Summary
A seizure-like event is first an airway, breathing, circulation and reversible-cause problem, then a diagnostic problem. Protect the patient from injury, position when feasible, provide oxygen and suction according to need, obtain bedside glucose immediately, establish access and time the event. Do not restrain limbs or place objects in the mouth. Generalised convulsive activity lasting five minutes, or recurrent seizures without recovery, should be treated as convulsive status epilepticus: emergency therapy begins at the five-minute operational threshold rather than waiting for thirty minutes. Follow the locally approved weight-based status protocol and escalate early if first-line treatment fails.
After stabilisation, decide whether the episode was an epileptic seizure, syncope, a functional or psychogenic nonepileptic seizure, or another paroxysmal event. Then ask whether an acute systemic or brain insult provoked it. Hypoglycaemia, sodium disturbance, infection, stroke, trauma, alcohol or sedative withdrawal, toxins, eclampsia and missed antiseizure medication can demand treatment different from chronic epilepsy. A first unprovoked seizure is not automatically epilepsy; recurrence risk, EEG, imaging and clinical context matter. Conversely, one seizure may satisfy an epilepsy definition when enduring recurrence risk is sufficiently high or an epilepsy syndrome is diagnosed.
The diagnostic foundation is a detailed witness account, video if safely recorded, examination, 12-lead ECG and targeted tests. EEG supports classification but a normal EEG does not exclude epilepsy. MRI using an epilepsy protocol is preferred for structural assessment when indicated; emergency CT answers acute questions. Provide driving, bathing, height, fire, sleep, alcohol, pregnancy and medication advice before discharge. How Common Is It?
Seizures are common enough that every emergency and general clinician must recognise them, yet the proportion caused by epilepsy varies sharply by setting. Emergency cohorts contain acute symptomatic seizures, syncope, metabolic events, intoxication and functional attacks as well as epilepsy. Population estimates also depend on case definitions, access to EEG and specialist review, and whether febrile seizures are included. These uncertainties make a witnessed convulsion neither proof of epilepsy nor a benign event. The immediate phenotype and precipitant are more useful than an unsupported prevalence figure.
Epilepsy contributes substantially to neurological disability worldwide, with a disproportionate treatment gap in resource-limited and rural settings. The burden includes injury, school and employment disruption, stigma, driving restrictions, medication adverse effects, pregnancy concerns and sudden unexpected death in epilepsy. Frequency alone incompletely describes harm: a single unpredictable generalised tonic-clonic seizure may have greater safety consequences than frequent brief aware focal events. Record event type, duration, recovery, nocturnal occurrence, injuries and the patient's own priorities.
In India, patients may first present to family, a pharmacy, a primary health centre, an obstetric unit or a district emergency department. Travel time and out-of-pocket costs can delay specialist assessment and MRI. Cultural interpretations may delay evidence-based care, while premature labelling can create avoidable stigma. A practical pathway therefore distinguishes the time-critical emergency, treats reversible causes, documents uncertainty honestly and establishes a named follow-up route. Epidemiology informs service planning; it must not substitute for glucose testing, ECG, pregnancy assessment or a careful description of the actual event.
Risk Factors
Risk should be separated into risk for an acute symptomatic seizure, risk for a first unprovoked seizure, and risk of recurrence after that event. Acute precipitants include hypoglycaemia; severe sodium, calcium or magnesium disturbance; uraemia; hepatic failure; hypoxia; fever or central nervous system infection; stroke; traumatic brain injury; hypertensive encephalopathy; eclampsia; alcohol or benzodiazepine withdrawal; stimulant or toxic ingestion; and medicines that lower seizure threshold. Ask about recent dose changes, missed antiseizure medicines, sleep deprivation, vomiting, fasting and complementary preparations. Fever in an adult is not a simple febrile seizure diagnosis.
An enduring susceptibility is more likely with a previous brain insult, remote stroke or trauma, developmental disorder, cerebral malformation, tumour, neurocutaneous syndrome, family history, prior unrecognised focal events or a recognised epilepsy syndrome. After a first unprovoked seizure, epileptiform EEG abnormalities, a relevant structural lesion, a nocturnal seizure and previous brain injury increase recurrence risk. These factors guide a shared treatment decision; they are not a mechanical scoring system and do not make every abnormal scan causal.
For people with established epilepsy, poor adherence, abrupt withdrawal, inadequate sleep, intercurrent illness, alcohol excess or withdrawal, interacting medicines and unsuitable drug selection can worsen control. Pregnancy changes pharmacokinetics and exposes both parent and fetus to risks from seizures and some medicines. Postpartum sleep loss compounds risk. Ask about depression, self-harm, substance use and access barriers without blame. Risk conversations must include water, heights, open flames, unsupervised machinery and nocturnal events, but should preserve independence through proportionate precautions rather than treating every person as continuously unsafe.
