Risk clusters around repeated wet work and contact with detergents, solvents, oils, foods, cement, metals, rubber chemicals, resins, plants, cosmetics and topical drugs. Healthcare workers, cleaners, cooks, hairdressers, beauticians, construction workers, mechanics, metal workers, farmers and florists are frequently exposed, although occupation does not prove causation. Domestic work may create the same cumulative water-and-detergent load without appearing on an employment record. Diagnosis
History
Build a timeline linking morphology and location to each exposure cycle. Record first onset, improvement on leave, relapse after return, weekends, seasons, new products, repairs, hobbies and household tasks. Ask the patient to describe a normal shift rather than merely naming a job. List cleansers, sanitizers, gloves, glues, oils, cutting fluids, cement, plants, dyes, cosmetics, footwear, jewellery and every topical medicine. Itch may dominate allergy; burning or stinging may suggest irritation, but neither is definitive. Note atopy, previous reactions, treatment response, contact urticaria, respiratory symptoms and effects on sleep, function and employment.
Examination
Inspect all skin, not only the complaint site, with consent and good light. Map dorsal versus palmar hands, finger webs, wrist cuffs, eyelids, hairline, ears, neck, axillae, feet and areas beneath jewellery or equipment. Acute disease may show oedema, papules, vesicles and ooze; chronic disease may show scale, fissures, hyperkeratosis and lichenification. In richly pigmented skin, erythema may be subtle while violaceous, grey-brown or post-inflammatory colour change is prominent. Sharp borders, sparing patterns, asymmetry and spread beyond direct contact generate hypotheses but do not prove mechanism. Look for crust, pustules, pain, lymphangitis and systemic illness.
Investigations
Patch testing by a trained service is indicated for suspected allergic disease, chronic or recurrent dermatitis, occupational cases and eczema that becomes unexpectedly treatment-resistant. Test a baseline series plus exposure-directed allergens and, when safe, relevant patient products. Standard readings are commonly made after removal and again later; delayed readings may uncover some allergens. Interpret reaction strength, irritancy and current, past or unknown relevance. Do not use patch tests to diagnose irritant dermatitis. Skin scraping or microscopy, bacterial or viral testing, biopsy and immediate-allergy testing are reserved for specific differentials. Differential Diagnosis
Atopic dermatitis can affect hands, face and flexures and may coexist with contact disease. A previous atopic history increases irritant susceptibility but does not explain every occupational flare. Dyshidrotic eczema produces deep vesicles on palms and sides of fingers; hyperkeratotic hand eczema produces thick scale and painful fissures. Neither morphology excludes allergy. Psoriasis tends toward well-demarcated plaques, nail pitting or characteristic disease elsewhere, yet palmar psoriasis may be difficult to distinguish clinically. Tinea is often asymmetric with an active border or associated foot and nail infection; microscopy is preferable to repeated steroid-antifungal combination use.
Scabies should be considered with nocturnal itch, household contacts, burrows or genital and web-space lesions. Impetigo, eczema herpeticum and cellulitis create different urgency. Seborrhoeic dermatitis, rosacea, photoaggravated eruptions, cutaneous lupus and dermatomyositis enter facial or exposed-site differentials. Venous eczema belongs in a swollen lower leg; stasis ulcer dressings and topical antibiotics can then add secondary allergy. Lichen simplex may reflect an itch-scratch cycle, but the trigger still needs investigation.
Contact urticaria occurs within minutes, causes wheals or swelling and may be immunological, for example with latex or food proteins. It is not assessed by ordinary delayed patch testing and systemic symptoms may progress to anaphylaxis. Airborne contact dermatitis must be distinguished from photodermatitis by exposed-site pattern and sparing. Management
Explain the working mechanism and make exposure control concrete. For an irritant, reduce cumulative dose by eliminating unnecessary wet work, lowering concentration, automating or enclosing the process, using tools instead of bare hands and allowing recovery periods. For a relevant allergen, provide its chemical name, synonyms, product categories and safe-substitution method. Ingredient lists and safety data sheets may change, so advice should include how to recheck. An occupational-health or workplace assessment can identify decanted agents, contaminated surfaces and tasks missed in clinic. Job change is a last resort after feasible controls, but prolonged uncontrolled exposure can entrench disease and threaten livelihood.
Restore barrier function with frequent fragrance-free emollient chosen for tolerability and task compatibility. Use a gentle soap substitute where appropriate, lukewarm water and careful drying, especially between fingers. An ointment may suit very dry hands at night; a lighter preparation may be practical during work. Cotton liners can reduce sweat beneath suitable protective gloves, but glove material and breakthrough time must match the chemical. Replace damaged or contaminated gloves. Barrier creams are not invisible gloves and should not justify continued contact with a potent sensitiser.
Suppress active inflammation with a topical corticosteroid of appropriate potency, vehicle, site and duration. Palms often need a stronger preparation than eyelids or flexures, under clinician direction. Wet compresses may soothe acute vesicular disease. Topical calcineurin inhibitors can be useful steroid-sparing options at selected thin-skin sites. Address sleep, pain, fissure care and mental or occupational harm. Systemic immunomodulation or phototherapy is specialist therapy for severe chronic disease after diagnosis, avoidance and topical technique have been revisited.
