Summary
Oedema is clinically detectable expansion of interstitial fluid; it is a sign, not a diagnosis. The first decision is whether the swelling represents an emergency. Sudden unilateral leg swelling may indicate deep-vein thrombosis, while erythema, warmth, pain and systemic illness raise concern for infection. Breathlessness, hypoxaemia, pulmonary crackles, raised jugular venous pressure or shock may indicate acute heart failure or another cardiopulmonary emergency. Facial or tongue swelling with stridor suggests angioedema. Hypertension, headache, visual symptoms or upper abdominal pain in pregnancy requires urgent obstetric assessment for pre-eclampsia.
After immediate threats are addressed, classify oedema by distribution, time course, pitting, symmetry and associated organ-system features. Generalised or bilateral dependent oedema commonly reflects heart failure, kidney disease, cirrhosis or severe hypoalbuminaemia, but venous insufficiency, medicines and immobility are frequent alternatives. Localised non-pitting swelling suggests lymphatic obstruction, although chronic venous oedema can become fibrotic.
Evaluation should connect history and examination to targeted tests: urinalysis and quantified protein for renal disease; creatinine and electrolytes; albumin and liver synthetic function; ECG, natriuretic peptide and echocardiography when heart failure is plausible; and compression ultrasonography when DVT is suspected. How Common Is It?
Oedema is encountered across primary care, emergency medicine, inpatient wards, obstetrics, nephrology, cardiology and hepatology, but there is no single useful population prevalence because studies count different phenomena. Dependent ankle swelling after prolonged standing, chronic venous disease, drug-related oedema, nephrotic syndrome and fluid overload in heart failure are biologically and prognostically different. A responsible guide therefore does not convert one hospital series into an Indian national estimate.
The conditions behind oedema are common. Chronic kidney disease, diabetes, hypertension, heart failure, chronic liver disease, venous disease and pregnancy-related hypertensive disorders all contribute to clinical workload in India. Their relative frequency changes with age, sex, pregnancy status, rural or urban setting, referral pattern and access to diagnostics. In children, nephrotic syndrome and nutritional disease change the differential; this guide focuses on adults and signals when paediatric review is needed.
Pre-test probability should come from the individual presentation. Unilateral swelling after surgery, immobilisation, cancer treatment or previous venous thromboembolism carries a different risk from painless bilateral ankle swelling after starting amlodipine. Ascites with sarcopenia and stigmata of chronic liver disease differs from abrupt facial puffiness, haematuria and hypertension. Clinical services should audit the diagnoses actually seen locally rather than assume a universal distribution. The important epidemiological lesson is that common benign mechanisms and time-critical disease overlap in appearance, so prevalence never replaces structured assessment.
Risk Factors
Risk assessment begins with the mechanism most likely to alter capillary hydrostatic pressure, plasma oncotic pressure, permeability or lymphatic drainage. For venous thrombosis, ask about previous DVT or pulmonary embolism, active cancer, recent operation or trauma, reduced mobility, long travel, pregnancy or puerperium, oestrogen exposure and thrombophilia. Varicose veins, obesity, prolonged standing, previous thrombosis and calf-muscle pump impairment favour chronic venous disease. Cellulitis risk increases with skin breaks, tinea pedis, ulcers, diabetes and pre-existing lymphoedema.
Cardiac risk includes ischaemic heart disease, hypertension, valvular disease, cardiomyopathy, arrhythmia, congenital disease and cardiotoxic treatment. Renal clues include diabetes, hypertension, systemic autoimmune disease, recent infection, nephrotoxic exposure and known CKD. Liver risk includes harmful alcohol use, chronic viral hepatitis, metabolic dysfunction-associated steatotic liver disease and hepatotoxic drugs. Malignancy, malabsorption, severe inflammation and urinary or gastrointestinal protein loss can lower oncotic pressure.
