Summary
Joint pain should first be localized and classified, not assigned a rheumatological label from a blood test. Confirm whether pain arises from the joint itself rather than bone, muscle, tendon, bursa, nerve or referred disease. Then classify by number of joints—monoarticular, oligoarticular or polyarticular—by duration, inflammatory versus mechanical features, symmetry, distribution and systemic involvement. Acute hot monoarthritis is septic arthritis until adequately assessed, even without fever. Crystal arthritis is common but crystals do not exclude simultaneous infection. Synovial-fluid aspiration is therefore central when infection or crystal disease is plausible.
Inflammatory pain commonly brings swelling, warmth, prolonged morning stiffness and improvement with movement; mechanical pain is more activity-related, with brief stiffness and less synovitis. These patterns guide but never absolutely diagnose. Persistent small-joint or multi-joint synovitis warrants urgent rheumatology referral even when rheumatoid factor, anti-CCP or inflammatory markers are normal. Chronic monoarthritis requires consideration of tuberculosis, indolent infection, internal derangement and neoplasm. Immunosuppression, prosthetic joints, injection drug use, recent procedure and extremes of age lower the threshold for infection pathways.
Assessment integrates history, full musculoskeletal and systemic examination, selective blood tests, aspiration and question-led imaging. Treatment depends on cause: septic arthritis requires urgent antimicrobial and drainage planning; inflammatory arthritis benefits from early disease-modifying care; osteoarthritis is managed around function, exercise and weight where relevant; and gout needs acute-flare care plus long-term urate strategy when indicated. Pregnancy, lactation, kidney or liver disease and infection risk change prescribing. How Common Is It?
Musculoskeletal pain is among the most frequent reasons for healthcare contact, but “joint pain” includes very different denominators: transient viral arthralgia, osteoarthritis, gout, inflammatory arthritis, trauma and dangerous infection. Osteoarthritis becomes increasingly common with age and prior joint injury. Gout prevalence rises with chronic kidney disease, obesity, hypertension, diuretic exposure and dietary or genetic factors. Rheumatoid arthritis is less common than osteoarthritis but causes disproportionate disability when persistent synovitis is not recognized and treated early. Septic arthritis is comparatively uncommon yet can rapidly destroy cartilage and cause sepsis, making its cost of delayed diagnosis much greater than its frequency suggests.
Indian case mix varies with age, occupation, rural or urban setting, access to rheumatology and endemic infection. Tuberculous arthritis is a particularly important chronic monoarthritis differential; delayed recognition can produce irreversible joint damage. Chikungunya and other viral infections may cause acute or persistent polyarthralgia. Rheumatic fever, brucellosis in exposed populations and bacterial infection must be considered from the history rather than through indiscriminate panels. Diabetes, chronic kidney disease and expanding use of corticosteroids, conventional DMARDs and biologics increase both inflammatory-disease complexity and infection vulnerability.
Published prevalence estimates cannot be transferred uncritically between community surveys and tertiary clinics. Pain-reporting instruments, joint examination, radiographic definitions and referral selection all change the result. Women, older adults, manual workers and patients with limited transport may be underrepresented in specialist cohorts. A useful service audit measures time from persistent synovitis to rheumatology, time from hot-joint presentation to aspiration and antibiotics, culture yield, TB diagnostic confirmation, functional outcomes and medicine monitoring rather than claiming that one national percentage captures the burden.
Risk Factors
For septic arthritis, risk rises with older age, diabetes, chronic kidney or liver disease, immunosuppression, inflammatory arthritis, skin infection, bacteraemia, injection drug use, recent joint surgery or injection, prosthetic material and previous joint damage. Gonococcal disease is relevant in the appropriate sexual history. Immunosuppressed patients may lack fever, leukocytosis or a striking synovial response. A prosthetic joint follows a specialist prosthetic-joint-infection pathway rather than an uncomplicated native-joint algorithm. Recent antibiotics can reduce culture yield and must be documented.
Crystal disease risk includes chronic kidney disease, diuretics, excess weight, hypertension, alcohol exposure, high cell turnover, transplantation and family history. Gout often affects the first metatarsophalangeal joint but can involve ankle, knee, wrist, hand or elbow. Calcium pyrophosphate deposition becomes more likely with age and selected metabolic disorders. Neither serum urate nor a stereotyped location can safely separate gout from infection during an acute flare.
