Summary
Blepharitis is inflammation centered on the eyelid margin and is a common cause of chronic irritation, burning, crusting, intermittent blur and red eyes. The practical distinction is between anterior disease at the lash line and posterior disease involving the meibomian glands, although mixed patterns are frequent. Anterior findings can include erythema, scale, lash debris and collarettes; posterior disease may show plugged gland orifices, altered meibum, telangiectatic margins and tear-film instability. Seborrhoeic skin disease, rosacea, Demodex infestation, altered bacterial colonization, allergy and meibomian-gland dysfunction can contribute, but a visible organism or positive culture is not required in every case.
The condition is usually chronic and relapsing rather than permanently cured by a short course of medicine. Diagnosis is clinical and depends on visual acuity, external and slit-lamp examination, lid eversion when indicated, and inspection of the cornea and tear film. Treatment targets the observed phenotype: safe warming and lid care, tear supplementation, management of associated skin or ocular-surface disease, and selected antimicrobial, anti-inflammatory or antiparasitic therapy under appropriate supervision.
Blepharitis should not become a label that delays recognition of microbial keratitis, herpes, uveitis, preseptal or orbital cellulitis, cicatrizing conjunctivitis or eyelid malignancy. Marked asymmetry, focal lash loss, architectural distortion, recurrent unifocal chalazion, conjunctival scarring or failure of reasonable treatment requires specialist assessment. How Common Is It?
Blepharitis occurs across childhood and adult life and is frequently identified in eye-care practice. Precise prevalence is difficult to state because anterior blepharitis, posterior blepharitis, meibomian-gland dysfunction, Demodex-associated disease and dry eye have overlapping definitions and are measured with different grading systems. The American Academy of Ophthalmology guideline describes it as a common chronic cause of ocular irritation but does not support applying a single clinic-derived percentage to every population. Community burden also differs from the selected population seen in a corneal service.
Posterior disease becomes more prominent with age, although meibomian abnormalities and blepharokeratoconjunctivitis can occur in children. Rosacea, seborrhoeic dermatitis and dry eye are frequent associations. Morning crusting or irritation may be prominent in blepharitis, whereas aqueous-deficient dry-eye discomfort often accumulates later in the day; this is a clue, not an absolute diagnostic rule. Many people have signs without major symptoms, and symptom intensity may reflect associated surface inflammation rather than the amount of lid debris alone.
India-specific national prevalence cannot be inferred from an international clinic guideline. Climate, dust, air pollution, cosmetic practices, skin disease, access to lid examination and patterns of self-medication may influence presentation and health-seeking. A person repeatedly buying antibiotic-steroid combinations may appear only after corneal or pressure complications. For service planning, locally collected data using explicit diagnostic criteria are more credible than extrapolating a foreign survey.
Risk Factors
Look for dermatological, ocular, environmental, medicinal and anatomical contributors. Facial rosacea can accompany posterior blepharitis and meibomian-gland dysfunction; seborrhoeic dermatitis often produces greasy scale around scalp, brows, ears and lashes. Atopic dermatitis, psoriasis and acne treatments may alter the lid or ocular surface. Demodex becomes more prevalent with age and is suggested by cylindrical lash collarettes, but infestation must be interpreted with the full examination. Diabetes, immunosuppression and poor general health can modify infection risk without making ordinary blepharitis an invasive infection.
Local risks include meibomian obstruction, incomplete blinking, lid malposition, contact-lens use, chronic preserved eye drops, ocular surgery and cosmetic products applied near gland openings. Ask about false lashes, adhesives, eyeliner at the waterline, kajal or surma, cleansers and shared cosmetics. These exposures should be discussed neutrally; the clinician must identify temporal association and contamination or toxicity risk rather than assume cultural practice is causal. Dust, smoke and prolonged visual tasks may worsen the tear-film component.
A history of recurrent chalazia, unilateral persistence, focal lash loss or previous eyelid tumour is not merely a risk profile for routine inflammation; it raises concern for masquerade disease. Children with chronic blepharitis may develop keratitis and amblyogenic visual loss. Patients with glaucoma surgery or advanced glaucoma need caution with vigorous lid pressure. Before treatment, record pregnancy, drug allergy, arrhythmia risk, photosensitivity, liver disease and interacting medicines if systemic antimicrobial therapy might be considered. Risk factors guide phenotype and safety; none substitutes for examining the lid margin and cornea.
