Summary
A bursa is a thin, fluid-containing sac that reduces friction where skin, tendon or muscle moves over bone. Bursitis means symptomatic inflammation or irritation of one of these sacs; it is a regional diagnosis, not a single disease. Common examples include prepatellar bursitis in front of the kneecap, olecranon bursitis over the elbow, greater-trochanteric bursal involvement within the broader lateral hip pain syndrome, subacromial-subdeltoid bursitis around the shoulder, pes anserine bursitis at the medial knee and retrocalcaneal bursitis behind the ankle. Location changes both the examination and the differential.
Most non-infective episodes follow repeated pressure, altered load, a direct blow or adjacent tendon dysfunction. Gout, calcium-pyrophosphate deposition, rheumatoid arthritis and other systemic disorders can also inflame a bursa. Superficial bursae are vulnerable to inoculation through a cut or abrasion; septic bursitis must be considered when swelling is hot, red, rapidly progressive, associated with fever or accompanied by a skin breach. It cannot be made safe by simply labelling all swelling as overuse.
Initial care for an uncomplicated mechanical presentation emphasizes temporary load modification, ice for comfort, protection from repeated pressure and graded restoration of movement and strength. Aspiration is selective: it is valuable when infection or crystals are plausible, but is not a routine ritual for every uncomplicated deep bursal syndrome. Corticosteroid injection must never precede reasonable exclusion of infection and should follow an anatomical diagnosis, discussion of transient benefit and harms, and appropriate expertise. How Common Is It?
Bursitis is common in clinical practice but poorly represented by one defensible population rate. Studies often group bursitis with shoulder impingement, greater trochanteric pain syndrome, occupational knee pain or nonspecific periarticular pain. Others identify fluid on ultrasound or magnetic resonance imaging even when it is not the source of symptoms. Conversely, primary-care episodes may resolve without imaging or formal coding. India has no comprehensive national surveillance system that separates each anatomical bursa, infective status and occupational exposure. A local hospital series or an overseas insurance database therefore cannot be converted into a national prevalence claim.
The distribution follows anatomy and activity. Prepatellar swelling is associated with frequent kneeling; olecranon disease with prolonged elbow pressure or minor trauma; subacromial-subdeltoid inflammation often coexists with rotator-cuff loading problems; pes anserine symptoms occur around the medial proximal tibia; and retrocalcaneal irritation can accompany footwear pressure or Achilles pathology. Age, sport, work and comorbidity change the mix. The 2025 systematic review of knee bursae found markedly uneven evidence: relatively better studies existed for pes anserine and osteoarthritis-associated presentations, while uncommon bursae relied heavily on imaging reports and small series.
The epidemiological lesson is to avoid equating an imaging observation with symptomatic disease. Bursal fluid, tendon degeneration and osteoarthritis frequently coexist, especially with increasing age. Record site, provoking activity, duration, functional limitation, systemic illness, skin integrity and whether infection was investigated. These variables are more transferable than a headline percentage. WHO burden estimates show musculoskeletal disorders are a major source of disability worldwide, but their aggregated figures do not provide a bursitis-specific Indian incidence.
Risk Factors
Mechanical risk reflects repeated compression, friction or sudden impact. Frequent kneeling can irritate the prepatellar or superficial infrapatellar bursa; leaning on a hard desk can affect the olecranon; poorly fitted footwear can compress the retrocalcaneal region; and abrupt increases in running, climbing, throwing or overhead work can overload tissues adjacent to a bursa. Weakness, restricted joint motion, altered gait, poor movement control and a rapid training-volume change may perpetuate symptoms. Obesity can increase load in some lower-limb presentations, but it should be addressed without blaming the patient or assuming it is the sole cause.
Systemic disease changes probability and management. Gout and calcium-pyrophosphate deposition can produce an acutely inflamed superficial bursa. Rheumatoid arthritis and other inflammatory arthritides may cause recurrent or multi-site disease. Diabetes, chronic kidney disease, immunosuppression and alcohol dependence increase concern for infection or impaired recovery. A recent injection, surgery, penetrating injury, insect bite, abrasion, ulcer or adjacent cellulitis creates an inoculation route. Repeated aspiration itself can introduce infection. Occupational exposure must be elicited specifically because many workers cannot simply stop kneeling, lifting or using an affected arm.
