Summary
Bell's palsy is an acute, usually unilateral, lower motor neuron facial nerve palsy for which no alternative cause is found after an appropriate history and examination. It is a diagnosis of exclusion, not a synonym for every drooping face. Weakness typically evolves over hours and reaches maximum severity within 72 hours, affecting the forehead, eye closure and lower face on the same side. Mild ear or retroauricular pain, altered taste, sound sensitivity or reduced tearing may occur. A progressive course over weeks, recurrent or bilateral palsy, additional neurological deficits, vesicles, significant ear disease, trauma or a mass requires another explanation.
The first safety tasks are to identify stroke or another central lesion and protect an eye that cannot close. Sudden facial weakness with arm or leg weakness, speech or language disturbance, gaze abnormality, severe imbalance, visual-field loss, severe headache, reduced consciousness or other focal signs requires an emergency stroke pathway. Forehead sparing supports a supranuclear lesion, but forehead movement alone is not a sufficient stroke rule-out: brainstem stroke can cause a complete peripheral-pattern palsy and some central lesions are clinically variable. When Bell's palsy is confidently diagnosed and contraindications have been considered, oral corticosteroids should be started as early as possible, ideally within 72 hours; benefit from later initiation is uncertain. Antiviral monotherapy is not recommended. Adding an antiviral to steroids may offer at most a modest or uncertain benefit, with some guidelines reserving discussion for severe or complete paralysis. Eye protection is mandatory when closure is incomplete: preservative-free lubricants by day, ointment and mechanical closure at night, avoidance of corneal trauma and urgent ophthalmic assessment for pain, photophobia, redness or reduced vision. How Common Is It?
Bell's palsy is the commonest diagnosis among acute peripheral facial palsies, but reported incidence varies with case definition, population and ascertainment. It can occur at any age. Many people recover substantial function spontaneously, especially when weakness is incomplete, yet prognosis is not uniformly benign. Complete paralysis, older age, severe pain, diabetes and lack of early improvement may be associated with poorer recovery, while electrodiagnostic severity can refine prognosis in selected complete palsies. Population statistics should not be used to reassure away a threatened cornea or an atypical presentation.
Recovery usually begins within weeks and continues over months. Some patients have persistent weakness, facial tightness, involuntary linked movements called synkinesis, crocodile tears, speech or eating difficulty, and psychological distress. The House-Brackmann scale and Sunnybrook system are common ways to document severity, but a scale number does not replace a description of brow elevation, gentle and forced eye closure, oral movement and resting symmetry. Baseline photographs or video may aid follow-up with informed consent and secure handling.
Pregnancy, particularly the third trimester and early postpartum period, is associated with increased presentation in observational literature, although causation and the influence of hypertensive disease are complex. Diabetes is also over-represented in some cohorts. These associations do not prove that a particular palsy is idiopathic. In an Indian clinic, the apparent frequency and causes depend on referral patterns, access to imaging, local infection epidemiology and whether ear, parotid and neurological causes are actively sought.
No current national Indian incidence is asserted from small hospital series. Educational emphasis should remain on a reproducible diagnostic pathway, time-sensitive steroid discussion, eye care and follow-up. A patient discharged with the label Bell's palsy but without documented eye closure, neurological examination and return precautions has not received a complete assessment, regardless of how common the condition is.
Risk Factors
Bell's palsy is idiopathic by definition, but several clinical contexts are associated with its occurrence or affect management. Diabetes mellitus can increase susceptibility to neuropathy and complicate corticosteroid treatment through hyperglycaemia. Check symptoms and known glycaemic status, and plan glucose monitoring when steroids are used. Hypertension, obesity and metabolic disease may coexist but do not establish causation. Immunocompromise broadens the differential to opportunistic infection, malignancy and inflammatory disease and lowers the threshold for specialist assessment.
Pregnancy and the early postpartum period require particular care. Acute facial palsy may still be Bell's palsy, but blood pressure, headache, visual symptoms, oedema and other features of pre-eclampsia must be assessed through obstetric pathways. Decisions about corticosteroids should consider gestational age, maternal glycaemia and obstetric risk while recognising that treatment benefit is time-dependent. Lactation questions should use current medicine information rather than unsupported blanket reassurance.