Review every medicine and its timing. Dihydropyridine calcium-channel blockers, non-steroidal anti-inflammatory drugs, corticosteroids, thiazolidinediones, hormones, gabapentinoids and some vasodilators can contribute. A drug association does not prove causality or justify abrupt withdrawal. Pregnancy changes plasma volume and venous pressure; physiological dependent swelling is common, but new facial or hand swelling with hypertension or symptoms is not safely dismissed. Lymphatic risk follows node dissection, radiotherapy, malignancy, recurrent infection, trauma and, in endemic settings, lymphatic filariasis. Hypothyroidism can cause non-pitting tissue change, but isolated ankle oedema without other features is nonspecific.
Diagnosis
History
Establish onset, progression, laterality, diurnal variation and whether elevation changes swelling. Ask about pain, redness, fever, trauma, breathlessness, orthopnoea, nocturnal dyspnoea, chest pain, palpitations, reduced exercise tolerance and rapid weight change. Renal questions include oliguria, frothy urine, haematuria, recent infection and nephrotoxic exposure. Ask about jaundice, abdominal distension, gastrointestinal loss, appetite and alcohol use. Record pregnancy status, fetal concerns and pre-eclampsia symptoms. Document drugs, operations, immobility, cancer, thrombosis and lymphatic injury.
Examination
Assess airway, breathing, circulation, oxygen saturation, temperature, blood pressure and mental state before examining the legs. Map symmetry and extent; compare circumference at reproducible points. Note pitting, tenderness, warmth, erythema, ulcers, varicosities, skin thickening and Stemmer sign without treating any single sign as diagnostic. Examine jugular venous pressure, heart sounds, rhythm, lungs and perfusion. Look for ascites, hepatosplenomegaly and chronic liver signs. Check face, sacrum and hands; measure weight. In pregnancy, follow the obstetric hypertension pathway.
Investigations
Select tests from the differential. Urinalysis, urine albumin- or protein-creatinine ratio, creatinine, electrolytes, albumin, liver tests, full blood count and glucose often form a useful core. ECG, chest imaging, natriuretic peptide and echocardiography support cardiac assessment when indicated. Compression ultrasonography is used for suspected DVT according to clinical probability and D-dimer strategy. Consider thyroid testing, inflammatory tests, venous or lymphatic imaging, hepatitis tests and renal immunology only when clinical features justify them. Severe hypoalbuminaemia requires a search for renal, hepatic, gastrointestinal, inflammatory or nutritional cause rather than reflex albumin infusion.
Differential Diagnosis
For unilateral acute leg swelling, prioritise DVT, cellulitis, trauma, ruptured Baker cyst, superficial thrombophlebitis, acute venous obstruction and compartment pathology. DVT may be painless and clinical examination cannot exclude it. Cellulitis is supported by tender expanding erythema, warmth, a portal of entry and sometimes fever; bilateral red legs are more often venous or inflammatory than simultaneous infection. Sudden whole-limb swelling, venous collateral veins or malignancy may indicate proximal iliac or caval obstruction.
Chronic unilateral or asymmetric swelling suggests post-thrombotic syndrome, chronic venous insufficiency, lymphoedema, pelvic mass or previous surgery and radiotherapy. Primary lymphoedema is less common. Lymphatic filariasis belongs in the Indian differential when exposure, geography and chronic limb or genital lymphoedema fit, but not every non-pitting leg is filarial. Lipedema usually spares the feet and is painful or tender; obesity can coexist with multiple mechanisms.
Generalised or bilateral oedema is organised by cardiac, renal, hepatic and protein-related causes. Heart failure is suggested by congestion, raised JVP and cardiopulmonary symptoms. Glomerular disease or nephrotic syndrome is supported by proteinuria, haematuria, hypertension or impaired renal function. Cirrhosis commonly produces ascites with portal-hypertension and synthetic-function features. Severe malnutrition, malabsorption or protein-losing enteropathy may lower albumin. Drugs, pregnancy, hypothyroidism, venous dependency and immobility are alternatives. Angioedema causes abrupt deeper swelling, often non-pitting, and airway involvement is an emergency. Always allow more than one cause, especially in older adults with CKD, heart disease and medicines that promote swelling.