Diagnosis
History
Reconstruct before, during and after the event from the patient and each witness. Ask about posture, exertion, pain, emotion, heat, prolonged standing, palpitations, chest pain and prodromal light-headedness for syncope; and about aura, forced head or eye deviation, focal onset, tonic stiffening, rhythmic jerking, cyanosis, tongue injury, incontinence, duration and post-event confusion for seizure. Pallor, sweating and rapid recovery support syncope but brief myoclonic jerks can occur. Prolonged fluctuating movements, tightly closed eyes or preserved interaction may suggest a functional seizure, yet no single sign is definitive and epilepsy can coexist. Seek a safe phone video. Establish fever, headache, pregnancy, substances, medicines, trauma and previous subtle focal episodes.
Examination
Repeat vital signs, consciousness and glucose while stabilising. Look for trauma, tongue injury, meningism, rash, pregnancy-related hypertension, toxidromes and focal neurological deficit. Examine the cardiovascular system and pulses; obtain orthostatic measurements only when safe. Persistent weakness may be postictal, but stroke remains a time-critical alternative. Papilloedema, fever, immunosuppression or sustained altered mental state changes the investigation pathway. A normal examination after recovery does not exclude epilepsy or cardiac syncope.
Investigations
Obtain a 12-lead ECG after a first suspected seizure because arrhythmia and channelopathy can mimic epilepsy. Use targeted full blood count, glucose, electrolytes including sodium and calcium, renal and liver tests, pregnancy test, toxicology and infection studies according to context. Emergency CT is appropriate for acute trauma, focal deficit, persistent altered state, anticoagulation or suspected haemorrhage; MRI with an epilepsy protocol better assesses enduring structural causes. EEG should support seizure classification, ideally promptly when requested, but must not be used to rule epilepsy out. Lumbar puncture follows appropriate assessment when infection or inflammation is suspected and must not delay empirical therapy in a critically ill patient.
Differential Diagnosis
Syncope is the most important common mimic. Vasovagal syncope often has a trigger, progressive warmth, nausea, sweating, visual dimming and pallor followed by brief loss of tone and rapid orientation. Convulsive movements do not exclude syncope. Sudden collapse during exertion or supine, injury without warning, a family history of young sudden death, abnormal ECG or palpitations demands a cardiac pathway. Hypoglycaemia can produce autonomic symptoms, confusion and convulsions; a delayed normal glucose does not prove it was normal at onset. Sleep disorders, migraine aura, transient ischaemic attacks, movement disorders and panic attacks can also resemble focal seizures.
Functional or psychogenic nonepileptic seizures are genuine, involuntary episodes, not deliberate behaviour. Semiology may include long duration, waxing and waning intensity, asynchronous movement or responsiveness, but bedside features overlap and should not be used to accuse or dismiss. The preferred diagnostic standard is capture of a typical event with video-EEG interpreted in clinical context. Explain uncertainty respectfully, screen for injury and comorbidity, and remember that epilepsy and functional seizures can coexist.
Within epileptic events, distinguish focal aware, focal impaired-awareness, focal-to-bilateral tonic-clonic and generalised-onset patterns because investigation and drug choice differ. Acute symptomatic seizures occur in close temporal association with an insult such as metabolic disturbance, infection, stroke or withdrawal; an unprovoked seizure occurs without such an immediate cause. Eclampsia, meningitis or encephalitis, cerebral venous thrombosis, neurocysticercosis, brain tumour and autoimmune encephalitis enter the differential only when history, examination and targeted testing support them. Avoid empiric long-term antiseizure treatment merely because an event looked dramatic.
Management
During an active convulsion, call for help, note onset time, remove hazards, support airway and breathing, check glucose and treat a documented low value. Do not force the mouth open. A convulsive seizure at or beyond five minutes, or serial seizures without recovery, is status epilepticus. If an immediately available individual emergency plan does not specify otherwise, use the approved first-line benzodiazepine route: buccal midazolam or rectal diazepam in the community, or intravenous lorazepam only when intravenous access and resuscitation facilities are immediately available. Correct reversible causes simultaneously. If convulsive status persists after two benzodiazepine doses, use one approved intravenous second-line medicine and involve anaesthesia or critical care early for airway support and refractory status. Consider eclampsia, toxin-specific treatment, infection, stroke and metabolic correction. Persistent impaired consciousness after motor activity stops may represent non-convulsive status and needs urgent specialist and EEG assessment.