Inflammatory polyarthritis risks and clues include personal or family autoimmunity, psoriasis, inflammatory bowel disease, uveitis, recent genitourinary or gastrointestinal infection, connective-tissue symptoms and smoking for rheumatoid arthritis. Osteoarthritis relates to age, prior injury, repetitive load, obesity for weight-bearing joints and anatomical factors; radiographic change and pain severity correlate imperfectly. Tuberculous arthritis risk includes TB exposure, prior disease, immunosuppression, malnutrition and residence or work in higher-transmission settings. Pregnancy and postpartum immune shifts can modify inflammatory disease, venous thrombosis can mimic limb pain, and medicine exposure may constrain treatment. Risk factors adjust probability and urgency; their absence never rules out infection, inflammatory disease or structural injury.
Diagnosis
History
Establish whether onset was hours, days or months; whether pain is mono-, oligo- or polyarticular; and whether stiffness, swelling, warmth or functional loss is present. Ask about trauma, fever, rigors, rash, weight loss, night sweats, eye symptoms, psoriasis, bowel or genitourinary symptoms, urethral or vaginal discharge and tick or travel exposure where relevant. Record prior flares, surgery, prosthesis, injections, infections and antibiotics. Review psoriasis, IBD, TB and family history. Medicines—including diuretics, anticoagulants, corticosteroids, DMARDs, biologics and supplements—pregnancy possibility and lactation must be explicit.
Examination
Perform vital signs and sepsis assessment before a complete musculoskeletal examination. Distinguish true synovitis from periarticular tenderness by looking for effusion, warmth, boggy swelling and restriction of both active and passive movement. Map every symptomatic and silent swollen joint, symmetry and small-joint involvement. Examine skin, nails, eyes, mouth, entheses, dactylitis, spine and sacroiliac region. Look for tophi, nodules, lymphadenopathy and hepatosplenomegaly. Neurovascular status and the joint above and below are essential after trauma.
Investigations
Aspirate an unexplained acute swollen joint urgently when infection is possible. Send adequate synovial fluid for Gram stain and culture, cell count with differential and crystal microscopy; blood-culture bottles may improve microbiological yield when locally validated. Obtain blood cultures before antibiotics if this does not delay sepsis care. Blood count, CRP, ESR, renal and liver profiles support context but cannot exclude infection. Rheumatoid factor and anti-CCP are used when clinical synovitis suggests rheumatoid arthritis; normal results do not cancel referral. Plain radiographs assess fracture, chronic damage and baseline structure. Ultrasound can confirm effusion and guide aspiration; MRI is reserved for defined questions such as occult infection, osteonecrosis, internal derangement or axial disease.
Differential Diagnosis
Acute monoarthritis is first divided into infection, crystals, trauma or haemarthrosis, and other inflammation. Septic arthritis includes common pyogenic organisms, gonococcal disease and infection seeded from skin, blood, procedure or surgery. Tuberculosis, fungal infection and brucellosis are typically more indolent but may accelerate in immunosuppression. Gout and calcium pyrophosphate arthritis can closely mimic infection, and either may coexist with positive cultures. Trauma, fracture, ligament or meniscal injury, osteonecrosis and anticoagulant-related haemarthrosis are structural alternatives. A Baker cyst rupture, cellulitis, bursitis or deep-vein thrombosis may be mislocalized as joint pain.
Polyarticular inflammatory disease includes rheumatoid arthritis, psoriatic arthritis, axial or peripheral spondyloarthritis, reactive arthritis, systemic lupus erythematosus, vasculitis and other connective-tissue disease. Viral causes include chikungunya, parvovirus, hepatitis B or C and others selected by exposure and syndrome. Rheumatic fever belongs in an age- and criterion-based assessment, not an isolated antistreptolysin titre. Disseminated gonococcal infection may cause migratory pain, tenosynovitis and skin lesions. Fever with rash or multiorgan involvement broadens the differential to sepsis, adult-onset Still disease and drug reactions.
Mechanical and periarticular causes include osteoarthritis, tendinopathy, bursitis, enthesopathy, hypermobility, referred spinal pain and neuropathy. Fibromyalgia causes widespread pain and tenderness without objective synovitis, but it can coexist with inflammatory disease. Chronic monoarthritis demands deliberate exclusion of tuberculosis, low-grade bacterial infection, foreign body, pigmented villonodular synovitis, neuropathic arthropathy and tumour. In children, transient synovitis cannot be assumed until septic hip and malignancy are considered. Pregnancy-related pelvic girdle pain is common, but a hot swollen joint, fever, focal bony pain or thrombotic features requires a separate urgent pathway.