Diagnosis
History
Ask about itching, burning, grittiness, crusting, sticky lashes, watering, photophobia and fluctuating vision. Define morning versus evening pattern, unilateral or bilateral involvement, recurrence, contact-lens use, skin symptoms, cosmetics and previous treatment. Clarify pain severity, true visual loss, discharge, fever, trauma, surgery and topical steroid exposure. Repeated focal swelling may represent chalazion, while rapid tender swelling can be hordeolum or cellulitis. Failure to improve, focal recurrence and loss of lashes must be documented rather than normalized as chronic blepharitis.
Examination
Measure monocular visual acuity. Inspect face and scalp for rosacea or seborrhoeic disease, then assess lids for erythema, oedema, telangiectasia, malposition, closure and blink. Under magnification, note the location and character of scale or collarettes, lash loss or misdirection, ulceration, gland plugging and meibum quality and expressibility. Examine conjunctiva, tear film and cornea with fluorescein when appropriate; look for punctate staining, infiltrate, phlyctenule, pannus or epithelial defect. Evert the lid when a foreign body, lesion or cicatrization is possible.
Investigations
Routine cases are diagnosed clinically. Lid-margin culture is reserved for severe recurrent anterior inflammation, unusual infection or non-response where the result could alter therapy. Lash epilation and microscopy can demonstrate Demodex when the diagnosis is uncertain; characteristic collarettes may provide a strong clinical clue. Biopsy is considered by an appropriate specialist for marked asymmetry, focal architectural loss, ciliary madarosis, recurrent unifocal chalazion or treatment resistance. Tear-film testing helps characterize associated dry eye. Corneal sampling follows microbial-keratitis protocols, not a routine blepharitis pathway.
Differential Diagnosis
Meibomian-gland dysfunction is a major posterior blepharitis phenotype but can be described separately as chronic diffuse gland abnormality with obstruction or altered secretion. Dry eye may coexist and can dominate symptoms. Allergic conjunctivitis causes prominent itch, papillae and chemosis; irritant or toxic conjunctivitis follows exposure to cosmetics, preserved drops or traditional preparations. Infectious conjunctivitis produces characteristic discharge and epidemiology, while a painful contact-lens wearer with focal corneal opacity may have microbial keratitis rather than uncomplicated lid disease.
A hordeolum is an acute tender infection involving a lash follicle or gland. A chalazion is a more chronic lipogranulomatous nodule and is often less tender. Preseptal cellulitis produces diffuse lid erythema and swelling; fever, proptosis, impaired ocular movement, diplopia or reduced vision raises orbital concern. Herpes simplex or zoster may cause vesicles, epithelial keratitis or reduced corneal sensation. Molluscum contagiosum on the lid can cause chronic unilateral follicular conjunctivitis. Pediculosis and contact dermatitis have different examination clues.
Sebaceous carcinoma can mimic unilateral blepharitis or recurrent chalazion, particularly with focal lash loss, thickening, yellowish change or conjunctival spread. Basal or squamous carcinoma and immune-mediated cicatrizing disease also enter the differential when anatomy is distorted or inflammation resists treatment. In children, blepharokeratoconjunctivitis may cause recurrent phlyctenules, keratitis, scarring and amblyopia. Uveitis, episcleritis and scleritis explain pain or photophobia not accounted for by the lid. A differential diagnosis is not a list alone: every feature must be reconciled with acuity, lid anatomy, surface findings and tempo.
Management
Set expectations that control usually requires repeated, technically correct care and review of associated conditions. Show the patient the relevant lid finding and agree a feasible regimen. Warm compresses are most useful when meibomian secretions are obstructed; temperature must be comfortably warm rather than hot, with clean material and no corneal pressure. Gentle lid massage may follow when appropriate. Anterior scale can be managed with careful lid-margin cleansing using a locally available, non-irritating method. Aggressive scrubbing can worsen dermatitis or epithelial injury, and vigorous pressure is unsuitable after filtering surgery or in selected glaucoma patients.
Treat associated dry eye with an appropriate lubricant and address rosacea, seborrhoeic dermatitis, allergy, exposure or lid malposition through the relevant pathway. Contact lenses should be paused during active painful inflammation and lens hygiene reviewed. Cosmetics or adhesives temporally associated with flares can be stopped and reintroduced cautiously only after recovery. Demodex-associated disease requires confirmation of the phenotype and a treatment whose formulation, ocular safety and Indian regulatory status are verified; concentrated essential oils near the eye can burn the surface.
A clinician may select topical antibiotic ointment for significant anterior bacterial disease, oral tetracycline-class treatment for selected posterior or rosacea-associated disease, or a short monitored anti-inflammatory course when benefits outweigh risks. These are not interchangeable default prescriptions. In-office heat, expression, light-based or exfoliation procedures have variable evidence and cost; the AAO notes insufficient independent randomized evidence to name one universally superior option. Review response by symptoms, acuity, lid signs, meibum and corneal involvement, and reconsider the diagnosis when improvement is absent.