Anatomical labels also conceal related pathology. Lateral hip pain may arise from gluteal tendinopathy rather than isolated trochanteric bursal inflammation; shoulder bursal change may accompany rotator-cuff disease; a popliteal cyst can communicate with the knee joint; and medial knee pain may arise from osteoarthritis or meniscal pathology. Recognizing these associations prevents endless injections into a structure that is not driving disability. Risk factors modify the diagnostic plan; none confirms bursitis and their absence does not exclude sepsis.
Diagnosis
History
Localize pain and swelling with one finger where possible, then establish onset, trauma, repetitive pressure, recent activity change and effect on sleep, work, walking or overhead reach. Ask whether pain occurs on compression, active movement, passive movement or at rest. Fever, rigors, malaise, rapid progression, a skin wound, recent injection and prior antibiotics raise concern for infection. Ask about gout, inflammatory arthritis, psoriasis, diabetes, kidney disease, immune-modifying medicines and previous similar episodes. For shoulder or hip symptoms, clarify weakness, instability, radicular symptoms and inability to bear weight.
Examination
Record vital signs when infection is conceivable. Inspect for focal swelling, erythema, abrasion, sinus or scar, comparing both sides and considering that redness is less conspicuous in deeply pigmented skin. Palpate the expected bursal site, adjacent tendon, bone and joint line. Assess active and passive range: relatively preserved joint motion with superficial focal tenderness supports a periarticular process, whereas severe pain through passive motion raises concern for septic arthritis or another intra-articular disorder. Examine strength, gait, neurovascular status and the joints above and below.
Investigations
Uncomplicated, typical mechanical bursitis is often a clinical diagnosis. Plain radiographs are useful after trauma or when fracture, calcification, foreign body or significant arthritis is possible. Ultrasound can identify fluid, synovial thickening and adjacent tendon abnormality and can guide a procedure; MRI is reserved for diagnostic uncertainty, deep disease, suspected tear, osteomyelitis or a mass. Aspirate a superficial bursa when infection or crystal disease is a material possibility, sending adequate fluid for cell differential, Gram stain, culture and crystal analysis. Blood count, CRP and blood cultures support evaluation when systemic infection is suspected but cannot independently rule it out.
Differential Diagnosis
The first distinction is periarticular bursal pain versus disease inside the joint. Septic arthritis usually produces marked pain with passive movement and demands urgent joint aspiration and source-control planning; a coexisting superficial bursa does not protect against it. Crystal arthritis may involve a joint, a bursa or both. Osteoarthritis can cause activity-related pain, stiffness and effusion, while inflammatory arthritis is suggested by prolonged morning stiffness, multiple swollen joints, enthesitis, psoriasis or systemic features. Haemarthrosis follows trauma, anticoagulation or a bleeding disorder and may require a separate pathway.
Site-specific alternatives are essential. An olecranon swelling may be a rheumatoid nodule, gouty tophus, abscess or traumatic haematoma. Anterior knee swelling may represent cellulitis, patellar tendon disease or joint effusion. Medial knee pain includes osteoarthritis, meniscal injury and medial collateral ligament pathology. Posterior knee fullness may be a popliteal cyst, deep-vein thrombosis, aneurysm or soft-tissue tumour. Lateral hip pain frequently reflects gluteus medius or minimus tendinopathy; hip osteoarthritis, lumbar radiculopathy, fracture and avascular necrosis must be considered. Shoulder symptoms overlap with rotator-cuff tear, calcific tendinopathy, adhesive capsulitis, glenohumeral or acromioclavicular disease and cervical radiculopathy.
Dangerous mimics include cellulitis, necrotising infection, osteomyelitis, fracture and malignancy. A rapidly enlarging, firm or unexplained mass, night pain unrelated to position, weight loss or neurological deficit is not routine bursitis. Ultrasound or MRI may show bursal fluid incidentally, so imaging must be reconciled with the pain location and examination. The diagnosis becomes stronger when anatomy, provoking mechanism, focal signs and response to a measured rehabilitation plan align.