After a clearly provoked seizure, treat the cause and decide whether short-term antiseizure treatment is needed; chronic therapy is not automatic. After a first unprovoked seizure, discuss recurrence risk, examination, EEG, imaging, patient values and consequences of another event. Immediate treatment reduces short-term recurrence but does not necessarily improve long-term remission; offer it when the balance is favourable rather than from fear alone. Established epilepsy treatment is matched to seizure type, syndrome, age, comorbidity, interactions and reproductive potential. Never stop maintenance therapy abruptly without a plan.
Before discharge, confirm recovery and supervision, document the event, tests and uncertainty, arrange urgent specialist assessment, and give written first aid. Advise calling emergency services for a seizure lasting five minutes, repeated events without recovery, serious injury, pregnancy, breathing difficulty, water-related events or a first seizure. Discuss adherence, sleep, alcohol, bathing rather than locking a bathroom, supervised swimming, heights, flames and machinery. A personalised emergency plan is essential for recurrent prolonged seizures.
Prescribing Information
Status treatment is protocol-driven because route, dose, weight, cardiorespiratory monitoring and previous doses determine safety. Benzodiazepines can depress respiration, especially after repeated dosing, yet undertreatment of ongoing convulsive status is also dangerous. Ensure ventilation equipment and trained support are available. After two benzodiazepine doses, the current guidelines second-line options are intravenous levetiracetam, phenytoin or sodium valproate; do not combine or repeat them empirically outside expert guidance. Their interaction, cardiac, hepatic and reproductive risks differ, so selection must follow the current local emergency formulary. Phenytoin administration requires cardiac and blood-pressure monitoring and is not suitable for every generalised epilepsy. Correct glucose with an appropriate protocol and address thiamine need in a person at risk of deficiency without delaying treatment of hypoglycaemia.
Long-term antiseizure prescribing follows seizure type. Sodium-channel medicines can worsen some generalised epilepsies; broad-spectrum medicines are not interchangeable. Review kidney and liver function, mood, cognition, bone health, weight, rash risk and interactions with contraception, anticoagulants, tuberculosis or HIV therapy. Explain common adverse effects and urgent warnings. New fever, mucosal lesions, facial swelling or a painful spreading rash after a recently started medicine requires immediate assessment for severe cutaneous adverse reaction, not routine continuation.
Reproductive safety requires a documented conversation before conception when possible and at every relevant review. Valproate carries major fetal and neurodevelopmental risk and must follow the current Indian product information and regulatory requirements; foreign safety programmes are evidence of that risk, not rules to import unchanged into Indian practice. It should not be initiated casually in anyone with pregnancy potential. Other medicines also require individual risk assessment. Do not abruptly stop treatment during pregnancy because uncontrolled tonic-clonic seizures can be dangerous. Coordinate neurology and obstetric care, consider preconception folate under local guidance, and monitor levels when clinically indicated. Use generic substitution, rescue formulations and brand continuity according to medicine-specific guidance and availability, not a universal rule.
When to Refer
Transfer immediately to an emergency-capable service for convulsive activity at five minutes; recurrent seizures without recovery; respiratory compromise; persistent altered consciousness; first seizure with pregnancy or puerperium; severe hypoglycaemia or electrolyte disturbance; suspected poisoning or withdrawal; fever with meningism; acute severe headache; new focal deficit; significant trauma; anticoagulation; or concern for stroke, haemorrhage, encephalitis or eclampsia. Refractory status requires critical-care and neurology support while treatment continues. Do not wait for imaging or a referral reply before initiating time-critical stabilisation and empirical treatment where indicated.
A first suspected unprovoked seizure needs prompt assessment by a clinician with epilepsy expertise. Refer urgently when diagnosis is uncertain, ECG is abnormal, examination is focal, events recur, recovery is incomplete or social circumstances make immediate safety difficult. Paediatric first seizures need age-appropriate expertise; neonatal events and infantile spasms are separate emergencies. Refer to cardiology for exertional or supine syncope, palpitations, a concerning family history or an abnormal ECG. Functional seizure suspicion warrants neurologic confirmation and psychologically informed care, not discharge without explanation.
Tertiary epilepsy referral is appropriate for drug-resistant epilepsy, uncertain seizure classification, discordant EEG or imaging, suspected surgical lesion, troublesome adverse effects, pregnancy complexity, severe psychiatric comorbidity or need for video-EEG. Two appropriately selected and tolerated medication schedules failing to achieve sustained freedom should prompt evaluation rather than endless substitutions. A referral should include witness descriptions, videos with consent, event frequency, injuries, ECG, laboratory results, imaging, EEG, complete drug history and the clinical question. Maintain rescue planning and prescription responsibility while the person waits.
