Summary
Infantile colic describes recurrent, prolonged crying, fussing or irritability in an otherwise healthy young infant when no obvious cause is found and caregivers cannot reliably prevent or settle the episodes. Rome IV clinical criteria require that symptoms start and stop before five months of age and that there is no failure to thrive, fever or illness. The older rule of threes is useful historically but is not required for ordinary clinical diagnosis; its time thresholds now mainly standardize research. Colic is therefore a diagnosis made after competent history and examination, not a label attached to every evening cry.
Crying commonly rises in early infancy, peaks around six to eight weeks and then falls, usually resolving by three to four months. The baby is well between episodes, feeds and grows appropriately, and examination is normal. Fever, lethargy, abnormal responsiveness, bilious or forceful vomiting, blood in stool, distension, poor feeding, dehydration, faltering growth, a weak or high-pitched continuous cry, respiratory difficulty, bruising or neurological signs demand another pathway.
Management begins with listening, observed feeding where possible, explanation of natural history, practical soothing options and a caregiver safety plan. Continuing breastfeeding is encouraged. Routine special formula changes, acid suppression, antispasmodics, simethicone, gripe water and indiscriminate maternal exclusion diets are unsupported or potentially harmful. A cow's-milk-protein elimination trial belongs only when additional allergy features make that diagnosis plausible and must include planned reintroduction or oral challenge. Selected strain-specific probiotics have only weak evidence in breastfed infants and uncertain benefit in formula-fed infants. Caregiver exhaustion and risk of shaking are clinical priorities, not footnotes.
How Common Is It?
Reported prevalence varies widely because studies use different definitions, ages, diaries and caregiver thresholds. Older research based on crying for at least three hours daily, three days weekly and three weeks identified a narrower group than Rome IV clinical practice. Cultural expectations, family support, feeding patterns, recall and access to care also influence who receives the label. A precise Indian national prevalence should not be asserted without representative contemporary surveillance; clinic and urban survey estimates cannot be generalized to all families.
The developmental pattern is more reliable than a single percentage. Crying normally increases during the first weeks after birth, often clusters in late afternoon or evening, peaks at about six to eight weeks and declines thereafter. Colic typically resolves by three to four months and, by Rome IV definition, begins and ends before five months. It can occur in breastfed and formula-fed infants and does not prove poor milk supply, abnormal parenting, intestinal damage or later behavioural disease.
Although self-limited for the infant, the family burden can be severe. Repeated sleepless episodes can amplify anxiety, postpartum depression, conflict, unsafe sleep practices, early breastfeeding cessation, unnecessary formula expenditure, repeated investigations and exposure to ineffective medicines. The number of hours cried does not measure caregiver resilience; a family with limited support may need urgent help even when the baby's examination is reassuring.
Clinicians should distinguish population reassurance from individual assessment. New crying in a neonate, a change from the infant's established pattern, or symptoms outside the expected age range has a different pre-test probability from a familiar evening pattern in a thriving ten-week-old. Colic remains common, but uncommon serious illness must be considered each time red flags or a changed pattern appears.
Risk Factors
No single proven cause explains infantile colic. Proposed mechanisms include developmental regulation of crying and sleep, immature gut signalling, altered motility, microbiome differences and caregiver-infant interaction under stress, but these are not validated bedside diagnostic tests. Parents should not be told that ordinary gas, their emotional state or one food definitely caused the crying. Tobacco smoke exposure is harmful for many reasons and has been associated with excessive crying in some studies, yet avoiding smoke is a general infant-safety intervention rather than a guaranteed colic cure.
Risk of harm often matters more than risk of developing colic. Prematurity, neonatal illness, poor weight gain or feeding difficulty lower the threshold for paediatric reassessment because crying may accompany disease. First-time parenthood, sleep deprivation, social isolation, financial stress, maternal pain, postpartum depression and an absent overnight support person increase caregiver vulnerability. Ask who can safely take over, not merely whether the family has relatives.
Feeding factors require nuance. Poor latch, fast milk flow, excessive air swallowing, overfeeding, rigid schedules or inappropriate formula preparation may cause distress but should be demonstrated through history or observed feed rather than assumed. Cow's-milk protein allergy can include crying, but isolated colic without eczema, blood or mucus in stool, vomiting, diarrhoea, food refusal, faltering growth or another allergic feature has low specificity. Lactose intolerance is uncommon as a primary cause in a thriving young infant; congenital lactase deficiency is a severe diarrhoeal illness, not typical evening colic.