Management
For a typical aseptic mechanical episode, explain the anatomical problem and agree on temporary reduction of the provoking load rather than complete immobilization. Protect a superficial bursa with padding, modify kneeling or leaning, use a wrapped cold pack briefly for comfort, and maintain pain-limited movement. Once irritability settles, graded exercise should correct adjacent strength, flexibility and movement deficits. A runner with lateral hip pain needs progressive hip-abductor loading and gait-context assessment; a worker with prepatellar symptoms needs practical knee protection and task breaks. Recovery is judged by function, not by disappearance of every imaging abnormality.
Persistent symptoms require a diagnostic reset: confirm the site, examine the related tendon and joint, review systemic disease and decide whether imaging or aspiration would change treatment. Aspiration may decompress a tense superficial bursa and provide diagnostic fluid, but recurrence and iatrogenic infection are possible. If infection is suspected, obtain appropriate samples and begin locally guided antibiotics with follow-up or hospital referral according to severity. Purulent, recurrent or poorly responding infection may need repeated drainage or surgery.
A carefully selected corticosteroid injection may offer short-term relief in some non-infective presentations, but evidence varies greatly by bursa. Use anatomical precision, sterile technique and shared decision-making; avoid injecting through cellulitis or when infection has not been reasonably excluded. Routine platelet-rich plasma or other marketed biologic injections cannot be generalized from small heterogeneous studies. Refractory mechanical bursitis occasionally merits orthopaedic review for bursectomy or correction of an underlying structural driver, but surgery is uncommon and should follow a confirmed diagnosis and adequate conservative care.
Prescribing Information
Analgesia should support sleep and activity while a mechanical plan addresses the cause. Paracetamol may be suitable for some adults, but dose ceilings, liver disease, low body weight, alcohol use and duplicate combination products must be checked. Oral non-steroidal anti-inflammatory drugs can reduce short-term pain yet may cause gastrointestinal bleeding, kidney injury, fluid retention, hypertension or cardiovascular harm. Review age, pregnancy, renal function, ulcer history, anticoagulants, antiplatelets and other interacting medicines; use the lowest effective exposure and follow current Indian product information and local prescribing policy. Topical NSAIDs may reduce systemic exposure for a superficial site but are not risk-free.
Do not prescribe antibiotics for uncomplicated mechanical bursitis. When septic bursitis is suspected, aspirate before antibiotics when this is safe and will not delay necessary care, then choose therapy from the likely organisms, allergy phenotype, renal function, recent healthcare and local antibiogram. Modify treatment to culture and susceptibility and arrange an explicit review. This guide deliberately omits a universal drug-and-duration recipe because severity, methicillin resistance, immune status, depth, response and local resistance differ. Systemic toxicity or concern for septic arthritis requires hospital management rather than repeated oral courses.
Corticosteroid injection is a procedure with medicine-related harms: infection, bleeding, skin depigmentation or atrophy, post-injection flare, transient hyperglycaemia and possible tendon injury when placed incorrectly. Obtain consent, check anticoagulation and diabetes, use sterile technique and document agent, dose, site and batch according to local governance. Never inject a potentially infected bursa. Evidence for PRP and other biologics remains site-specific and heterogeneous; cost and marketing should not be mistaken for proven superiority.
When to Refer
Arrange same-day emergency or hospital assessment for systemic toxicity, rapidly spreading erythema, severe rest pain, inability to move the joint, suspected septic arthritis, significant immune compromise with infection features, or a deep-site infection that cannot be safely sampled. A painful hip with fever, an acutely hot joint or neurological or vascular abnormality is not managed as routine outpatient bursitis. Communicate vital signs, site, symptom tempo, skin breach, comorbidities, medicines, samples already obtained and any antimicrobial exposure. Do not allow transfer logistics to delay stabilization.
Refer to orthopaedics, rheumatology, sports medicine or a suitably trained musculoskeletal service when the diagnosis remains uncertain, symptoms recur despite well-delivered load modification and rehabilitation, a tendon tear or structural lesion is suspected, or aspiration or image-guided intervention is being considered. Rheumatology is particularly relevant for recurrent multi-site bursitis, inflammatory arthritis, gout or unusual crystal findings. Infectious-diseases or microbiology advice is valuable for resistant organisms, immune suppression, treatment failure or culture-negative infection after prior antibiotics.
