Rifampicin is the backbone of modern tuberculosis treatment and the single most important drug in anti-TB chemotherapy. India bears the world's highest TB burden, making rifampicin prescribing knowledge essential for every Indian clinician.
Clinical Pharmacology Overview
Rifampicin inhibits bacterial DNA-dependent RNA polymerase by binding to its beta subunit, blocking RNA transcription. It is bactericidal against actively multiplying M. tuberculosis and, critically, against semi-dormant persisters -- the property that allows short-course chemotherapy. It has broad antimycobacterial activity and is also active against many gram-positive and some gram-negative bacteria.
Pharmacokinetics: Oral bioavailability is 90-95% on an empty stomach (reduced by food). Protein binding is 80%. Metabolised in the liver to desacetyl rifampicin (also active). Half-life is 2-5 hours (increases initially, decreases with repeated dosing due to auto-induction of its own metabolism). Eliminated primarily in bile (enterohepatic circulation) and urine.

Approved Indications in Indian Clinical Practice
- Pulmonary and extrapulmonary tuberculosis (as part of multidrug regimen)
- Latent tuberculosis infection (LTBI) prophylaxis
- Leprosy (multibacillary and paucibacillary -- monthly supervised dose in MDT)
- Staphylococcal prosthetic device infections (with another agent)
- Brucellosis (combination therapy)
- Meningococcal and H. influenzae type b prophylaxis
- Legionella infections (combination)
Dosing in Adults (TB)
NTEP (National Tuberculosis Elimination Programme) regimen:
Intensive phase (2 months): Rifampicin 450 mg (body weight below 60 kg) or 600 mg (60 kg and above) daily, in FDC with isoniazid, pyrazinamide, and ethambutol.
Continuation phase (4 months): Rifampicin + isoniazid daily.
Total duration: 6 months for drug-sensitive TB.
Administer on an empty stomach (1 hour before or 2 hours after meals) for optimal absorption.
Leprosy (WHO MDT): 600 mg once monthly (supervised).
Meningococcal prophylaxis: 600 mg every 12 hours for 2 days.
Important Drug Interactions
Rifampicin is the most potent CYP enzyme inducer in clinical use. Critical interactions:
- Oral contraceptives: FAILURE -- use alternative contraception
- Warfarin: markedly reduced effect -- dose increase needed, monitor INR frequently
- Protease inhibitors (ritonavir, lopinavir): contraindicated -- use rifabutin instead
- NNRTIs: efavirenz is preferred (dose adjustment may be needed); nevirapine levels are significantly reduced
- Corticosteroids: reduced efficacy -- may need to double steroid dose
- Antifungals (ketoconazole, itraconazole, voriconazole): markedly reduced levels -- avoid or use alternatives
- Cyclosporine, tacrolimus: drastically reduced levels -- transplant rejection risk
- Methadone: reduced levels -- withdrawal symptoms
- Oral hypoglycaemics (sulfonylureas, meglitinides): reduced efficacy
- Phenytoin: complex -- levels may rise initially then fall
Side Effects and Monitoring
Common: orange-red discolouration of urine, tears, sweat, saliva (harmless, warn patient). GI upset, nausea.
Serious: hepatotoxicity (especially in combination with isoniazid and pyrazinamide), thrombocytopenia (immune-mediated, often with intermittent dosing), flu-like syndrome (with intermittent/irregular dosing), acute renal failure (rare, intermittent dosing).
Drug-induced liver injury: monitor LFTs at baseline and monthly for the first 3 months (or earlier if symptomatic). Stop all hepatotoxic anti-TB drugs if ALT exceeds 5x ULN (asymptomatic) or 3x ULN with symptoms.
Monitoring: LFTs, CBC, serum creatinine at baseline and periodically. Educate about orange discolouration and contact lens staining. Check for drug interactions at every visit.
Special Populations
Pregnancy: Category C. Generally considered safe in pregnancy for TB treatment (the risk of untreated TB in pregnancy far outweighs drug risk). Included in NTEP pregnancy TB treatment regimens. Crosses the placenta.
Renal impairment: No dose adjustment for GFR above 30 mL/min (primarily hepatic/biliary elimination). Some guidelines suggest caution at GFR below 30.
Hepatic impairment: Use with caution. Increased hepatotoxicity risk. Monitor LFTs closely. In severe hepatic disease, modified regimens may exclude rifampicin.
HIV co-infection: Use efavirenz-based ART. Protease inhibitors are contraindicated. Refer to NTEP/NACO guidelines for TB-HIV co-treatment timing.

Indian Regulatory Status
Rifampicin is a Schedule H drug under the Drugs and Cosmetics Act. It is included in the NLEM of India. Under the NTEP (formerly RNTCP), rifampicin-containing FDCs are provided free of charge through the government programme. India transitioned from intermittent (thrice-weekly) to daily rifampicin-containing regimens in 2017 under NTEP. The CDSCO mandates that rifampicin be sold only as part of anti-TB FDCs (not as single-drug formulation to the public) to prevent monotherapy and resistance development.

Why is rifampicin the most important anti-TB drug?
Rifampicin is bactericidal against both actively growing and semi-dormant (persister) M. tuberculosis bacilli. Its ability to kill persisters is what allows shortening of TB treatment from 18 months to 6 months. Loss of rifampicin from a regimen is the defining event of MDR-TB.
Why does rifampicin cause so many drug interactions?
Rifampicin is the most potent known inducer of hepatic CYP enzymes (especially CYP3A4, CYP2C9, CYP2C19) and P-glycoprotein. It accelerates the metabolism of hundreds of drugs including antiretrovirals, oral contraceptives, warfarin, steroids, antifungals, and immunosuppressants. This induction takes 1-2 weeks to develop and 2 weeks to resolve after stopping.
Can rifampicin be used with antiretroviral therapy?
Rifampicin significantly reduces levels of protease inhibitors (contraindicated combination) and NNRTIs. Efavirenz-based ART is the preferred regimen with rifampicin-containing TB treatment. Alternatively, rifabutin (a weaker CYP inducer) can replace rifampicin. The NTEP and NACO guidelines provide specific recommendations for TB-HIV co-treatment.
Why do body fluids turn orange-red on rifampicin?
Rifampicin and its metabolites are orange-red in colour. This causes harmless discolouration of urine, tears, sweat, saliva, and sputum. Patients must be warned to prevent alarm and to avoid permanent staining of contact lenses. This discolouration is also used as a rough adherence marker.