Iatrogenic risks include serial formula switches, stopping breastfeeding, diluting feeds, herbal preparations of uncertain composition, honey, unsafe water, acid-suppressing medicine, sedating antihistamines, anticholinergic drugs and forceful manipulation. Repeatedly telling a distressed caregiver that nothing is wrong without offering support can increase risk of shaking or unsafe sleep.
Diagnosis
Infantile colic is a clinical diagnosis in a well, normally growing infant after age-appropriate exclusion of illness and injury. Rome IV replaces rigid counting for ordinary practice with a pattern-based definition, while formal research still uses caregiver report and a prospective diary threshold.
History
Clarify age at onset, timing, duration, consolability and whether the baby returns to baseline. Ask fever, poor feeding, weak suck, vomiting including bilious or projectile episodes, stool blood or mucus, diarrhoea, constipation, urine output, rash, cough, breathing difficulty, falls, medicines and sick contacts. Review birth, prematurity, newborn screening, immunization and growth. Take a precise feeding history: breast or formula, latch, let-down, volumes, intervals, preparation, water source, bottle flow and recent changes. Ask directly how caregivers cope, whether anyone fears losing control, and whether the infant has ever been shaken.
Examination
Observe colour, interaction, cry, consolability, hydration, temperature, respiratory effort and a feed when feasible. Plot weight, length and head circumference. Examine fontanelle, eyes, mouth, ears, heart, lungs, abdomen, groins, genitals, anus, skin, digits for hair tourniquet and the neurological and musculoskeletal systems. Undress the infant fully enough to detect bruising, swelling, hernia or injury.
Investigations
A thriving infant with a classic pattern and normal examination usually needs no laboratory or imaging test. Test only a specific alternative: urine for suspected UTI, glucose or electrolytes for an unwell infant, stool assessment for significant bleeding or diarrhoea, or imaging for obstruction, pyloric stenosis, intussusception, injury or another defined concern. Response to simethicone, formula or maternal restriction does not confirm colic. A short crying and feeding diary can reveal pattern and support follow-up without turning normal variation into continuous surveillance.
Differential Diagnosis
Start with time-critical causes. Infection includes sepsis, urinary tract infection, meningitis, otitis and respiratory disease; young infants may have poor feeding, hypothermia or altered behaviour rather than obvious fever. Bilious vomiting suggests intestinal obstruction or malrotation until assessed. Progressive projectile non-bilious vomiting with hunger and dehydration raises pyloric stenosis. Episodic severe pain, pallor, vomiting and bloody stool can signal intussusception, although the classic triad may be incomplete. Incarcerated hernia, testicular torsion and acute abdominal disease require urgent examination.
Occult injury must be considered when the history is inconsistent, the cry changes, or bruising, swelling, tenderness, reduced limb movement, seizures, apnoea or altered consciousness is present. Examine for hair tourniquet, corneal abrasion, oral injury and non-accidental trauma. A safeguarding concern should trigger a protected multidisciplinary pathway, not confrontation or a private promise of secrecy.
Feeding-related differentials include inadequate transfer, oversupply with fast let-down, overfeeding, incorrect formula concentration, swallowing dysfunction and oral anomalies. Gastro-oesophageal reflux is common and usually physiological; crying alone does not diagnose reflux disease, and acid suppression is not a colic test. Cow's-milk protein allergy becomes more plausible with eczema, blood or mucus, recurrent vomiting, diarrhoea, feed refusal, faltering growth or immediate allergic symptoms. Lactose intolerance, primary constipation and simple gas are overdiagnosed.
Other considerations include urinary retention, diaper dermatitis, candidiasis, fracture, vaccination-related discomfort, medication exposure and neurological or metabolic disease. Normal developmental crying, hunger, overtiredness and a need for contact are not diseases. Maternal depression, anxiety or psychosis does not cause colic, but it affects safety and capacity to cope and deserves parallel assessment. Symptoms starting after five months or persisting beyond the expected course require diagnostic review rather than indefinite reuse of the label.