Recent viral symptoms are frequently reported, and latent herpes-virus reactivation is a proposed mechanism, but routine viral testing does not confirm Bell's palsy. Severe otalgia with auricular, canal or oral vesicles suggests herpes zoster oticus, or Ramsay Hunt syndrome, which often has a worse prognosis and may include hearing loss or vertigo. Lyme neuroborreliosis should be considered after plausible tick exposure or travel/residence in an endemic area, especially with bilateral palsy, meningitic symptoms or erythema migrans. Testing without compatible exposure produces misleading results.
Trauma, recent ear surgery, chronic otitis, cholesteatoma and parotid disease are alternative causes rather than risk factors for idiopathic palsy. Recurrent ipsilateral palsy deserves renewed investigation. Children require paediatric assessment because aetiology, evidence for steroids and dosing differ from adults. Practical risk includes inability to obtain lubricants, tape or urgent eye review; clinicians should solve this at first contact rather than assume a pharmacy and ophthalmologist are readily available.
Diagnosis
Bell's palsy is diagnosed clinically after confirming an acute peripheral-pattern facial paresis and finding no more likely cause. Establish the time of onset and time to maximal weakness because a gradual progression over weeks is atypical and because steroid evidence is strongest within 72 hours. Document severity and whether the eye closes completely. Do not order broad tests or imaging routinely in a typical new unilateral palsy, but investigate when history or examination identifies a competing diagnosis.
History
Ask exact onset, progression and which function was first noticed. Clarify forehead movement, eye irritation or exposure, drooling, speech, food trapping, taste, hyperacusis, tearing, facial numbness and ear or mastoid pain. Ask about limb weakness or numbness, dysarthria versus aphasia, diplopia, dysphagia, vertigo, ataxia, severe headache, seizure and altered consciousness. Examine timing of rash or vesicles, hearing loss, fever, recent infection, tick exposure or travel, trauma, dental or ear procedures, cancer, parotid swelling and previous episodes. Record pregnancy or postpartum status, diabetes, hypertension, immunosuppression and medicines.
Examination
Perform an immediate neurological screen: mental status and language, pupils, eye movements, visual fields, facial sensation, palate and tongue, speech, limb power and sensation, coordination and gait when safe. Observe the face at rest and during brow elevation, gentle and tight eye closure, showing teeth, pursing lips and puffing cheeks. A peripheral lesion usually weakens upper and lower face. Inspect the cornea for exposure, test visual acuity when possible and assess Bell's phenomenon without forcibly manipulating an injured eye. Examine the pinna, canal and oral cavity for vesicles; assess hearing, vestibular signs, mastoid and middle ear; and palpate the parotid and neck.
Investigations
Routine blood tests, Lyme serology, viral serology, CT and MRI are not indicated for a typical first episode. Check capillary or laboratory glucose when diabetes is known, suspected or steroid risk warrants it. Order Lyme testing only with epidemiological and clinical plausibility using the current local algorithm. Imaging with contrast along the facial-nerve course is considered for progressive, recurrent, bilateral, segmentally unusual or non-resolving palsy; multiple cranial neuropathies; suspected tumour, temporal-bone disease or brainstem lesion; or other red flags. Electrodiagnostic testing is not routine for incomplete palsy but can support prognosis and specialist decisions in complete paralysis.
Differential Diagnosis
Stroke is the urgent central differential. A hemispheric corticobulbar lesion commonly weakens the contralateral lower face with relative forehead preservation, but this is not absolute. A pontine stroke can damage the facial nucleus or fascicle and cause complete ipsilateral upper and lower facial weakness, usually with other brainstem findings. Sudden onset, arm or leg deficit, gaze palsy, diplopia, dysphagia, ataxia, language disturbance, neglect, visual-field defect or severe headache mandates emergency assessment. Normal limb power does not excuse failure to examine the other cranial nerves and coordination.
Ramsay Hunt syndrome causes peripheral facial palsy with prominent ear pain and herpetic vesicles in the pinna, canal, tympanic membrane or mouth; rash can occasionally appear after weakness or be absent. Hearing loss, tinnitus and vertigo support involvement beyond uncomplicated Bell's palsy. Acute or chronic otitis media, mastoiditis and cholesteatoma may cause pain, fever, discharge, conductive hearing loss or destructive ear findings and need ENT care. Malignant otitis externa is a critical consideration in a patient with diabetes or immunocompromise and severe persistent otalgia.
