Diclofenac is one of the most widely used NSAIDs in India across oral, injectable, and topical formulations. It is effective for pain and inflammation but carries significant cardiovascular and gastrointestinal risks that must be weighed carefully.
Clinical Pharmacology Overview
Diclofenac is a phenylacetic acid derivative that non-selectively inhibits cyclooxygenase (COX-1 and COX-2), with relative COX-2 selectivity. It reduces prostaglandin synthesis, thereby producing analgesic, anti-inflammatory, and antipyretic effects. It also inhibits the lipoxygenase pathway to some extent.
Pharmacokinetics: Oral bioavailability is approximately 50-60% (significant first-pass metabolism). Highly protein-bound (more than 99%). Metabolised by CYP2C9 and CYP3A4. Half-life is 1-2 hours, but anti-inflammatory effect persists longer due to accumulation in synovial fluid. Excreted in urine (65%) and bile (35%).

Approved Indications in Indian Clinical Practice
- Rheumatoid arthritis and osteoarthritis
- Ankylosing spondylitis
- Acute musculoskeletal pain and soft tissue injuries
- Post-operative pain
- Renal colic (IM diclofenac is commonly used in Indian emergency departments)
- Dysmenorrhoea
- Acute gout
- Dental pain
- Topical: localised musculoskeletal pain
Dosing in Adults
Oral: 50 mg two to three times daily with meals. Maximum: 150 mg/day.
Sustained release: 100 mg once daily.
IM injection: 75 mg deep IM into the gluteal muscle. May repeat once after 30 minutes if needed. Do not use IM formulation for more than 2 days.
Topical: apply gel (1%) to affected area three to four times daily.
Rectal suppository: 50-100 mg at bedtime (useful when oral route is not available).
Always use the lowest effective dose for the shortest duration.
Important Drug Interactions
- Anticoagulants (warfarin, heparin, DOACs): increased bleeding risk
- Aspirin: reduced cardioprotective effect of low-dose aspirin (pharmacodynamic interaction at COX-1)
- ACE inhibitors, ARBs: reduced antihypertensive effect, increased renal impairment risk
- Diuretics: reduced diuretic effect, increased nephrotoxicity risk
- Lithium: increased lithium levels
- Methotrexate: reduced renal clearance of methotrexate
- SSRIs: increased GI bleeding risk
- Corticosteroids: increased GI ulcer and bleeding risk
- Cyclosporine: increased nephrotoxicity
Side Effects and Monitoring
Common: dyspepsia, nausea, abdominal pain, diarrhoea, headache.
Serious: GI bleeding and perforation, cardiovascular events (MI, stroke), renal impairment (especially in volume-depleted patients), hepatotoxicity (monitor LFTs with chronic use), Stevens-Johnson syndrome (rare), acute generalised exanthematous pustulosis.
Cardiovascular: diclofenac has cardiovascular risk comparable to COX-2 selective inhibitors. Avoid in established CVD.
Monitoring: blood pressure, renal function, LFTs for chronic use. H. pylori testing and eradication before long-term NSAID therapy. Consider gastroprotection (PPI) for high-risk patients.
Special Populations
Pregnancy: Category C (first and second trimester), Category D (third trimester). Avoid after 30 weeks gestation -- risk of premature closure of ductus arteriosus. NSAIDs may also impair fertility.
Renal impairment: Avoid in severe renal impairment (GFR < 30). Use with caution and adequate hydration in moderate impairment. NSAIDs reduce renal blood flow via prostaglandin inhibition.
Hepatic impairment: Use with caution. Diclofenac is associated with hepatotoxicity. Monitor LFTs.
Elderly: Highest risk group for GI bleeding and renal impairment. Use lowest dose, shortest duration, with gastroprotection.

Indian Regulatory Status
Diclofenac for human use is a Schedule H drug under the Drugs and Cosmetics Act. Veterinary diclofenac was banned by the Government of India in 2006 to protect endangered Gyps vulture species (meloxicam was approved as a veterinary alternative). Human formulations remain available as tablets, capsules, injections, suppositories, and topical preparations. Included in the NLEM for select formulations. FDCs of diclofenac with paracetamol are among the most prescribed analgesic combinations in India (Schedule H).

What is the cardiovascular risk of diclofenac?
Diclofenac carries a cardiovascular risk similar to selective COX-2 inhibitors (rofecoxib class). The SOS and CNT meta-analyses showed approximately 40% increased risk of major vascular events. It should be avoided in patients with established cardiovascular disease and used at the lowest effective dose for the shortest duration.
What is the maximum dose of diclofenac?
The maximum recommended daily dose is 150 mg (orally) in divided doses. The CDSCO and European regulatory authorities have reduced the recommended dose ceiling. Use the lowest effective dose for the shortest possible duration. The injectable form should not be used for more than 2 days.
Can diclofenac be used in dengue fever?
NSAIDs including diclofenac are CONTRAINDICATED in suspected dengue fever because they impair platelet function and increase the risk of bleeding, which is already a major concern in dengue haemorrhagic fever. Paracetamol is the recommended antipyretic and analgesic in dengue.