Management
First acknowledge the distress and explain what was checked. Reassurance should be specific: the baby is afebrile, hydrated, feeding, growing and normal on examination; describe the expected peak and decline, the red flags and the planned review. Ask what the family most needs tonight. Arrange protected sleep shifts, help from a trusted adult and follow-up rather than giving a long list of unproven products. If overwhelmed, the caregiver may place the baby on the back in a safe empty cot and step away briefly while another adult is contacted. Never shake, strike or smother an infant.
Support responsive feeding. Continue breastfeeding; observe latch, positioning and milk transfer, address painful feeding or fast flow, and avoid routine maternal dietary restriction. For formula-fed infants, verify correct preparation, scoop-to-water ratio, safe water, teat flow and realistic volumes. ESPGHAN advises against specialized colic formulas for most infants and against switching a breastfed infant to formula. Repeated changes make response uninterpretable and increase cost.
Soothing is individualized: holding, skin-to-skin contact while awake, gentle rhythmic movement, quiet repetitive sound, reduced overstimulation, burping when needed, a walk or pram ride, and a predictable sleep routine may help even if none stops every episode. Follow safe-sleep guidance; sofas, pillows, prone unsupervised sleep and sedating remedies are not treatments.
When additional features support cow's-milk protein allergy, a clinician may supervise a time-limited two-to-four-week elimination followed by planned reintroduction or oral food challenge. Breastfeeding continues while maternal dairy exclusion receives calcium and nutritional support; formula-fed infants generally need an appropriate extensively hydrolysed formula, not partially hydrolysed, lactose-free, goat or routine soy formula. Probiotic evidence is strain- and feeding-specific and weak; any trial needs the exact strain, time limit, safety assessment and stop rule. Natural improvement must not be misattributed automatically to the last product tried.
Prescribing Information
Routine drug treatment is not recommended for infantile colic. Simethicone has not shown benefit over placebo in the Cochrane evidence base; a seemingly favourable response during a naturally improving condition does not establish efficacy. Dicyclomine and related anticholinergic antispasmodics can cause serious sedation, apnoea and neurological or anticholinergic adverse effects and should not be used for young-infant colic. Acid-suppressing medicines do not treat developmental crying and expose infants to harm when prescribed without true gastro-oesophageal reflux disease. Antibiotics, antihistamines, sedatives and analgesics are not diagnostic trials.
Gripe water and herbal mixtures vary in ingredients, concentration, sugar, alcohol, sodium bicarbonate and contamination control. They lack reliable evidence and may displace feeds or delay evaluation. Honey is prohibited in infants under twelve months because of botulism risk. Sucrose can have a limited procedural-analgesia role under protocol but is not a chronic colic treatment. Lactase studies are inconsistent; lactose-free formula is not routine because primary lactose intolerance is not the usual mechanism.
ESPGHAN gives a weak, moderate-certainty recommendation that clinicians may consider specific probiotics—Limosilactobacillus reuteri DSM 17938 or Bifidobacterium animalis subsp. lactis BB-12—in breastfed infants, based on exact studied strains and courses. It makes no recommendation for L. reuteri DSM 17938 in formula-fed infants because evidence is insufficient and does not recommend probiotics for prevention. A generic product labelled probiotic is not interchangeable with a studied strain. Quality, storage, viable count, cost and immune or central-line risk need review.
Cow's-milk-protein elimination is a diagnostic-management trial only when the phenotype supports allergy. Document baseline symptoms, choose the appropriate diet or formula, protect maternal and infant nutrition, review at two to four weeks and plan reintroduction or supervised challenge. Indefinite avoidance after spontaneous improvement creates nutritional and diagnostic harm.
When to Refer
Arrange immediate paediatric or emergency assessment for fever in a young infant according to age-specific protocol, appearing ill, cyanosis, apnoea, respiratory distress, weak or high-pitched continuous cry, reduced responsiveness, seizure, bulging fontanelle, bilious vomiting, significant dehydration, abdominal distension, major stool blood, suspected obstruction, incarcerated hernia, testicular torsion, injury or safeguarding concern. A caregiver report that the baby is different from usual deserves weight even when one isolated sign is absent.
