How Common Is It?

Hypertension may coexist with diabetes, central adiposity, dyslipidaemia, smoking or reduced kidney function, creating greater absolute risk than any isolated abnormality suggests. South Asian populations often develop diabetes and cardiovascular disease at younger ages and lower body mass index than some populations used to derive older Western equations. That observation supports earlier attention to risk factors; it does not justify adding an arbitrary multiplier to a calculator.
The Ministry of Health and Family Welfare's NP-NCD operational framework describes population-based screening and longitudinal care for adults through India's primary-care system. Screening identifies people who need confirmation and management; it is not the same as a completed risk assessment. Record tobacco form, duration and recency of cessation; hypertension and treatment; diabetes type and duration; kidney disease; pregnancy-related vascular history; inflammatory disease; HIV; severe mental illness; medicines; diet, activity and alcohol; and premature cardiovascular events or markedly raised cholesterol in relatives. Ask what the person understands and what prevention outcomes matter to them.
Examination
Measure blood pressure using a validated device, correct cuff and standardized technique, repeating or confirming outside the clinic according to the hypertension pathway. Record pulse, weight and an appropriate adiposity measure. Look for signs that change the pathway: tendon xanthomata, corneal arcus at an unusually young age, absent peripheral pulses, vascular bruits, heart failure, endocrine disease or treatment toxicity. Examination does not diagnose future cardiovascular disease, but it can reveal established disease or secondary causes that make a generic risk score inappropriate.
Investigations

Differential Diagnosis
A high estimated risk is not a diagnosis of occult coronary disease. Separate asymptomatic primary prevention from established atherosclerotic cardiovascular disease, which follows secondary-prevention pathways. Chest pressure, exertional dyspnoea, focal neurological symptoms, claudication or acute vascular symptoms require diagnostic assessment rather than a calculator. Similarly, severe blood-pressure elevation with acute target-organ injury is an emergency; it is not managed by scheduling a routine risk review.
Distinguish common polygenic risk from familial hypercholesterolaemia and other inherited lipid disorders. Explain that relative risk reduction produces a larger absolute benefit when baseline risk is higher. Explore priorities, health literacy, cost, medicine access and readiness; document a follow-up plan rather than issuing generic lifestyle advice.
Support complete tobacco cessation, avoidance of smokeless tobacco and second-hand smoke, a diet emphasizing vegetables, fruit, pulses, whole grains, nuts and unsaturated fats while limiting trans fat, excess saturated fat, salt and highly processed foods, and regular physical activity adapted to ability. Address weight without stigma. Confirm and treat hypertension, diabetes and chronic kidney disease through their condition-specific pathways. These measures are complementary; a statin does not neutralize smoking or uncontrolled blood pressure.

Prescribing Information
Statin choice and intensity must follow a current local formulary, indication, age, pregnancy potential, comorbidity, interaction profile and baseline assessment. Before initiation, document the prevention indication, full lipid profile, blood pressure, smoking and alcohol status, diabetes status, renal function and transaminases; obtain thyroid-stimulating hormone when clinically suggested. Ask about unexplained generalized muscle symptoms. A modest stable transaminase elevation or previous non-specific muscle pain should not be converted automatically into permanent statin exclusion, but active liver disease, substantial interaction or suspected myopathy requires individualized review.

Red Flags
Do not continue routine risk scoring when the person has possible acute cardiovascular disease. Central chest pressure, autonomic symptoms, acute breathlessness, syncope with cardiovascular features, new focal neurological deficit, sudden monocular visual loss, a cold painful pulseless limb, or severe hypertension with encephalopathy, pulmonary oedema, acute kidney injury or other target-organ injury needs emergency assessment. A prevention percentage must never be used to rule out an acute event.
Laboratory red flags include a markedly abnormal lipid profile suggestive of an inherited disorder, very high triglycerides with abdominal pain or pancreatitis risk, unexpected renal deterioration, significant transaminase abnormality, or severe hyperglycaemia with acute metabolic symptoms. Clinical clues such as tendon xanthomata, premature corneal arcus, multiple relatives with early coronary disease or sudden death, and cardiovascular disease at a young age require a familial pathway.
During treatment, generalized muscle pain with objective weakness, dark urine or systemic illness requires urgent assessment for myopathy or rhabdomyolysis, especially with interacting medicines.
