Azithromycin is an azalide macrolide antibiotic with unique tissue-concentrating pharmacokinetics that allow short-course therapy. It is one of the most prescribed antibiotics in India, making rational use and stewardship essential.
Clinical Pharmacology Overview
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, blocking translocation. It is bacteriostatic at usual concentrations but can be bactericidal at high tissue concentrations against certain organisms. It has excellent intracellular penetration and concentrates within phagocytes, which deliver it to sites of infection.
Pharmacokinetics: Oral bioavailability is approximately 37%. Absorbed better on an empty stomach (capsules) or unaffected by food (tablets and suspension). Minimal protein binding (7-50%). Extremely high volume of distribution (31 L/kg) with tissue concentrations 10-100 times higher than plasma. Tissue half-life is 68 hours, allowing short-course regimens. Primarily eliminated unchanged in bile and faeces.

Approved Indications in Indian Clinical Practice
- Community-acquired pneumonia
- Acute bacterial exacerbation of COPD
- Acute bacterial sinusitis
- Pharyngitis/tonsillitis (when beta-lactam allergic)
- Uncomplicated genital Chlamydia trachomatis
- Chancroid
- Enteric fever (as second-line)
- Traveller's diarrhoea
- MAC prophylaxis in HIV/AIDS
- Trachoma
Dosing in Adults
3-day regimen: 500 mg once daily for 3 days (total 1.5 g).
5-day regimen: 500 mg on day 1, then 250 mg once daily on days 2-5 (total 1.5 g).
Chlamydia: 1 g orally as a single dose.
Enteric fever: 500 mg once daily for 7 days.
MAC prophylaxis: 1200 mg orally once weekly.
Timing: capsules should be taken 1 hour before or 2 hours after meals. Tablets and suspension can be taken with food.
Important Drug Interactions
- QT-prolonging drugs (amiodarone, sotalol, fluoroquinolones, ondansetron): additive QT prolongation risk
- Warfarin: may enhance anticoagulant effect (monitor INR)
- Antacids (aluminium/magnesium): reduce absorption if taken simultaneously (separate by 2 hours)
- Cyclosporine: azithromycin may raise cyclosporine levels
- Unlike erythromycin and clarithromycin, azithromycin does NOT significantly inhibit CYP3A4, resulting in fewer drug interactions
Side Effects and Monitoring
Common: diarrhoea, nausea, abdominal pain, vomiting (GI effects are milder than erythromycin).
Serious: QT prolongation and risk of torsades de pointes (especially in patients with pre-existing cardiac conditions or electrolyte disturbances), hepatotoxicity (rare, cholestatic), C. difficile-associated diarrhoea.
Rare: hearing loss (usually reversible, associated with high doses or prolonged use).
Monitoring: ECG in patients with cardiac risk factors. LFTs if prolonged use or symptoms of hepatotoxicity. No routine monitoring needed for short courses in healthy patients.
Special Populations
Pregnancy: Category B. Considered safe. Drug of choice for Chlamydia in pregnancy (single 1 g dose).
Renal impairment: No dose adjustment needed (primarily hepatic/biliary elimination).
Hepatic impairment: Use with caution. Avoid in severe hepatic disease due to risk of hepatotoxicity.
Elderly: Higher risk of QT prolongation. Use with caution, especially with concurrent QT-prolonging medications.

Indian Regulatory Status
Azithromycin is a Schedule H1 drug under the Drugs and Cosmetics Act, reflecting the regulatory intent to limit over-the-counter dispensing and promote antimicrobial stewardship. Schedule H1 requires the pharmacist to maintain a record of the sale with the prescriber's details. It is included in the NLEM of India. The Indian National Action Plan on Antimicrobial Resistance emphasises rational macrolide use.
Frequently Asked Questions
Why does azithromycin only need a 3-day or 5-day course?
Azithromycin has a tissue half-life of 68 hours and concentrates heavily inside cells (especially phagocytes). A 3-day course maintains therapeutic tissue levels for 7-10 days after the last dose, effectively making it equivalent to a longer course of other antibiotics.
Does azithromycin interact with other drugs like erythromycin does?
Azithromycin has far fewer drug interactions than erythromycin or clarithromycin because it does not significantly inhibit CYP3A4. However, it still carries a risk of QT prolongation and should be used cautiously with other QT-prolonging drugs.
Is azithromycin safe in pregnancy?
Azithromycin is Category B and is generally considered safe in pregnancy. It is used for Chlamydia infections in pregnant women as a single 1 g dose. It should still be used only when clearly indicated.
Why was azithromycin overused during COVID-19?
Azithromycin was widely prescribed during COVID-19 based on early, low-quality data suggesting antiviral and anti-inflammatory effects. Subsequent large trials (RECOVERY, PRINCIPLE) showed no benefit. The ICMR subsequently removed it from COVID-19 treatment guidelines. This episode is an antimicrobial stewardship teaching point.