Lyme disease is considered with relevant geographic exposure, tick bite, erythema migrans, radicular pain, meningitic symptoms or bilateral facial palsy. In much of India, testing should be driven by travel and exposure rather than imported screening habits. HIV, tuberculosis, sarcoidosis, Guillain-Barre syndrome and other inflammatory or infectious disorders enter the differential with bilateral palsy, systemic features or multiple neuropathies. In a child, infection, trauma and congenital or neoplastic causes deserve age-specific review.
Slow progression, twitching before weakness, recurrent same-side palsy, a parotid or skull-base mass, other cranial neuropathies or failure to recover raises neoplasm, including facial-nerve schwannoma or parotid malignancy. Trauma may injure the nerve in the temporal bone. Myasthenia gravis can cause variable facial and eyelid weakness but typically preserves sensation and pupils and does not fit a fixed isolated seventh-nerve palsy. Functional facial symptoms are diagnosed positively by specialists, never merely because initial imaging is normal.
Management
First decide whether the patient needs an emergency stroke or other acute pathway. For a clinically typical Bell's palsy, explain the diagnosis, uncertainty and expected course; document facial severity and eye closure; start eye protection immediately; and discuss corticosteroids within the therapeutic window. Arrange follow-up rather than offering reassurance alone. Patients need written instructions identifying neurological and ocular deterioration and a contact route if recovery does not begin.
Offer an oral corticosteroid to appropriate adults as soon as possible and within 72 hours of onset after considering contraindications. This guide does not provide a dose, taper or extension schedule: the prescriber must use the current institutional formulary and the patient's age, comorbidity and indication. Evidence is strongest in adults. For children, pregnancy, poorly controlled diabetes, active infection, peptic ulcer risk or severe psychiatric history, seek appropriate paediatric, obstetric or medical advice without unnecessary delay.
Protect the cornea whenever blink or closure is incomplete. Use frequent preservative-free artificial tears during waking hours and lubricating ointment at night. Demonstrate safe eyelid taping or a moisture chamber so lashes do not abrade the cornea and tape does not press directly on the globe. Glasses reduce wind and dust exposure; contact lenses should be avoided until ocular-surface safety is confirmed. Eye pain, foreign-body sensation, photophobia, marked redness or reduced vision needs same-day ophthalmic assessment. Severe exposure may require temporary eyelid procedures or weights under specialist care.
Do not prescribe antiviral monotherapy. Discuss adding an oral antiviral to steroids mainly in severe or complete palsy, making clear that any incremental benefit is uncertain and probably modest. Vesicular zoster-pattern disease follows an urgent Ramsay Hunt pathway rather than the same evidence assumption. Facial exercises or neuromuscular retraining may help persistent weakness or synkinesis when delivered by an experienced therapist; forceful electrical stimulation lacks established routine benefit. Acupuncture evidence is insufficient for a confident recommendation. Surgery is not routine. Address eating, oral hygiene, speech, work and psychological impact.
Prescribing Information
Corticosteroid prescribing should record onset time, diagnostic confidence, facial severity, contraindication review and the formulary-selected regimen. Start within 72 hours where possible; evidence for starting later is limited, so late presentation requires individual judgement rather than a promise of equivalent benefit. Check diabetes history and arrange glucose monitoring when the local steroid protocol or the patient's risk warrants it. Use the current formulary and patient-specific review for infection, ulcer, psychiatric and immunosuppression risks. Do not prolong or repeat a course because recovery is slow without reassessment.
Antiviral monotherapy should not be used for Bell's palsy. If combination therapy is chosen for severe or complete paralysis, use a current institutional formulary and obtain renal, interaction and pregnancy advice where applicable. Explain that evidence does not establish a large recovery advantage and that treatment does not replace steroids or eye protection. Suspected Ramsay Hunt syndrome, immunocompromise or disseminated zoster requires diagnosis-specific urgent assessment rather than extrapolation from a Bell's-palsy regimen.
Ocular medicines are active treatment, not cosmetic comfort. Use preservative-free lubrication and mechanical protection as advised by the eye-care pathway; confirm that any taped closure can be reopened easily and that the patient or caregiver can inspect the eye. New pain, photophobia, redness or reduced vision requires same-day ophthalmic assessment rather than unsupervised escalation of eye drops. Topical antibiotics and any anaesthetic drops require an ophthalmic indication.
