or dyspepsia before conception, symptoms in an earlier pregnancy, multiparity, obesity and large meals may increase susceptibility. Symptoms are often provoked by lying down soon after eating, late-night meals, high-fat foods, personally identified spicy foods, caffeine, smoking and tight clothing; triggers vary and indiscriminate dietary restriction is unnecessary. Review medicines because iron preparations, non-steroidal anti-inflammatory drugs, corticosteroids, calcium-channel blockers and other agents may aggravate upper gastrointestinal symptoms or mucosal injury. Do not advise stopping a prescribed drug without assessing why it is needed. Separate risk of reflux from risk of serious disease. Gallstones are more likely in pregnancy, and obesity, rapid weight change or prior biliary symptoms strengthen that possibility. Hypertension, renal disease, diabetes, autoimmune disease, multiple pregnancy and previous hypertensive pregnancy increase concern for pre-eclampsia, while thrombosis history, immobility and dyspnoea raise concern for pulmonary embolism. Previous ulcer disease, H pylori infection, anticoagulant use or gastrointestinal bleeding changes the investigation threshold. The absence of risk factors does not exclude either obstetric emergencies or ectopic pregnancy early in gestation. A useful assessment therefore records gestational and medical risk alongside the symptom pattern rather than constructing a checklist that labels every high-risk patient as diseased.
Diagnosis
History
Clarify gestation and pregnancy location where relevant, onset, site, character, radiation, relation to food and posture, nocturnal symptoms, regurgitation, vomiting, oral intake and response to remedies. Ask about haematemesis, melaena, dysphagia, odynophagia, weight loss, fever, jaundice, pruritus, diarrhoea, urinary symptoms and medicine or supplement use. Screen deliberately for headache, visual disturbance, facial or hand swelling, reduced fetal movements when applicable, vaginal bleeding, unilateral early-pregnancy pain, breathlessness, pleuritic pain, diaphoresis and exertional discomfort. ### Examination Record temperature, pulse, blood pressure, respiratory rate, oxygen saturation and hydration. Examine heart and lungs, assess pallor and jaundice, and palpate the abdomen for epigastric or right-upper-quadrant tenderness, guarding, masses and uterine findings appropriate to gestation. Check oedema and repeat blood pressure correctly when hypertensive disease is possible. ### Investigations Typical mild reflux without alarm features usually needs none. Target tests to the differential: full blood count, liver and renal profile, lipase, urinalysis and protein assessment, ECG and cardiac testing, or obstetric assessment as indicated. Ultrasound is first-line for suspected biliary disease and obstetric concerns. If testing for H pylori will change management, use a validated breath or stool-antigen pathway and account for PPI-related false negatives. Endoscopy is reserved for bleeding, obstruction, significant dysphagia, persistent unexplained symptoms or another compelling indication after obstetric, anaesthetic and gastroenterology planning.
Differential Diagnosis
Uncomplicated gastro-oesophageal reflux usually causes retrosternal burning, sour regurgitation or post-meal epigastric discomfort that worsens supine and improves with antacid treatment. Functional dyspepsia may produce early satiety and upper abdominal discomfort without structural disease, but should not be assumed during a first severe presentation. Hyperemesis gravidarum is dominated by persistent vomiting, failure of intake, dehydration, weight loss and functional impairment, usually beginning before 16 weeks; epigastric burning may be secondary to repeated emesis. Peptic ulcer disease or erosive oesophagitis becomes more plausible with focal pain, melaena, haematemesis, anaemia or NSAID exposure. Biliary colic causes episodic right-upper-quadrant or epigastric pain, often after food and sometimes radiating to the back or shoulder; fever, persistent tenderness or jaundice suggests cholecystitis or obstruction. Pancreatitis produces sustained severe epigastric pain, commonly radiating to the back, with vomiting and systemic illness. After 20 weeks, epigastric or right-upper-quadrant pain with hypertension, headache, visual symptoms, thrombocytopenia or abnormal liver tests may represent severe pre-eclampsia or HELLP and needs urgent obstetric assessment. Acute fatty liver is a late-pregnancy systemic illness with nausea, abdominal pain, jaundice, hypoglycaemia or coagulopathy. Pulmonary embolism and acute coronary syndrome can present with chest or epigastric discomfort, dyspnoea, tachycardia, syncope or diaphoresis. Early-pregnancy pain also requires consideration of ectopic pregnancy, especially with bleeding or an unconfirmed intrauterine pregnancy.
