Ciprofloxacin is a second-generation fluoroquinolone with excellent gram-negative activity and unique anti-Pseudomonal coverage among oral antibiotics. Rising resistance in India and global safety warnings demand judicious prescribing.
Clinical Pharmacology Overview
Ciprofloxacin inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination. It is bactericidal and exhibits concentration-dependent killing. It has excellent gram-negative coverage including Pseudomonas aeruginosa, and moderate gram-positive activity (poor against Streptococcus pneumoniae -- unlike respiratory fluoroquinolones).
Pharmacokinetics: Oral bioavailability is 70-80%. Widely distributed with a volume of distribution of 2-3 L/kg. Moderate protein binding (20-40%). Partially metabolised hepatically and eliminated both renally (40-50%) and via faeces. Half-life is 4-6 hours. Achieves high concentrations in urine, bile, lung tissue, and prostate.

Approved Indications in Indian Clinical Practice
- Complicated urinary tract infections and pyelonephritis
- Chronic bacterial prostatitis
- Lower respiratory tract infections (not first-line for community-acquired pneumonia due to poor pneumococcal coverage)
- Bone and joint infections (one of few oral agents with bone penetration)
- Intra-abdominal infections (with metronidazole)
- Infectious diarrhoea (bacterial gastroenteritis, traveller's diarrhoea)
- Anthrax (post-exposure prophylaxis and treatment)
- Pseudomonas infections (one of few oral anti-pseudomonal agents)
- Ophthalmic and otic infections (topical formulations)
Dosing in Adults
UTI (uncomplicated): 250-500 mg orally twice daily for 3 days.
UTI (complicated) and pyelonephritis: 500 mg orally twice daily for 7-14 days.
Respiratory and systemic infections: 500-750 mg orally twice daily.
Bone and joint: 500-750 mg orally twice daily for 4-6 weeks.
IV: 200-400 mg every 8-12 hours (for patients unable to take oral).
Dose adjustment in renal impairment: GFR 30-50: usual dose; GFR 5-29: 250-500 mg every 12-18 hours.
Important Drug Interactions
- Antacids, calcium, iron, zinc, sucralfate: chelation reduces absorption by up to 90% -- separate by 2 hours
- Theophylline: ciprofloxacin inhibits CYP1A2, raising theophylline levels and seizure risk
- Tizanidine: contraindicated combination; ciprofloxacin massively increases tizanidine levels
- Warfarin: enhanced anticoagulant effect
- NSAIDs: increased CNS stimulation and seizure risk
- Cyclosporine: increased nephrotoxicity risk
- Corticosteroids: increased tendon rupture risk
Side Effects and Monitoring
Common: nausea, diarrhoea, headache, dizziness, insomnia.
Serious (boxed warning-level): tendon rupture (especially Achilles tendon, risk increased with corticosteroids and age over 60), peripheral neuropathy (may be irreversible), CNS effects (seizures, psychosis, raised intracranial pressure), aortic aneurysm/dissection.
Other: QT prolongation, C. difficile-associated diarrhoea, photosensitivity, crystalluria (ensure adequate hydration), dysglycaemia.
Monitoring: clinical response. Renal function if prolonged use. Advise patients to report tendon pain, numbness/tingling, or CNS symptoms immediately.
Special Populations
Pregnancy: Category C. Avoid in pregnancy due to potential cartilage toxicity. Use only if no safer alternative exists.
Children: Generally avoided under 18 years. Specific exceptions exist (complicated UTI, anthrax, cystic fibrosis).
Renal impairment: Dose reduction required when GFR < 30 mL/min.
Elderly: Higher risk of tendon rupture, especially if co-prescribed corticosteroids. Use with caution and at reduced doses.

Indian Regulatory Status
Ciprofloxacin is a Schedule H1 drug, reflecting heightened regulatory concern about antimicrobial resistance. Schedule H1 requires that the dispensing pharmacist maintain a register with the prescriber's name, patient name, and drug details. It is included in the NLEM. The ICMR's antimicrobial stewardship guidelines recommend restricting fluoroquinolone use to documented infections without safer alternatives. Topical ophthalmic preparations are Schedule H.
Frequently Asked Questions
Why are fluoroquinolones restricted in many countries?
The US FDA and EMA have issued boxed warnings for fluoroquinolones due to serious adverse effects including tendon rupture, peripheral neuropathy, CNS effects, and aortic aneurysm. They should be reserved for infections without safer alternatives. In India, ciprofloxacin remains widely used but stewardship is increasingly emphasised.
Can ciprofloxacin be used in children?
Ciprofloxacin is generally avoided in children under 18 due to the risk of cartilage damage in weight-bearing joints (demonstrated in juvenile animal studies). Exceptions include complicated UTI, anthrax post-exposure prophylaxis, and cystic fibrosis-related Pseudomonas infections where benefits outweigh risks.
Why should ciprofloxacin not be taken with milk or antacids?
Divalent and trivalent cations (calcium in milk, aluminium/magnesium in antacids, iron, zinc) chelate ciprofloxacin in the GI tract, forming insoluble complexes that dramatically reduce absorption. Separate ciprofloxacin from these substances by at least 2 hours.
Is ciprofloxacin effective against typhoid in India?
Ciprofloxacin was the drug of choice for enteric fever in India for decades, but fluoroquinolone resistance in S. Typhi has risen dramatically. Many Indian isolates now show decreased ciprofloxacin susceptibility (DCS). Current guidelines prefer ceftriaxone or azithromycin for empirical treatment of enteric fever.