Pregnancy and breastfeeding decisions require current obstetric and medicine-information review. Do not state that every drug is risk-free, and do not withhold a time-sensitive treatment solely because of pregnancy without balancing maternal benefit, gestation and alternatives. For children, avoid extrapolating adult doses and certainty. Analgesia for ear or facial pain should follow age, pregnancy, renal, hepatic and gastrointestinal considerations. Steroids or antivirals do not treat stroke, tumour, bacterial ear infection or Lyme disease; those diagnoses need their own pathways.
When to Refer
Activate emergency stroke services for sudden facial weakness with any central neurological feature or diagnostic uncertainty that cannot be safely resolved. Emergency assessment is also appropriate for severe headache, reduced consciousness, seizure, meningism, rapidly evolving multiple cranial neuropathies or acute brainstem signs. Send patients with mastoid swelling, systemic toxicity, severe diabetic otalgia or suspected invasive ear infection through urgent ENT or emergency pathways. Do not delay transfer for routine outpatient facial photographs or serology.
Arrange same-day ophthalmology when incomplete closure is accompanied by eye pain, photophobia, redness, foreign-body sensation, reduced vision, corneal opacity or suspected abrasion. Urgent eye input is also appropriate when sensation is impaired, the patient cannot perform protection, exposure is severe, or previous ocular surgery or disease increases risk. Immediate lubrication and mechanical protection continue during referral.
Refer urgently to ENT, neurology or an appropriate facial-nerve service for gradual progression beyond 72 hours, bilateral palsy, recurrent ipsilateral episodes, a parotid or neck mass, significant hearing loss or vestibular symptoms, vesicles, chronic ear discharge, trauma, multiple cranial neuropathies, systemic inflammatory features or suspected malignancy. Pregnancy with hypertension or symptoms of pre-eclampsia needs obstetric assessment. Suspected Lyme disease requires infectious-disease or protocol-guided evaluation based on exposure.
Reassess any patient with new or worsening neurological findings, new ocular symptoms or incomplete facial recovery at three months; earlier specialist review is reasonable when no recovery begins within several weeks or the original diagnosis is uncertain. Persistent weakness, contracture or synkinesis benefits from a multidisciplinary facial-nerve team, therapist and sometimes ophthalmic or reconstructive expertise. A referral should include onset and peak timing, severity scale plus functional description, eye closure, full neurological and ear findings, pregnancy and diabetes status, treatment dates and recovery trajectory.
Red Flags
Stroke red flags are sudden facial weakness plus arm or leg weakness or numbness, speech or language difficulty, gaze deviation, diplopia, dysphagia, severe imbalance, visual-field loss, neglect, thunderclap or severe new headache, collapse or altered consciousness. Use the local emergency number and stroke pathway. Do not reassure solely because the forehead is weak or because the patient is young. A pontine lesion can produce a peripheral-pattern face with other brainstem deficits, and time-critical reperfusion decisions depend on rapid specialist assessment.
Eye red flags include inability to close the eye, increasing pain, grittiness that persists after lubrication, photophobia, marked redness, discharge, a visible corneal spot, reduced or blurred vision and loss of corneal sensation. Exposure keratopathy can progress to ulceration and sight loss. Apply lubrication and protect the eye while arranging urgent ophthalmic review. Never patch a painful red eye without inspection and a clear plan, and never use home topical anaesthetic.
Alternative-cause red flags include progression over weeks, recurrent same-side palsy, bilateral weakness, multiple cranial neuropathies, limb areflexia or weakness, meningism, a parotid mass, unexplained weight loss, severe ear pain, vesicles, hearing loss, vertigo, chronic discharge, mastoid swelling, recent head injury or cancer. Fever and toxic appearance are not features to simply absorb into an idiopathic label. Immunocompromise lowers the threshold for investigation.
Treatment red flags include marked hyperglycaemia, severe mood or behavioural change, gastrointestinal bleeding, allergic reaction or renal symptoms after medicine. Pregnancy-associated facial palsy with high blood pressure, severe headache, visual disturbance, epigastric pain or sudden swelling requires urgent obstetric assessment. Patients and families should know that worsening facial weakness alone may occur during the first 72 hours, but any new neurological or ocular feature changes urgency.