Management
First treat instability or the alternative diagnosis; do not conduct a prolonged reflux trial in a patient with haemorrhage, severe hypertension, hypoxaemia, peritonism or shock. For a stable typical pattern, explain the pregnancy physiology and agree realistic goals: comfortable eating, adequate sleep and retention of necessary medicines. Suggest smaller meals, avoiding lying down for about three hours after eating, remaining upright after meals, elevating the head and shoulders for nocturnal symptoms and trying the left lateral position. Reduce only foods or drinks that reproducibly trigger symptoms; avoid nutritionally restrictive lists. Smoking cessation and alcohol avoidance have wider pregnancy benefits. If measures are insufficient, consider a locally available pregnancy-compatible antacid or alginate. Check the actual ingredients, renal function, sodium load and other medicines, and separate an antacid from iron or folic acid by at least two hours because absorption can be reduced. Persistent symptoms justify clinician review for an H2-receptor antagonist or PPI rather than repeated uncontrolled over-the-counter use. Reassess adherence, timing and diagnosis before escalation. Omeprazole is commonly preferred when a PPI is needed because pregnancy exposure data are most extensive. Use the lowest effective regimen for the shortest clinically appropriate period, while recognising that ulcer disease or severe oesophagitis may require sustained therapy. Arrange follow-up rather than issuing indefinite repeats. Provide written advice on bleeding, dysphagia, persistent vomiting, severe pain, hypertension symptoms, breathlessness and reduced fetal movement.
Prescribing Information
Antacids neutralise gastric acid and alginates form a reflux barrier; both act quickly but do not diagnose the cause. Product composition matters. Ask a pharmacist or obstetric prescriber to select a preparation suitable for pregnancy and comorbidity rather than treating every antacid as interchangeable. Avoid excessive or prolonged unsupervised use, account for sodium in patients with hypertension or oedema, and consider magnesium or aluminium accumulation in significant renal impairment. Sodium bicarbonate and older high-dose magnesium trisilicate preparations should not be routine pregnancy choices. Antacids can reduce absorption of iron, folate and several other medicines, so use a two-hour separation from antenatal supplements and check medicine-specific intervals. If an H2-receptor antagonist is chosen, use a product supported by current local formulary and interaction guidance; famotidine is a common option where available. For a PPI, UKTIS/BUMPS reports no good evidence that omeprazole or other PPIs increase miscarriage, congenital anomaly, stillbirth, preterm birth or low birth weight, while acknowledging limitations of observational evidence. Omeprazole is preferred when clinically appropriate because it has more safety information. Do not promise zero fetal risk. Review interacting medicines and confirm that symptoms justify treatment. H pylori eradication requires a combination regimen whose antibiotic and sometimes bismuth components need pregnancy-specific scrutiny; do not copy a non-pregnant regimen or treat on serology alone. Defer eradication when clinically safe, or obtain obstetric, gastroenterology and antimicrobial advice when ulcer complications make treatment necessary.