Management
An unstable patient with suspected joint infection receives ABCDE care, cultures and sepsis management while orthopaedic and microbiology or infectious-disease teams are contacted. In a stable patient, aspirate before starting antibiotics to preserve culture yield; in sepsis or septic shock, do not delay antibiotics for aspiration that cannot be performed immediately. Drainage strategy—serial aspiration, arthroscopic lavage or open surgery—depends on joint, stage, organism, response and local expertise. Reassess pain, inflammatory markers, culture results and function because persistent infection may require repeat surgery. Begin protected mobilization and rehabilitation when infection control permits.
For crystal arthritis, first exclude infection, then choose an anti-inflammatory strategy around comorbidity, medicines and preference. Long-term gout management is not simply avoiding purine-rich foods: discuss urate-lowering therapy and treat-to-target follow-up when guideline indications are met. Persistent inflammatory synovitis needs early rheumatology review for diagnosis and disease-modifying therapy; repeated steroid bursts or analgesics should not substitute for that referral. Before immunosuppression, assess infection and perform regimen-specific screening, vaccination and laboratory monitoring.
Osteoarthritis management is guided by symptoms and function rather than imaging severity. Education, tailored therapeutic exercise and weight management when relevant are core treatments. A brief initial increase in discomfort with exercise does not mean structural harm, but the programme should be adapted to capability and red flags. Trauma requires protection, imaging and specialist care according to injury. Chronic pain management should address sleep, mood, work, safeguarding and goals without implying that pain is imaginary. Discharge after an undifferentiated acute flare requires improving physiology, a credible plan, explicit return advice and reliable review; inability to bear weight, increasing swelling, fever or systemic illness should prompt reassessment.
Prescribing Information
Do not prescribe through the diagnosis. For osteoarthritis, current guidelines places medicines alongside therapeutic exercise and recommends the lowest effective dose for the shortest possible time. It offers a topical NSAID for knee osteoarthritis, considers topical NSAIDs at other sites, and advises that an oral NSAID should follow an assessment of gastrointestinal, renal, liver and cardiovascular toxicity, pregnancy, age, current medicines and comorbidity, with gastroprotection. It does not recommend routine paracetamol or weak opioids, and does not offer strong opioids for osteoarthritis. Use current Indian product information for exact doses, contraindications and interactions.
For a gout flare, current guidelines says to consider an NSAID, colchicine or a short course of an oral corticosteroid after accounting for comorbidity, other medicines and patient preference. When starting or titrating urate-lowering therapy, it recommends colchicine for flare prevention while the serum-urate target is reached; if that is unsuitable, it considers a low-dose NSAID or oral corticosteroid, with individual adverse-effect review. In a hot swollen joint, septic arthritis must remain under active assessment before corticosteroid injection or repeat anti-inflammatory treatment. Do not use antibiotics or antituberculous therapy as a diagnostic trial for unexplained chronic joint pain.
DMARDs and biologics require an expert-led or clearly defined shared-care system; the monitoring, infection screen, vaccination plan and pregnancy advice are medicine specific. The BSR monitoring guideline places csDMARD initiation under expert supervision and requires counselling about fertility, pregnancy and breastfeeding. Do not stop or continue an antirheumatic agent reflexively in pregnancy or lactation: obtain current agent-specific rheumatology-obstetric advice. A fever or new hot joint in an immunosuppressed person requires urgent assessment rather than self-escalation of corticosteroids. For suspected septic arthritis, SANJO advises aspiration and blood cultures before antibiotics when this does not delay care, but immediate empiric treatment in sepsis or septic shock.
When to Refer
Refer immediately to emergency, orthopaedic and infection services for an acutely hot swollen joint when septic arthritis is possible, especially with sepsis, immunosuppression, prosthesis, recent procedure or inability to bear weight. Aspiration and referral should not be deferred because fever is absent, serum urate is high or crystals are found. A child with hip pain, fever, restricted movement or refusal to walk needs an urgent paediatric orthopaedic pathway. Trauma with deformity, neurovascular compromise, open injury or suspected compartment syndrome is also immediate.