Prescribing Information
Every prescription should state the exact medicine, formulation, laterality, amount, frequency, duration and review plan. Topical antibiotic ointment can remain on the lid margin longer than a drop but may blur vision; it should be selected for an identified indication, allergy history and local susceptibility context. Routine repeated antibiotic courses encourage adverse effects and antimicrobial pressure. Antibiotic-steroid combination drops obtained without examination are especially risky because steroid can worsen herpetic or microbial keratitis, raise intraocular pressure and accelerate cataract. If steroid is prescribed, document the indication and arrange pressure and surface review.
Oral doxycycline or related tetracycline-class treatment is sometimes used for meibomian or rosacea-associated inflammation. Dose and duration vary across studies and products; check current Indian labeling and local guidance. Consider pregnancy, breastfeeding, age, oesophageal irritation, photosensitivity, gastrointestinal intolerance and interactions. Oral azithromycin has regimen-specific evidence but can prolong the QT interval and interact with other medicines; cardiovascular risk and stewardship must be assessed. A platform guide should never turn these options into one-size-fits-all dosing.
Antiparasitic treatment for Demodex is formulation-specific. Do not recommend household insecticide, veterinary ivermectin, undiluted tea-tree oil or an overseas product whose ocular formulation is unavailable locally. Preservative-free tears may reduce associated surface irritation but do not treat every lid driver. Advise clean hands, avoidance of bottle-tip contact, separate personal towels and safe disposal dates. Topical anaesthetic must not be supplied for home relief. Persistent pain or reduced vision requires examination rather than escalation by telephone.
When to Refer
Refer routinely to eye care when the diagnosis is uncertain, symptoms recur despite correct lid care, contact-lens use complicates assessment, or visual function is affected. Ophthalmology review is appropriate for moderate or severe corneal staining, recurrent phlyctenules, infiltrates, reduced corneal sensation, trichiasis, lid malposition, recurrent chalazia, difficult Demodex disease, significant rosacea-associated keratitis or a planned intraocular procedure with uncontrolled lid inflammation. Children with recurrent redness, photophobia or reduced acuity need prompt ophthalmic assessment because corneal disease and amblyopia can develop.
Expedite oculoplastic or ocular-oncology assessment for marked asymmetry, focal lash loss, ulceration, bleeding, architectural distortion, recurrent unifocal chalazion, firm thickening or lack of response to an adequate treatment trial. Conjunctival scarring, forniceal shortening or symblepharon warrants corneal or external-disease review for immune-mediated cicatrizing disease. Dermatology can help with severe rosacea, atopy, psoriasis or seborrhoeic disease, but referral must not replace ocular examination.
Same-day ophthalmic care is needed for reduced vision, moderate or severe pain, marked photophobia, focal corneal opacity, epithelial defect in a contact-lens wearer, severe chronic redness with deterioration, or suspected herpes or uveitis. Fever with proptosis, painful or restricted eye movement, diplopia or optic dysfunction is an orbital emergency. Referral advice in India should name an available facility and urgency, with interim instructions that do not delay emergency treatment.
Red Flags
The cornea determines urgency. Pain, photophobia, reduced acuity, a focal infiltrate, epithelial defect, stromal haze or anterior-chamber reaction suggests keratitis or intraocular inflammation rather than simple margin irritation. Contact-lens wear increases concern for microbial keratitis. Avoid patching and empiric steroid, stop lens wear, and arrange urgent slit-lamp assessment. Vesicles on the lid or forehead, reduced corneal sensation or a dendritic epithelial pattern raises herpes infection. Chemical exposure requires immediate irrigation before detailed documentation.
Orbital warning signs include proptosis, impaired or painful ocular movement, diplopia, reduced colour vision, afferent pupillary defect, severe headache or systemic toxicity. These require emergency imaging and specialist pathways, not an oral-antibiotic trial labeled as blepharitis. Rapid diffuse lid swelling without orbital signs may still be preseptal cellulitis and needs clinical assessment. In children, persistent photophobia, corneal vascularization or declining vision can mark blepharokeratoconjunctivitis with amblyopia risk.