Management
Treat instability first. Pulmonary oedema or acute heart failure requires monitored emergency care, oxygen only when hypoxaemic, ventilatory support when needed, and cause-directed treatment. Suspected DVT follows a validated diagnostic and anticoagulation pathway, accounting for bleeding risk and renal function. Cellulitis needs appropriate antimicrobial assessment and source care. Airway-threatening angioedema demands emergency airway planning and cause-specific treatment. Suspected pre-eclampsia is transferred into urgent obstetric care.
For stable generalised oedema, management addresses the driver and congestion. In heart failure, optimise guideline-directed disease treatment and use loop diuretic therapy to relieve fluid retention with monitoring of weight, symptoms, blood pressure, renal function and electrolytes. In CKD or nephrotic syndrome, quantify protein loss, manage sodium balance and blood pressure, review nephrotoxins and refer for cause-specific renal treatment. Cirrhotic ascites requires specialist-informed sodium management, diuretics in appropriate ratios, monitoring and consideration of diagnostic paracentesis, large-volume paracentesis or transplant evaluation according to presentation.
Conservative measures suit selected dependent or venous oedema: movement, calf exercises, elevation, skin care and correctly assessed compression. Do not start compression when severe arterial disease, acute untreated DVT, decompensated heart failure or infection makes it unsafe. Drug-related oedema is managed by reviewing indication, dose and alternatives with the prescriber, not by adding a diuretic automatically. Daily weights and fluid balance can support monitoring but require consistent technique. Albumin is not a nutritional tonic or routine response to a low laboratory value. Explain the diagnosis, expected tempo, return precautions and who will review results.
Prescribing Information
Diuretics require an indication and a monitoring plan. Loop diuretics can cause hypovolaemia, hypotension, kidney injury, hyponatraemia, hypokalaemia or hypomagnesaemia; response varies with renal function, gut absorption and concurrent sodium intake. Record baseline blood pressure, creatinine and electrolytes, reassess after initiation or dose change, and give clear sick-day and deterioration advice consistent with the patient's wider regimen. Potassium-sparing agents can cause dangerous hyperkalaemia, especially with CKD, ACE inhibitors, angiotensin-receptor blockers or potassium supplements.
Spironolactone and loop-diuretic strategies for cirrhotic ascites should be supervised with weight, renal function, sodium and potassium monitoring. Rapid fluid removal can precipitate encephalopathy, kidney dysfunction or circulatory failure. Intravenous albumin has defined uses in specialist cirrhosis pathways and selected clinical settings; visible oedema or hypoalbuminaemia alone is not a prescription. In nephrotic syndrome, diuresis can reduce an already limited effective circulating volume, so dosing requires clinical judgement.
For amlodipine-associated oedema, review temporal relationship, dose, blood pressure goals and alternatives. It reflects preferential arteriolar dilation and may respond poorly to simply escalating a loop diuretic. Do not advise abrupt cessation of antihypertensives, corticosteroids or other essential medicines. NSAIDs can worsen sodium retention, heart failure and kidney function and should be reviewed. Anticoagulation for confirmed or strongly suspected VTE follows a verified local protocol, renal and hepatic function, pregnancy status, interactions and bleeding risk. Compression hosiery is a medical intervention requiring correct size and arterial assessment. This guide does not provide a patient-specific drug or fluid prescription.
When to Refer
Arrange same-day emergency assessment for oedema with hypoxaemia, severe breathlessness, chest pain, syncope, shock, rapidly progressive airway or facial swelling, suspected DVT or pulmonary embolism, sepsis, compartment concern, acute kidney injury with oliguria, severe electrolyte disturbance, or hypertensive symptoms in pregnancy. A tense painful limb, neurological compromise or rapidly spreading skin change should not wait for routine imaging. Children with generalised oedema require paediatric assessment because causes and treatment differ.