Red Flags
The first red flag is time: generalised convulsive activity lasting five minutes is status epilepticus for emergency action. Recurrent seizures without return to baseline, increasing frequency, respiratory compromise, cyanosis, hypotension, hyperthermia or prolonged confusion require immediate escalation. Subtle facial twitching, eye deviation or fluctuating awareness after apparent cessation may represent ongoing non-convulsive seizure activity. Check glucose during the event when possible and never assume a known epilepsy diagnosis explains a new pattern.
Fever, headache, neck stiffness, petechiae, immunosuppression or altered behaviour raises concern for central nervous system infection. Sudden headache, focal deficit, anticoagulation, trauma or persistent depressed consciousness raises haemorrhage or stroke concern. Severe hypertension, visual symptoms, epigastric pain, oedema, pregnancy or recent delivery must trigger an eclampsia pathway; magnesium-based obstetric protocols differ from routine epilepsy treatment. Toxic syndromes, alcohol or sedative withdrawal, severe sodium disturbance, hypocalcaemia and renal or hepatic failure can recur unless the cause is corrected.
Collapse during exertion, while supine or without prodrome; chest pain; palpitations; an abnormal ECG; or sudden death in a young relative may indicate malignant arrhythmia rather than epilepsy. Persistent unilateral weakness should not be attributed to Todd paresis until stroke has been assessed. Serious injury, drowning exposure, aspiration or inability to return to a safe supervised environment changes disposition. In established epilepsy, a newly painful widespread rash, mucosal erosion, fever or facial swelling after medication is an emergency drug-reaction signal. Written safety-netting should name these features and the destination, not merely say to return if worse.
Indian Clinical Context
At a primary health centre or small hospital, the highest-value actions are often simple: time the seizure, protect airway and circulation, check capillary glucose, establish access, identify pregnancy, use the approved status protocol and arrange transfer with a clear handover. Lack of EEG or MRI does not justify delaying benzodiazepine treatment for status or antibiotics for suspected meningitis. It does justify documenting the unanswered question and choosing the safest referral pathway. Tele-neurology can support triage, but it cannot replace resuscitation or examination in an unstable patient.
India's differential includes common global causes plus context-dependent infection, neurocysticercosis, tuberculosis, malaria, Japanese encephalitis and toxic exposures. None should be diagnosed from geography alone. Fever, immune status, focal findings, imaging and laboratory evidence should drive testing. Review traditional, over-the-counter and veterinary products because ingredients or contaminants may be relevant. When cost restricts testing, prioritise bedside glucose, ECG, targeted electrolytes and acute imaging according to red flags; avoid a broad low-yield panel that leaves no funds for follow-up.
Driving advice must be explicit but legally accurate. Do not invent a universal seizure-free interval. Explain that a seizure can affect fitness to drive, advise stopping driving until medically and legally cleared, and check the current Central Motor Vehicles Rules, licensing forms, transport authority process and specialist recommendation applicable to that person. Discuss occupational machinery, commercial driving and work at height separately. Address stigma in the patient's language and include family only with consent. Generic availability and interactions with antitubercular or antiretroviral medicines require careful prescribing. Establish who will obtain EEG, imaging and medicines, and who will contact the patient with results.

A learner should communicate first aid without myths, give proportionate driving and water-safety advice, and create an access-aware plan. Case discussion can test seizure versus convulsive syncope, pregnancy, alcohol withdrawal, neuroinfection, focal lesion and a normal routine EEG. Competence is demonstrated by prioritising threats, naming uncertainty and arranging continuity, not by reciting every antiseizure medicine or ordering CT for all events.

Functional seizures are involuntary and ideally confirmed by capturing a typical event on video-EEG; they may coexist with epilepsy. Immediate antiseizure treatment after a first unprovoked event lowers early recurrence but does not guarantee better long-term remission. Driving rules are jurisdictional: advise cessation pending current legal and specialist clearance rather than memorising a foreign interval. Drug choice depends on seizure type, pregnancy potential, comorbidity and interactions.

When the history supports an epileptic seizure, EEG can support classification and recurrence assessment, ideally performed promptly. Sleep-deprived, ambulatory or video-EEG may be considered by specialists if uncertainty continues. The diagnosis remains clinical and integrates witness history, ECG, imaging and reversible-cause assessment.
What advice should be given about driving after a first seizure in India?
Advise the person not to drive until medical assessment and the applicable legal or licensing requirements have been clarified. Do not quote a universal foreign seizure-free interval. Review the current Central Motor Vehicles Rules and licensing forms, the relevant transport authority process, seizure cause and recurrence risk, and specialist advice. Commercial driving and safety-critical work require separate consideration and careful documentation.
