Pantoprazole is one of the most prescribed proton pump inhibitors in India, used extensively for acid-related disorders. This guide covers its pharmacology, appropriate use, long-term safety concerns, and NEET PG relevance.
Clinical Pharmacology Overview
Pantoprazole is a substituted benzimidazole that irreversibly inhibits the hydrogen-potassium ATPase (proton pump) on the parietal cell apical membrane. It is a prodrug that requires activation in the acidic environment of the parietal cell canaliculus. Once activated, it forms covalent disulphide bonds with cysteine residues on the proton pump, permanently inactivating it. New pump synthesis (half-life approximately 50 hours) is required to restore acid secretion.
Pharmacokinetics: Bioavailability is approximately 77% (among the highest of PPIs). Protein binding is 98%. Extensively metabolised by CYP2C19 and CYP3A4 in the liver. Half-life is 1-2 hours, but duration of acid suppression is 24 hours due to irreversible pump inhibition. Peak acid suppression occurs at 2-6 hours.

Approved Indications in Indian Clinical Practice
- Gastro-oesophageal reflux disease (GERD)
- Peptic ulcer disease (gastric and duodenal ulcers)
- H. pylori eradication (as part of triple or quadruple therapy)
- Zollinger-Ellison syndrome
- NSAID-induced gastropathy (treatment and prophylaxis)
- Stress ulcer prophylaxis in critically ill patients
- Upper GI bleeding (IV formulation)
Dosing in Adults
GERD and peptic ulcer: 40 mg orally once daily before breakfast for 4-8 weeks.
Maintenance therapy: 20 mg once daily.
H. pylori eradication: 40 mg twice daily as part of triple therapy (with amoxicillin 1 g BD + clarithromycin 500 mg BD) for 14 days.
Zollinger-Ellison syndrome: 40-80 mg twice daily (doses up to 240 mg/day may be needed).
IV for GI bleeding: 80 mg bolus followed by 8 mg/hour continuous infusion for 72 hours after endoscopic haemostasis.
Timing: Always administer 30-60 minutes before a meal for optimal efficacy.
Important Drug Interactions
- Clopidogrel: PPIs (especially omeprazole) may reduce clopidogrel activation via CYP2C19 inhibition. Pantoprazole has the weakest interaction and is the preferred PPI with clopidogrel.
- Methotrexate: PPIs may reduce methotrexate clearance, risking toxicity
- Ketoconazole, itraconazole: reduced absorption due to elevated gastric pH
- Iron, calcium, vitamin B12: reduced absorption with chronic PPI use
- Atazanavir: significantly reduced absorption -- avoid concurrent use
Side Effects and Monitoring
Common: headache, diarrhoea, nausea, abdominal pain, flatulence.
Long-term risks: hypomagnesaemia (can cause arrhythmias and seizures), vitamin B12 deficiency, increased fracture risk (hip, wrist, spine), C. difficile-associated diarrhoea, community-acquired pneumonia, potential chronic kidney disease.
Rare: acute interstitial nephritis, subacute cutaneous lupus erythematosus.
Monitoring: magnesium levels if on long-term therapy (especially with diuretics), B12 if symptomatic, bone density in high-risk patients on long-term use.
Special Populations
Pregnancy: Category B. Generally acceptable when a PPI is needed. Omeprazole has more safety data; pantoprazole is a reasonable alternative.
Renal impairment: No dose adjustment required.
Hepatic impairment: Maximum dose 20 mg/day in severe hepatic impairment. Half-life is prolonged in cirrhosis.
Elderly: No specific dose adjustment but limit duration. Higher risk of fractures and hypomagnesaemia.

Indian Regulatory Status
Pantoprazole is a Schedule H drug under the Drugs and Cosmetics Act. It is included in the NLEM of India. The CDSCO permits its sale only with a valid prescription. Fixed-dose combinations of pantoprazole with domperidone (e.g., Pan-D) are among the most prescribed FDCs in India and are Schedule H. The DCGI has periodically reviewed PPI-domperidone FDCs for rational drug use.
Frequently Asked Questions
Why should pantoprazole be taken before meals?
PPIs inhibit only actively secreting proton pumps. Taking pantoprazole 30-60 minutes before a meal ensures peak drug levels coincide with meal-stimulated acid secretion, maximising the number of pumps inhibited.
What are the risks of long-term PPI use?
Long-term risks include hypomagnesaemia, vitamin B12 deficiency, increased fracture risk (especially hip), C. difficile infection, community-acquired pneumonia, and chronic kidney disease. These risks must be weighed against benefits, and PPIs should be used at the lowest effective dose for the shortest necessary duration.
Is pantoprazole safe in pregnancy?
Pantoprazole is Category B. While data is more limited than for omeprazole, it is generally considered acceptable when a PPI is needed. Omeprazole has the most pregnancy data among PPIs.
Can pantoprazole be given IV?
Yes. IV pantoprazole 40 mg is available and commonly used in Indian hospitals for upper GI bleeding, stress ulcer prophylaxis, and when oral administration is not possible. Continuous infusion (8 mg/hour after 80 mg bolus) is used after endoscopic haemostasis for bleeding peptic ulcers.