Refer urgently to nephrology when oedema accompanies rapidly worsening renal function, nephritic urine, heavy proteinuria with complications, suspected systemic glomerulonephritis, refractory hypertension or diagnostic uncertainty requiring biopsy. Cardiology review is appropriate for new heart failure, significant valve disease, arrhythmia, persistent congestion despite treatment or unclear cardiac function. Hepatology or gastroenterology review is needed for new decompensated cirrhosis, ascites requiring paracentesis, suspected spontaneous bacterial peritonitis, encephalopathy, bleeding or transplant consideration.
Vascular or lymphoedema services help with chronic unilateral swelling, venous ulceration, recurrent cellulitis, suspected proximal obstruction, post-thrombotic syndrome or uncertain lymphoedema. Consider oncology when obstruction or unexplained weight loss raises concern for malignancy. In India, referral feasibility varies across health and wellness centres, district hospitals, medical colleges and private services. State the question, severity, renal and liver function, medicines, pregnancy status and tests already performed. Confirm transport and follow-up rather than assuming that a referral letter completes the pathway.
Red Flags
Pulmonary congestion is the most immediate danger: severe dyspnoea, low oxygen saturation, pink frothy sputum, diffuse crackles, exhaustion, altered consciousness or poor perfusion requires resuscitation and urgent escalation. Oedema with pleuritic pain, haemoptysis, tachycardia, syncope or unexplained hypoxaemia raises concern for pulmonary embolism. Acute unilateral swelling with pain or risk factors needs objective DVT assessment; neither a negative Homans sign nor preserved walking ability rules it out.
Pain out of proportion, tense compartments, sensory change, weakness or absent pulses suggests limb-threatening pathology. Fever, toxicity, rapidly extending erythema, bullae, crepitus or severe tenderness raises concern for necrotising infection. Facial, tongue or laryngeal swelling, voice change, drooling or stridor threatens the airway. Sudden oliguria, hyperkalaemic features, uraemic symptoms or severe hypertension may indicate acute renal decompensation.
In liver disease, haematemesis, melaena, confusion, fever with ascites, hypotension or worsening jaundice needs urgent care. Severe hyponatraemia can cause confusion or seizures. In pregnancy or the puerperium, hypertension with severe headache, visual disturbance, epigastric or right-upper-quadrant pain, breathlessness, reduced fetal movement or neurological symptoms requires an obstetric emergency pathway. Rapid unexplained weight gain is supportive but not diagnostic. Finally, apparent oedema accompanied by weight loss, lymphadenopathy, unilateral whole-limb swelling or abdominal mass may indicate malignancy or central obstruction and warrants expedited investigation.
Indian Clinical Context
An Indian pathway must accommodate variable access to laboratory testing, Doppler ultrasonography, echocardiography, nephrology, hepatology and monitored beds. At first contact, reliable vital signs, urine dipstick, pregnancy assessment, medication reconciliation and recognition of breathlessness, DVT, infection or hypertensive pregnancy can be more valuable than an indiscriminate panel. If advanced tests are unavailable, document the working differential, stabilise within competence and arrange a named referral destination. Do not let inability to measure BNP delay transfer of a clinically unstable patient.
Diabetes and hypertension are important contributors to renal and cardiac disease, while chronic liver disease has alcohol-related, viral and metabolic causes. Lymphatic filariasis remains relevant in endemic districts, but diagnosis should be based on compatible epidemiology and clinical assessment rather than stereotype. Amlodipine is widely used and often contributes to dependent ankle swelling; the medicine history prevents unnecessary heart-failure labelling while not excluding coexistent disease. Pregnancy-related swelling must be linked to blood pressure, symptoms and urine assessment.
Advice must reflect heat, travel distance, outdoor work, footwear, ability to elevate the limb, medicine affordability and access to repeat electrolytes. Compression products may be unavailable or incorrectly fitted. Patients with limited literacy need demonstrated skin care, weight monitoring and red-flag instructions in an understood language. Government, charitable and private services coexist, and local protocols determine where ultrasonography, paracentesis or specialty review occurs.