Prescribing Information
Prescribe the preparation, body site, frequency, course and review point rather than writing only topical steroid. Potency is site-dependent: an underpowered product may prolong inflammation, while excessive potency or duration on face, eyelids, genital skin or flexures increases atrophy and other local harm. Demonstrate fingertip-unit style quantities or another clear application method. Ointments are often useful on dry fissured skin; creams may be preferred for acute or hairy areas but can contain preservatives that themselves sensitise. Check every vehicle and co-prescribed topical for suspected allergens.
Emollients should be bland, acceptable and used often enough to matter. Burning after application may reflect fissured skin, an irritating ingredient or allergy and deserves review. Paraffin-containing products can increase fabric flammability even after drying; counsel about naked flames, smoking, laundering and contaminated dressings. Antihistamines do not remove delayed contact inflammation, although a sedating agent may sometimes be considered briefly for sleep after individual risk assessment. Avoid routine topical antihistamines and sensitising topical antibiotics. Treat bacterial infection only when clinical features support it and follow local antimicrobial guidance; colonisation or ooze alone is not an automatic indication.
A short systemic corticosteroid course is occasionally used for severe widespread allergic disease, but rebound can occur if it is stopped too quickly while exposure continues. Diabetes, infection, hypertension and other risks need assessment. Ciclosporin, methotrexate, alitretinoin where available, or other systemic options require dermatologist selection, baseline tests and monitoring; evidence is often derived from chronic hand eczema rather than pure contact disease. Do not compound multiple potent steroids, antifungals and antibiotics empirically. Prescribing cannot compensate for unidentified exposure.
When to Refer
Refer to dermatology or a cutaneous-allergy service when allergic contact dermatitis is suspected, the eruption is chronic or recurrent, treatment response is poor, the distribution is occupational or unusual, or the person may need patch testing beyond a baseline series. Early referral is valuable for healthcare, construction, salon, manufacturing, agricultural and food workers because delay can allow sensitisation, chronic fissuring and loss of employment. Send photographs, the timeline, prior prescriptions, ingredient labels, safety data sheets and, after checking service instructions, samples or packaging of suspected products.
Occupational-health involvement is appropriate when symptoms track work, controls are inadequate, colleagues are affected or temporary modification is needed. The clinical note should describe tasks, agents, timing, objective distribution and functional restriction without asserting legal causation beyond the evidence. Workplace review can evaluate elimination, substitution, engineering control, wash facilities, glove selection, training and surveillance. Social-work or employment advice may help where informal work or loss of wages threatens adherence.
Urgent same-day assessment is needed for rapidly spreading painful erythema, fever, purulence with systemic illness, extensive blistering, facial swelling, mucosal involvement, ocular symptoms or suspected eczema herpeticum. Immediate emergency care is required for contact urticaria with breathing difficulty, hypotension or widespread systemic features. Refer severe, widespread or function-limiting dermatitis needing systemic treatment, and disease on eyelids, genital skin or severely fissured hands when safe topical management is uncertain. Children, pregnancy and major immunosuppression may alter testing and prescribing decisions. A negative standard series does not end investigation when history strongly implicates an untested occupational or product allergen.
Red Flags
Pain out of proportion, fever, rapidly advancing warmth, lymphangitis, purulent drainage or haemodynamic disturbance suggests invasive infection rather than an uncomplicated eczema flare. Grouped painful vesicles, punched-out erosions or systemic malaise can indicate eczema herpeticum and require urgent antiviral assessment. Widespread epidermal detachment, target lesions, mucosal erosions or a new high-risk medicine points away from simple contact dermatitis toward a severe cutaneous adverse reaction. Eye pain, photophobia, reduced vision or marked periocular swelling needs urgent ocular assessment.
An immediate wheal, lip or tongue swelling, wheeze, dizziness or collapse after latex, food or another contact suggests immediate hypersensitivity. Remove exposure and manage as possible anaphylaxis according to emergency protocols; do not wait for a patch-test appointment. Chemical burns require immediate decontamination using the substance-specific safety process, not neutralisation experiments or topical steroid alone. Cement can cause serious alkaline injury, and contamination trapped in boots or gloves prolongs exposure.
Clinical red flags also include repeated potent steroid use without a diagnosis, skin atrophy, glaucoma risk from periocular application, extensive secondary infection, or prolonged unsupervised combination creams. Occupational coercion, inability to access clean water, unaffordable protective equipment and threatened job loss affect safety and require documentation. If several coworkers develop similar disease, consider a shared exposure and notify appropriate workplace channels. Persistent dermatitis despite apparently complete allergen avoidance should trigger review of synonyms, cross-reactors, hidden exposure, glove allergy, mixed irritation and alternative diagnoses rather than escalation to indefinite immunosuppression.

Indian exposure assessment should be local and task-specific. Wet cement can combine alkaline irritation with chromate allergy; construction workers may handle it without intact boots or gloves. Farmers may contact pesticides, fertilisers, rubber, plants and airborne Parthenium. Healthcare and domestic workers may have intense handwashing, sanitizer, detergent and glove exposure.