Same-day assessment is appropriate for persistent poor feeding, reduced urine, repeated forceful vomiting, blood or mucus in stool with systemic symptoms, new bruising, worsening rash, excessive sleepiness, pain on handling, or a caregiver who cannot maintain safety. Faltering weight, an abnormal head-growth trajectory, persistent diarrhoea, recurrent vomiting, eczema with suspected food allergy or symptoms outside the expected colic age range need planned paediatric evaluation.
Refer for feeding support when latch, transfer, pain, oversupply, bottle technique or formula preparation is uncertain. A trained lactation professional can help without substituting for medical assessment. Paediatric gastroenterology or allergy referral is reasonable for severe or diagnostically uncertain cow's-milk protein allergy, poor growth, gastrointestinal bleeding, multiple food restrictions or failure of a properly conducted elimination and challenge pathway.
Family referral is equally important. Screen the birthing parent and primary caregiver for depression, anxiety, intrusive thoughts, psychosis, suicidality and sleep deprivation; connect urgently to mental-health care when risk is present. Social work, community nursing or safeguarding teams may be needed where support is absent or violence is feared. Routine colic that remains burdensome still merits follow-up within days to review growth, feeding, coping and the diagnosis. Give a named return route rather than saying to wait until the baby grows out of it.
Red Flags
A diagnosis of colic requires absence of illness. Red flags include fever or hypothermia, an infant who looks pale, mottled, blue or seriously unwell, reduced responsiveness, weak suck, persistent feed refusal, dehydration, fewer wet nappies, grunting, respiratory rate above the expected range, chest indrawing, apnoea, seizures, a bulging fontanelle or a weak, abnormally high-pitched or continuous cry. In early infancy, deterioration can be subtle; do not use a normal period between cries to overrule concerning observations.
Gastrointestinal alarms are green bilious vomit, progressive projectile vomiting, haematemesis, significant rectal blood, tense distension, a mass, peritonism, persistent diarrhoea, no stool with vomiting, or poor weight gain. Bilious vomiting is obstruction until proved otherwise. Pyloric stenosis and intussusception have age and pattern clues but incomplete textbook presentations occur. A groin swelling, scrotal redness, testicular asymmetry or a tender irreducible hernia requires urgent surgical assessment.
Safeguarding red flags include bruising in a non-mobile infant, oral injury, scalp swelling, unexplained fracture, pain on movement, altered consciousness, apnoea, retinal or neurological symptoms, inconsistent history, delayed presentation, caregiver intoxication, escalating anger or admission that the baby was shaken. Respond calmly, ensure immediate safety and follow local child-protection procedure. Never send an at-risk caregiver home with reassurance alone.
Family danger signs include suicidal thoughts, postpartum psychosis, hallucinations, intent to harm the baby, domestic violence or inability to stay awake safely while holding the infant. The safest immediate step may be another sober adult taking over while emergency help is arranged. Any major change in crying pattern, persistence beyond five months, or failure to follow the expected natural history should reopen the diagnosis rather than produce stronger colic medication.
Indian Clinical Context
In India, infant crying may be managed simultaneously by family advice, pharmacies, paediatric clinics and traditional remedies. Ask neutrally what has already been given; shaming families can conceal honey, gripe water, herbal mixtures, ghutti, janam ghutti, sedating medicines or incorrectly prepared formula. Explain specific harms and provide an achievable alternative. Over-the-counter availability does not establish infant safety or efficacy. Forceful massage, neck manipulation, smoke exposure and unsafe swaddling should be addressed directly.
Protect breastfeeding from commercial pressure. A crying baby is often interpreted as evidence of insufficient milk, yet adequacy is assessed through observed transfer, swallowing, urine output and growth, not crying alone. Where families use top feeds, check water safety, sterilization, scoop accuracy and storage. Dilution risks hyponatraemia and undernutrition; concentration risks dehydration. Do not recommend expensive specialty formula without a defined indication, trial period and review.
Cow's-milk protein allergy is commonly overdiagnosed when isolated crying is treated as allergy. Require additional compatible features, exclude emergencies, and close the diagnostic loop with planned reintroduction or challenge. Maternal dairy exclusion can be nutritionally difficult in vegetarian households and needs protein, calcium, vitamin D and iodine review. Arrange shifts so an exhausted caregiver can sleep, and avoid holding the infant on a sofa or bed when likely to doze.