When to Refer
Send the patient for immediate emergency or obstetric assessment when there is haematemesis, melaena with circulatory concern, syncope, hypotension, tachycardia with pallor, severe or escalating chest or epigastric pain, guarding, hypoxaemia, severe breathlessness, jaundice with systemic illness, or suspected pre-eclampsia or HELLP. After 20 weeks, new epigastric or right-upper-quadrant pain accompanied by headache, visual disturbance or raised blood pressure belongs in an obstetric pathway on the same day. Early-pregnancy pain or bleeding with no confirmed intrauterine pregnancy needs an ectopic-pregnancy pathway. Refer urgently to gastroenterology or acute medicine for significant acute gastrointestinal bleeding, progressive dysphagia, persistent vomiting, unexplained anaemia, weight loss, a palpable mass or symptoms that remain unexplained after an appropriate initial assessment. A biliary pattern with fever, jaundice or continuous pain needs surgical or medical assessment rather than routine reflux follow-up. Non-urgent specialist review is reasonable for refractory symptoms despite correct use of stepwise treatment, recurrent ulcer disease, suspected H pylori requiring pregnancy-specific treatment, or a compelling indication for endoscopy. Referral timing must reflect distance and transport. In an Indian rural or remote setting, a stable patient can deteriorate during transfer; communicate with the receiving facility, document vital signs, secure appropriate accompaniment and continue indicated resuscitation. Referral is not a substitute for stabilisation, and a temporary response to antacid does not rule out cardiac, biliary or obstetric disease.
Red Flags
Red flags are action triggers, not items to collect before acting. Gastrointestinal danger features include vomiting blood, coffee-ground emesis, black tarry stool, progressive dysphagia, painful swallowing, persistent vomiting, inability to retain fluid, unexplained anaemia or weight loss, severe focal tenderness, guarding and abdominal distension. Haemodynamic danger includes collapse, confusion, cool peripheries, oliguria, tachycardia or hypotension; normal blood pressure early in haemorrhage does not guarantee safety. Obstetric danger after 20 weeks includes severe or persistent epigastric or right-upper-quadrant pain, blood pressure elevation, severe headache, visual symptoms, sudden swelling, reduced fetal movements, bleeding or seizures. Consider HELLP even when the presenting label is indigestion, and obtain platelets and liver tests when clinical concern exists. Cardiopulmonary red flags include breathlessness, pleuritic or exertional chest pain, hypoxaemia, haemoptysis, unilateral leg swelling, diaphoresis or syncope. Hepatobiliary and pancreatic warning signs include jaundice, fever, pain radiating to the back, peritonism and systemic toxicity. In the first trimester, unilateral pelvic or lower abdominal pain, shoulder-tip pain, vaginal bleeding, dizziness or an unlocated pregnancy raises concern for ectopic pregnancy. A symptom that is new, unusually intense, unrelated to meals, resistant to reasonable therapy or accompanied by maternal concern deserves reassessment. Safety-net instructions should specify where to go at any hour, not merely advise the patient to return if worse.
Indian Clinical Context
Care must work across pharmacies, sub-centres, primary health centres, private clinics, district hospitals and tertiary obstetric units. Over-the-counter availability means patients may take several differently branded mixtures containing overlapping antacids, alginates, local anaesthetic, prokinetic or acid-suppressing drugs. Reconcile ingredients, dose frequency and traditional or herbal products without judgement. Product quality, sodium content and interaction advice should be checked rather than inferred from the brand. Iron and calcium use is common in antenatal programmes; explicitly scheduling antacid away from iron and folic acid may improve both symptom control and supplement absorption. H pylori prevalence and antimicrobial resistance make imported eradication recipes unsafe. Testing and therapy should follow local laboratory performance and current antimicrobial policy, with pregnancy-compatible agents chosen by obstetric and gastrointestinal teams when treatment cannot wait. Upper gastrointestinal endoscopy and emergency ultrasound may be unavailable locally. Referral plans should therefore account for travel time, cost, escort, blood availability and the ability to return. Teleconsultation can support decision-making but cannot replace examination of severe pain or abnormal vital signs. Give advice in the patient's preferred language, include family only with consent and avoid normalising suffering as an inevitable part of pregnancy. Record gestation, blood pressure, symptom pattern, oral intake, medicines, alarm features, plan and emergency route. Exact prescribing, pre-eclampsia assessment and transfer procedures remain governed by the state, institution and current obstetric formulary.