Physiotherapy referral is appropriate when weakness, movement control, gait, work exposure or return-to-sport planning needs structured progression. Occupational-health input can translate a diagnosis into workable padding, task rotation and graded return rather than simply issuing prolonged rest. Consider oncology or sarcoma-pathway referral for a growing or atypical mass, constitutional symptoms or imaging that is not characteristic of fluid. Referral urgency depends on harm if delayed, not merely on pain score. Every referral should state the clinical question and who will monitor the patient while waiting.
Red Flags
Fever, rigors, tachycardia, hypotension, confusion or rapidly worsening local inflammation suggests infection with possible systemic involvement. A superficial bursa that is intensely warm and tender after an abrasion, puncture, injection or draining lesion requires prompt assessment for sepsis. Absence of fever does not reassure in older, diabetic, kidney-disease or immunosuppressed patients. Marked pain during passive joint movement, inability to bear weight or a toxic child raises concern for septic arthritis or osteomyelitis rather than isolated bursitis. Aspiration or antibiotic decisions should not be deferred merely because an initial CRP is modest.
Trauma-related deformity, bony tenderness, extensive bruising, sudden loss of active movement or neurovascular deficit may indicate fracture, dislocation, tendon rupture or compartment pathology. Calf swelling with dyspnoea, chest pain or haemodynamic symptoms requires evaluation for venous thromboembolism, not attribution to a popliteal cyst. A pulsatile popliteal mass is vascular until assessed. Severe night pain, progressive enlargement, weight loss or a solid component on imaging must trigger a tumour-aware pathway.
During treatment, spreading erythema, new fever, increasing drainage, worsening function, rising inflammatory markers or failure after correctly chosen antibiotics signals inadequate drainage, resistant infection or wrong diagnosis. After injection, escalating pain, fever, neurological symptoms, extensive bruising or hyperglycaemic decompensation needs urgent review. Repeated short-lived benefit from procedures is itself a warning to reconsider anatomy and systemic disease. A bursal label should never close diagnostic reasoning when the trajectory contradicts an uncomplicated mechanical disorder.
Indian Clinical Context
Indian practice spans tertiary musculoskeletal units, district hospitals, small clinics and workplaces where imaging, sterile aspiration, culture and physiotherapy are unevenly available. That variability should alter referral logistics, not the threshold for recognizing serious infection. A provider without safe aspiration or follow-up capacity should not inject an undiagnosed hot swelling. Stabilize systemic illness, protect microbiological yield when feasible and transfer with a clear handover. Local antibiograms and state or institutional antimicrobial policies take priority over regimens from foreign patient-information pages.
Occupational histories are especially relevant in farming, construction, floor work, domestic work, plumbing, tailoring, driving and manual material handling. Advice to rest may be financially impossible and clinically incomplete. Co-design padding, task rotation, brief recovery periods and graded return, documenting any restrictions. Footwear, training surfaces, long travel and access to rehabilitation can perpetuate lower-limb symptoms. Diabetes, chronic kidney disease, tuberculosis risk and unsupervised medicine use may broaden the differential, although chronic bursal swelling alone is not diagnostic of tuberculosis. Obtain tissue or fluid through an appropriate pathway when infection is suspected.

The NMC curriculum defines undergraduate educational outcomes rather than a treatment standard. Learners should explain why incidental fluid does not prove the pain generator, why crystals do not automatically exclude infection, and why steroid injection is contraindicated when infection remains plausible. Frequently Asked Questions
Does fluid seen in a bursa on ultrasound prove that it is causing the pain?
No. Small amounts of bursal fluid and adjacent tendon changes can be incidental, especially with age or osteoarthritis. The imaging location must match focal tenderness, provoking movement and the clinical course. If it does not, reconsider tendon, joint, referred-pain, infection and mass differentials before treating the scan.
When should a clinician aspirate a suspected bursa rather than observe it?
Aspiration is most useful when septic bursitis or crystal disease is a meaningful possibility, when a tense superficial collection needs diagnostic decompression, or when the result will change care. It is not automatically required for every typical mechanical episode. Sterile technique, correct anatomy, adequate samples and a follow-up plan are essential.

Normal early blood tests also do not exclude infection. Clinical concern should prompt appropriate aspiration, culture, antimicrobial planning and escalation rather than reassurance from one absent sign.