Indian Clinical Context
Acute facial weakness in India may present first to a pharmacy, local practitioner, emergency department, general physician, paediatrician, ENT clinic, neurologist or ophthalmologist. A low-resource first assessment can still be safe if it documents onset, forehead and eye closure, full neurological screen, ear and oral vesicles, hearing symptoms, parotid findings, pregnancy and diabetes, then provides immediate eye protection. Routine CT for every typical palsy wastes resources and exposes patients without improving diagnostic yield; failure to image an atypical or central presentation is equally unsafe.
Access to preservative-free tears, ointment and specialist eye review varies. The clinician should confirm what is locally available, demonstrate taping with the actual materials and supply an interim plan. Dust, heat, two-wheeler travel and occupational exposure can worsen ocular drying; protective glasses and work modification may be important. A vague instruction to keep the eye covered is inadequate and can lead to pressure or abrasion. Family members can assist at night after teach-back.
Diabetes is common and may be undiagnosed. Glucose assessment and monitoring should be proportionate when prescribing high-dose corticosteroids, with a clear escalation plan rather than automatic exclusion. Pregnancy requires coordination with obstetrics, particularly to check blood pressure and pre-eclampsia symptoms. Lyme testing should not be ordered reflexively from overseas algorithms; establish travel or residence in an endemic region, tick exposure and compatible systemic features. Conversely, tuberculosis, HIV, leprosy and locally relevant infections or inflammatory disorders may enter the differential when the presentation is bilateral, recurrent, chronic or multisystem.
Patients may seek massage, electrical stimulation, Ayurveda or other traditional treatments. Ask respectfully, advise against forceful manipulation, burns or substances placed in the eye, and explain that such measures must not delay steroids within the evidence window, corneal protection or stroke assessment. No national Indian drug regimen is asserted here; follow current approved formulary and institutional pathways. Documentation and translated instructions are especially important when patients travel long distances for review.
NMC Competency Mapping
Bell's palsy integrates neurological localisation, cranial-nerve examination, emergency stroke recognition, ENT and ocular examination, rational investigation, therapeutics and communication. The current NMC CBME curriculum should be mapped using the institution’s official version rather than attaching an unverified Bell's-palsy-specific code. The learner should distinguish a descriptive peripheral facial palsy from the etiological diagnosis of Bell's palsy and explain why idiopathic disease remains a diagnosis of exclusion.
At Know and Know How levels, learners should describe the facial nerve’s motor and parasympathetic functions, compare supranuclear with nuclear or infranuclear patterns, and recognise that forehead findings are helpful but not absolute. They should understand spontaneous recovery, predictors of poorer outcome, the 72-hour steroid evidence window, the limited additional certainty for antivirals and the mechanism of exposure keratopathy.
At Show How level, learners should examine facial movements systematically, document eye closure, complete a focused cranial and limb examination, inspect the ear and oral cavity, assess hearing and identify a parotid mass. They should demonstrate lubrication and safe nocturnal eyelid closure, counsel a person with diabetes about monitoring and explain red flags in plain language. Starting steroids, managing pregnancy, ordering imaging and treating zoster or Lyme disease are supervised clinical decisions.
Assessment can use an OSCE in which a patient labelled with Bell's palsy has subtle diplopia and gait ataxia, rewarding escalation rather than formulaic treatment. Another station can test corneal-protection teaching. Written questions should avoid the false absolute that forehead involvement always excludes stroke. Curriculum records should map relevant institutional neurology, ENT, ophthalmology, emergency medicine and pharmacology outcomes and retain the curriculum version used.

Combination antiviral and steroid therapy may be discussed for severe or complete paralysis, but any incremental benefit is small or uncertain. Routine laboratory testing, viral serology and imaging are unnecessary in a classic first episode; image or test atypical cases according to the suspected cause.
Incomplete eye closure demands daytime preservative-free lubricant, night ointment and safe mechanical closure or moisture protection. Eye pain, photophobia, redness or reduced vision is an ophthalmic urgency. Review new neurological symptoms, worsening ocular symptoms or incomplete recovery by three months. Synkinesis is involuntary movement accompanying a voluntary facial movement and may respond to specialist facial retraining; decompression surgery and electrical stimulation are not routine first-line care.