Refer persistent synovitis of undetermined cause urgently to rheumatology. Small joints of hands or feet, more than one joint, or delayed presentation strengthen urgency, but normal CRP, ESR, rheumatoid factor or anti-CCP must not delay referral. Same-day or rapid specialist advice is warranted for systemic vasculitis features, rapidly progressive weakness, eye inflammation, major organ involvement or cervical myelopathy symptoms in rheumatoid disease. Chronic monoarthritis with weight loss, night sweats, sinus, draining lesion, TB risk or destructive imaging needs coordinated orthopaedic, rheumatology and infection assessment with tissue or fluid diagnosis.
Pregnancy or planned conception in a person using DMARDs, biologics or recurrent anti-inflammatory therapy requires rheumatology and obstetric medication review. Transfer should be capability-based when aspiration, microbiology, paediatric orthopaedics, arthroscopy or specialist imaging is unavailable. Confirm the accepting clinician and send antibiotic history, immunosuppression, prosthesis details, aspiration results and imaging. Routine referral is appropriate for uncertain recurrent gout, refractory symptoms, atypical osteoarthritis, progressive deformity, occupational limitation and rehabilitation needs. Surgery decisions should be based on pain, function and clinical appropriateness, not withheld solely because of age, disability or body size.
Red Flags
A hot, swollen, intensely painful joint with restricted passive movement is septic arthritis until assessed, whether or not fever is present. Sepsis physiology—hypotension, tachypnoea, altered mentation, oliguria, mottling or rising lactate—requires immediate resuscitation and antibiotics after rapidly obtainable cultures. Immunosuppression, diabetes, a prosthetic joint, recent surgery or injection, skin infection and bacteraemia amplify risk. Crystals do not neutralize these warnings. Persistent symptoms after aspiration, antibiotics or washout may indicate treatment failure and need repeat specialist review.
Structural red flags include inability to bear weight after trauma, deformity, open injury, severe pain with passive stretch, tense swelling, neurological deficit, absent pulse or a cool limb. Sudden hip pain with steroid exposure, alcohol excess or pregnancy-related risk can indicate osteonecrosis. Focal night pain, unexplained fracture, mass, weight loss or persistent bony tenderness raises malignancy or osteomyelitis concern. Back or neck pain with weakness, sphincter symptoms or saddle sensory change follows a spinal emergency pathway rather than a peripheral-joint algorithm.
Systemic inflammatory warnings include visual loss or painful red eye, haemoptysis, glomerular features, mononeuritis, digital ischaemia, severe rash, myocarditis symptoms or rapidly progressive muscle weakness. Pregnancy adds medication toxicity, venous thrombosis and infection considerations; fever, unilateral leg swelling or cardiopulmonary symptoms are not routine arthritis. Chronic monoarthritis with TB exposure, night sweats, sinus formation or gradual destruction requires prompt tissue and microbiological evaluation. A normal white count, CRP, ESR or early radiograph does not exclude infection, and a positive autoimmune antibody without compatible clinical disease should not divert attention from a dangerous alternative.
Indian Clinical Context
Indian joint-pain pathways must distinguish widespread mechanical and inflammatory disease from infections whose delay causes preventable disability. Tuberculous arthritis often presents as chronic monoarthritis with pain, swelling and progressive loss of movement; constitutional symptoms may be absent. Diagnosis should pursue synovial fluid or tissue for microbiology and histopathology as appropriate, with imaging used to define extent. Empirical anti-TB therapy for every chronic swollen joint is unsafe because bacterial infection, crystal disease, inflammatory arthritis and malignancy can mimic it, while incomplete or inappropriate treatment promotes toxicity and resistance. Link confirmed disease to NTEP pathways.
Chikungunya can produce prolonged inflammatory polyarthralgia after acute fever, but the label should rest on epidemiology and appropriate testing, not be used indefinitely for every seronegative arthritis. Consider rheumatic fever, hepatitis-related disease, brucellosis and other infections only when the clinical and exposure pattern supports them. Ask non-judgementally about pharmacy purchases, traditional treatments and prior injections, and treat a new hot joint or systemic illness as a safety problem rather than an opportunity for repeated unsupervised injections.
Access differs sharply between urban tertiary centres and facilities without microscopy, culture, ultrasound guidance or rheumatology. A hot joint still needs an urgent pathway: aspirate using trained sterile technique where safe, obtain blood cultures, begin sepsis care and transfer for drainage if local capability is inadequate. Chronic inflammatory disease needs shared-care monitoring that is feasible near the patient, with clear responsibility for blood results and medicine interruption during serious infection. Physiotherapy advice must fit work, travel, floor sitting and home conditions. Use plain-language explanations and avoid framing exercise as joint wear. Document disability, financial constraints and transport because they determine whether a theoretically correct follow-up plan is safe.