Masquerade signs are equally important even when pain is absent. Unilateral treatment-resistant inflammation, focal ciliary madarosis, recurrent chalazion in the same location, thickening, ulceration, bleeding, yellowish tissue or loss of normal lid architecture may indicate malignancy. Forniceal shortening and conjunctival scarring suggest cicatrizing disease. Do not repeatedly cauterize, inject or prescribe steroid without reconsidering the diagnosis. Photograph or document the lesion, avoid false reassurance and refer through an appropriate oculoplastic, oncology or ocular-surface service.
Indian Clinical Context
Primary eye-care availability varies substantially across Indian districts. The Ministry of Health and Family Welfare framework supports visual-acuity assessment, recognition of common eye conditions and referral from community or Health and Wellness Centre levels to linked vision centres and higher facilities. It does not establish a national blepharitis prescription or assure microscopy, corneal culture, biopsy, meibography or oculoplastic oncology in every location. International AAO and TFOS documents therefore provide evidence-informed principles, while current Indian labeling, antimicrobial policy, scope of practice and local referral arrangements govern implementation.
Cost and product identity deserve explicit attention. Patients may cycle through branded lubricants or antibiotic-steroid combinations without a lid examination. Ask them to bring bottles, cosmetics and traditional preparations; ingredients matter more than cap colour. Kajal, surma, eyeliner, lash adhesive and shared cosmetics should be discussed without stigma, focusing on placement, contamination, temporal association and ocular toxicity. Instructions for warming and cleansing must use water and material that are clean, comfortably warm and realistically available.
Dust, smoke, heat, air conditioning and prolonged screen work may amplify surface symptoms but should not become a substitute diagnosis. Explain the chronic course in the patient's preferred language and demonstrate technique. For urgent corneal disease or suspected malignancy, identify a medical college, district eye unit, charitable hospital or private service with the required capability, not simply the nearest optical shop. Record transport and follow-up barriers. Foreign medicine availability and device-based procedures must never be assumed or marketed as necessary care.

OP3.1 adds the skill of history taking in a patient with red eye and enumeration of causes. A good clinical presentation therefore identifies why keratitis, conjunctivitis, uveitis, cellulitis or malignancy is less or more likely, rather than merely naming blepharitis.
Reading does not certify slit-lamp competence, meibomian expression, lash epilation, corneal scraping or biopsy. Treatment discussions must include chronicity, adherence, medicine harms, antimicrobial stewardship and referral triggers. In examinations, distinguish hordeolum from chalazion, identify the malignant masquerade, and state that reduced vision, photophobia or corneal opacity changes urgency. Supervised bedside teaching remains essential for all procedural findings.

Anterior blepharitis affects the lash-bearing margin and produces scale, crusting, collarettes and erythema. Posterior blepharitis involves meibomian-gland dysfunction, with capped orifices, altered secretions, telangiectasia and an unstable lipid layer. Mixed disease is common. Meibomian glands are modified sebaceous glands in the tarsal plates; their lipid reduces tear evaporation. Rosacea favors posterior disease, while seborrhoeic dermatitis can affect lashes, brows, scalp and nasolabial areas. Topical antibiotic, oral tetracycline-class medicine, anti-inflammatory or antiparasitic therapy requires a verified indication and safety review. Steroid can raise pressure and worsen infection. The high-yield response to pain, photophobia, focal corneal opacity or reduced acuity is urgent corneal assessment. Frequently Asked Questions
Is blepharitis contagious and does every patient need an antibiotic?
Most chronic blepharitis is not managed as a simple contagious infection, and antibiotics are not required for every case. Treatment depends on whether anterior bacterial features, meibomian dysfunction, rosacea, Demodex or another driver is present. Purulent discharge or contagious exposure warrants assessment for conjunctivitis or another diagnosis.
How should a warm compress be used without harming the eye?
Use clean material at a comfortably warm, never scalding, temperature and follow the clinician's advised duration and frequency. Avoid forceful pressure, particularly after glaucoma filtering surgery or when pressure is painful. The aim is gentle warming of obstructed secretions, not squeezing the eyeball or abrading inflamed skin.
Do lash collarettes prove that Demodex is causing all symptoms?
Collarettes are a strong clinical clue to Demodex-associated blepharitis, but symptoms can also come from dry eye, allergy, rosacea or corneal disease. Magnified examination and, in selected cases, lash microscopy help confirmation. Treatment must use an ocular-safe formulation whose availability and labeling are verified locally.
Which blepharitis features could indicate a tumour or other masquerade?
Persistent one-sided disease, focal loss of lashes, recurrent chalazion in the same site, ulceration, bleeding, firm thickening, yellowish change, distorted lid architecture or conjunctival scarring requires specialist assessment. Repeated antibiotics or steroid should not postpone examination and possible biopsy when these atypical features are present.
