Tramadol is a centrally acting analgesic with a unique dual mechanism that makes it useful for moderate pain but also introduces risks not typical of other opioids, including seizures and serotonin syndrome.
Clinical Pharmacology Overview
Tramadol has a dual mechanism of action: it is a weak mu-opioid receptor agonist and also inhibits reuptake of serotonin (5-HT) and norepinephrine (NE). The (+)-enantiomer predominantly inhibits serotonin reuptake and has higher affinity for the mu receptor, while the (-)-enantiomer predominantly inhibits norepinephrine reuptake. The active metabolite O-desmethyltramadol (M1), formed by CYP2D6, has 6 times greater mu-opioid affinity than the parent compound.
Pharmacokinetics: Oral bioavailability is 68-75% (increases to nearly 100% with repeated dosing due to saturation of first-pass metabolism). Protein binding is 20%. Metabolised by CYP2D6 (to active M1 metabolite) and CYP3A4. Half-life is 5-7 hours. Renally excreted (90%).

Approved Indications in Indian Clinical Practice
- Moderate to moderately severe pain
- Post-operative pain (step-down from injectable opioids)
- Chronic pain (musculoskeletal, neuropathic -- with caution regarding dependence)
- Cancer pain (as part of WHO pain ladder -- Step 2)
- Dental pain
- Labour analgesia (used in some Indian centres, though not first-line)
Dosing in Adults
Immediate-release: 50-100 mg orally every 4-6 hours as needed. Start at 25 mg and titrate to reduce GI side effects. Maximum: 400 mg/day.
Extended-release: 100 mg once daily, titrate by 100 mg every 5 days. Maximum: 300 mg/day.
IV/IM: 50-100 mg every 4-6 hours. Maximum: 600 mg/day (injectable).
Tramadol + paracetamol (37.5/325 mg): 1-2 tablets every 4-6 hours. Maximum: 8 tablets/day.
Elderly (over 75 years): maximum 300 mg/day. Start at the lowest dose.
Important Drug Interactions
- MAO inhibitors: CONTRAINDICATED -- risk of fatal serotonin syndrome (wait 14 days after MAOI discontinuation)
- SSRIs, SNRIs, TCAs, triptans: increased serotonin syndrome risk
- Carbamazepine: CYP3A4 inducer, reduces tramadol efficacy AND increases seizure risk
- CYP2D6 inhibitors (fluoxetine, paroxetine, quinidine): reduce conversion to active M1 metabolite, reducing efficacy
- Ondansetron: may reduce tramadol analgesic efficacy (5-HT3 antagonism)
- Warfarin: enhanced anticoagulant effect reported
- Benzodiazepines: respiratory depression risk (lower than with strong opioids but still relevant)
Side Effects and Monitoring
Common: nausea, vomiting, dizziness, constipation, headache, drowsiness, dry mouth.
Serious: seizures (dose-dependent, especially above 400 mg/day), serotonin syndrome, respiratory depression (less than morphine but not negligible), dependence and withdrawal syndrome, anaphylactoid reactions.
Withdrawal: atypical opioid withdrawal plus anxiety, panic attacks, paraesthesia -- reflecting the serotonin/norepinephrine component.
Monitoring: pain scores, sedation level, respiratory rate. Watch for seizure activity. Screen for serotonergic co-medications. Monitor for signs of dependence with prolonged use.
Special Populations
Pregnancy: Category C. Not recommended in pregnancy. Chronic use in late pregnancy can cause neonatal withdrawal syndrome. Neonatal seizures have been reported.
Renal impairment: Extend dosing interval to every 12 hours when GFR < 30 mL/min. Maximum 200 mg/day. Avoid in severe renal failure (not dialysable efficiently).
Hepatic impairment: Reduce dose. Avoid in severe hepatic impairment. Extended-release formulation is contraindicated.
Elderly: Start low (25 mg), maximum 300 mg/day. Higher sensitivity to CNS effects and seizure risk.
CYP2D6 ultra-rapid metabolisers: increased conversion to M1, higher opioid effect and toxicity risk.

It is NOT currently listed under the NDPS Act, though its abuse potential is recognised. Some Indian states have imposed additional restrictions. Tramadol-paracetamol combinations (Ultracet) are also Schedule H1. The CDSCO monitors tramadol prescribing trends as part of opioid pharmacovigilance.

The seizure risk increases at doses above 400 mg/day, in patients with epilepsy, and with concurrent serotonergic or epileptogenic drugs. Seizures can occur even at therapeutic doses.
What is the serotonin syndrome risk with tramadol?
Tramadol's serotonin reuptake inhibition can cause serotonin syndrome when combined with SSRIs, SNRIs, MAO inhibitors, triptans, or other serotonergic drugs. Symptoms include agitation, hyperthermia, clonus, diaphoresis, and in severe cases, rhabdomyolysis and death. The combination with MAO inhibitors is contraindicated.
How does tramadol compare to other opioids?
Tramadol is a weak mu-opioid agonist with approximately one-tenth the potency of morphine. Its analgesic effect comes from both opioid receptor binding and monoamine reuptake inhibition. It has lower abuse potential than strong opioids but dependence does occur, especially with prolonged use.