Undergraduate competence is bounded. This guide does not credential independent joint aspiration, ultrasound guidance, intra-articular injection, operative washout, prosthetic-joint-infection management, DMARD initiation or modification of antituberculous regimens. These need supervision, local protocols and assessed procedural skills. Faculties should verify exact codes and outcomes in the current NMC curriculum instead of inventing a single universal “joint pain” competency. The safe graduate outcome is syndrome classification, early stabilization, recognition of septic and structural emergencies, rational first investigations, safe analgesic reasoning and timely referral for disease-modifying or definitive care.
Key Exam Pearls for NEET PG
For acute monoarthritis, septic arthritis and crystals are the first major branches. Aspirate an unexplained hot swollen joint: send cell count and differential, Gram stain and culture, plus polarized microscopy for crystals. No synovial white-cell threshold alone perfectly confirms or excludes infection. Urate crystals are needle shaped and strongly negatively birefringent; calcium pyrophosphate crystals are rhomboid and weakly positively birefringent. Crystal identification does not exclude concurrent sepsis. Serum urate may fall during an acute gout flare, so a normal value does not rule gout out.
Inflammatory arthritis causes synovitis, prolonged morning stiffness and functional loss, whereas osteoarthritis is usually activity related with brief stiffness. Osteoarthritis in a typical older adult is often a clinical diagnosis and routine imaging may not be required. Rheumatoid arthritis commonly produces symmetric small-joint polyarthritis but can present variably. Rheumatoid factor is not specific and anti-CCP is more specific, yet negative serology or normal inflammatory markers does not cancel urgent referral for persistent synovitis. Spondyloarthritis clues include inflammatory back pain, enthesitis, dactylitis, psoriasis, uveitis and inflammatory bowel disease.
TB arthritis is typically chronic monoarthritis and requires microbiological or tissue confirmation whenever feasible. A child with fever and restricted hip movement has septic arthritis until excluded; transient synovitis is a diagnosis made after risk assessment. In septic arthritis, obtain cultures and aspirate before antibiotics if the patient is stable, but never delay antibiotics in sepsis or shock. In gout, acute anti-inflammatory treatment and long-term urate-lowering strategy answer different problems. In pregnancy, medicine safety is agent specific; uncontrolled inflammation and abrupt withdrawal can also harm. Examination questions reward identifying the dangerous hot joint, choosing aspiration, and referring persistent inflammatory synovitis before waiting for serology.
Frequently Asked Questions
Can crystals in synovial fluid rule out septic arthritis?
No. Monosodium urate or calcium pyrophosphate crystals can coexist with bacterial infection. Continue culture and clinical assessment, especially when the patient is systemically unwell, immunosuppressed, has a prosthesis or recent procedure, or fails to improve as expected. A crystal result explains one mechanism but does not sterilize the joint or replace microbiological testing and follow-up.
Should antibiotics wait until after aspiration of a hot swollen joint?
In a haemodynamically stable patient, obtain synovial fluid and blood cultures before antibiotics whenever this can be done promptly, because prior treatment can reduce culture yield. In sepsis or septic shock, do not delay appropriate antibiotics for an aspiration that is not immediately available. Resuscitation, sampling, antimicrobial therapy and urgent orthopaedic source-control planning should proceed as one coordinated pathway.
Do negative rheumatoid factor and anti-CCP tests exclude rheumatoid arthritis?
No. Rheumatoid arthritis is a clinical diagnosis supported by examination and investigations, and seronegative disease occurs. Persistent synovitis, particularly in more than one joint or the small joints of hands or feet, warrants urgent rheumatology referral even with normal acute-phase markers and negative antibodies. Primary-care tests should not create a delay in specialist assessment or early disease-modifying treatment.
When should tuberculosis be suspected in chronic joint pain in India?
Consider osteoarticular tuberculosis in chronic monoarthritis or focal bone and joint disease with progressive restriction, destructive imaging, sinus formation, TB exposure, previous disease, immunosuppression or constitutional features. Absence of fever or weight loss does not exclude it. Seek appropriate synovial or tissue microbiology and histopathology rather than using an empirical treatment response as the diagnostic test, and link confirmed disease to NTEP care.
