Dosing in Adults
GERD and peptic ulcer: 20 mg orally once daily before breakfast for 4-8 weeks.
Maintenance: 10-20 mg once daily.
H. pylori eradication: 20 mg twice daily as part of triple therapy for 14 days.
Zollinger-Ellison syndrome: 60 mg once daily initially; doses up to 100 mg/day or 60 mg twice daily may be needed.
NSAID ulcer prophylaxis: 10-20 mg once daily.
Timing: 30-60 minutes before a meal, preferably before breakfast.
Important Drug Interactions
- Clopidogrel: rabeprazole has the least interaction with clopidogrel among PPIs (similar to pantoprazole) due to minimal CYP2C19 inhibition
- Ketoconazole, itraconazole: reduced absorption due to elevated gastric pH
- Methotrexate: potential for increased methotrexate levels
- Atazanavir: reduced absorption -- avoid combination

Pregnancy: Category B. Limited human data. Use only if clearly needed. Omeprazole has the most pregnancy safety data among PPIs.
Renal impairment: No dose adjustment required.
Hepatic impairment: Use with caution in severe hepatic impairment. No formal dose adjustment but clearance is reduced.
Elderly: No dose adjustment. Monitor for long-term adverse effects (fractures, infections).
