Clinical Pharmacology Overview
Metformin is a biguanide that primarily reduces hepatic glucose production via activation of AMP-activated protein kinase (AMPK). It also enhances peripheral insulin sensitivity and reduces intestinal glucose absorption. Unlike sulfonylureas, metformin does not stimulate insulin secretion and therefore carries minimal risk of hypoglycaemia when used as monotherapy.
Pharmacokinetics: Oral bioavailability is 50-60%. It is not protein-bound and is excreted unchanged by the kidneys via tubular secretion. The plasma half-life is approximately 6 hours. Steady-state concentrations are reached within 24-48 hours.

- Type 2 diabetes mellitus as first-line monotherapy
- Type 2 diabetes as part of combination therapy with sulfonylureas, DPP-4 inhibitors, SGLT2 inhibitors, thiazolidinediones, or insulin
- Polycystic ovary syndrome (off-label but widely used in India for insulin resistance and anovulation)
- Pre-diabetes with high-risk features (off-label)
Dosing in Adults
Immediate-release: Start at 500 mg once or twice daily with meals. Titrate by 500 mg every 1-2 weeks as tolerated. Usual effective dose is 1500-2000 mg per day in divided doses. Maximum: 2550 mg per day.
Extended-release (SR/XR): Start at 500 mg once daily with the evening meal. Titrate by 500 mg weekly. Maximum: 2000 mg once daily.
Gradual dose titration minimises gastrointestinal side effects, which are the most common reason for discontinuation.
Important Drug Interactions
- Iodinated contrast media: withhold metformin 48 hours before and after contrast procedures due to risk of lactic acidosis in the setting of contrast-induced nephropathy
- Alcohol: increased risk of lactic acidosis
- Carbonic anhydrase inhibitors (topiramate, acetazolamide): may increase metformin levels
- Cimetidine: competes for renal tubular secretion, raising metformin levels
- Corticosteroids: antagonise glycaemic control
Side Effects and Monitoring
Common: nausea, diarrhoea, abdominal discomfort, metallic taste. These are dose-related and often resolve with slow titration.
Serious: lactic acidosis (rare but potentially fatal, mainly in patients with renal impairment, hepatic failure, or tissue hypoxia).
Long-term: vitamin B12 deficiency (monitor levels periodically, especially in patients on high doses or with peripheral neuropathy).
Monitoring: HbA1c every 3-6 months, serum creatinine and eGFR at least annually (more frequently if borderline), vitamin B12 levels annually after 3-4 years of use.
Special Populations
Pregnancy: Category B. Not recommended in pregnancy in Indian practice; insulin is preferred. However, some guidelines permit metformin in gestational diabetes when insulin is refused or unavailable.
Renal impairment: Contraindicated if eGFR < 30 mL/min/1.73 m2. Use with caution and dose reduction at eGFR 30-45. No dose adjustment needed above eGFR 45.
Elderly: Start at lower doses. Monitor renal function more frequently as age-related decline in GFR increases accumulation risk.
Hepatic impairment: Avoid in clinical or laboratory evidence of hepatic disease due to impaired lactate clearance.
NEET PG High-Yield Points
- Mechanism: activates AMPK, reduces hepatic gluconeogenesis
- Does NOT cause hypoglycaemia as monotherapy (a recurring MCQ theme)
